3.Significance of islet auto-antibodies and ?-cell function in patients with ketosis-prone diabetes

Objective To investigate the clinical characteristics of islet auto-antibodies and ?-cell function in patients with ketosis-prone diabetes(KPD). Methods A total of 78 patients with KPD were divided into four categorical groups based on the presence or absence of auto-antibodies (A+ or A-) and of ?-cell functional reserve(?+ ?-):A+?+(group 1,n=13),A-?-(group 2,n=18),A-?+(group 3,n=28),A+?-(group 4,n=19).Islet auto-antibodies,including glutamic acid decarboxylase antibody(GAD-Ab),islet cell antibody(ICA)and insulin autoantibody(IAA) were measured. The clinical characteristics,biochemical parameters and serum peptide were compared among four groups. Results Compared with group 1,2 and 4,group 3 showed the oldest age at onset,and higher levels of BMI,hypertension,serum triglyceride,cholesterol,fasting and postprandial C-peptide. Patients in group 2 had the youngest age at onset,the lowest level of serum C peptide. The phenotypes of patients in group 1 and group 4 were intermediate between group 2 and group 3. Conclusions About 39.7% of patients with KPD have positive islet auto-antibodies. Patients with KPD show significantly different levels of ?-cell function,clinical characteristics and biochemical parameters,which may need different therapeutic strategies.
4.Clinical analysis of three-dimensional conformal radiotherapy plus chemotherapy for locally advanced non-small cell lung cancer:study of 56 patients

Objective To analyze the efficacy,complications and prognostic factors of three-dimensional conformal radiotherapy(3 DCRT)for locally advanced non-small cell lung cancer(NSCLC).Methods 56 patients who were treated either by radiotherapy alone(14 patients)or radiotherapy plus chemotherapy(42 patients)from Jan.2005 to Feb.2008 were enrolled.The patient cohort consisted of 36 men and 20 women,median age 62,median total dose 60 Gy.Results The following-up rate was 96%.The number of patients completed follow-up were 14 and 10,respectively at 3-year and 5-year.The response rate of 3DCRT was 70%.with complete mmission 9%and partial remission 61%.The 1-、3-、and 5-.year survival rates were 62%、25% and 17%,respectively,and the median survival time(MST)was 20 months.By logrank test,clinical stage,KPS perfomance,tumor volume,radiation dose,treatment regimen and response to treatment showed statistically dramatic impact on overall survival.By Cox muhivariable regression,the independent adverse prognostic factors by both univariate analysis and multivariate analysis were clinical stage,treatment type,and response to treatment.Grade 2 acute radiation pneumonitis was observed in 1 patient and grade 3 in 1 patient.Late grade 2 lung injury developed in 1 patient,and grade 3 in 1 patient.Acute grade 1 radiation esophagitis were observed in 20 patients.and above Grade 2 in 5patients.Acute grade 1+2 hematologic toxicity developed in 15 patients,and above Grade 2 developed in 4 patients.Conclusions 3 DCRT was feasible in the treatment of locally advanced NSCLC with acceptable normal tissue toxicity.Relative early stage.radio-chemotherapy with total radiation dosage≥60 Gy and good immediate tumor response are favorable prognostic factors for overall survival.
5.Progress in the study of liver ischemia reperfusion injury

Liver ischemia reperfusion injury is a common pathophysiological process in hepatic surgery,which is the consequence of multiple mechanisms such as oxidative stress,inflammatory reaction,apoptosis,and autophagy and so on.However,the molecular mechanisms behind these processes have not been completely elucidated,the current treatment of liver I/R injury is merely supportive care,and thus new therapeutic strategies are needed.The recent studies indicate that the ischemic precondition and pharmacological precondition may protect the liver against hepatic ischemiareperfusion injury.It will be an important clinical significance of further studying its molecular mechanisms and protective strategies.
6.Study on Mongolian Medicine Prescription Classification Method Based on Fuzzy C-means Algorithm

