1.Predicting Coronary Atherosclerosis by Right Subclavian Artery Ultrasound and Lipid Levels

Purpose To analyze the correlation of right subclavian artery ultrasound and individual blood lipid level with coronary atherosclerosis (CA) using carotid ultrasound as control so as to explore the clinical value of them in predicting CA.Materials and Methods Coronary CT angiography (CCTA) and right subclavian artery/neck vessel ultrasound in 55 patients were retrospectively analyzed.Gensini score was used to calculate the degree of coronary artery lesions.According to the result of CCTA,patients were divided into normal group,mild coronary atherosclerosis group,severe coronary artery stenosis group.The intina-media thickness (IMT) at the origin of right subclavian artery and carotid artery bifurcation on ultrasound was measured to evaluate carotid lesions.The correlation between the IMT and Gensini score of coronary artery was analyzed.The blood lipid levels of the 3 groups were analyzed,and the efficacy of right subclavian artery,carotid artery ultrasound and blood lipid in predicting CA was evaluated.Results ① The IMT at right subclavian artery and carotid artery bifurcation were positively correlated with Gensini score,and the former had higher correlation than the latter (r=0.636,r=0.462,P<0.01);the prediction efficacy of the IMT at right subclavian artery was slightly higher than that at carotid artery bifurcation (with sensitivity and specificity of 78.3%,64.5%,and 77.5%,60.0% respectively).② Compared with normal control group,the levels of low-density lipoprotein cholesterol,triglycerides,total cholesterol were higher,but the level of highdensity lipoprotein cholesterol (HDL-C) was lower in mild coronary atherosclerosis and severe coronary artery stenosis group,and the difference between the severe coronary artery stenosis group and normal groups was bigger,among which the level of triglycerides had the largest difference among the three groups with no statistical significance (P>0.05).The HDL-C had a more significant correlation with Gensini score than other index of blood lipid (r=-0.151).③ The combination of IMT and HDL-C had a sensitivity of 81.0% and specificity of 66.7% in predicting CA,which were higher than that of each individual index (with sensitivity of 78.3%,64.5% and specificity of 64.3%,55.5%respectively).Conclusion The IMT at right subclavian artery has a better predictive value in determining the degree of coronary artery lesions than that at carotid artery bifurcation.The combined application of blood lipid levels and the IMT at right subclavian artery can improve the sensitivity and specificity of predicting CA.
2.Comparative study of neurological soft signs in remittent schizophrenia and non-psychotic first-degree relatives

Objective To examine the difference of neurological soft signs (NSS) among the remittent schizophrenia patients,first-degree non-psychotic relatives of schizophrenia patients and healthy controls.Methods The Cambridge Neurological Inventory (CNI) (the Chinese version) was administered to 86 remittent schizophrenia patients (patient group),86 first-degree non-psychotic relatives of schizophrenia patients (relative group) and 86 healthy controls (control group).Results Compared with relative group,NSS total-score,the subscore of motor coordination and sensory integration were significantly higher in patient group (P<0.01).Compared with control group,NSS total-score,the subscore of motor coordination and sensory integration were significantly higher in patient group (P<0.01).Compared with control group,NSS total-score and the subscore of motor coordination were significantly higher in relative group (P<0.01).Conclusions The levels of NSS in remittent schizophrenia patients and their first-degree non-psychotic relatives are higher than normal control,and the patients have more NSS than their relatives.The motor coordination nay be a potential endophenotype for schizophrenia.
3.The feasibility of dual-source CT using low voltage scan and iterative reconstruction combine with split-bolus injection to reduce radiation dose in urography

Objective To evaluate the image quality,diagnosis accuracy and dose reduction of split-bolus CT urography (CTU) with low voltage scan and sinogram affirmed iterative reconstruction (SAFIRE).Methods A total of 80 cases of consecutive patients with confirmed or suspected urinary system disease needed CTU examination were divided into two groups (control group and test group) by using random number table.In control group,convention scan (120 kV) with one time injection was used.But low voltage scan (80 kV) with SAFIRE algorithm and split-bolus injection (SBI) was used in experiment group.The radiation dose,image quality and diagnosis accuracy were compared.Results A total of 77 cases completed CTU examination successfully in the two groups,including 39 cases in control group and 38 cases in test group.The effective dose reduced from (26.68 ± 4.07) in control group to (3.93 ± 0.85) mSv in test group (t =-33.78,P < 0.05).Subjective image quality score was (4.49 ± 0.79) in control group and (4.39 ± 1.53) in test group,with no significantly statistical difference (Z =2.71,P > 0.05).Signal-to-noise ratio (SNR) of objective image quality in test group was higher than that in control group (127.3±15.9 vs.109.6 ± 13.2,t =4.49,P<0.05).But there was no significantly statistical difference in contrast-to-noise ratio (CNR) between control group(100.8 ± 12.9)and test group (109.0 ± 14.4,P > 0.05).For diagnosis accuracy,no statistical difference were found between two groups(84.62% and 81.58%,P > 0.05).Conclusions The combination of low voltage scan with SAFIRE algorithm and split-bolus injection CTU could reduce the radiation dose significantly,but the objective image quality,CNR (except SNR) of subjective image quality and diagnosis accuracy were all unaffected obviously.
4.Acute renal failure in systemic lupus erythematosus

