1.Significance of Changes of Endogenous Hydrogen Sulfide in Rats with Doxorubicin-Induced Heart Failure

Objective To investigate the changes in hydrogen sulfide(H_2S) level in plasma and myocardial tissues of doxorubicin-induced heart failure rats,and to study the role of H_2S in the development of doxorubicin-induced heart failure.Methods Male Wistar rats were randomly divided into 2 groups as follows:the doxorubicin group(n=12),in which 2.5 mg/kg of doxorubicin was injected intraperitoneal once a week for 10 weeks(total dose of 25 mg/kg);the control group(n=9),in which an(equivalent) volume of physiological saline was administered weekly for a total of 10 weeks.The observation of behavior was taken at the same time.Hemodynamic and echocardiographic measurements were obtained 10 weeks after treatment.Meanwhile,H_2S concentrations in serum and myocardial tissues were evaluated by modified sulfide electrode method.The changes of H_2S level between 2 groups were analyzed.Results The rats treated with doxorubicin showed inanimate behavior,decrease of the body temperature,activities and food intake,faster breathing,significant loss of weight,the cumulative mortality was 33%.Left ventricular systolic pressure(LVSP),difference of left ventricular pressure[△LVP=LVSP-left ventricular diastolic pressure(LVDP)] and left ventricular peak rate of contraction(+LV dp/dtmax),left ventricular peak rate of relaxation(-LV dp/dtmax)were significantly reduced in the group of doxorubicin rats(P
2.Effects of 5-HT and Norepinephrine on the Neuronal Activities of the Nucleus Tractus Solitaril in Medullary Slices

The relationship between 5-HT receptor and a-adrenoceptor was investigated in rat medullary slice preparations. In 57 nucleus tractus solitarii (NTS) neurons, 47 (82.5%) responded to both serotonin (5-HT) and norepinephrine (NE). After synapse transmission was blocked by perfusion of artificial cerebrospinal fluid (ACSF) with low calcium and high magnesium, 5 in 8 neurons were still sensitive to both 5-HT and NE. On the other hand, 3 in 5 neurons responding only to 5-HT or NA were sensitive to both 5-HT and NE after the synapse transmission blockage. In some neurons which exhibited excitatory or inhibitory responses to 5-HT or NE alone, the responses were inversed when perfused with 5-HT and NE in combination. These results suggest that there may exist an inhibitory interaction between 5-HT receptors and a-adrenoceptors in NTS and that most 5-NT and a-adrenoceptors coexist in the same NTS neurons.
3.In vitro study on new bioactive bone screws affecting biological behavior of osteoblasts

Objective To detect the effect of a new bioactive bone screws composed of nanohydroxyapatite/polyamid 66/glass fiber (n-HA/PA66/GF) biomaterial on biological behavior of osteoblasts with a view to a later clinical application of the screws.Methods Mice osteoblasts were co-cultured with n-HA/PA66/GF biomaterial or its extract.Cell growth was detected under a microscope,cell proliferation using MTT assay,apoptosis by flow cytometry,osteocalcin by ELISA method,cell migration using Transwell invasion assay,cell adhesion and growth by scanning electron microscope,and cytoskeleton and actin filament distribution using immunofluorescene.Results Direct contact test demonstrated n-HA/PA66/GF material had no obvious cytotoxicity to osteoblasts.Extract of n-HA/PA66/GF material stimulated osteoblast proliferative activity over time with absorbance value of 0.96 ± 0.14,1.54 ± 0.15,and 2.39 ±0.27 respectively after 2,4,and 6 hours of coculture (P < 0.05).The n-HA/PA66/GF material induced more osteoblasts to entering S period without obvious influence on apoptosis rate and promoted osteocalcin secretion.Migrated cells in medium supplied with n-HA/PA66/GF material or FBS was 8.73 ± 3.26 and 9.47 ± 3.29 in each visual field,but there was no significant difference (P > 0.05).Osteoblasts had a regular form on the surface of n-HA/PA66/GF material,closely adhered to the surface,and reproduced and aggregated with each other to form stratified cell layers.However,nHA/PA66/GF material exhibited no obvious influence on distribution of cytoskeleton and actin filament.Conclusion The new n-HA/PA66/GF screws has excellent cytocompatibility with positive regulatory effect on cell growth,proliferation,secretion,adhesion,cycle,and osteocalcin secretion.
4.Influence factors on prognosis of the patients with capillary leak syndrome

