2.Rapid determination of ATP, ADP, AMP and phosphate in drug by 31P NMR spectroscopy.

The content of ATP, ADP, AMP, sodium phosphate and sodium pyrophosphate were determined by 31P NMR, the linear range of ATP, ADP and AMP were found to be 0.004-0.080 mol x L(-1), sodium phosphate and sodium pyrophosphate were 0.005-0.100 mol x L(-1). The RSD were 0.40%-1.30%, the recovery were 96.9% - 105.2%. The method has been applied to the determination of ATP injection. The impurities of ATP injection were ADP and sodium phosphate. The content of ATP is in line with the requirement of the pharmacopoeia. The results indicated that the method is of good reproducibility, high accuracy, rapid and simple operation, without pretreatment and interference of other elements, 31P NMR is a new and reliable method of analyzing ATP, ADP, AMP and phosphate.

3.Analysis on the frequency of urinary iodine in a population-based intervention study

Objective Through a two-year follow up program,this study was to analyze the urinary iodine frequency of a cohort in the intervention trial,concerning different doses of salt iodization,so as to explore the selection of appropriate concentration of salt iodization.Methods A multistage cluster sampling method was used to select three townships in two countries for community intervention with different doses [ ( 15 ± 5 ) mg/kg,(25 ± 5 ) mg/kg,( 35 ± 5 ) mg/kg ] of salt iodization.Results After intervention,the median of urinary iodine was reduced among the population.The urinary iodine frequencies of (15 ± 5) mg/kg and (25 ± 5) mg/kg among groups of children were mainly concentrated in 100-200 μg/L and 200-300 μg/L paragraphs in A county.While the 300 μg/Lparagraph had an overall decline in B county,the 100 μtg/L and 200 μg/L paragraph ratio increased but the trend seemed to be slow.The 100-300 μg/L paragraph of the four treatment groups took a larger proportion and kept smooth in a more ideal state.However,the control group still maintained at above 250 μg/L level.Conclusion The iodine supplementation should be gradually implemented in Chongqing.The doses of salt iodization should be reduced from the current (35 ± 15)mg/kg to (25 ± 5) mg/kg in the economically developed areas.At the same time,we need to continuously follow the changes of the condition.

5.A survey on the agricultural profession-related injuries among 11 902 rural residents in Shandong province

Objective To understand the prevalence and risk factors of agricultural activities related injuries among rural residents in Shandong province. Methods A retrospective investigation was conducted among agricultural profession-related workers in 20 villages with multistage cluster sampling method in Shandong province. Four times face-to-face interview were conducted by trained interviewers, including 32 students and local medical personnel under constructed questionnaires.Accidental injuries occurred in the activities or in the agricultural profession-related jobs were recorded, from May 1st 2009 to April 30st 2010. Data was input and analyzed by SPSS 13.0 statistical software. Results A total of 837 cases reported at least 1 job-related injury out of the 11 902 people who had been surveyed in one year. The crude incidence rate was 7.03% and the standardized incidence rate was 7.36%, higher in males (9.01%) than in females (4.10%), X2= 105.53, P=0.000.Children and adolescents (≤ 14 aged) had the higher incidence rate (9.50%), X2=9.70, P=0.008.People working in the area of commercial service related to agricultural products had the highest incidence rate (12.94%). In particular, those occupations that related to agricultural construction or materials appeared to have had higher incidence rates as 16.80% and 15.59% respectively, than other kinds of jobs (X2= 167.30, P=0.000). There were higher proportion of injuries occurred in the roads (28.79%), in the fields(28.08%) during labor work(38.00%) transportation(27.97%), respectively.The seasonality of agricultural injures mostly occurred between June and August, accounted for 47.43%. Major external causes related to injuries were instruments or tools (31.42%) being used,transportation (24.13%) and falls (20.19%). Wounds on limbs took the majority (56.39%). The accidental self-inflicted injury occupied 76.82%, while accidents to passive injuries occupied 11.47%,other kinds accounted for 11.71%. Most of the accidents caused mild or moderate damage, accounted for 60.22% and 30.34% respectively. 7 cases died of injuries, with the fatality rate as 0.84% and the mortality rate was 58.81 per one hundred thousand. 72 cases ended up in disability caused by injuries,with the morbidity rate as 6.05 per thousand. Answers to the major internal causes of injuries appeared to be: "did not know how to protect oneself" (29.87%) with females (45.41%) in particular,followed by "over fatigue" and "inappropriate treatment" which took the 2nd place in males. Major natural environmental factors of injuries would relate to "high air temperature and humidity but lower wind velocity"(14.93%), "unclear signs on the country road"(12.19%), "the sky was cloudy or dark"(10.87%), "slippery road caused by rain or snow" (10.51%), "kids were unintended" (10.27%) etc.Most of the wounded received treatment clinically (50.18%), with the percentile (P50) of medical cost as 182.76 RMB (Yuan). The proportion of inpatient was 27.72% and with percentile of time as P50=7.57 day and cost as P50=2840.00 RMB Yuan. The wounded had a rest of P50=5.9 days, with an indirect cost as P50=233.16 RMB Yuan. Conclusion The results of this study indicated that the incidence of agricultural profession-related injuries was high, with serious harm. Behavioral intervention and awareness of injuries should be enhanced, together with the improvement of environmental condition.
6.Retrospective study on clinical application of biovalves in 52 cases.

