1.Effect of myopic defocus on static visual acuity and dynamic visual acuity

Background Researches have suggested that the defocus can induce the change of static visual acuity,but whether it produce influence on dynamic visual acuity is not clear.Objective This study was to investigate the impact of myopic defocus on static visual acuity and dynamic visual acuity and explore the essential difierence between static visual acuity and dynamic visual acuity. Methods Forty volunteers were enrolled in this trial.including 20 adults with the age of 27.4±1.64 years and 20 children with the age of 11.70+1.49 years.All the eyes of subjects received regular examined to excluded the eye disease with the best corrected vision of ≥1.0 D,astigmatism of ≤0.75 D and anisometropia <1.50 D.+1.00 D,+1.50 D,+2.00 D,+2.50 D slasses were ware respectively for the defocus on the foundation of full correction.Dynamic visual acuity was inspected by using selfmade DVA-I training software.and static visual acuity wag tested by static visual acuity chart (Precision Vision,CAT.NO.2125).This clinical trial complied with the Helsinki Declaration and obtained the approval of Ethic Committee of Wenzhou Medical College.Written informed consent was received from each individual prior to the protocol. Results The dynamic and static visual acuities were gradually decreased with the elevation of defocus (F=506.907,P=0.000).No significant differences were found between static visual acuity and dynamic visual acuity in adult or children at various defocus(P>0.05).The regression linear analysis showed that a positive correlation between static visual acuity with defoeus(R2=0.819,t=26.72,P=0.000) or dynamic visual acuity with defoeus(R2=0.826,t=27.42,P=0.000).The slope and intercept between defocus with static visual acuity were steeper than that between defocus and dynamic visual acuity (slope:F=34.18,P=0.000;intercept:F=1005.56,P=0.001). Conclusion The effect of defocus on static visual acuity is different from that of dynamic visual acuity.It could be speculated that dynamic visual acuity is related not only to refractive systems but also other factor.
2.Effects of Ang-Ⅱ on c-fos mRNA and pro-α1ⅠmRNA of cadiac FBC in rat embryo

Objective To observe the effects of Ang-Ⅱ on expressions of c-fos mRNA and pro-α1[Ⅰ]mRNA in fibroblasts (FBC) of rat embryo in the course of collagen synthesis.Methods The change of free Ca2+([Ca2+]i) was tested in fluorometry and the expressions of c-fos mRNA and pro-α1[Ⅰ]mRNA were tested by RT-PCR method in incubated FBC.Results The level of[Ca2+]i in FBC and the expressions of c-fos mRNA and pro-α1[Ⅰ]mRNA were enhanced with the increase of Ang-Ⅱ concentration(10-8~10-5and 10-7~10-6mol/L).Conclusions The expressions of c-fos mRNA and pro-α1[Ⅰ]mRNA were induced by Ang-Ⅱ activating signal system in FBC.
3.A comparative study on accommodative microfluctuation between emmetropic and myopic school-aged children

Background The incidence of myopia is increase.Some researches documented that formation of myopia is closely related with weakness of the accommodative response,enhancement of accommodative lag and accommodative microfluctuations in short-distance use of eyes.However,there still is controversy.Objective This study sought to compare the accommodative microfluctuations among emmetropic and myopic school-aged children at reading,and to discuss its potential relationship with the onset and development of myopia.Methods A casecontrolled study was designed.Eighty-nine children aged 8-12 years old were recruited in this study,including 47emmetropic children and 42 myopic children.Refractive error were checked by subjective refraction in phoropters and binocular vision and stereopsis were examined in all the subjects.A Grand Seiko WAM5500 auto-refractor was used to measure the accommodative responses and accommodative microfluctuations with different stimulus in 40 cm and 25 cm.The differences in accommodative response and accommodative microfluctuations at 25 cm and 40 cm reading distance were compared between the emmetropic children and myopic children using independent sample t test,and change of accommodative microfluctuations in myopic children at 25 cm and 40 cm reading distance was evaluated by paired t test.Results When the reading distance was 25 cm and 40 cm,the accommodative responses of emmetropes were (2.67 ±0.31) D and (1.70 ±0.23) D,and they were higher than (2.31 ± 0.33) D and (1.49 ±0.24) D of myopes,showing significant differences (t =5.330,P =0.000; t =4.140,P =0.000).Accommodative microfluctuation of myopes was(0.35 ±0.16)D in 25 cm reading distance and that of emmtropes was(0.26±0.08)D,with significant difference between them (t =3.180,P =0.002).However,there was not significant difference in accommodative microfluctuation at 40 cm reading distance between the myopic children and emmtropie children [(0.27±0.10) D vs.(0.24±0.09) D] (P=0.220).In myopic children,the accommodative microfluctuation at 25 cm reading distance was(0.35±0.16) D,showing a much increase than(0.27±0.10) D at 40 cm reading distance(t=3.850,P =0.000),but an insignificant difference in the accommodative mierofluctuations was seen between the 25 cm and 40 cm reading distance in the emmetropic children (P =0.145).Conclusions With the increased accommodative stimulus,myopic children present lower accommodative responses and larger accommodative microfluctuations.
4.Maxent modeling for predicting the global potential distribution of talaromycosis marneffei

