2.Changes in epidemiology and clinical characteristics of cervical carcinoma over the past 50 years

Objective To evaluate the changes in the epidemiology and clinical characteristics of cervical carcinoma over the past 50 years, and to explore the equitable treatment corresponding to these changes. Methods The clinical and pathological data of 1557 patients with invasive cervical carcinoma from January 1955 to December 2004 were retrospectively analyzed. Results (1) The average age of cervical carcinoma onset gradually decreased over the past 50 years, from 56.27?8.45 in years 1955~1964 to 43.81?8.9 in years 1995~2004. At the same time, the radio of clinically early (stages Ⅰ-Ⅱ) and non-squamous cancer also steadily increased. (2) Young patients (≤35 year old) had higher ratios of non-squamous, poorly differentiated and late stage (Ⅲ-Ⅳ) cancers, with a higher rate of lymphatic metastasis compared with other ages. (3) Because of the changes in epidemiology and clinical characteristics of cervical carcinoma, it′s necessary to alter the traditional model of treatment and to explore equitable therapy corresponding to these changes. Conclusions The average age of cervical carcinoma onset gradually decreased and simultaneously the ratio of clinically early (stages Ⅰ-Ⅱ) and non-squamous cancer also steadily increased over the last 50 years. Patients aged 35 and younger of cervical carcinoma had some obvious clinical characteristics compared with that of older patients. Preservation of reproductive endocrine function ought to be deliberated on in cervical carcinoma treatment of women with fecundity. 4.Effect of Kangaroo care on diminishing neonatal pain during heel lancing

Objective To investigate the effect of Kangaroo care(KC)on diminishing neonatal pain during heel lancing. Methods Sixty neonates were divided randomly into two equal groups:intervention group and control group,which 30 cases in each group.In the intervention group,neonates were treated with KC(neonates were put in direct and continuous skin-to-skin contact with his/her mom)from 20 mins before heel lancing to 1 mins after the end of the lancing.In the control group,neonates were treated with regular care.The differences in heart rate,blood oxygen saturation,facial pain expression,crying duration and DAN(Douleur Aigue Nouveaune)scores between two groups were compared at 7 time points from 10s before the heel lancing to 10s after the lancing. Results The results of repeated measu te at 7 time points after heel lancing.There were significant interaction effect between time and main effect(P<0.01),which showed that the heart rate of two groups were not equal at different time points.Compared with the time effects(P<0.01)which showed that two groups of neonatal heart rate of the two groups would accelerate with time extension. There were significant differences(all P<0.05)in heart rate at 7 time points between two groups and smaller heart rate varied range in intervention group.The results of repeated measures analysis for neonatal blood oxygen saturation at 7 time points after heel lancing. There were significant interaction effect between time and main effects(P<0.01),which showed that the blood oxygen saturation of two groups were not equal at different time points.Compared with the time effect(P<0.01),which showed that two groups of neonatal blood oxygen saturation would reduce with time extension.There were significant differences(all P<0.05)in blood oxygen saturation of two groups at 30s,40s and 50s after heel lancing and smaller blood oxygen saturation varied range in intervention group.The crying duration and DAN scores in intervention group were significantly reduced compared with the controls(all P<0.05).Conclusions
Heel lancing could lead to moderate to severe pain of neonates.KC could effectively diminish the neonatal pain degree caused by heel lancing.
5.Laparoscopic Modified Soave Procedure for 20 Cases of Hirschsprung′s Disease in Infants

Objective To summarize the treatment results,operative complications and experiences of laparoscopic modified pull-through(Soave)precedure.Methods Twenty children with Hirschsprung disease(HD),16 males and 4 females,with age from 3.5 months to 4 years old,underwent laparoscopic modified pull-through(Soave)precedure under whole anesthesia and tracheal intubation,and the pathological colon was freed by ultrasonic surgical knife.Results All cases were operated on successfully.The average operating time was 132 min.The average resection colon was 35 cm and the average volume of bleeding was about 14 mL without blood transfusion.Two patients had postoperative enterocolitis;3 patients were no complications postoperation,such as constipation,soiling,intestinal obstruction,preianal intestine prolapse or cuff narrowing,and all patients had 1-2 bowel movements per day during a period of 6 months to 3 years follow-up.Conclusion Laparoscopic modified pull-through(Soave)precedure has smaller incision,less bleeding,quick recovery and less complications.

