3.Study on Extraction Technology of Polysaccharides from Gossypium herbaceum L. by Pectinase Hydrolysis

OBJECTIVE:To optimize the extraction technology of polysaccharides from Gossypium herbaceum L. by pectinase hydrolysis. METHODS:Polysaccharides were extracted from G. herbaceum L. by pectinase hydrolysis combined with traditional hot water extraction. With the extraction rate of polysaccharides as the evaluated index,single factor test and orthogonal design were applied to investigate the effects of 4 factors including enzymolysis temperature,enzymolysis time,the amount of enzyme and pH value on polysaccharides extraction rate,and verification tests were conducted. RESULTS:The optimal extraction technolo-gy was as follows as enzymolysis temperature of 40 ℃,enzymolysis time of 150 min,enzyme accounting for 2.0%,pH value for enzymolysis of 4.6. Under the above conditions,the average extraction rate of polysaccharides from G. herbaceum L. was 2.474%(RSD=3.34%,n=5). CONCLUSIONS:Pectinase hydrolysis is an effective method to extract polysaccharides from G. herbace-um L.. The optimal extraction technology is reasonable and feasible.
4.Effects of Perioperative Parenteral Nutrition Under Different Osmotic Pressure on the Incidence of Phlebitis

Objective To explore ways to decrease peripheral vascular complications during parenteral nutrition through peripheral veins. Methods According to the support sequences of giving peripheral parenteral nutrition, ninty perioperative cases were randomly divided into low(20. 3 kPa·h-1 ),medium(27. 1 kPa·h-1 ) and high(33. 9 kPa·h-1 ) osmotic pressure rates(n=30 in each group). The infusion pump was used to control the rate of infusion. The total parenteral nutrient was infused through BD24G venous indwelling needles. The pain ratings at injection sites were evaluated after parenteral nutrition everyday,the incidences of phlebitis were observed three days later after removing the indwelling needles. ResultsUnder the low osmotic pressure,the pain was mild,while for the medium osmotic pressure,the pain became more obvious. The pain was aggravated obviously under high osmotic pressure. There were only two cases of over grade-II phlebitis under low osmotic pressure rate,9 cases under medium osmotic pressure rate and 14 cases under high osmotic pressure rate, in which a grade-Ⅲ phlebitis developed. Conclusion Decreasing the osmotic pressure rate through peripheral venous is an effective method to avoid or reduce the incidences of peripheral vascular complications with the peripheral parenteral nutrition support.
5.Preliminary exploration of Internet-based practice of stomatology hospitals

Internet-based practice of stomatology hospitals plays an important role in promoting hierarchical medical system and overcoming the social problems of " high costs and difficulties of medical services" . The authors introduced the practice of free online consultations at a tertiary stomatology hospital during this period. They also analyzed such characteristics of the practice as the doctors′ response rate, doctors′ response timeliness, as well as the specialties and timeframe distribution of patient consultations. The hospital provides online consultations on Guangdong cloud-based hospital platform in two modes. For example, the normal consultation mode operates in a many-many manner, with services provided by attendings and associate chief physicians appointed less than three years. In off hours, patients can also raise questions online. The expert consultation mode offers consultations during business hours in a one-to-one manner, with services provided by associate chief physicians appointed over three years and chief physicians. From February 10 to March 22 2020, 2 156 persons/times of consultations were served in the normal mode, and 1 043 persons/times received in the expert mode. The response timelines of the latter (16.19 min) was much better than the former (27.10 min). In terms of consultation workload of sub-specialties, oral medicine ranked the top (69.30%), while the consultations of the normal mode in work hours accounted for only 73.47% of total consultation workload. It is recommended to optimize resources deployment, introduce artificial intelligent inquiry service, explore an online-offline combined stomatological service, and improve performance management. These measures, when in place, can promote sustainable development of Internet-based stomatology hospitals.
6.Overview of CDISC standard and implementation in China.

CDISC standard has become a set of global data standards that can be used in clinical study, covering the full life cycle of clinical researches. After nearly 20 years of development and continuous version upgrades, CDISC standard can improve the quality and efficiency of clinical research and drug review, and to facilitate all stakeholders involved in researches to exchange the study data and communicate the outcomes. CDISC standard has been or is to be adopted as standard format in data submission by multiple regulatory authorities, and more widely implemented by the global pharmaceutical community. CDISC standard is gradually adopted in China. The feasibility and roadmap of CDISC standard as the Chinese data submission format requirements are undergoing exploration and piloting further.

