Objective: To observe the clinical efficacy of grain-sized moxibustion in treating chemotherapy-induced myelosuppression for non-small cell lung cancer (NSCLC) and its effect on quality of life (QOL). Methods: Eighty NSCLC patients admitted to the Inpatient Department of Zhejiang Cancer Hospital between September 2016 and March 2018 were recruited and divided into an observation group and a control group by random number method, with 40 cases in each group. The two groups both received chemotherapy with paclitaxel plus cisplatin (TP regimen). The control group received oral administration of leucogen tablets starting from the first day of chemotherapy, 20 mg each time, three times a day, for consecutive 14 d; the observation group was additionally given grain-sized moxibustion, once a day, five days per week at a two-day interval, until the fourteenth day. The myelosuppression severity was observed and compared between the two groups prior to chemotherapy, at the 3rd, 7th and 14th days of chemotherapy; the QOL in the two groups was evaluated before chemotherapy, at the 14th and 21st days of chemotherapy. Results: Regarding myelosuppression, the peripheral blood indicators increased significantly at the 3rd day of chemotherapy in both groups (P<0.05 or P<0.01); at the 7th and 14th days of chemotherapy, the peripheral blood indicators presented a decreasing tendency in the two groups, but the level in the observation group was still significantly higher than that before chemotherapy (P<0.01); at the 3rd, 7th and 14th days of chemotherapy, the peripheral blood indicators in the observation group were higher than those in the control group (P<0.05 or P<0.01); the occurrence rate of myelosuppression in the observation group was significantly lower than that in the control group (P<0.01). The QOL score in the observation group was markedly higher than that in the control group at the 14th and 21st days of chemotherapy (both P<0.05). Conclusion: Grain-sized moxibustion can effectively improve myelosuppression after chemotherapy for NSCLC, reducing its occurrence and enhancing the patient's QOL.

Objective To evaluate the expression quantity of T lymphoma invasion and metastasis inducing factor 1(Tiara 1)in gastric cancer tissues,tissues around the cancer over 3 cm and tissues of gastric benign lesions,and to analyze the correlation between amount of Tiaml and gastric cancer biological behaviours.Methods Real-time fluorescence quantitive RT-PCR(FQ-RT-PCR)was used to detect the expression level of Tiaml mRNA in 66 cases of gastric cancer tissues,tissues around the cancer over 3 cm and the gastric tissues of 11 cases with benign lesion.Results Positive rate of Tiam1 mRNA expression in gastric cancer tissues,tissues around the cancer was 92.4%,19.7%,respectively,and only one case express Tiaml mRNA in 11 cases gastric benign lesion tissues.There was significant difference(P0.05),on the other hand,the expression quantity of Tiaml mRNA in gastric cancer was significantly correlated with metastasis into lymph node,the extent of invasion,differentiated degree of histologic type,and the stages of TNM(P

Objective To investigate the clinical significance of myasthenia gravis (MG) associated autoantibodies.Methods Titin,ryanodine receptor (RyR)and acetylcholine receptor (AChR) antibodies were examined in the sera of 74 myasthenia gravies patients by ELISA.Results AChR,Titin, RyR antibodies were detected in 77.0% (57/74),39.2% (29/74) and 32.4% (24/74) of the MG patients,respectively.For thymoma MG,AChR,Titin and RyR antibodies were detected in 76.2% (16/21),71.4% (15/21) and 52.4% (11/21) respectively.For late onset MG,Titin and RyR antibodies were detected in 77.3% (17/22) and 50.0% (11/22) respectively.With respect to the modified Osserman classification,the positve rate for Titin and RyR antibodies is much higher in more severe patients (X~2= 16.094,P=0.001;X~2=11.226,P=0.011).Titin antibodies was significantly related with RyR antibodies (r=0.380,P=0.001).Conclusions Titin and RyR antibodies show high sensitivity for thymoma MG,and the combination of serological and radiological testing can increase both sensitivity and specificity in diagnosis of thymoma MG.The levels of the two antibodies may serve as important prognosis markers in MG.The induction of the immune response against Titin and RyR and the possible pathogenic effects of the two antibodies will be further studied.