Objective To understand the clinical features of acute renal failure(ARF)as the initial presentation of systemic lupus erythematosus(SLE).Methods Eight cases of ARF in SLE from Jan 1995 to Apt 2006 were investigated,descriptive analysis and literature review were performed.Results①The symp- tom of ARF in SLE was mainly oliguria,with severe accompany symptoms and complications.②The level of leucocyte and hemoglobin was low in laboratory tests,also the complement level decreased significantly.The most frequent renal pathology was typeⅣ,Ⅳ+ⅤLN.③Large dose steroid and CTX were the mainstay of treatment.In addition,SCUF,CVVHDF and hemodialysis could be used for lethal conditions.Conclusion ARF can be the first manifestation of SLE and it usually represents more severe disease and more complica- tions.Large dose steroid and CTX can improve prognois.In cases refractory to steroid and if the effect is obso- lete,CTX treatment SCUF,CVVHDF and hemodialysis can be use.
5.Analysis of urinary iodine monitoring results of school-age children of Yi County in Hebei Province in 1998-2011

Objective To investigate and evaluate the iodine nutritional status of school-age children in Yi county of Heibei Province,and to provide scientific basis for the development of control measures to iodine deficiency disorders.Methods Retrospectinely analyzed school-age children urinary iodine monitoring results of Yi County of Hebei Province from 1998 to 2011.One township was selected randomly in each of the five directions (east,west,south,north and center) from 1998 to 2011.One elementary school was randomly chosen in each of the five townships.And at least 20 urine samples of 8-10 years old children were randomly collected in each school selected.Urinary iodine contents were determined through arsenic cerium catalytic spectrophotometric method.Urinary iodine determination standard according to The Standard of Iodine Deficiency Disorders Elinination (GB 16006-2008) and The Iodine Deficiency Disorders Elinination Guide.Results A total of 1725 children urinary samples were collected from 1998 to 2011,urinary median was 243.26 μg/L,each of the median urinary iodine was higher than 100 μg/L.The ratio of urinary iodine less than 100 μg/L was < 50% over the years,and less than 50 μg/L was < 20%.Overall,the highest value of urinary iodine stayed at approximately 200 μg/L,showing positively skewed distribution that was skewed to the right.Urinary iodine level was compared among different ages,gender and ethnicity,and the difference was not statistically significant(P > 0.05).Conclusions In recent years,the iodine intake of school-age children of Yi County can meet the body needs,and the iodine nutrition is at an appropriate level.
7.Assessment of effect of laboratory external quality assessment program on cytogenetic testing

Objective To analyze the change of chromosome test level after participating in the inter‐laboratory proficiency test(PT) program of the College of American Pathologists (CAP) .Methods The results for participating in CAP PT during 2011-2014 were analyzed ,the accuracy of the cytogenetic chromosome quality control test in this laboratory was obtained accord‐ing to the CAP evaluation results ,and the effect of CAP external quality assessment item on increasing the chromosome detection level in this laboratory was evaluated by analyzing the chromosome band levels before and after participating in CAP .Results This laboratory participated in CAP PT test for 10 times during 2011-2014 ,a total of 59 cases were analyzed ,the accuracy rate for jud‐ging chromosome karyotype was 100% ,the karyotype description accuracy rate was 95 .1% .The chromosome test results of clinical cases in this laboratory displayed that peripheral blood chromosome abnormal detection rate was 18 .9% and bone marrow chromo‐some abnormal detection rate was 25 .9% ,the abnormal rate of newly diagnosed leukemia was 66 .8% ;the detection failure rates of peripheral blood chromosome and bone marrow chromosome were 0 .5% and 5 .0% respectively ;the detection failure rates of pe‐ripheral blood chromosome and bone marrow chromosome after participating in CAP were decreased ,the chromosome band average level was improved ,showing statistical difference compared with those before participating in CAP (P<0 .01) .Conclusion Partici‐pating in high quality external laboratory assessment item can increase the clinical analytic level of cytogenetic chromosome test .
8.Study on clinical efficacy of combination of traditional Chinese medicine and western medicine in treatment of pit viper bites and peripheral blood inflammatory factors.

