Chinese Journal of Minimally Invasive Surgery 2001;0(04):-
Ojective To investigate the value of preoperative B-type ultrasonicd examination in the choice of incision of minicholecystectomy. Methods Before performing minicholecystectomy the size of gallbladder and the thickness of abdominal wall measured by B-type ultrasonography the projections postion of gallbladder bottom and neck on abdominal wall were marked in 95 patlents. Results The incision of 82 cases were determined by the linea formed with linking the projection of gallbladder bottom to neck on abdominal wall. The procedure with two aperture was chosen in 11 cases and the incision was extended in 2 cases out of them. The operation visual field was clear and common bile duct and hepatic artery and port vein could be differentiated. The incidenec of complications was 5.3%(5/95) and no severe complication occurred. Conclusions Preoperative B-type ultrasonogrophy is a simple and effective method for the choice of incision of minicholecystectomy in the term of individuation

Objective To investigate the best pathologically diagnostic method for breast cancer before surgery. Methods Wet fine needle aspiration cytology was conducted in 68 cases of probable breast cancer by using a 10 ml syringe with size 7 needle. Results Malignant cells were found in 19 cases, without any false positive cases.Of the remaining 49 cases of negative cytological findings: 46 cases underwent segmental mastectomy,1 of which was postoperatively diagnosed as having fibrosarcoma by paraffin section examination and the rest of which were all demonstrated the presence of benign tumors;3 cases without surgical intervention were followed for (11~20) months and no evidences of breast cancer were found. The sensitivity was 95 0% (19/20),the specificity was 100% (48/48),the accuracy rate was 98 5% (67/68),and the false negative rate and false positive rate were 5 0% (1/20) and 0 (0/48),respectively.Of the 19 cases of positive cytological findings,no implantation metastasis in needle passage occurred. There were no complications in the study. Conclusions Wet fine needle aspiration cytology is safe, economical, convenient, quick, almost painless, and accurate, being the method of first choice in the diagnosis of breast cancer.

Objective To investigate the relationship of insulin resistance and secretion during late pregnancy in women with glucose intolerance. Methods Immunoenzymetric assay was used to measure the fasting serum insulin levels in 122 pregnant women which including of 36 pregnant women with gestational diabetes mellitus(GDM),34 pregnant women with gestational impaired glucose tolerance(GIGT),and 52 pregnant women with normal glucose tolerance(NGT).The fasting plasma glucose levels were measured by glucose oxidase method. The insulin sensitivity index(ISI) and islet secretive function index(IFI) were compared between the three groups. Results ISI had an increasing trend from NGT group,GIGT group to GDM group(P

Objective: To search for a best method for management mandible fractures by evaluating the effects of different treatments. Methods: We retrospectively analyzed 148 cases of mandible fractures treated in our department from January 1996 to June 2007.Results: Among the total number,134 cases were restored to normal occlusion, while 6 cases experienced local occlusive disfunction and 8 malocclusion. The effect of treatment was correlated with the types of fracture and methods of diaplasis. Conclusion: Mandible fractures should be treated with a new concept of combined and sequential multidisciplinary methods.Sound diaplasis followed by reliable fixation can produce a satisfying curative effect. At present, intermaxillary elastic traction with internal titanium plate fixation is the most effective method for the management of mandible fractures.

Objective To explore the application of the virtual private network(VPN) to picture archiving and communication system(PACS) for the remote access and management of PACS.Methods The client-ports(namely the remote computers) were connected to Internet by broadband such as ADSL,and the web server of PACS was connected to Internet by broadband router with VPN function so that the remote computers could access the web server of PACS remotely.By the VPN technology, the system administrators of PACS could diagnose and maintain the general malfunctions of the radiology's local network, and thus the remote management came true. Results The remote access and management of PACS had been available in our hospital for half a year since the application of the VPN technology in PACS.By more convenient medical services the work efficiency had been greatly improved. Conclusion The application of VPN technology to PACS,which is characterized by low expense, dependable performance, simple maintenance, can realize remote access and management of PACS.

Journal of Medical Postgraduates 2003;0(09):-
Objective: Maxillary and mandible fractures are severe intermaxillary injuries, frequently involving other vital organs, and usually difficult to be treated. This study discusses the treatment of maxillary and mandible fractures. Methods: We retrospectively analyzed the clinical data of 39 cases of maxillary fractures and/or mandible fractures. Results: Of the 39 cases,34 males,5 females and 84.64% aged from 20 to 50,87.17% were caused by vehicle accidents, and 94.87% were accompanied by injuries of other parts of the body. After surgical treatment,78.95% gained normal or basically normal ocular esthetic results and 68% achieved good or improved facial esthetic results. Conclusion: The results of treatment are closely related with the type and severity of fracture and the time and plan of treatment.Preoperative CT scanning and facial profile three-dimensional reconstruction are necessitated for a thorough evaluation of the fractures and an analysis of the causes and extent of facial and ocular changes. The best treatment plan for maxillary and/or mandible fractures would include anatomic reduction by surgery, intermaxillary elastic traction and rigid internal fixation with the Ti plate.

Aim To study the effect of Lacidophilus exopolysaccharides(LAEPS) on immunity. Methods Effects of LAEPS on immunity were investigated by delayed type hypersensitivity reaction, haemolytic plaque assay and macrophage function assay in mice. Results LAEPS ip 7~8 day promoted delayed type hypersensitivity reaction, increased haemolytic plaque and enhanced macrophage function in a dose-dependent manner. Conclusion LAEPS is able to enhances immunity.

Objective To investigate the association between the Adiponectin rs266729 and rs2241766 gene polymorphisms and nonalcoholic fatty liver disease in the Han Chinese population residing in Qingdao. Methods Adiponectin rs266729 and rs2241766 gene polymorphisms were genotyped in patients with NAFLD (n = 336) and healthy controls (n = 280) using polymerase chain reaction (PCR). Serum lipid profiles and adiponectin levels were determined using biochemical methods. Statistical analyses were performed using Pearson Chi square test, logistic regression analysis, t test, linear regression analysis. Results We found a significant association between the Adiponectin rs266729 genotype frequencies and allele frequencies between NAFLD pa-tients and controls (χ2= 9.929, P = 0.007; χ2= 9.809, P = 0.002). After adjustment of confounding factor, the rs266729 G allele was associated with an increased risk of NAFLD compared to the C allele (OR = 1.410, 95%CI: 1.082-1.831, P = 0.008) No significant differences were found in the rs2241766 genotype frequencies and allele frequencies between NAFLD population and the controls (OR = 1.410, 95%CI: 1.082-1.831, P = 0.008). Conclusion The Han Chinese in Qingdao carrying the rs266729 G allele are at increased risk of NAFLD.
