1. Role of bone marrow cells in renewal of renal tubular epithelial cells under physiological condition

Objective: To observe whether bone marrow cells (BMCs) can differentiate into the renal tubular epithelial cells under physiological condition. Methods: Transgenic C57BL/6 mice labeled with green flouresent protein (GFP) served as bone marrow donors and C57BL/6 mice without fluorescence labeling were divided into 3 groups: normal group (N, n=15), total body irradiation group (TBI, n=15), and bone marrow transplantation (BMT, n=15) group. The mice in Group N received no treatment, those in Group TBI were irradiated with lethal dose of γ rays but received no transplant (sacrificed within 1 week after irradiation), and those in Group BMT were transplanted with BMCs after the irradiation (sacrificed at 58 and 84 days after transplantation). Blood samples were obtained from all 3 groups for routine analysis and analysis of urea nitrogen and plasma creatinine. Meanwhile, mice kidneys were harvested for H-E staining. The amount of GFP positive BMCs in mice of BMT group was determined by flow cytometry; the distribution and amount of GFP positive cells in renal tissues were observed by immunohistochemistry. Results: There was no difference in the levels of urea nitrogen and plasma creatinine between the 3 groups. Lethal-dose total body irradiation resulted in no significant change in renal histological structure and function, but RBC, WBC and PLT of the mice in TBI group were markedly reduced compared with those in the other 2 groups (P<0.05). Green fluorescence was seen in renal tubular epithelium of mice on 56 and 84 days after transplantation by fluorescent microscope and laser scanning confocal microscope. Flow cytometry result showed that GFP positive BMCs were (78.75 ± 5.99)% and (79.58±4.60)% in recipient mice on 56 and 84 days after transplantation, respectively; and immunohistochemistry staining showed that GFP positive cells in renal tubular epithelial of the recipient mice were (4.17±1.66)% and (3.88±1.35)% , respectively (P>0.05). Conclusion: Bone marrow cells ma y participate in renewal of tubular epithelial cells even without the renal tubular damage.

According to the characteristics of the etiology and pathogenesis of child cerebral palsy, on the basis of "regulating the mind in treatment of all kinds of diseases" and "regulating the functions of five zang organs with back-shu points", Professor DONG Gui-rong applied the penetrating needling technique on the scalp points and acupuncture at back-shu points of five zang organs in the treatment of child cerebral palsy. The valuable clinical experiences have been summarized as "regulating the mind with scalp needling technique, benefiting the brain and opening the orifice", "regulating five zang organs with back-shu points" and "integration of acupuncture and rehabilitation, and function reconstruction". Two effective cases were introduced.

5.Correlation between Fibrinogen in Pleural Effusion and Pleural Adhesion in Tuberculosis Pleurisy

ObjectiveTo investigate the corelation between fibrinogen in pleural effusion and pleural adhesion in tuberculosis exudative pleurisy.Methods234 cases of primary tuberculosis pleurisy were divided into 3 groups (A、B、C) according to their level of fibrinogen in pleural effusion from low to high. The incidence rates of pleural adhesion were assessed during the course of treatment and after treatment.ResultsThe incidence rate of pleural adhesion in the course of treatment were as those: group A 10.5%, group B 32.3%, group C 54.5%. After treatment, it was as those: group A 10.5%, group B 16.9%, group C 42.4%. Whenever, there was significant difference between group C and group A or B (P<0.05).ConclusionThe level of fibrinogen in pleural effusion may be associated with pleural adhesion, which hinder the recovery of patients.

TTI gene coding for Tsetse thrombin inhibitor was modified with E.coli bias codon and expressed in Escherichia coli with high efficiency.Recombinant protein was purified to more than 98% purity.Assay for enzyme activity determination was set up.The result showed that the fusion protein exhibited inhibiting activity for thrombin.Inhibitory rate of purified TTI was 73% when concentration of thrombin and substrate was 10U/ml and 250?mol/L respectively.Inhibition pattern was determined as competitive with Ki at 35?mol/L.

?-lactamase inhibitor research is popular for its potential on ?-lactam antibiotics resistant strain.A ?-lactamase binding peptide SIPIS04-01 was obtained by the yeast two-hybid system.In vitro assay showed that it can inhibit the ?-lactamase activity.In order to improve the expression level of the recombinant peptide,a two-copy expression plasmid pYG563 was constructed by random orientation tandem repeat method after codon modification,the two-copy plasmid was successfully expressed and the product was increased by 48.4% than that of one-copy plasmid.Purified peptide showed inhibitory activity against TEM-1 ?-lactamase in vitro and the inhhibitory constant Ki was measured.
8.Observation of long-term efficacy and life quality in allergic rhinitis treated with acupoint catgut embedding therapy combined with acupuncture-moxibustion therapy.