Abstract: Objective To predict the potential distribution of talaromycosis marneffei (TSM) and analyze its driving factors, so as to provide evidence for the surveillance and prevention of this disease. Methods The data of all laboratory-confirmed, non-duplicating TSM published in the English and Chinese literature from the first case in January 1964 to December 2018 was collected. A Maxent ecology model using environmental variables, Rhizomys distribution and HIV/AIDS epidemic was developed to forecast ecological niche of TSM worldwide, as well as identify the driving factors. Results A total of 705 articles (477 in Chinese and 228 in English) were obtained during the study period. After excluding imported cases, a total of 100 foci information were included in the model. The area under the receiver operating characteristic (ROC) curve (AUC) of the model was 0.997 for the training set and 0.991 for the test set. Maxent model revealed that Rhizomys distribution, mean temperature of warmest quarter, precipitation of wettest month, HIV/AIDS epidemic and mean temperature of driest quarter were the top 5 important variables affecting TSM distribution. In addition to identifying traditional TSM endemic areas (South of the Yangtze River in China, Southeast Asian, North and Northeast India), other potential endemic areas were also identified, including parts of the North of the Yangtze River, Central America, West Coast of Africa, East Coast of South America, the Korean Peninsula and Japan. Conclusion Our finding has discovered hidden high-risk areas and provided insights about driving factors of TSM distribution, which will help inform surveillance strategies and improve the effectiveness of public health interventions against TM infections.
5.Efficacy of highly active antiretroviral therapy for childhood acquired immunodeficiency syndrome.

OBJECTIVETo investigate the efficacy of highly active antiretroviral therapy (HAART) for acquired immunodeficiency syndrome (AIDS) in children.
METHODSThe clinical data of 38 children (2-15 years old) with AIDS from a region of Rwanda and who had received HAART were retrospectively reviewed.
RESULTSAll of 13 children with anemia showed improved anemia symptoms after HAART. The hemoglobin contents returned to normal levels in 12 children with mild or moderate anemia. CD4 T lymphocytes increased by 24%-1 181% in 5 out of 6 cases with severe immunodeficiency after HAART. During the HAART, the weight gain averaged 2.3 kg yearly. The growth and development in 5 out of 8 children with delayed growth restored the levels of normal children of the same age after HAART.
CONCLUSIONSHAART can improve the health status in children with AIDS and is effective for childhood AIDS.

6.Depression of the Thoracolumbar Posterior Vertebral Body on the Estimation of Cement Leakage in Vertebroplasty and Kyphoplasty Operations.

BACKGROUNDThe cross-section of thoracolumbar vertebral body is kidney-shaped with depressed posterior boundary. The anterior wall of the vertebral canal is separated from the posterior wall of the vertebral body on the lateral X-ray image. This study was designed to determine the sagittal distance between the anterior border of the vertebral canal and the posterior border of the vertebral body (DBCV) and to analyze the potential role of DBCV in the estimation of cement leakage during percutaneous vertebroplasty (PVP) or percutaneous kyphoplasty (PKP).
METHODSWe retrospectively recruited 233 patients who had osteoporotic vertebral compression fractures and were treated with PVP or PKP. Computed tomography images of T11-L2 normal vertebrae were measured to obtain DBCV. The distance from cement to the posterior wall of the vertebral body (DCPW) of thoracolumbar vertebrae was measured from C-arm images. The selected vertebrae were divided into two groups according to DCPW, with the fracture levels, fracture grades and leakage rates of the two groups compared. A relative operating characteristic (ROC) curve was applied to determine whether the DCPW difference can be used to estimate the degree of cement leakage. The data were processed by statistical software SPSS version 21.0 using independent sample t-test and Chi-square tests.
RESULTSThe maximum DBCV was 6.40 mm and the average DBCV was 3.74 ± 0.95 mm. DBCV appeared to be longer in males than in females, but the difference was not statistically significant. The average DCPW of type-B leakage vertebrae (2.59 ± 1.20 mm) was shorter than that of other vertebrae (7.83 ± 2.38 mm, P < 0.001). The leakage rate of group DCPW ≤6.40 mm was lower than that of group DCPW >6.40 mm for type-C and type-S, but much higher for type-B. ROC curve revealed that DCPW only has a predictive value for type-B leakage (area under the curve: 0.98, 95% confidence interval: 0.95-0.99, P < 0.001), and when the cut-off value was 4.05 mm, the diagnostic sensitivity and the specificity were 94.87% and 93.02%, respectively.
CONCLUSIONSDepression of the thoracolumbar posterior vertebral body may be informative for the estimation of cement location on C-arm images. To reduce type-B leakage, DCPW should be made longer than DBCV on C-arm images for safety during PVP or PKP.

