1.The role of transbronchial needle aspiration cytology in the staging and typing of lung cancer

Objective To explore the clinical role of transbranchial needle aspiration(TBNA) in the lymph node staging of lung cancer.Methods To forty-six cases of lung cancer with suspected lymph node metastasis by chest CT scan,the TBNA was performed before operation.The cytological results were compared with post-operative pathology.Results In total 94 groups mediastinal nodes,fifty-nine groups were positive by TBNA,sixty-seven groups were diagnosed lymph node metastasis by pathology,the positive accuracy was 88% with the total coincidence rate of 82.6% by cytolagical typing.The overall accurate rate of c-N by TBNA was 86.9%(40/46) compared with p-N.Conclusion TBNA for lymph node staging and pathologic typing of lung cancer is a safe,simple and economic method with high accuracy and high clinical application value.
2.Common Gram-negative Bacilli Infection and Antibiotic Resistance Analysis in Lower Respiratory Tract Disease among Aged People in Chengdu Region

OBJECTIVE To monitor drug resistance of the 5 kinds of Gram negative bacilli isolated from the old patients with lower respiratory tracts infection in Chengdu region. METHODS From Jan 2002 to Jun 2004,we collected the sputum samples of the old patients who were diagnosed lower respiratory tract infection with Gram negative bacilli,which were analyzed with automatic VITEK32 and 532 strains of common Gram negative bacilli were identified. RESULTS The most common 5 kinds of Gram negative bacilli,which were cultured among old patients in Chengdu region with lower respiratory tracts infection,were: Escherichia coli(27.44%),Klebsiella pneumoniae(20.49%),Pseudomonas aeruginosa(18.23%),Acinetobacter baumannii(17.67%),and Enterobacter cloacae(16.17%).For above bacteria,the ratio of the resistance to amoxicillin/clavulanic acid,ampicillin,cefazolin,compound sulfamethoxazole was over 40%.And the isolating rate of ESBLs produced by (E.coli) and K.pneumoniae was 20.78%. CONCLUSIONS Monitoring the common bacteria and ESBLs-producing bacteria precisely can provide therapy,guide to early cure and control lower respiratory tracts infection of aged patients effectively.

Objective To explore the diagnostic value of human telomerase reverse transcriptase (hTERT) and proliferating cell nuclear antigen (PCNA) in pleural and peritoneal effusion. ObjectiveTo compare the effects of preventative intra-aortic balloon pump (IABP) insertion with intra-or post-operative IABP insertion in high-risk patients undergoing coronary artery bypass grafting (CABG).MethodsFrom Jan 2008 to May 2011,one hundred and four patients received CABG or off-pump CABG (OPCABG) and IABP therapy in our hospital.The enrolled criteria of IABP insertion included left ventricular ejection fraction (EF) less than 0.40,unstable angina,AMI,left main stenosis,emergency CABG,refractory ventricular arrhythmia.Group1 included thirty eight patients with preoperative IABP insertion,thirty one for intra-operative IABP insertion ( group 2 ) and thirty four for postoperative IABP insertion ( group 3 ).The indications for IABP insertion for group 2 and 3 were unstable hemodynamics,failure to wean off cardiopulmonary bypass and low output syndrome during or after operation.Clinical data including operative mortality,ventricular fibrillation,chest drainage,hospital stay,ICU stay,ventilator supporting time,IABP supporting time,EF improvement rate,mechanical assist device and Inotropic drugs utilizations were analyzed among three groups.ResultsGroup 1 demonstrated good therapeutic results.There were significant statistic differences in operative mortality ( group 1 2.6%,group 2 12.9%,Group 3 47.1%),hospital stay[group 1 (23.6 ± 9.8) days,group 2 (21.5 ±9.7) days,group 3 (28.9 ±13.3) days],ICU stay[group 1 (2.3 ± 1.1 ) days,group 2 (3.5 ± 1.5 ) days,group 3 (5.2 ± 3.4) days],ventilator supporting time [group 1 (29.5 ± 23.0) hours,group 2 (38.7 ± 20.6) hours,group 3 (84.1 ± 48.0) hours],IABP supporting time [group 1 (77.0 ± 43.7 ) hours,group 2 ( 93.8 ± 44.8 ) hours,group 3 ( 121.5 ± 71.7 ) hours],EF improvement rate [group 1 (7.5 ± 7.2),group 2 ( 8.5 ± 7.5 ),group 3 (2.0 ± 6.7)],inotropic drugs utilization[group 1 ( 3.7 ± 4.9) days,group2 (6.2±4.6) days,group3 (10.8±5.4) days](P<0.05).ConclusionComparing with intra- or post-operative IABP insertion,high-risk patients undergoing CABG could significantly benefit from preventative IABP insertion.Early IABP insertion was recommended for high-risk patients undergoing coronary artery bypass grafting.
5.Correlation among unexplained oligospermatism,azoospermatism and micro-deletion in AZF gene

Objective To investigate the association among oligospermatism,azoospermatism in male sterility patients and micro-deletion in AZF gene of Y chromosome and to establish a integrate clinical diagnostic method for screening micro-deletion in AZF gene in Chinese.Methods PCR method was used to detect micro-deletion in AZF gene in 62 oligospermatism and azoospermatism patients and 20 normal male controls.Results 13.64%(6/44) of oligospermatism patients and 22.22%(4/18) of azoospermatism patients presented micro-deletion.However,micro-deletion was not found in 20 normal male controls.Furthermore,micro-deletion occur mostly in AZFc region.Conclusion Micro-deletion of AZF gene from Y chromosome is one of the major risks for oligospermatism and azoospermatism.
