Objective To observe the clinical effects of unilateral cleft lip repairmen via improved Millard technique. Methods 64 cases of patients with unilateral cleft lip were collected from July 2014 to June 2015. The traditional Millard technique was improved by moving traditional fixed point of columella nasi to close with the lateral nasal columella roots, and the moving range was adjusted according to the nasal columella deformity and nostril size. Meanwhile ,the nasal floor and the alar basal incisions were not designed. Instead ,fully free of affected side orbicularis oris muscles were applied to making apposition suture with the healthy side. Results It showed that anatomic form of the upper lip and the philtral column of patients ,whose unilateral cleft lips were repaired by the improved Millard technique, were normal, and both sides of the lip peak were basically symmetrical after 1 ~ 12 month(s)follow-up. Conclusions Compared with the traditional Millard technique, the improved technique could bring a better effect on appearance of the patients. And it will be one of the ideal repairing techniques for unilateral cleft lip.

Objective To detect the influence of the perfusion quantity and distribution of bone cement by percutaneous ky-phoplasty(PKP) on the early treatment result of thoracolumbar osteoporotic compression fractures(OVCF) .Methods From May 2011 to May 2013 ,62 cases of osteoporotic fractures of thoracic or lumber vertebra were treated by PKP .CT scans were performed postoperatively to analysis the distribution of the bone cement in the vertebra .According to the bone cement distribution on the transverse plane CT film ,the results were classified into four degrees :excellence ,good ,fair and poor .The cases were followed-up regularly .Preoperative and postoperative visual analogue scale(VAS) ,oswestry dysfunction index(ODI) ,height of the operated ver-tebra ,cobb angle ,the incidences of complications during and after the surgery were compared between groups of different degrees of bone cement distribution and different amount of bone cement injection .Results Among the 62 cases ,the follow-up time ranged from 3 to 36 months[average(10 .5 ± 5 .3)months] .In all of the cases ,there was statistically significant difference between the pre-operative and postoperative VAS scoring(P< 0 .05) .3 months after suergery ,there were no statistically significant influence on the results of VAS scoring ,the ODI scoring ,the height lost of the operated vertebra and the improvement of the Cobb angle(P> 0 .05) . In cases of bone cement injection more than 5 mL ,adjacent vertebra fractures happened in 3 cases 6 months postoperatively and 6 cases 12 months postoperatively .In cases of bone cement injection less than 4 mL ,there were only 2 cases of adjacent vertebra frac-tures happened 12 months posoperatively .The degree of vertebra height lost between the bone cement excellent group and poor group was statistically significant in 6 months and 12 months postoperatively .In cases when the distribution of bone cement was ex-cellent ,the improvement of pain and function was significantly different(P< 0 .05) .Conclusion OVCF is treated by PKP .Through conventional operation ,the ultra-early(within 3 months)efficacy is excellent ,in cases of different amount of bone cement injection and different degree of bone cement distribution .However ,with appropriate amount of bone cement ,the more eventfully and sym-metrically the distribution of the bone cement is ,the better of the early clinical results ,probably .

Objective:To explore the effect of nurse led cooperative integrated nursing model in children with hypospadias, provide reference for the integration of collaborative medical care and patient care, improve the nursing level of nurses and the quality of child care. Methods:By adopting a quasi experimental study method, from January 2020 to December 2021, the clinical data of 84 male children undergoing hypospadias surgery in the Department of Urology, Children ′s Hospital of Nanjing Medical University were retrospectively analyzed, and 84 accompanying staff were included in the study. Among them, 42 children admitted from January to December 2020 and 42 accompanying staff served as the control group, and 42 children admitted from January to December 2021 and 42 accompanying staff served as the intervention group. The control group adopted the routine perioperative nursing mode, and the intervention group adopted the nurse-led collaborative integrated nursing mode. The anxiety and satisfaction of the caregivers in the two groups, the medical fear, medical compliance and postoperative pain of the children in the two groups, and the incidence of postoperative complications of the children in the two groups were compared. Results:After intervention, the satisfaction score of the intervention group ′s accompanying staff and the score of the patient ′s medical compliance were (96.46 ± 3.27) and (2.93 ± 0.89) points. The control group ′s scores were (85.24 ± 5.71) and (1.75 ± 0.63) points. The differences between the two groups were statistically significant ( t=9.52, -8.40, both P<0.05). The anxiety score of the accompanying staff in the intervention group was (44.33 ± 2.43) points, and the medical fear score and postoperative pain score of the patients were (20.76 ± 2.92) and (3.06 ± 0.57) points, respectively. The control group′s scores were (67.11 ± 3.36), (33.58 ± 3.84) and (6.24 ± 0.71) points, respectively. The differences between the two groups were statistically significant ( t=23.47, 12.51, 22.66, all P<0.05). The total incidence of postoperative complications in the intervention group was 4.76% (2/42), while in the control group was 52.38% (22/42). The difference was statistically significant ( χ2=23.33, P<0.05). Conclusions:The nurse led collaborative integrated nursing mode has a positive effect on relieving the negative emotions of caregivers and children, improving the satisfaction of hospitalization, improving the quality of care for children, and reducing the incidence of postoperative complications of children.

Objective:To investigate the application of preauricular fascial pedicle mandibular flap for wound healing in upper-middle face of skin tumour. Methods:Select upper-middle face of skin tumour patients who were treated in department of oncology, first affiliated hospital of Bengbu medical college from December 2018 to October 2020..According to the defect site and scope, primary repair of a large range of skin and soft tissues of face by using preauricular fascial pedicle mandibular flap. Results:The group inclouded 10 cases of skin tumour, 5 males and 5 females, aged from 55 to 86 years, mandibular flap were all developed well and the configuration and function were satisfactory. After 3- 22 months of follow-up, no local recurrence and complications, such as difficulty of eyelid closure and deviation of mouth angle. Conclusions:Preauricular fascial pedicle mandibular flap technique with simple operation, safety and reliability. The flap with long pedicle and large area can be cut and it also can achieve 180° rotation to repair postoperative tissue defect wound in upper-middle face of skin cancer, lead to good effects of appearance and functionality.

Objective:To investigate the application of the preauricular fascial pedicled mandibular flap for wound healing after upper-middle face skin tumor resection. Methods:The patients with upper-middle face skin tumor were selected, who were treated in the Department of Oncology, First Affiliated Hospital of Bengbu Medical College from December 2018 to October 2020. According to the location and size of the defects, large defects of facial skin and soft tissues were primarily repaired by using the preauricular fascial pedicled mandibular flap. Results:This study included 10 cases of skin tumour, 5 males and 5 females, aged from 55 to 86 years. The mandibular flaps were all developed well and the appearance and function were satisfactory. After 3-22 months of follow-up, no local recurrence and complications occurred, such as difficulty of eyelid closure and deviation of commissure. Conclusions:Preauricular fascial pedicled mandibular flap technique has such advantages as simple operation, safety and reliability. The flap can be obtained with a long pedicle and a large area and it also can achieve a maximum of 180° rotation to repair postoperative tissue defects in upper-middle face of skin cancer resection, leading to good outcome of appearance and functionality.