OBJECTIVE:
To summarize the clinical application of bioprosthetic valve replacement.
METHODS:
Fifty two patients, aged 13-73(52.4+/-14.0) years, underwent cardiac valve replacement with biovalves from June 2002 to June 2006 in our hospital. Fifty three tissue valve replacements combined with 8 mechanical valve replacements were performed. Other procedures were also carried out if appropriate, including bidirectional Glenn shunt in 1 patient with double outlet of right ventricle and pulmonary artery stenosis; interruption of accessory pathway in 1 patient with Wolff-Parkinson-White syndrome; coronary artery bypass graft in 5 with severe coronary artery disease; and atrial or ventricular repair in 4 with congenital septum defects.
RESULTS:
One patient died in hospital due to multiple organ failure. The hospital mortality rate was 1.9%. The mean follow-up was 25 months. Two patients lost follow-up and the follow-up rate was 96%. Forty one patients returned to NYHA class I, 9 to class II, and 1 to class III. Echocardiography showed the implanted bioprosthesis functioned well without stenosis or regurgitation. There was no structural deterioration with freedom from thromboembolism and anticoagulant hemorrhage in all patients.
CONCLUSION
The efficacy of biovalves in heart valve replacement is satisfactory.

7.Experiences of prevention and control of hospital infection in the emergency wards:the designated hospital treating new novel coronavirus disease in Shanghai as an example

Since the outbreak of novel coronavirus pneumonia, the World Health Organization and the National Health Commission have issued multiple guidance documents.Medical facilities across the nation have actively implemented the countermeasures in response to the epidemic.However, in the real scenario of prevention and control, hospital infection remains a great concern.It is crucial to formulate measures and procedures based on standard prevention and transmission route prevention to avoid cross infection between medical workers and patients.Based on the practice, we summarize the institutional management, layout process, prevention and control of hospital infection in the emergency wards for further discussion and improvement.
8.Contrastive study of three different treatments of unstable distal ulna and radius fractures in older children