Objective To discuss influence factors on prognosis of the patients with capillary leak syndrome (CLS) in ICU.Methods The clinical data of 191 patients with CLS in ICU were reviewed,and the patients were divided into three groups according to prognosis:death group ( n =37),cured group ( n =132) and non-healed group (n =22).The clinical data of death group were compared with those of cured group at admission,during the course of CLS and before discharging from hospital.Results Compared with the cured group,the central venous pressure and serum albumin decreased ( P < 0.01 ) ; anion gap,triglycerides,pressure adjusted heart rate (PAHR) and oxygenation index were lower ( P < 0.01 or P < 0.05) ; serum glucose and SIRS score increased ( P < 0.01 ) in death group.There was higher rate of poor renal function at admission in death group than that in other groups ( P < 0.01 ).There were more many patients treated with intravenous administration of hydroxyethyl starch,ulinastatin and continuous blood filtration therapy in cured group than those in other groups ( P < 0.05).Conclusions The factors influencing the outcomes of the patients with CLS were hypovolemia,severe hypoproteinemia,interior milieu disorder,malnutrition,hypoxemia,renal injury and severe systemic inflammatory response.The outcomes of patients with CLS in ICU could be improved by using hydroxyethyl starch,ulinastatin and continuous blood filtration therapy.
5.Cytomegalovirus pneumonia after allogeneic hematopoietic stem cell transplantation

To compare the clinical features and prognosis in patients with cytomegalovirus pneumonia from other pneumonia after allogeneic hematopoietic stem cell transplantation (allo-HSCT). A total of 118 patients with pulmonary complications after allo-HSCT from March 2016 to June 2019 were analyzed retrospectively, who were divided into cytomegalovirus (CMV) pneumonia group ( n=34) and the non-CMV pneumonia group ( n=84). Compared with non-CMV pneumonia group, CMV pneumonia group presented earlier median onset time (1.8 vs.6.0 months, P=0.015) after allo-HSCT, more dyspnea (41.2% vs. 19.0%, P=0.012), hypoxemia (38.2% vs. 13.1%, P=0.006), and interstitial pneumonia (82.4% vs. 23.8%, P<0.01).The incidence of CMV-viremia and serum viral load in CMV pneumonia group were significantly higher than those in non-CMV pneumonia group. Consistently, and the development of mixed infection in CMV pneumonia group was higher than that of non-CMV pneumonia group (41.2% vs. 16.7%, P=0.013). The median follow-up time was 12.8 (0.4-46.5) months. The 1-year attributable mortality in CMV pneumonia group was significantly higher than that in non-CMV pneumonia group (26.5% vs. 10.7%, P=0.004), while the 1-year overall survival rate was significantly lower than that in non-CMV pneumonia group (61.8% vs. 85.7%, P=0.001). Reduced-intensity conditioning (RIC)( P=0.036), high flow ventilation ( P=0.033) and negative CMV-viremia ( P=0.009) were unfavorable prognostic factors of patients with CMV pneumonia. Compared with those with non-CMV pneumonia, patients with CMV pneumonia had more characteristic clinical manifestations and imaging features. However, due to the higher incidence of mixed infections, the causes of pneumonia need to be identified by bronchoscopic alveolar lavage. In conclusion, patients with CMV pneumonia have worse clinical outcome. RIC, high flow ventilation and negative CMV-viremia are adverse prognostic factors for CMV pneumonia.
6.Case Report of Brucellar Meningitis in Children and Literature Review