9.Risk factors and prognosis of perioperative hidden blood loss in hip replacement patients

BACKGROUND:Hidden blood loss, a frequent occurrence fol owing artificial joint replacement, greatly affects the recovery from total hip arthroplasty. Many factors have been shown to have a correlation with the hidden blood loss, but the specific mechanism is not yet clear.
OBJECTIVE:To investigate the correlation of patients’ baseline (sex, age, underlying disease, obesity), prosthesis types, surgical time with hidden blood loss fol owing total hip arthroplasty, as wel as the association between hidden blood loss and prognosis.
METHODS:Ninety patients undergoing total hip arthroplasty were enrol ed in this study. Using Gross formula, we calculated the total blood loss according to height, weight, and pre-and post-operative hematocrit, and subtracted the dominant loss of blood to get the quantitative value of hidden blood loss. According to the criterion, the 90 patients were divided to two groups:group I:volume of hidden blood loss>480 mL, group II:volume of hidden blood loss<480 mL. Then we analyze the difference in the gender, age, underlying disease, type of prosthesis, surgical time between the two groups, and whether there was a correlation between these factors and hidden blood loss. Al the patients were fol owed for 1 year. Perioperative complications and survival curves were observed and monitored in the two groups.
RESULTS AND CONCLUSION:There were 39 patients in the group I, including 64.1%males, 64.1%hypertension patients, 35.9%diabetic patients, 7.7%patients receiving cement prosthesis;while, there were 51 patients in the group II, including 37.3%males, 25.5%hypertension patients, 5.9%diabetic patients, 35.3%patients receiving cemented prosthesis, showing a significant difference between the two groups. Based on these experimental findings, age, hypertension, diabetes mel itus, type of prosthesis were shown to be factors independently associated with hidden blood loss;however, obesity and smoking exhibited no correlation with hidden blood loss. In addition, a statistical difference in the survival rate was found at admission and during the 1-year fol ow-up.
10.Expressions of heat shock protein in uterine cervix with different pathological changes

Objective To study the expressions of main subtypes of heat shock protein (HSP) in uterine cervix with different pathological changes. Method 478 cases of cervical biopsy specimen were divided into invasive carcinoma of cervix group (63 cases), cervical intraepithelial neoplasia group (CIN, 106 cases), chronic cervicitis group (293 cases) and normal uterine cervix (16 cases) according to pathological diagnosis. The expression levels of HSP70, HSP90α and HSP90β mRNA were detected by quantitative RT-PCR with specific complex cRNA as internal control. Results (1) The expressions of HSP70, HSP90α and HSP90β mRNA were significantly downtrend stepwise in invasive carcinoma of cervix, CIN, chronic cervicitis and normal cervix tissue. (P<0.01, respectively). (2)In the invasive carcinoma of cervix group, the expression level of HSP90β mRNA was higher in advanced stage (FIGOⅡb-Ⅳ) compared with incipient (FIGOⅠa-Ⅱa) cancer of the cervix. (P<0.05). (3)The expressions of HSP70 and HSP90β mRNA were each higher in poorly differentiated tumor than in well-differentiated tumor. (P<0.05, respectively). (4)The expression levels of all three HSP mRNA had no significant differences were observed with different histological types of cervical cancer. (P>0.05). Conclusions Heat shock protein may play some important roles in malignant transformation of cervix cell and aggravation of cervix cancer. HSP70 and HSP90α may promote cancer cell transition and proliferation, and HSP90β may participate in cell differentiation.
