1.Anesthesia for Pheochromocytoma Removal A Case Report.

A 54 year-old woman underwent resection of pheochromocytoma under general anesthesia. Following induction of anesthesia with intravenous thiopental sodium, succinylcholine iodide was administered intravenously and endotracheal intubation was performed. Anesthesia was maintained with nitrous oxide, oxygen and diethyl ether using a semi-closed carbon dioxide absorption circle system. The course of anesthesia was rather stormy, reflected by hypertension, arrhythmia and hypotension, but the patient tolerated anesthesia well.

2.A Note on Nitschkia confertula.

A fungus that grew over bed-logs of shiitake (Lentinula edodes) and caused damage was isolated from mushroom-growing farms. The fungus produced extensive mat-like dark subiculum with ascomata in it and was identified as Nitschkia confertula. This is the first report in Korea and morphological characteristics are fully described.

3.A Case of Ophthalmoplegia Caused by Herpes Zoster Ophthalmicus.

Ophthalmoplegia may accompany herpes zoster ophthalmicus(HZO). A 60-year-old woman with rheumatoid arthritis suffered from HZO in the right side consecutively developed a mydriasis and abduction deficit in the right eye. CSF examination revealed pleocytosis and increased protein, while magnetic resonance imaging of the brain was normal. She was treated with intravenous acyclovir for 7 days. Ophthalmoplegia was completely resolved over the following 8 weeks, but mydriasis persisted.

Collagenous colitis is an uncommon condition charaeterized clinically by diarrhea and weight loss and histologically by thickening of the subepithelial collagen band with chromic inflammation. Laboratory tests of blood, urine and stool, and colonscopic findings are usually normal. The etiology of collagenous colitis is unknown. We report a case of collagenous colitis improved after treatment with sulfasalazine with review of literatures.

6.The Significance of Mucosal Biopsy in Normal Colonoscopy.

Many patients of gastroenterology clinics complain symptoms requiring colonic investigations. Radiological examinations may fail to detect early inflammatory bowel diseases and small neoplasms or polyps. And therefore colonoscopies are performed in preference to barium enema. Recently several reports raised a question regarding whether biopsy is necessary in a macroscopically normal colon. To determine what proportion of apparently normal colons is histologically abnormal, to measure the thickness of subepithelial basement membrane(SEBM) in normal colons, and to see whether the thiekness varies according to the different areas of the large bowel we prospectively analyzed 100 consecutive subjects with normal laboratory findings and normal mucosa on colonoscopy. Significant histologic abnormalities were not detected in all 100 cases. Thickness of SEBM (mean+SD) was 1.6+0.4 um at hepatic flexure, 1.6+0.4um at splenic flexure and 1.7+0.4 pm at sigmoid colon. The SEBM was significantly thicker at the sigmoid colon than at the hepatic flexure. Range of thickness of normal SEBM was 0.8 to 2.5 pm(mean+2SD). Maximum thickness of SEBM was 3 pm. The result of this study suggests that doing colonoscopic biopsies in all normal colons do not seem to be essential in Koreans yet.

7.Content Analysis of the Health Counseling by the Intranet in a University: 2000-2004.

OBJECTIVES: The research was aimed at analyzing the contents of university intranet for systematically execution of the healthy information provision and healthy consultation services from 2000 January to 2004 December. METHODS: We have analyzed 300(28.3%) the instances of accomplished health consultation cases from the whole 1,059 instances which were the replied in a university intranet. RESULTS: According to the contents of health consultation in ICPC code, a general symptom 91 cases(30.3%) was most, muscle-skeletal system 44 cases(14.7%), and digestive system 43 cases(14.3%) in order of cases of health counselling. The symptoms and complaint with 155 cases(51.7%) were most in the distribution of the health counselling contents by 17 charter of ICPC. The most common reason of counselling by 17 charter of ICPC were as follows; questions about the symptom and diagnosis(118 cases, 39.3%), the preventive and treatment methods(91 cases, 30.0%), and medical fee(20cases, 6.7%) in order. We mainly answer on the content of health counselling were as follows; make an offer of medical information and knowledge(48.3%), recommend visit clinic or hospital(23.7%), guide to treatment(12.7%), and so on. CONCLUSIONS: This research showed that the program of health counselling may not meet completely the high quality and adequate distribution of health counselling by the intranet in a university by content analysis. The finding suggests that health counselling by intranet in a university may be used to supplement of systemic improvement on the intranet Q/A format from current lack of essential health information and security for the quality of the health counselling.

