1.A Case of Follicular Mucinosis.

A 17-year-old male had a coin sized, follicular plaque on the glabella for 3 months. Histopathologic examination revealed reticular degeneration in the pilosebaceous follicles and amorphous homogenous materials between the degenerated cells. Alcian blue and toluidine blue stained the material as blue and metachromatically purple, respectively. The material was subsequently confirmed as acid mucopolysaccharide. Two months after the initial visit, the lesion showed a tendancy to heal spontaneously. According to the data obtained, the case was considered as an acute benign form of follicular mucinosis.

2.The effect of digital rectal examination on serum levels of prostate -specific antigen.

The prostate-specific antigen (PSA) has been considered a useful tumor marker in monitoring responses, disease progression and recurrence after therapy in patients with prostatic cancer. However. The effect of digital rectal examination on serum PSA determination has remained controversial. The author. therefore. investigated the influence of examination on seurm PSA values. Blood samples were taken before (T0) and 5 minutes (T1), 1 hour (T2), 24 hours (Ts) and I week (T4) after digital rectal examination and the concentration of serum PSA was measured using a Tandeme radioimmunometric assay in 49 persons including 7 patients with prostatic cancer. 27 patients with BPH and 15 controls free of prostatic disorder, The following results were obtained. 1. In normal control group. the mean (+/-SD) PSA values before and 5 minutes, 1 hour. 24 hours and l week after digital rectal examination were 1.51+/-1.12, 2.12+/-1.57. 2.53+/-1.62, 2.39+/-1.41 and 1.97+/-1.20 (ng/ml) respectively. Although serum PSA values measured at 5 minutes. 1 hour, 24 hours and 1 week after digital rectal examination were significantly elevated (p<0.05), most of post-examination PSA values remained within the normal range. 2. In benign prostatic hyperplasia. statistically significant differences between PSA values before and 1 hour and 24 hours after digital rectal examination were observed (p<0.05), while in prostatic cancer group. there were no significant differences between paired pre-examination and post-examination mean PSA values. 3. There were statistically significant differences between PSA values before and 5 minutes, 1 hour, 24 hours and 1 week after rectal examination in normal-value group (<4.0 ng/ml) and between PSA values before and 1 hour after rectal examination in high-value group (>4.0 ng /ml). These results suggest that the evaluation of the concentration or PSA level for prostatic cancer suspected should be done before or immediately after prostatic manipulation.

3.A Clinical Study of 32 Patients with Sweet' s Syndrome.

BACKGROUND: The clinical study of Sweets syndrome has never been reported in Korea. OBJECTIVE: The purpose of this study was to elucidate the clinical observations and associated diseases of Sweets syndrome. METHODS: During a 5-year-period from Jan. 1990 to Dec. 1994, 32 patients were evaluated in regard to age incidence, sex ratio, location of skin lesions, and associated diseases. RESULTS & CONCLUSIONS : 1, The age distribution was in the range of 21-70 years. Most patients were between 31 to 40 years of age. There were 12 male patients and 20 female giving a sex distribution of 1.7: 1 ratio in favor of the fernale. 2. The most cornmon site of iavolvement was the arm(37.5%). 3. ESR was increased in 21 patients(65.6%) and leukocytosis was present in 13 patients(40.6) 4. The most common associated disease was Behcets disease(31.2%). 5. Steroids were used mos frequently for treatment of Sweets syndrome with 12 patients(37.5%). Recurrence occurred in 7 patients(21.9%). The results show that the iniidence of Sweets syndrome in men in Korea is not as rre as was previously thought and tfiat there are many associated diseases especially Behcets disease.

5.A Case of Acquired Digital Fibrokeratoma.

Acquired digital fibrokeratoma is an uncommon, solitary, round, firm, more or less hyperkeratotic projection, most frequently situated on fingers or toes but occasionally on the other locations. It arises out of a collarette of slightly raised skin and may have slight or great resemblance to a rudimentary supernumerary digit or may be mistaken for some other more ordinary condition such as an odd cutareous horn. We experienced a case of acquired digital fibrokeratoma on the right second finger in a 25-year-old male. We treated it with total excision.

7.Multiple Perifollicular Fibromas.

We report a case of multiple perifollicular fibromas. The patient was a 23-year-old female who had numerous skin colored perifollicular papules of 10 years' duration on her left cheek without any symptoms. Some of the perifollicular papules had a central comedo. A skin biopsy specimen of a perifollicular papule showed dense concentric fibrous sheaths surrounding the hair follicles.

8.Untrastructure of Melanocyte in Penile Melanosis.

Penile melanosis is a new disease entity which is benign and different from pigmented nevus and acral lentiginous melanoma in situ. A 20-year-old man had hyperpigmented macules on the glans penis and penile shaft for 5 years. Clinical and histologic findings were consistent with penile melanosis and giant melanosome complexes were observed ultrastructurally.

9. The individuals with rs224175 T/T genotype of the TGF-Β1 gene have an increased risk for developing IIBV infecton-associated diseases

Objective: To assess the association of three new single nucleotide polymorphism (SPN) sites of the transforming growth factor β1 (TGF-β1) gene, which is prevalent in Asia, with the susceptibility to the chronic hepatitis B infection-related liver disease. Methods: The genomic DNA of 129 cases suffering from the chronic HBV peripheral blood infection (74 cases with chronic type B hepatitis, 41 cases with liver cirrhosis and 14 cases with liver cancer) as well as 41 healthy volunteers were prepared. The genotypes of the TGF-β1 gene at rs2241715, rs2241716 and rs4803455 sites were detected by an allotype-specific PCR method. Results: There was a significant difference in the genotypes for rs2241715 and allele frequencies between healthy volunteers and patients with chronic type B hepatitis, liver cirrhosis, and liver cancer (P < 0.05). The individuals with rs2241715 T/T genotype had a 2.974-fold increased risk for developing chronic type B hepatitis (95% CI = 1.209-7.314, P =0.018) and 3.228-fold increased risk for developing liver cirrhosis (95% CI = 1.201-8.675, P = 0.020), while no significant correlation was observed in both genotypes and allele frequencies at rs2241716 and rs4803455 sites. Conclusion: The TGF-β1 rs2241715 T/T genotype: a predisposing factor for the chronic HBV infection-related diseases.
10.A Case of Keratosis Palmoplantaris Nummularis.

A case of 39-year-old male with keratosis palmoplantaria nummularis was reported. The skin lesions were characterized by the presence of painful focal keratoses on the plantar pressure points. Biopsy of a keratotic plantar lesion revealed excessive hyperkerstosis, hypergranulosis, focal hypogranulosis, and acanthosis in the epidermis. We could observe aymptomatic relief of this patient who was treated with mixture of corticosteroid, 25% urea and 2.5% salicylic acid.