Objective:To compare the clinical effect of manual reduction and plaster fixation, closed reduction and percutaneous K-wire fixation, as well as open reduction plate and elastic intramedullary pin fixation in the treatment of unstable distal ulna and radius fractures in older children.Methods:The clinical data of 110 cases treated in Department of Orthopedics, Children′s Hospital of Nanjing Medical University from January 2014 to December 2017, for unstable fractures of the distal ulna and radius were retrospectively reviewed.There were 73 males, 37 females, aged from 10 to 14 years[(11.6±2.3) years old]. According to the different treatments, the 110 cases were divided into 3 groups which included manual reduction group (48 patients), closed reduction and percutaneous K-wire fixation group (35 patients, K-wire fixation group) and open reduction with plate and elastic stable intramedullary nail fixation group (27 patients, open reduction group). All cases were treated with plaster external fixation.After the fracture healing, the plaster was removed and then exercises were begun.The data of operative time, fracture healing time, displacement rate and wrist function after treatment were recorded in the three groups and compared statistically.Results:There was no significant difference in general data between 3 groups before operation (all P>0.05). For the manual reduction group, K-wire fixation group and open reduction group, the operation time was (20.2±8.4) min, (35.4±12.2) min and (68.5±20.4) min, respectively; the clinical fracture healing time was (42.1±3.8) d, (44.2±4.3)d and (48.4±5.3) d; the cost of treatment was (2 000.4±551.3) Yuan, (8 000.2±1 151.6) Yuan and (20 010.7±2 453.3) Yuan, respectively.There were significant differences in operation time and cost treatment among the 3 groups ( χ2=11.266, F=58.427, all P<0.05). The Berton score of excellent and good were 89.6% (43/48 cases), 91.4% (32/35 cases) and 92.6% (25/27 cases) 3 months later after surgery in the manual reduction group, K-wire fixation group and open reduction group.The mean follow-up was (11.2±3.7) months(range 6-18 months). In the manual reduction group, there were 6 cases with fracture displacement within 2 weeks after the manual reduction.There were 3 cases with symptoms of caudal irritation in K-wire fixation group, which happened in 1 case in the open reduction group.No complications such as nonunion and iatrogenic nerve injury were found during the follow-ups. Conclusion:There is no significant difference in the wrist function among the 3 methods for the treatment of distal radius and ulna fractures in older children.The manual reduction therapy has a high fracture displacement rate.The open reduction therapy causes maximal operative trauma and costs highly.The percutaneous K-wire method is minimally invasive and has shorter operative time and a lower fracture displacement rate.Therefore, percutaneous K-wire is the optimal treatment for distal ulna and radius fractures.
9.Finite element analysis of the influence of anchorage number and location on Bankart repair strength of shoulder joint.

OBJECTIVE:
To study the influence of the number and position of anchor nails on the Bankart repair by the finite element analysis.
METHODS:
The 3D CT data of shoulder joints including normal adult volunteers were imported into Mimics 10.0 and Geomagic 2012 software to reconstruct the related tissues, and then meshed in Hypermesh 13.0 software to give material attributes. The position and number of MPC constraints were adjusted in Abaqus 6.14 to establish finite element model of shoulder joint under 6 working conditions including A model(normal control model), B model (Bankart damage control model), C model (1 anchor), D model (2 anchors), E model (3 anchors), F model (4 anchors). The humerus external rotation and forward load were added to simulate the fear test of the shoulder joint forward instability.
RESULTS:
(1)Normally, the inferior glenohumeral ligament complex accompanied by the external rotation of the humerus head has a wrinkle and a significant stress concentration around its anterior and posterior bundle terminations.(2)The stress of the anterior bundle and posterior bundle of the inferior glenohumeral ligament were increased by 52.33% and 45.67% respectively after Bankart's injury.(3)In each model of anchor repair, the stress concentration was obvious at the anchor site, and the stress of anterior and posterior bundle ligaments could be reduced; there were no obvious differences between C, D models and B model(>0.05), and there were significant differences between E, F models and B model(<0.05).
CONCLUSIONS
One or two anchors can reduce the stress of the anterior and posterior bundles of the inferior glenohumeral ligament in the repair of Bankart's injury, but when the angle of rotation is large, the stress concentration in the anchor position increases significantly, leading to the increases of the risk of failure. The application of three or four anchors can enhance the constraints on the inferior glenohumeral ligament and reduce the stress of the anchor itself through the "load sharing" effect between anchors, so as to ensure the stability of the shoulder joint and provide a mechanical environment for the early rehabilitation of patients.

10.Effect of overexpression of Smad7 gene on cell proliferation.

OBJECTIVETo study the effect of overexpression of Smad7 gene on cell proliferation in human bronchial epithelial cell lines.
METHODSHuman bronchial epithelial cell lines, BEP2D and BERP35T2 cells, were cotransfected with the mammalian expression vectors PCISmad7.neo and pMyc-SEAP, the latter was ac-myc cis-acting enhancer element fused with alkaline phosphatase (SEAP) reporter gene. Expression of c-myc, p15 and p21 mRNA was detected by RT-PCR before and after stable transfection of Smad7 into BEP2D and BERP35T2 cells in order to study the regulation of TGF-beta-mediated growth inhibition.
RESULTSAfter BEP2D and BERP35T2 cells transfected with Smad7, the transcriptional activity of c-myc was significantly increased. Smad7 overexpressing cells showed upregulation of c-myc expression and downregulation of p15 and p21 expression, which contributed to the loss of TGF-beta responses in these cells.
CONCLUSIONOverexpression of Smad7 may facilitate cell proliferation by antagonizing TGF-beta-mediated antiproliferative gene responses.