OBJECTIVETo investigate the protection effects of electroacupuncture on injury of lipid peroxidation induced by liver ischemia in septic rats.

METHODSForty-eight male SD rats were subjected to sepsis induced by cecal ligation and puncture (CLP), and were randomly divided into a Sham operation group (group A), a CLP model group (group B), a CLP model plus electroacupuncture at "Zusanli" (ST 36) group (group C), a CLP model plus electroacupuncture at the shame acupoint (group D), a vagotomy plus CLP model group (group E) and CLP model plus electroacupuncture group after vagotomy (group F), 8 rats in each group. CLP was performed in group E and group F after the abdominal vagotomy. Bilateral "Zusanli"(ST 36) points and the shame acupoint were electroacupunctured (2 mA, 2/100 Hz) for 1 hour in group C, group F and group D, respectively. The hepatic blood flow (HBF) was detected by a laser-Doppler flowmetry at 6 h after CLP. The plasma activity of alanine aminotransferase (ALT) was also determined and specimens of liver were harvested for evaluation of malondialdehyde (MDA), xanthine oxidase (XOD) and assessment of the rate of water content.

RESULTSThe blood flow of the liver was (56.97 +/- 11.95) U in group C which was significantly lower than (80.12 +/- 19.57) U in group A but higher than (42.61 +/- 10.97) U in group B, (44.53 +/- 9.23) U in group D, (30.05 +/- 4.46) U in group E and (30.46 +/- 6.38) U in group F (all P < 0.05) 6 h after CLP. Meanwhile, the levels of MDA, XOD, ALT and the rates of water content in liver in group C were all significantly higher than those in group A, but lower than those in the other four groups (all P < 0.05). The levels of MDA, XOD, ALT and the rates of water content in liver in group E and group F were all significantly higher than those in group D (all P < 0.05), while the blood flow of the liver lower than that in group D (P < 0.05), and with no significant differences in all above measurements between group E and group F (all P > 0. 05).

CONCLUSIONElectroacupuncture at "Zusanli" (ST 36) can promote hepatic blood flow, inhibit lipid peroxidation and alleviate hepatic edema and dysfunction in septic rats, which might be related with the completeness of cranial nerve.

OBJECTIVETo analyze the relationship between tumor necrosis factor-alpha (TNFalpha) gene promoter -308 polymorphism and myasthenia gravis (MG) in Chinese and analyze secretion of TNFalpha in peripheral blood mononuclear cells (PBMC) in MG patients.

METHODSA biallelic polymorphism at position -308 in the promoter of TNFalpha gene was screened by PCR amplification and NcoI recognition site. One hundred and twenty-three MG cases and 115 healthy controls were included in this study. MG patients were classified to different groups according to clinical type, age at onset, and sex respectively. PBMC were isolated from 20 patients and 20 healthy controls, and then cultured in the presence or absence of phytohemagglutinin (PHA) and acetycholine receptors (AchR). The supernatants were harvested after incubation and stored until TNFalpha was assayed by enzyme-linked immunosorbent assay.

RESULTSThe frequency of TNFalpha-308 allele 2 (A) was found significantly increase in MG patients and showed a trend especially in late onset (> or =40 years) and male patients (P < 0.05). The allele A had no relationship with thymic pathogenesis in MG patients. But frequency of allele A was significantly higher in general type than in ocular type (P < 0.05). MG patients had a higher inducible level of TNFalpha by PHA and AchR, and could be down regulated after treatment.

CONCLUSIONPolymorphism in TNFalpha gene promoter -308 is associated with onset of MG. The microsatellite allele TNFalpha2 confer risk for the development of MG in Chinese patients. MG patients have a higher inducible level of TNFalpha.