7.Expression of Rabies Virus Glycoprotein Gene in Saccharomyces cerevisiae

To obtain non-pathogenic rabies virus glycoprotein(RV-G),we expressed RV-G in Saccaromyces Cerevisiae(S.cerevisiae).In our study,tat-G fusion gene was cloned into the expression vector pYes2.0,which allows expression of a foreign gene in the yeast cells under the control of GAl1 promoter.Transforma-tion was performed by using lithium-treated yeast cells and several Ura+-tranformants were isolated.Ac-cording to the relative mobility in SDS-PAGE,we know probably two forms(designated as yGI and yGⅡ) of RV-G analogues produced in S.cerevisiae,their molecular weights were estimated as 66 kD and 56 kD,respectively.On the other hand,there was a specific band about 56 kD shown in western blot result.Com-bining precursors’ achievements,we will draw a conclusion that trans-membrane domain(TD) and cyto-plasmic domain have a negative regulation on RV-G antigen immunogenicity in S.cerevisiae.
8.Colorectal cancer survival analysis in major areas in shanghai China

Background and purpose:The incidence and mortality of colorectal cancer increased gradually in China, cancer survival rate plays an important role in guiding cancer prevention and treatment.Methods:Data of colorectal cancer cases diagnosed during 2002-2006, follow-up information and death report were from Shanghai Cancer Registry, Life Table and EdererⅡ were used to calculate observed survival (OS) and relative survival (RS), the related demographic characteristics and status were also analyzed to relfect the survival situations of the colorectal cancer survivors in major areas in Shanghai.Results:In this study, 16 682 colon cancer cases and 11 906 rectum cancer cases were included in analysis, 5-year OS rates for colon cancer and rectum cancer were 48.84% and 51.65%, RS rates were 70.50% and 71.31%. Signiifcant difference in survival had been found among the various diagnostic stages, the survival rate in stageⅠ was much higher than those in stageⅢ and stageⅣ. The survival of colorectal cancer patients with different gender, age, tumor histological and residential areas were also found with differences, female was higher than male, age ranged from >44-54 years had the highest survival rates than other age groups. The survival of epithelial tumors was higher than other histologic types in colon cancer, and the survival of non-epithelial tumors was higher than other histologic types in rectum cancer. Over the past 3 decades, the 5-year OS increased dramatically in Shanghai. Conclusion:The level of survival of colorectal cancer in Shanghai is similar with it in developed counties. Colorectal cancer survival rate with different population characteristics may provide suggestions and evidence for further improvement of early screening, diagnosis and treatment.
9.The patterns of head and neck cancer incidence in Shanghai from 2003 to 2012

Background and purpose:Head and neck cancer is common worldwide. This article aimed to describe the patterns of incidence of head and neck cancer and their changes in Shanghai from 2003 to 2012, in order to provide reference for prevention programs, research and control strategies on cancer.Methods:The data of lip, oral cavity and pharynx cancer cases were collected by the Shanghai Cancer Registry. The distributions of Shanghai lip, oral cavity and pharynx cancer incidences from 2003 to 2012 were described. The patterns were described according to gender, age, basis of diagnosis, histological type, diagnostic stage in detail. We compared incidences of recent 5 years (from 2008 to 2012) with those of previous 5 years (from 2003 to 2007).Results:On average from 2003 to 2012, 1105 new cases of head and neck cancer were diagnosed in Shanghai each year, with 2.08% among the total cancer cases. The crude rate was 8.01 per 100000 and the age-standardized rate was 4.45 per 100000. Nasopharyngeal cancer was the major subtype of the head and neck cancer, with 50.81% among the total head and neck cases. The crude and age-standardized rates among males were higher than those among females. The histologically verified percentage was 85.77%. The squamous carcinoma was the major histological type, with 57.58% among the total cases. The age-stan-dardized rate of nasopharyngeal cancer was in decline.Conclusion:The incidence of head and neck cancer was low in Shanghai during the past 10 years. Male cases were more than female cases. The major histological type was squamous carcinoma. Half of new cases were nasopharyngeal cancer which appeared to affect patients at a relatively young age. Patients with nasopharyngeal cancer were diagnosed at relatively advanced stages.
10.Survival analysis of patients with gastric cancer in Shanghai

Background and purpose:Incidence and mortality of gastric cancer have increased gradually in China. Population-based cancer survival rate can relfect improvements in cancer diagnosis and treatment, as well as provide evidence for gastric cancer prevention and control.Methods:Data from gastric cancer incidence and follow-up between 2002 and 2006 were collected by the Shanghai Cancer Registry. Life table and EdererⅡ were used to calculate observed survival (OS) and relative survival (RS).Results:A total number of 28 243 gastric cancer cases were documented between 2002 and 2006 in Shanghai. The 5-year OS and RS of gastric cancer were 32.15% and 45.81%, respectively. Survival rates of gastric cancer varied by age, residence, diagnostic stage, and tumor histological type, but not by gender. Patients aged 35-44 years who were diagnosed at stageⅠ or with tubular adenocarcinoma, had the highest survival rates. Moreover, patients in rural areas had higher 5-year OS than those in urban areas. During the period 2002-2006,both 5-year OS and RS of gastric cancer increased dramatically, as compared with 1972-1976. Conclusion:Survival rates of gastric cancer varied by age, residence, diagnostic stage, and tumor histological type. The 5-year OS and RS signiifcantly increased in Shanghai over the past three decades, but are still far below developed countries. Early diagnosis of gastric cancer would be an effective strategy to improve survival and prognosis of patients with gastric cancer.