OBJECTIVETo observe the impacts of acupoint catgut embedding therapy and acupuncture-moxibustion therapy on the long-term efficacy and patient's life quality in the treatment of allergic rhinitis.
METHODSSixty-nine patients were randomized into the combined acupuncture-moxibustion and acupoint catgut embedding therapy group (combined therapy group, 36 cases) and an acupuncture-moxibustion group (33 cases). In the acupuncture-moxibustion group, acupuncture was applied at Yingxiang (LI 20), Shangyingxiang (EX-HN 8), Yintang (GV 29), Shangxing (GV 23), Tongtian (BL 7) and Zusanli (ST 36). Moxibustion was applied at Zusanli (ST 36), Feishu (BL 13), Dazhui (GV 14) and Fengmen (BL 12). In the combined therapy group, on the basis of the treatment as acupuncture-moxibustion group, the catgut embedding therapy was applied at Feishu (BL 13), Fengmen (BL 12), Pishu (BL 20), Shenshu (BL 23), Zhongwan (CV 12) and Qihai (CV 6). The treatment duration was 4 weeks in the two groups. The clinical efficacy of allergic rhinitis and rhinoconjunctivitis quality of life questionnaire (RQLQ) score were observed before and after treatment as well as in the 4-weeks follow-up after the end of treatment respectively.
RESULTSThe markedly effective rate was 72.7% (24/33) in the combined therapy group and 48.4% (15/31) in the acupuncture-moxibustion group after treatment. The efficacy was similar between the two groups (P > 0.05). It was 57.6% (19/33) in the combined therapy group and was 22. 6% (7/31) in the 4-week follow-up after treatment, indicating the long-term efficacy in the combined therapy group was superior to that in the acupuncture-moxibustion group (P<0. 05). Scores of RQLO after treatment and in 4-week follow-up after treatment in both groups were improved as compared with those before treatment (all P < 0.05). In 4-week follow-up, the improvements in sleep and affection in the combined therapy group were superior to the acupuncture-moxibustion group (3.27 +/- 3.23 vs 4.61 +/- 3.56, 3.48 +/- 3.67 vs 5.81 +/- 4.15, both P < 0.05).
CONCLUSIONThe acupoint catgut embedding therapy combined with acupuncture-moxibustion therapy are safe and effective in the treatment of allergic rhinitis and display the more roles in the long-term efficacy.

9.Risk factors of pinhole infection in the fractured lower limbs after external fixation: a case-control study.

OBJECTIVETo investigate the risk factors of pinhole infection in the fractured lower limbs after external fixation.
METHODSThe case-control study was designed. From May 2009 to May 2014, the clinical data of 272 patients with lower limb fracture treated by external fixation device were collected. All the patients were divided into two groups according to post-operative pinhole infection. There were 29 cases in the case group including 23 males and 6 females. The age of patients in case group ranged from 25 to 77 years old,with the average age of (53.41 ± 12.77) years old. There were 243 cases in control group including 217 males and 26 females. The age of patients in the control group ranged from 27 to 78 years old, with the average age of (48.71 ± 11.87) years old. There were nine risk factors observed in our study including age, gender, fixed time by external fixation device, diabetes, time in bed, smoking, operation condition of other parts in the body, infection condition of other parts in the body.
RESULTSThe results of univariate analysis showed that there were statistically significant differences among age (χ² = 15.708, P < 0.001), fixed time by external fixation device (χ² = 11.940, P < 0.001), severity of the lower limb fracture (χ² =15.438, P < 0.001), diabetes (χ² = 8.519, P = 0.004) and time in bed (χ² = 7.165, P = 0.007) between case group and control group. The results of Logistic regression analysis showed that the risk factors of pinhole infection after fixed by external fixation device in the lower limb fracture were the advanced age (OR = 8.327, P < 0.001), fixed time by external fixation device (OR = 6.795, P < 0.001), diabetes (OR = 4.965, P = 0.001) and time in bed (OR = 4.864, P = 0.008).
CONCLUSIONThe advanced age, long fixed time, diabetes and long time in bed could increase the risk of pinhole infection after external fixation in the lower limbs with fracture.

10.Infliximab versus placebo in rheumatoid arthritis patients receiving concomitant methotrexate:a randomized double blind phase Ⅲ trial in China

Objective To evaluate the efficacy and safety of infliximab plus methotrexate combination therpy in Chinese with rheumatoid arthritis patients.Methods This was a double-blind placebo-controlled phaseⅢclinical trial,173 patients who had active rheumatoid arthritis were randomised to placebo(n=86)or infliximab(n=87)group on a background of a stable dosage of methotrexate.Patients were assessed at weeks 0,2,6,14 and 18.Results At week 2,the American College of Rheumatology(20)response criteria,which represent a 20% improvement from baseline,the same results with swollen joint count,tender joint count,du- ration of morning stiffness,VAS score,CRP,ESR were achieved in 52.9% of patients,compared with 14.0% of patients receiving placebo plus methotrexate.A 20% improvement was achieved in 75.9% of infliximab plus methotrexate at week 18,compared with 48.8% of patients on placebo plus methotrexate(P=0.0003).A 50% improvement was achieved in 43.7% of infliximab plus methotrexate at week 18,compared with 25.6% of pa- tients on placebo plus methotrexate(P=0.011).Infliximab was well-tolerated;withdrawals for adverse events as well as the occurrence of serious adverse events or serious infections were similar to those in the placebo group.There was only one case of tuberculosis in the treatment group.Conclusion Treatment with infliximab plus methotrexate is more effective than methotrexate alone in patients with active rheumatoid arthritis.It has rapid onset of effect and the efficacy is persistent.