7.Clinical characteristics, diagnosis and treatment of hepatoid adenocarcinoma of the stomach.

OBJECTIVETo investigate the clinical characteristics,diagnosis and treatment of hepatoid adenocarcinoma of the stomach.
METHODSClinical data of 13 hepatoid adenocarcinomas of the stomach, collected from 201 cases of gastric cancer, were analyzed retrospectively.
RESULTSOf the 201 gastric carcinomas, there were 13 AFP-producing adenocarcinomas of the stomach, the positive rate was 6.5%. Morphologically, the tumor cells formed glandular, medullary and linear structures. Of the 13 hepatoid adenocarcinomas of the stomach, 10 cases were in gastric antrum and 10 cases were poorly differentiated. The metastasis rates of liver and lymph node in hepatoid adenocarcinoma of stomach were higher than those in non-hepatoid adenocarcinoma of stomach. The treatment of hepatoid adenocarcinoma of stomach depended mainly on radical resection, and adjuvant chemotherapy was needed.The prognosis of hepatoid adenocarcinoma of stomach was poor.
CONCLUSIONHepatoid adenocarcinoma of the stomach has its own special tumor biological behavior and poor prognosis. Special attention should be paid to this disease.

8.Effect of the sera of rabbits fed with Tongxinluo on MMP-9 and TIMP-1 expression and secretion in U937 monocyte-derived macrophages.

OBJECTIVETo investigate the effect of the sera of rabbits fed with Tongxinluo on the expression and secretion of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) in U937 monocyte-derived macrophages.
METHODSAtherosclerosis was induced in rabbits by high-cholesterol feeding, and the serum was obtained from the rabbits after administration of the aqueous solution of Tongxinluo or simvastatin by gavage. U937 monocyte-derived macrophages were incubated with the sera at different concentrations for 24 hours, and the changes in MMP-9 and TIMP-1 gene expression and secretion were detected by RT-PCR and enzyme-linked immunosorbent assay, respectively.
RESULTSThe serum of rabbits fed with Tongxinluo concentration-dependently inhibited the expression and secretion of MMP-9 in U937 macrophages, but did not affect TIMP-1 expression or secretion.
CONCLUSIONTongxinluo may stabilize the atherosclerotic plaques by inhibiting the expression and secretion of MMP-9.

9.Digital anatomy of the perforator flap in the thigh.

OBJECTIVETo provide algorithmic morphological data that enables safe elevation of the flow-through perforator flap, chimeric perforator flap in the thigh.
METHODS15 fresh cadavers were injected with a modified lead oxide-gelatin mixture for three-dimensional reconstruction using a spiral computed tomography scanner and specialized volume-rendering software (MIMICS). All of specimens were then dissected by layers. Angiography and photography were used to document the precise course, size, location, and type of individual perforators in the thigh region. The surface areas of cutaneous territories and perforator zones were measured and calculate with Photoshop and Scion Image.
RESULTSThe main artery supplying the thigh is femoral artery. There are (41 +/- 4.0) perforators whose outer diameters > or = 0.5 mm. These perforators have a superficial pedicle length of (4.2 +/- 1.7) cm. The average outer diameter is (0.8 +/- 0.1) mm. Each perforator supplies an average area of (44 +/- 6.4) cm2. There are lots of truly anastomoses among perforaors to form a subcutaneous network in the thigh.
CONCLUSIONSThe volume rendering technique is very useful for showing the subcutaneous network and preoperative flap design. The thigh appears to have the greatest potential for harvesting new or modified perforator flaps, especially, flow-through perforator flap or chimeric perforator flap.

10.Infection of Chlamydia trachomatis and apoptosis of spermatogenic cells.

OBJECTIVETo evaluate the relationship between infection of Chlamydia Trachomatis(Ct) and apoptosis of spermatogenic cells.
METHODSApoptotic spermatogenic cells were examined by Wright-Giemsa staining and the terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate(dUTP)-biotin nick-end labeling(TUNEL) technique.
RESULTSApoptosis rate of Ct infective group was significantly higher than that of normal group(P < 0.01).
CONCLUSIONSCt infection may cause the apoptosis of spermatogenic cells, which affords an objective evidence for illustrating the mechanism of Ct-infection-induced male infertility.
