1.Characteristics of volunteer blood donors with regular apheresis platelets in Zhoushan City

Objective:
To investigate the characteristics of volunteer blood donors with regular apheresis platelets in Zhoushan City, Zhejiang Province, so as to provide insights into recruitment of regular blood donors.
Methods:
Gender, age, occupation, educational level, and volume and times of blood donation were collected from volunteer blood donors with apheresis platelets in Zhoushan Central Blood Station during the period from 2019 to 2021 through the Zhejiang Provincial Blood Management Information System BIS version 3.0, and the willingness of blood donation, routes for acquiring blood donation knowledge and cause of blood donation were collected from volunteer blood donors with regular apheresis platelets using questionnaire surveys. The factors affecting regular apheresis platelets were identified among volunteer blood donors using a multivariable logistic regression model.
Results:
Totally 236 volunteer blood donors participated in regular apheresis platelets in Zhoushan City from 2019 to 2021, accounting for 51.08% of all volunteer blood donors with regular apheresis platelets, and there were 1 488 blood donations, accounting for 71.23% of all volunteer blood donations with apheresis platelets. A total of 2 086.00 U platelets were donated, accounting for 71.82% of all volumes of volunteer blood donations with apheresis platelets. The volunteer blood donors included 160 men (67.80%) and 76 women (32.20%) and had an age of 35 (17) years. There were 63 employees (26.69%) and 111 with a bachelor degree and above (47.03%). Multivariable logistic regression analysis showed a high proportion of regular blood donations among participants at ages of 31 to 60 years (OR:1.767-3.257, 95%CI: 1.010-6.250), university students (OR=2.598, 95%CI: 1.238-5.450) and civil servants (OR=2.733, 95%CI: 1.193-6.263). There were 228 participants with willingness to continue to participate in volunteer blood donation with regular apheresis platelets (96.61%), and 215 participants with a history of participating in volunteer blood donation surveys (91.10%). Internet was the predominant route for acquiring knowledge about apheresis platelet donation (41.10%), and “think that blood donation is a meaningful thing” was the predominant cause of participating in volunteer apheresis platelet donation (33.05%).
Conclusions
There is a high proportion of regular blood donation among civil servants and university students.
2.Application of titanium mesh fusion device in treatment of thoracolumbar burst fractures

Objective To explore the surgical method, clinical result, biomechanical property, and surgical indication of treatment of thoracolumbar burst fractures with titanium mesh fusion device. Methods 22 patients with the thoracolumbar burst fracture and paralysis were treated by anterior decompression and spinal stability reconstruction with titanium mesh fusion device and a titanium plate. In this group, the fracture was at the 12th thoracic vertebra in 7 cases, the 1st lumbar in 14, and the 2nd lumbar in 1. According to Frankel’s classification, 4 cases were of Grade A, 6 of Grade B, and 12 of Grade C. 20 patients experienced the kyphosis deformity. The kyphosis angles varied from 12 to 29 degrees with a mean of 19.5 degrees. Results The period of postoperative follow up varied from 0.6 to 2 years with an average of 1.2 years. The neural function was improved for more than 1 grade. 15 cases could control urination after the surgery. The kyphosis angles were reduced to 3 to 8 degrees with a mean of 5 degrees. The complications caused by the implants were not found. Conclusions To treat the thoracolumbar burst fractures with paralysis, the compressive elements from anterior side of the spinal cord should be completely removed. Since the titanium mesh fusion device plus a plate can reconstruct the spinal stability that satisfies spinal biomechanical property, it presents important application value.
3.Ventral release and posterior screw/rod implant fusion for irreducible atlantoaxial dislocation:one-year follow-up