Objective:To explore influence of multi-stage psychological intervention on long-term prognosis of patients with coronary heart disease(CHD)undergoing revascularization therapy.Methods:A total of 160 CHD patients, who suffered from depression after revascularization in our hospital,were selected.According to random number ta-ble,they were randomly and equally divided into routine psychological nursing group and multi-stage psychological nursing group,both groups were followed up for two years.Scores of self-rating anxiety scale(SAS),self-rating de-pression scale(SDS),Gensini score and incidence rate of main adverse cardiovascular events(MACE)were com-pared between two groups at enrollment and two years after enrollment.Results:Compared with enrollment,two years later,all scores significantly reduced in two groups(P=0.001 all);compared with routine psychological nurs-ing group,there were significant reductions in scores of SAS[(65.74 ± 3.95)scores vs.(61.13 ± 3.87)scores],SDS [(70.14 ± 5.69)scores vs.(65.59 ± 5.70)scores]and Gensini[(40.33 ± 4.30)scores vs.(35.14 ± 3.61)scores]in multi-stage psychological nursing group,P=0.001 all;Incidence rate of MACE in multi-stage psychological nursing group was significantly lower than that of routine psychological nursing group(13.75% vs.31.25%,P=0.008). Conclusion:Multi-stage psychological intervention can significantly relieve anxiety and depression state,reduce inci-dence of major adverse cardiovascular events in patients with coronary heart disease after revascularization.

OBJECTIVETo study the expression of MUM-1/IRF4 and its significance in follicular lymphoma.

METHODSNinety-eight cases of follicular lymphoma were enrolled into the study. They were graded according to the 2008 WHO criteria. The expression of MUM-1/IRF4 protein and other markers (CD10, bcl-6, bcl-2 and Ki-67) was studied using tissue microarray and immunohistochemistry.

RESULTSAmongst the 98 cases studied, there were 24 grade 1 cases, 30 grade 2 cases, 26 grade 3A cases and 18 were grade 3B cases. The rates of expression of MUM-1/IRF4, CD10, bcl-6, bcl-2 and Ki-67 (≥ 25%) were 39.8% (39/98), 62.2% (61/98), 80.6% (79/98), 87.8% (86/98) and 50.0% (49/98), respectively. MUM-1/IRF4 predominantly expressed in high-grade follicular lymphoma and showed a significantly positive correlation with lymphoma grade (r = 0.628, P = 0.000) and Ki-67 index (r = 0.473, P = 0.000). MUM-1/IRF4 expression had a significantly negative correlation with CD10 expression (r = -0.597, P = 0.000), but no correlation with bcl-6 and bcl-2 expression.

CONCLUSIONSMUM-1/IRF4 expression is significantly higher in high-grade follicular lymphoma, indicating that these cases have a high proliferative activity, more aggressive behavior and poorer prognosis. MUM-1/IRF4, when strongly expressed, is another helpful marker for the diagnosis of high-grade follicular lymphoma.

OBJECTIVETo evaluate the enhancing effects of ginsenoside Rg3 combined with mitomycin C and tegafur (MF) on postoperative chemotherapy in advanced gastric cancer.

METHODSSeventy-one postoperative patients with advanced gastric cancer were randomly divided into two groups, the control group (n=33), which received treatment with only MF (Mitomycin C+Tegafur), and the trial group (n=38), which were treated with ginsenoside Rg3+MF. The serum VEGF levels in the control group and trial group were detected preoperatively and postoperatively, meanwhile, the serum VEGF levels in 30 healthy persons were detected as comparison. The relations between patients survival and serum VEGF levels were analyzed.

RESULTSThe levels of serum VEGF in advanced gastric cancer were higher than those in healthy persons [(297.8+/-129.6) pg/ml vs (212.3+/-67.5) pg/ml] (P<0.01), and were correlated with the depth of tumor invasion, lymph node metastasis, tumor size > 4 cm and TNM stage (P<0.05). Fourteen weeks after operation, the levels of serum VEGF in trial group decreased below those of preoperation and approached to normal range, while in the control group, the levels of serum VEGF decreased near those of preoperation only. The median survival of patients in trial group and control group were 40 and 25 months respectively. The survival rate of patients in trial group was significantly higher than that in control group (P=0.047).

CONCLUSIONThe combined application of ginsenoside Rg3+MF chemotherapy can decrease the concentration of serum VEGF and improve the survival rate in advanced gastric cancer patients.