OBJECTIVETo observe the clinical efficacy of combination of traditional Chinese medicine and western medicine in the treatment of patient bitten by agkistrodon halys pallas, and the changes in peripheral blood inflammatory factors (hs-PCR, IL-6, TNF-alpha).
METHODNinty-eight patients were divided into three groups according to their hospitalization dates: the western medicine group, in which 32 patients were treated with antivenom serum (6 000 U) for five days, once every day; the traditional Chinese medicine group, in which 32 patients were treated with anti pit viper No. 2 concentrated decoction (300 mL), twice to three times every day, for five days; and the combined traditional Chinese medicine and western medicine group, in which 35 patients were treated with the combination of Chinese and Western medicine treatment described above. Then blood samples of all of patients were obtained, and serum factors (hs-PCR, IL-6, TNF-alpha) in peripheral blood were measured by Elisa assay. Another 30 health volunteers were chosen as the normal control group.
RESULTThe serum inflammatory factors were significantly higher in all patients of the three groups than that in healthy control before treatment (P < 0.01), and decreased significantly after treatment. In particular, the more remarkable reduction was found in the combined traditional Chinese medicine and western medicine group compared with the western medicine group and the traditional medicine group (P < 0.01). Symptom elimination in the three groups was superior to the western medicine group and the traditional medicine group at the first day and the third day of treatment (P < 0.05, P < 0.01). Total clinical effective rate was 100% in the combined traditional Chinese medicine and western medicine group, 84. 37% in the traditional medicine group and 65.62% in the western medicine group, the clinical effective rate of the combined traditional Chinese medicine and western medicine group was notably superior to that of the western medicine group and the traditional medicine group (P < 0.01).
CONCLUSIONThe serum inflammatory factors increased significantly in patients bitten by agkistrodon halys pallas. Treatment with the combined traditional Chinese medicine and western medicine can significantly decrease the serum inflammatory factors, and increase clinical effect, with more obvious clinical efficacy compared with the western medicine group and the traditional medicine group.

9.Effect of 5-HT7 receptor agonist on pyramidal neurons in the medial frontal cortex in a rat model of Parkinson's disease.

OBJECTIVETo investigate the activity of pyramidal neurons in the medial prefrontal cortex (mPFC) of normal and 6-OHDA-lesioned rats and the responses of the neurons to 5-hydroxytryptamine-7 (5-HT(7)) receptor stimulation.
METHODSThe changes in spontaneous firing of the pyramidal neurons in the mPFC in response to 5-HT(7) receptor stimulation were observed by extracellular recording in normal and 6-OHDA-lesioned rats.
RESULTSBoth systemic and local administration of 5-HT(7) receptor agonist AS 19 resulted in 3 response patterns (excitation, inhibition and no change) of the pyramidal neurons in the mPFC of normal and 6-OHDA-lesioned rats. In normal rats, the predominant response of the pyramidal neurons to AS 19 stimulation was excitatory, and the inhibitory effect of systemically administered AS 19 was reversed by GABAA receptor antagonist picrotoxinin. In the lesioned rats, systemic administration of AS 19 also increased the mean firing rate of the pyramidal neurons, but the cumulative dose for producing excitation was higher than that in normal rats. Systemic administration of AS 19 produced an inhibitory effect in the lesioned rats, which was partially reversed by picrotoxinin. Local administration of AS 19 at the same dose did not change the ?ring rate of the neurons in the lesioned rats.
CONCLUSIONThe activity of mPFC pyramidal neurons is directly or indirectly regulated by 5-HT7 receptor, and degeneration of the nigrostriatal pathway leads to decreased response of these neurons to AS 19.

10.Protection of lung function by introducing single photon emission computed tomography lung perfusion image into radiotherapy plan of lung cancer.

BACKGROUNDThe lung functional status could be displayed on lung perfusion images. With the images, the radiotherapy plans of lung cancer could be guided to more optimized. This study aimed to assess quantitatively the impact of incorporating functional lung imaging into 3-dimensional conformal radiotherapy (3DCRT) and intensity-modulated radiation therapy (IMRT) planning for non-small cell lung cancer (NSCLC).
METHODSTen patients with NSCLC who had undergone radiotherapy were included in this study. Before radiotherapy, each patient underwent CT simulation and lung perfusion imaging with single photon emission computed tomography (SPECT). The SPECT images were registered with simulation planning CT and used to contour functional lung (lung-F) and non-functional lung (lung-NF). Two 3DCRT plans and two IMRT plans were designed and compared in each patient: two anatomic plans using simulation CT alone and two functional plans using SPECT-CT in addition to the simulation CT. Dosimetric parameters of the four types of plans were compared in terms of tumor coverage and avoidance of normal tissues. Total radiation dose was set at 66 Gy (2 Gy x 33 fractions).
RESULTSIn incorporating perfusion information in 3DCRT and IMRT planning, the reductions on average in the mean doses to the functional lung in the functional plan were 168 cGy and 89 cGy, respectively, compared with those in the anatomic plans. The median reductions in the percentage of volume irradiated with > 5 Gy, > 10 Gy, > 20 Gy, > 30 Gy and > 40 Gy for functional lung in the functional plans were 6.50%, 10.21%, 14.02%, 22.30% and 23.46% in 3DCRT planning, respectively, and 3.05%, 15.52%, 14.16%, 4.87%, and 3.33% in IMRT planning, respectively. No greater degree of sparing of the functional lung was achieved in functional IMRT than in 3DCRT.
CONCLUSIONFunction-guided 3DCRT and IMRT plannings both appear to be effective in preserving functional lung in NSCLC patients.