BACKGROUND:Transoral ventral release and posterior fusion have predominated in the treatment of irreducible atlantoaxial dislocation, but there is no consistent conclusion on the clinical efficacy.
OBJECTIVE:To explore the clinical outcomes of transoral ventral release and posterior fusion and screw/rod implantation in the treatment of irreducible atlantoaxial dislocation.
METHODS:A total of 32 patients with irreducible atlantoaxial dislocation undergoing thetransoral ventral release and posterior fusion were selected. After treatment, they received cervical anteroposterior and lateral digital DR and cervical MRI examinations to understand the conditions of nerve compression and bone fusion. The recovery of nerve function was evaluated using Japanese Orthopaedic Association before treatment, 6 months after treatment and during final fol ow-up.
RESULTS AND CONCLUSION:Post-treatment, 29 patients were fol owed-up for an average period of 12 months. (1) Al the patients obtained perfect atlantoaxial joint reduction and bone fusion. This achieved reduction and reconstruction of spinal column stability. (2) Spinal compression was obviously lessened after treatment in al patients, and nerve functions were improved to different degrees. Significant differences in Japanese Orthopaedic Association score were detected between 6 months post-treatment, final fol ow-up and pre-treatment (P<0.05). (3) There were no serious intraoperative complications such as spinal cord or vertebral artery injuries. Postoperative complications such as infection or burst were also not found. (4) Imaging evaluation revealed that transoral ventral release and posterior fusion is safe and effective for treatment of irreducible atlantoaxial dislocation.
4.Investigation on nutritional health knowledge among regular unpaid blood donors in Zhoushan City

Objective :
To investigate the awareness of nutritional health knowledge regular unpaid blood donors in Zhoushan City, Zhejiang Province, so as to provide insights into nutrition and health education among unpaid blood donors.
Methods:
The regular unpaid blood donors were sampled using a convenient sampling method from Zhoushan Central Blood Station from January 1, 2021 to June 30, 2022. The nutritional health knowledge was investigated using the Chinese Nutrition Health Knowledge Questionnaire for Adults compiled by Chinese Center for Disease Control and Prevention, and the awareness and source of nutritional health knowledge was descriptively analyzed.
Results:
Totally 526 questionnaires were allocated, and 502 valid questionnaires were recovered, with an effective recovery rate of 95.44%. The respondents included 240 men (47.81%) and 262 women (52.19%), and included 343 individuals at ages of 18 to 44 years (68.33%), and 159 individuals at ages of 45 years and older (31.67%). The overall awareness of nutritional health knowledge was 14.54% among regular unpaid blood donors in Zhoushan City, and a higher awareness was seen among female regular unpaid blood donors (17.56%) than among males (11.25%) (P<0.05), while the awareness of nutritional health knowledge was significantly higher among respondents at ages of 18 to 44 years than among those at ages of 45 years and older (16.91% vs. 9.43%, P<0.05). The awareness of nutritional health knowledge was significantly higher among respondents with an educational level of junior college and above than among those with an educational level of high school/technical secondary school/technical school (17.24% vs. 12.22%, P<0.05), and the awareness was significantly higher among respondents with healthcare-related occupations than among those with other occupations (16.44% vs. 14.22%, P<0.05). Wechat, Weibo, Tik Tok and Kuaishou were main routes to obtain nutritional health knowledge (83.86%).
Conclusions
The awareness of nutritional health knowledge is low among regular unpaid blood donors in Zhoushan City. Men, middle-aged and elderly residents and residents with a low educational level are targets that should be given a high priority for nutritional health education, and new media may be fully utilized for nutritional health education.

6.Surgical treatment for aortoiliac occlusive disease:prevention and management of early complications

Objective To study the treatment of a or toiliac occlusive disease and prevention and management of early complications. Methods From Mar 1998 to Mar 2005, 83 aortoiliac arterial occlusion patients were treated with embolectom y by Forgaty catheter as the main therapy for acute occlusion. In cases of chron ic occlusion percutaneous transluminal angioplasty and stenting, prosthesis graf t bypass surgery as well as proximal endovascular procedure combining with dista l arterial reconstructions were adopted on case to case basis. Results There was acute aortoiliac occlus ion in 14 cases, 6 cases (42.9%) suffered from early complications after embole ctomy, with mortality of 7.1%. In chronic aortoiliac occlusion of 69 cases,va rious complications developed in 16 cases(23.2%),of which aortoiliac arteri al stent placement in 17 cases with complications of 23.5%;prosthesis bypass in 23 cases, with complications of 30.4%, and mortality of 4.3%;aortoili ac stenting combining with femoral-popliteal bypass in 29 cases(58 legs)with complications in 5 cases(17.2%). Conclusion The expansion and development of minimally invasive tech niques along with minimal invasive surgery may reduce surgical risks,decrease m obidity and mortality rate.
7.Atypical polypoid adenomyoma of the uterus: a clinicopathological review of 27 cases