Objective To explore the clinical characteristics and therapy of brucellar meningitis in children.Method One case of brucellar meningitis was reported and relevant literatures were reviewed.A 10 years old boy was admitted to hospital with a history of half a month fever and cough and 3 days headache.His family raised sheep,but he denied contacting closely with sheep and eating raw goat's milk and mutton.Results The course of this reported child was subacute.Clinical manifestations included fever,headache and meningeal irritation sign positive.The cerebrospinal fluid(CSF)findings were similar to those of tuberculous meningitis,where WBC count was elevated with predominant lymphocytes,glucose and chloride decreased and protein increased.The serum bacterium burgeri antibody was positive,agglutination titer was 1160.After brucellar meningitis was diagnosed,three antibiotics including rifampin,minocycline and ceftriaxone sodium and other symptomatic treatment were used.The CSF findings were normal 2 weeks later.This child taked orally minocycline,rifampin and trimethoprim-sulfamethoxazole for 8 weeks post discharge,the recovery was good.The domestic and foreign documents showed that the incidence of the nervous system complications of brucellosis in children was lower than that in adult.The main clinical manifestations were acute meningitis /meningocephalitis.Brucellar meningitis could be misdiagnosed as tuberculous meningitis,because the course of disease,clinical manifestations and CSF findings were similar.Therapeutic alliance including 3 kinds of antibiotics,which could easily pass through blood-brain barrier,was used to treat brucellar meningitis.The total course of treatment shouldn't be shorter than 8 weeks.Conclusions Brucellar meningitis should be screened through epidemiological investigation,serum agglutination test and cerebrospinal fluid culture,in case the reason of meningitis of meningoencephalitis is unknown,especially its clinical manifestation resembles to tuberculous meningitis.
7.Effects on application of 2 kinds of vein detaining needle pricking to senile patients

Objective To investigate the pricking method of vein detaining needle during intravenous transfusion in senile patient,improve achievement ratio.Methods A total of 600 senile patients were divided into 2 groups,each group 300 patients.Control group posted trocar sheath and needle in blood vessel respectively,and experimental group posted sheath and needle in blood vessel together during pricking.Pricking success ratios were compared.Results The success ratios of 2 groups were analyzed statistically;there was significant difference between them.Conclusions To posted sheath and needle in blood vessel together is easy and convenient,which can raise pricking success ratio,and relieve the pain caused by repeated pricking,and gave the resources.
8.Expressions of transient receptor potential A1 and related inflammatory factors in the rat model of prostatic inflammation.

OBJECTIVETo explore the molecular mechanism of pain associated with chronic prostatitis and chronic pelvic pain syndrome (CP/CPPS) in the rat model of prostatic inflammation.
METHODSThirty-six male SD rats were equally randomized to an experimental and a control group, the former injected with 50 μl of 3% λ-carrageenan into the ventral prostate to make the model of non-bacterial prostatic inflammation, while the latter with the same volume of sterile saline solution. At 1, 2 and 4 weeks after modeling, the prostate, L6-S1 dorsal root ganglion (DRG) and spinal cord were harvested for examination of the expressions of the nerve growth factor (NGF), transient receptor potential ankyrin 1 (TRPA1), and calcitonin-gene-related peptide (CGRP) by immunohistochemistry and Western blot.
RESULTSThe expressions of NGF, TRPA1 and CGRP in the prostatic tissue were all significantly increased in the experimental group as compared with the control (P <0.05), with a gradual decrease with the prolonging of time (P <0.05). In the L6-S1 DRG and spinal cord, the expressions of NGF, TRPA1 and CGRP exhibited no significant differences between the experimental and control groups at 1 week after modeling (P >0.05) and kept at high levels in the experimental group at 2 and 4 weeks, though not significantly different from those at 1 week (P >0.05). Statistically significant differences were observed in the expressions of the three proteins in the experimental rats among different time points (P <0.05), but not between the two groups at any time point (P >0.05).
CONCLUSIONThe molecular mechanism of CP/CPPS can be evaluated in the rat model of prostatic inflammation established by injecting λ-carrageenan into the prostate. TRPA1 may play an important role in connecting the upstream and down-stream pathways of CP/CPPS-associated pain.

10.Analysis and preventive measures of company disputes

Objective To analysis the causes of disputes between professional caregivers and patients and relatives, correctly recognize its characteristics, perfect hospital management, take active measures to reduce and avoid disputes between companies and patients and to imprive the company quality. Methods Totally, 146 disputes were analyzed between January and October, 2009. Results The responsibilities of both professional caregivers and patients can not be ignored among the disputes. Conclusions Perfecting management system and improving company quality can make the accompanying staff work better for patients and social services.
