1.THE INFLUENCE OF DIFFERENT CULTURAL CONDITIONS ONEXPRESSION OF ScFv GENES AGAINST ALPHA-TOXIN OFCLOSTRIDIUM PERFRINGENS TYPE A

The ScFv gene containing Nco I and Bam HI was amplified by PCR, and inserted into the prokaryotic expression vector pHOG21 with Nco I and Bam HI digestion. After screening, high-level expression recombinant XL1-Blue (pHOG2E3) were obtained, and the different expression levels of ScFv were detected by SDS-PAGE and EUSA. When the strain was induced by 0.5 mmol/LIPTG and 0.4mol/L sucrose in the culture medium LB at 37℃ for 6 hours, the high level production of ScFv protein was obtained. The molecular weight of the expressed ScFv is 31, 000 D. The ScFv was mainly in the form of inclusion body, but it could also be in the culture medium and soluble periplasmic content. Above all, the ScFv protein could neutralize the phospholipase C activities of alpha-toxin of Gostridium perfrigens type A.

4.Clinical Significance of P- Selectin Expression in Children with Viral Encephalitis

Objective To study the clinical significance of P - selection expression in children with viral encephalitis and the correlation between this expression and the cerebral infarction with critical viral encephalitis. Methods Flow cytometric was employed to detect the expression of P- selection on the surface of platelet membrane in 44 children with viral encephalitis(20 light patients and 24 critical patients) and 20 healthy control children. The area of the cerebral infarction was determined by computed tomographic scan in 20 patients with critical viral encephalitis. The correlation between the two variables was analyzed. Results The expressions of P - selection on the surface of platelet membrane on less than 5 days and on 2 weeks after the onset of viral encephalitis were significantly higher in critical patients than those in normal control children and light patients( P

Objective To explore the clinical significance of monocyte chemoattractant protein-1(MCP-1)in children with Henoch-Schonlein purpura nephritis(HSPN).At the same time compare the association between serum and urine MCP-1,to investigate the impact of the both on them in children with HSPN.Methods Serum and urine MCP-1 were measured by enzyme linked immunosorbent assay in 50 children with Henoch-Schonlein purpura(HSP)(25 cases of them patients with renal injures),and 25 healthy children,the changes of serum and urine MCP-1 were compared;at the same time serum urea nitrogen,creatinine,urinary albumin,urine N-acetyl-D-glucosaminidase(NAG),urine ?2-MG,24 hours urinary levels of protein were investigated in children with HSPN by analyzing the correlation between these indicators and serum and urine MCP-1;urine MCP-1 in HSPN group were measured in recovery period,and were compared with urine MCP-1 in HSP group and HSPN group in acute period.Results 1.The expressions of urine MCP-1 was significantly higher in HSPN group than those in HSP group and healthy controls(P0.05).2.Urine MCP-1 levels were associated with proteinuria in children with HSPN,but serum MCP-1 levels had nothing to do with HSPN.3.There was a close correlation between urine MCP-1 expression and urinary albumin,urine NAG,urine ?2-MG and 24 hours urinary levels of protein,but the expression of urine MCP-1 levels were not correlated with the serum urea nitrogen and creatinine.4.There was statistical significance in urine MCP-1 in acute and recovery periods with HSPN group(P

7.Clinical randomized controlled study on acupuncture for treatment of peripheral neuropathy induced by chemotherapeutic drugs.

OBJECTIVETo seek the effective treatment for peripheral neuropathy induced by chemotherapeutic drugs.
METHODSSixty-four cases of peripheral neuropathy induced by Paclitaxel or Oxaliplatin were randomly divided into an acupuncture group and a medication group, 32 cases in each group. The acupuncture group was treated with therapeutic principle of dredging meridians and collaterals, tonifying qi and eliminating blood stasis, supplementing liver and kidney, nourishing blood and tendon. Hegu (LI 4), Taichong (LR 3), Zusanli (ST 36), Qihai (CV 6) and Quchi (LI 11) etc. were selected. The medication group was treated with intramuscular injection of Cobamamide. The neurotoxicity of two groups was compared with questionnaire of peripheral neuropathy induced by chemotherapeutic drugs before and after treatment.
RESULTSThe total effective rate for sensory nerve disorder of acupuncture group was 66.7% (20/30), which was superior to that of 40.0% (12/30) in medication group (P < 0.05).
CONCLUSIONAcupuncture is more effective than Cobamamide for treatment of peripheral neuropathy induced by chemotherapeutic drugs, especially for moderate and severe sensory nerve disorder induced by paclitaxel.