Objective To classify Mongolian medicine prescription by using fuzzy c-means algorithm (FCM) and hard c-means algorithm (HCM); To explore the rationality of two kinds of clustering algorithm. Methods 27 Mongolian medicine prescriptions for treating Heiyi disease from Chuan Tong Meng Yao Yu Fang Ji were set as experimental data, and the data were preprocessed first. MS Visual Studio 2010 platform was used, and C# language was used for research and development. Chinese version and Mogolian version were implemented with WindowFrom and WPF technology, respectively. The medicine prescriptions were classified into 3, 4, 5, and 6 types by using FCM and HCM. Results All categorization with zero classification showed the existence of inclusion phenomena. The medicine in the classification results obtained by the two kinds of clustering algorithm did not exist cross. FCM could produce clustering results with smaller quantity difference and the more uniform classification compared with HCM. Conclusion The two algorithms are correct and reasonable, in which FCM algorithm has better clustering effect, and can be widely used in Mongolian prescription analysis, with a purpose to provide data supports for the research and development of new medicine.

Hydrogen sulfide is found to be the third gasotransmitter.It exerts extensive biological effects in nerve system,cardiovascular system,gastrointestinal system,and so on.In cardiovascular system,it exerts the function of vasorelaxation,inhibition of vascular smooth mu-scle cells and cardioprotection.It is also concerted with many cardiovascular diseases such as pulmonary hypertension,hypertension,coronary heart disease,atherosclerosis.This review discusses the latest research progress in the significance of hydrogen sulfide in cardiovascular diseases.
9.Treatment for pancreatic pseudocysts, report of 53 cases

Objective To evaluate the treatment and clinical outcome of pancreatic pseudocysts.Methods Clinical data of 53 pancreatic pseudocyst patients treated from Jan 2008 to Oct 2014 was retrospectively analyzed.Results 9 cases underwent CT-guided percutaneous catheter drainage,among them 3 cases underwent reoperation.8 cases underwent endoscopic ultrasonography-guided transmural drainage,including transgastric approach in 7,transduodenal approach in 1,Procedures in all these 8 cases were successful and pseudocyst disappeared completely without complication.Surgical procedures were performed in 36 cases,3 by external drainage,1 of the 3 underwent reoperation;11 cases underwent cystgastrostomy,with 2 suffering from postopertative gastrointestinal bleeding;3 cases underwent cystduodenostomy;19 cases underwent Roux-en-Y cystojejunostomy,with 1 suffering from postopertative gastrointestinal bleeding.Follow-up ranged from 4 months to 6 years,there was no cyst recurrence.Conclusion The treatment of pancreatic pseudocysts is diversified and should be individualized.Internal drainage is the mainstay of treatment,preferably through endoscopic ultrasonography-guided transmural drainage,leaving difficult cases to open cystojejunostomy.
10.Prophylactic cranial irradiation for non-small cell lung cancer: a systematic review

ObjectiveTo determine whether prophylactic cranial irradiation (PCI) has a role in the management of patients with non-small cell lung carcinoma (NSCLC)treated with radical intent.MethodsWe searched The Cochrane Library,MEDLINE,EMbase,CBM,CNKI and VIP.The quality of the includedstudieswascriticallyevaluated.Dataanalyseswereperformed usingtheCochrane Collaboration's RevMan 5.1 software.ResultsFour randomized controlled trials involving 905 patients met the inclusion criteria.The results meta-analyses showed the incidence of brain metastases was lower in PCI group compared with the observation group ( x2 =1.98,P =0.000 ) ; but there is no evidence of 1-year overall survival (OS) benefit ( x2 =1.12,P =0.880).Only RTOG 2009 provides prospective data:There were no significant differences in global cognitive function (P =0.600) or ADL ( P =0.880) after PCI,but there was a significant decline in immediate recall (P=0.030) and delayed recall (P =0.008 ) at 1 year,At 1 year,there was no significant differences in QOL after PCI ( P =0.050).Conclusions This systematic review show significantly decreases the risk of BM without improving 1-year OS in NSCLC patient receiving prophylactic cranial irradiation.There is insufficient evidence to support the use of PCI in clinical practice.Where possible,patients should be offered entry into a clinical trial.