Objective To investigate the clinical and pathological characteristics of atypical polypoid adenomyoma (APA) for improvement of the diagnosis, different diagnosis and treatment of the disease. Methods The clinical data, pathological characteristics, and the follow-up information were retrospectively analyzed in 27 cases of APA admitted in Peking Univeristy People′s Hospital from 2007 to 2016. Results The median age was 42.6 years old (range 25-60 years old). Fifteen patients were nullipara, 2 patients were postmenopausal. The most common presenting symptom was abnormal uterine bleeding (81%,22/27). Leisions were obtained by using hysteroscopy in 23 cases, hysterectomy 3 cases and dilatation and curettage 1 case. Fertility preserving treatments were performed in 10 patients who had strong desire for fertility, among which 1 case progressed into endometrial carcinoma. Among 15 patients underwent hysterectomy and (or) bilateral salpingo-oophorectomy, 9 cases of them had endometrial atypical hyperplasia. Endometrial carcinoma along with APA were found in three patients, 2 cases of them underwent hysterectomy and bilateral salpingo-oophorectomy and pelvic lymphadenectomy, the other one received medication for fertility preservation. Follow up information were available in 24 cases(89%,24/27)with a median follow up of 46 months (range 4-108 months), 1 case recurred and 1 case progressed into endometrial carcinoma. One case died of other malignancy, while the other patients were alive. Conclusions APA is a rare uterine neoplasm mixed with epithelial and mesenchymal component. It occurs mostly in childbearing-age women and its diagnosis is dependent on pathology. Although it′s clinical course is benign, there is risk of co-existance of endometrial carcinoma and endometrial atypical hyperplasia. For those who has desire of fertility, the treatment strategy is completely removed the lesion and closely followed up. For those who do not desire to preserve fertility, hysterectomy may be an option.
8.Effects of Community-based Rehabilitation on Schizophrenia

Objective To explore the effects of the community-based rehabilitation on schizophrenia. Methods 100 patients with schizophrenia from 4 communities were assigned to intervention group (n=50) and control group (n=50). The intervention group accepted comprehensive intervention for 6 months. All the patients were followed up for 12 months, and assessed with the Positive and Negative Symptoms Scale (PANSS), Personal and Social Function Scale (PSP), the General Quality of Life Inventory (GQOLI-74). Results 48 cases of the intervention group and 44 of the control group finished the study. The scores of negative symptoms and general spirit pathology were less in the intervention group than in the control group (P<0.01). The score of PSP was more in the intervention group than in the control group (P<0.01), as well as the total score and score of dimensions of physical function, mental health and social function (P<0.05). Conclusion Community-based rehabilitation can control the mental symptoms, and improve the social function and the quality of life of patients with schizophrenia.

10.Treatment-emergent central sleep apnea: a unique sleep-disordered breathing.

Treatment-emergent central sleep apnea (TECSA) is a specific form of sleep-disordered breathing, characterized by the emergence or persistence of central apneas during treatment for obstructive sleep apnea. The purpose of this review was to summarize the definition, epidemiology, potential mechanisms, clinical characteristics, and treatment of TECSA. We searched for relevant articles up to January 31, 2020, in the PubMed database. The prevalence of TECSA varied widely in different studies. The potential mechanisms leading to TECSA included ventilatory control instability, low arousal threshold, activation of lung stretch receptors, and prolonged circulation time. TECSA may be a self-limited disorder in some patients and could be resolved spontaneously over time with ongoing treatment of continuous positive airway pressure (CPAP). However, central apneas persist even with the regular CPAP therapy in some patients, and new treatment approaches such as adaptive servo-ventilation may be necessary. We concluded that several questions regarding TECSA remain, despite the findings of many studies, and it is necessary to carry out large surveys with basic scientific design and clinical trials for TECSA to clarify these irregularities. Further, it will be vital to evaluate the baseline demographic and polysomnographic data of TECSA patients more carefully and comprehensively.