OBJECTIVETo evaluate therapeutic effects of Rigidfix cross pins combined with Intrafix pins for the reconstruction of anterior or posterior cruciate ligament under arthroscopy.
METHODSFrom January 2009 to June 2010,34 patients with anterior or posterior cruciate ligament injuries were divided into two groups : group A and group B. There were 24 patients in group A, including 19 males and 5 females,with an average age of (31.83±9.57) years old. The patients in group A were treated with anterior cruciate ligament reconstruction under arthroscopy; Rigidfix cross pins and Intrafix pins were used to fix femoral and tibial side respectively. Among the 10 patients in group B, 8 patients were male and 2 patients were female, with an average age of (27.20+7.59) years old. The patients in group B were treated with posterior cruciate ligament reconstruction under arthroscope; Intrafix pins and Rigidifix cross pins were used to fix femoral and tibial side sepectively. The drawer test and Lachaman test were used to evaluate postoperative knee stability. All the patients were followed up at least 18 months. Lysholm and Tegner knee scores were used to evaluate the clinical therapeutic effects.
RESULTSAll the 34 patients were followed up, and the duration ranged from 18 to 26 months,with an average of (20.79±2.39) months. All the patients obtained good pain relief and knee stability. In group A,Lysholm scores significantly increased from 43.04±7.57 preoperatively to 85.41±4.68, 92.50±3.05, and 93.45±2.57 at 6,12, and 18 months postoperatively; Tegner scores significantly increased from 2.62±0.92 preoperatively to 7.45±1.14, 8.58±0.77, and 8.95±0.55 at 6, 12, and 18 months postoperatively. In group B,Lysholm scores significantly increased from 46.20±8.27 preoperatively to 86.40±5.14,90.40±2.67,and 92.00±3.85 at 6,12,and 18 months postoperatively ;Tegner scores significantly increased from 2.00±0.66 preoperatively to 7.10±0.99, 8.60±0.84, and 8.80±0.42 at 6,12, and 18 months postoperatively. There were no differences in Lysholm and Tegner scores between group A and B at different times during follow-up. Lysholm scores of all patients significantly increased from 43.97±7.79 preoperatively to 85.70±4.76,91.88±3.06,and 93.02±3.01 at 6,12,and 18 months postoperatively. Tegner scores of all patients significantly increased from 2.44±0.89 preoperatively to 7.35±1.09, 8.58±0.78, and 8.91±0.51 at 6,12,and 18 months postoperatively. During the follow-up period,there were no serious immunological rejection and complications.
CONCLUSIONReconstruction of anterior or posterior cruciate ligament under arthroscopy with Rigidfix cross pins and Intrafix pins fixation is feasible therapy for anterior or posterior cruciate ligament injuries, and the fixation is rigid. The therapy restores knee stability and provides a satisfactory short-term results.

9.A study on the lingual shape and position of the teeth in Chinese with normal occlusion.

OBJECTIVETo measure the lingual surfaces of the teeth in normal occlusion and obtain the data for lingual brackets' bonding and modifying for Chinese.
METHODS50 casts of nonorthodontic subjects with ideal occlusion were examined using the YM-2115 3D measurement instrument. The data was calculated by the relevant software and analysed statistically.
RESULTSThe reference values of the lingual surfaces of the teeth in normal occlusion were obtained. These include crown angulation(tip), crown inclination(torque), offset angles of molars and crown facial prominence.
CONCLUSIONSCompared with the data of foreigners, the characteristics of the lingual surface of the teeth in Chinese normal occlusion has many differences. The accurate placement of the lingual orthodontic brackets should be assisted by laboratory techniques because of the highly variable morphology of the lingual surfaces of the teeth.

10.Assessment application of the modified CURB-65 score for emergency community-acquired pneumonia

Objective To evaluate the clinical application value of modified CURB-65 score for assessing severity of community-acquired pneumonia (CAP) in emergency patients.Methods During the period from May 2011 to May 2012,198 emergency patients with CAP enrolled in this study were evaluated by CURB-65 score and modified CURB-65 score,respectively.Based on the severity of CAP,patients were divided into mild pneumonia group (Group A,n =107) and severe pneumonia group (Group B,n =91).The clinical status and biomarkers (the white blood cell count,procalcitonin,pneumonia severity index,hospitalization days,and hospitalization expenses) were recorded and compared with t test.Group B was divided into survived-subgroup (n =62) and death-subgroup (n =29).The differences in CURB-65 score and modified CURB-65 scere between the two groups were compared with t test.The correlation of CURB-65 score and modified CURB-65 score with procalcitonin,pneumonia severity index,hospitalization days,and hospitalization expenses were determined with Pearson rank correlation method.Results The procalcitonin,pneumonia severity index,hospitalization days,hospitalization expenses,modified CURB-65 score and CURB-65 score in Group B were significantly higher than those in Group A [(3.70 ± 0.83) vs.(1.27±0.24),t=28.91,P<0.01; (121.33±16.74) vs.(73.79±9.21),t=25.23,P<0.01;(25.79±10.13) vs.(14.85 ±6.83),t=9.02,P<0.01; (22.71 ±3.84) vs.(9.83 ±1.24),t=32.76,P<0.01; (3.69±1.03) vs.(3.32±1.06),t=2.48,P<0.05; (4.21±1.13) vs.(3.41±0.96),t =5.39,P<0.01],while no significant difference was observed in the white blood cell count between GroupA and B (17.58 ±5.99 vs.16.86±4.41,t =0.97,P>0.05).For Group B,the modified CURB-65 score of death-subgroup was significantly higher than that of survived-subgroup [(4.75± ± 1.17) vs.(4.01 ± 1.09),t =2.95,P < 0.01],whilc no significant difference was observed in the CURB-65 score between the death-subgroup and survived-subgroup (4.01 ± 1.15 vs.3.58 ±0.97,t =1.86,P > 0.05).The CURB-65 score positive correlated with the procalcitonin (r =-0.803,P =0.025),and had no obvious correlation with the pneumonia severity index,hospitalization days,and hospitalization expenses (r=0.621,P=0.320; r=0.701,P=0.231; r=0.675,P=0.256); The modified CURB-65 score significantly positively correlated with the procalcitonin,pneumonia severity index,hospitalization days,and hospitalization expenses (r =0.951,P =0.003 ; r =0.965,P =0.002 ; r =0.947,P =0.004 ; r =0.961,P =0.002).Conclusions Compared with the CURB-65 score,the modified CURB-65 score is more efficient in evaluating the severity and prognoses of CAP for emergency patients.