8.Erroneously Recorded Esophageal Retrograde Peristalsis due to a Manometric Catheter Inadvertently Hooked in the Esophagus.

Esophageal retrograde peristalsis is known to be absent normally. This rare finding, if it happens, usually suggests a possibility that proximal ends of the manometric catheter are connected to the pressure transducers in a reverse order. We report a case showing repeated retrograde peristalsis during an esophageal manometric examination due to an inadventently hooked catheter in the esophagus during insertion. We suggest that if a retrograde peristalsis is repeatedly observed during a manometric session, an erroneous recording due to a hooked catheter in the esophageal lumen should be considered as one possibility.

9.Root Resorption in Streptozotocin-induced Diabetic Rats with Ligature-induced Periodontitis.

To determine the effect of diabetes on root resorption in periodontitis, we investigated odontoclast formation and root resorption in diabetic rats with periodontitis. Odontoclast formation was observed in three groups of F344 rats: Controls (C) were normal rats without diabetes or periodontitis; the periodontitis (P) group had mandibular first molars to be ligatured; the periodontitis with diabetes (PD) group was intravenously administered streptozotocin (50 mg/kg) to induce diabetes and had mandibular first molars to be ligatured. On days 3, 10, and 20 after ligature, tumor necrosis factor (TNF)-alpha and receptor activator of nuclear factor-kappaB ligand (RANKL) expression, odontoclast formation, and root resorption areas were evaluated by immunohistochemistry, tartrate-resistant acid phosphatase staining, and hematoxylin and eosin staining, respectively. The PD group showed frequent urination, weight loss, and hyperglycemia. Numbers of TNF-alpha- and RANKL-positive cells were higher in the P and PD groups than in the C group. It was more prevalent in PD group on day 3. Odontoclast formation was greater in the P and PD groups than in the C group on days 3 and 10, then decreased to same level as the C group by day 20. Root resorption in the PD and P groups showed increases on days 3 and 10, respectively, compared to the C group. These results suggest that diabetes may transiently increase root resorption on day 3 with high expression of TNF-alpha and RANKL after periodontitis induction. This study could aid the understanding of root resorption in diabetic patients with periodontitis.

10.The Incidence of Gastroesophageal Reflux Disease and the Effect of Cisapride in Patients with Epigastric Soreness.

BACKROUND/AIMS: It has been thought that in many of those who complain of "epigastric soreness," their symptom is actually "heartburn" secondary to gastroesophageal reflux disease (GERD). This study was undertaken to determine the incidence of GERD in patients who complain of epigastric soreness and to evaluate the effect of cisapride tartrate on their symptoms. METHODS: A total 107 patients who visited tertiary referral hospitals and complained of epigastric soreness were enrolled. We evaluated their symptoms and performed esophagogastroduodenoscopies (EGD). In 53 of the 107 patients who showed a normal EGD, gastroesophageal reflux studies (24 hour ambulatory pH monitoring, Bernstein test, modified Bernstein test) were performed. RESULTS: Of the 107 patients, 36 had organic gastroduodenal diseases, 18 had reflux esophagitis, and 53 showed a normal EGD finding. Of these 53 patients, the gastroesophageal reflux studies were normal in 23 patients and abnormal in 30. Forty eight (44.9%) of 107 patients showed reflux-associated epigastric soreness. There was no difference in demographic characteristics among reflux-associated, gastroduodenal, and functional epigastric soreness groups. Among clinical characteristics of the 3 groups, the only difference found was the time of the symptoms during a day. In the reflux-associated epigastric soreness group, the symptoms were more severe during daytime. After cisapride tartrate administration for 4 weeks, symptom scores of reflux-associated and functional epigastric soreness groups improved from 9.2+/-3.9 and 10.4+/-3.9 to 4.0+/-3.8 and 3.8+/-2.2, respectively. CONCLUSIONS: We found a great number of patients having GERD among those complaining of "epigastric soreness." Cisapride tartrate was effective in relieving epigastric soreness in reflux-associated and functional epigastric soreness groups.
