BACKGROUND: Allergic diseases are characterized by immediate - and late - phase reactions to various allergens by the selective activation of a subset of CD4 + T cells. In response to allergen, T cells isolated from atopic donors are biased to low levels of IFN - y and high levels of IL - 4, and vice versa by T cells from non - atopic donor. Objective : The aim of this study was to evaluate the patterns of IL - 4 and IFN - y production after Der p I stimulation and the effect on the cytokine production from T cells by budesonide, disodium cromoglycate and cyclosporin A in atopic and non - atopic individuals. MATERIAL AND METHOD: Seven Der p I specific atopic and 7 non - atopic individuals were selected. We decided the 50% inhibiting concentration of each immunomodulator by lymphocyte proliferation assay, and measured their effects on the cytokine production in vitro by intracellular IL - 4, IFN - y staining and flow cytometry. Results and CONCLUSION: There was significant difference on stimulation index ( SI ) of production of IFN - y as well as IL-4 after Der p I stimulation between atopic and non-atopic individuals ( IL - 4 ; 1.57 +/- 0.7 : 0.98 +/- 0.2, p = 0.026, IFN - r : 1.45 +/- 0.5 : 0.95 +/- 0.2, p = 0.048 ). The synthesis of IL - 4, and IFN - r were significantly inhibited after the stimulation of every immunomodulators in atopic individuals, DSCG couldnt inhibit IL - 4 and IFN - r in nonatopic individuals. There was no significant difference in the inhibiting effect of these immunomodulators in both of them.

5.Closed Intramedullary Nailing of Diaphyseal Forearm Fracture in Adolescence

Fractures of the shaft of the radius and ulna occur commonly in children and are usually treated by closed reduction and plaster cast immobilization. Anatomic reduction is seldom necessary because of the remodelling potential in the child under 10 years of age, whereas the bones of children older than 10 years of age have less capacity to remodel and the diaphyseal fracture is unstable. In case of either unacceptable reduction or unstable fractures in adolescent patients, an operative treatment is required. In five children older than 12 years of age for whom conservative treatment had hailed, we treated a closed intramedullary nailing using a distal radial and proximal ulnar approach, and followed up for 1 year or more. All fractures healed within 6 weeks. No nonunion, cross-union or refrature occured. Another advantages of this method are negliable cosmestic defect and easy removal of the internal fixation device under local anesthesia. We think that closed intramedullary nailing with rush pin is a safe and reliable method to treat unstable forearm fracture in children older than 12 years of age.

6.Palisaded Encapsulated Neuroma.

Palisaded encapsulated neuroma (PEN) is a distinctive benign cutaneous tumor that was first described by Reed et al. in 1972. We have recently experienced two cases of PEN. Clinically, these lesions were asymptomatic, flesh-colored or reddish brown papules or nodules. Light microscopy revealed several lobulated, partially encapsulated dermal tumors composed of interlacing Schwann cell fascicles. In some areas, palisading of nuclei was observed, but this was not characteristic. On Bodian staining, moderate to large numbers of tiny axons were observed. Electron microscopy demonstrated many myelinated axons incompletely invested by Schwan cell cytoplasm.

Small cell carcinoma of the ovary has been identified as a specific histopathologic entity. The first II cases were reported by Dickersin et a1, 1982, Since then only 80 cases were reported in literature. This rare and highly aggresive melignancy primarily affeets ehildren and young women(10-40) and grows rapidly, with hypercalcemia occuring frequently. Abscence of effective treatment results in early mortality. Recently we have experienced one case of primary sma11 cell carcinoma of the ovary, which is presented with a brief review of the literature.

9.Radiolucent Zones Surrounding Transpedicular Screws After Lumbar Spinal Instrumented Fusion.

STUDY DESIGNS: A retrospective study for radiographic and clinical assessment. OBJECTIVES: To clarify the clinical significance of the radiolucent zones surrounding transpedicular screws that occasionally appears following lumbar spinal instrumented fusion. SUMMARY OF LITERATURE REVIEW: that the formation of radiolucent zones surrounding pedicular screws are significantly frequent after transpedicular fixation. MATERIALS AND METHOD: 88 cases, age 50 or older, which underwent lumbar spinal fusion with transpedicular screws, between January 1999 and December 2002, were included in this investigation. The postoperative radiographs of all patients were analyzed for radiolucent zones around the transpedicular screws. These radiolucent zones were evaluated in relation the number of fusion levels, the existence of osteoporosis, and the fusion status and satisfaction rates. RESULTS: Radiolucent zones were observed in 30 cases (34%, 30/88), 13 (43%, 13/30) of which disappeared during the follow-up period. The average number of fixation levels in the cases with and without radiolucent zones were 2.33 (range 1-4, SD 0.94) and 1.74 (range 1-4, SD 0.82), respectively. Osteoporosis was found to accompany 43.3 and 20.7% of the cases with and without radiolucent zones, with the latter cases showing a statistically significant higher fusion rate and greater patient satisfaction. CONCLUSION: Radiolucent zones, a frequent finding following pedicle screw fixation, resulted in less favorable outcomes. Surgeons should be alert to radiolucent zones and their transformation during follow-up. Methods for improving the stability of the interface between the pedicle screw and vertebral bone will require further research.

10.External Fixator and External Fixator Supplemented with K-wire in the Treatment of Distal Radius Fractures.

PURPOSE: To evaluate the differences of the outcome between external fixator and external fixator supplemented with K-wire in the treatment of distal radius fractures. MATERIALS AND METHODS: Twenty-one cases which underwent external fixation (external fixation group) and 28 cases, external fixation supplemented with K-wire (external fixation with K-wire group), were analyzed. Radial length, radial inclination and volar tilt were compared in preoperative, immediate postoperative radiographs, and radiographs after removal of external fixator between two groups. And functional outcome including flexion, extension, pronation and supination of wrist were compared between two groups and wrist pain, as well. RESULT: Radial length and radial inclination in the postoperative radiographs and radiographs after removal of external fixator showed no difference between two groups, but volar tilt of external fixation group measured 2.1+/-4.2 degrees, 1.3+/-3.8 degrees and external fixation with K-wire group, 8.8+/-2.3 degrees, 8.5+/-2.4 degrees respectively, so that external fixation with K-wire group showed better reduction and maintenance. Wrist flexion and extension about postoperative 6 months measured 25.6+/-8.2 degrees, 25.1+/-10.2 degrees, respectively, in external fixation group and 42.5+/-15.2 degrees, 33.6+/-9.5 degrees in external fixation with K-wire group, so that external fixation with K-wire group showed better functional results. CONCLUSION: In the treatment of distal radius fractures, to obtain better reduction and function result, external fixations supplemented with K-wire need to be taken into consideration.
