1.Quantity change and clinical significance of CD14- CD11b+ CD33+ myeloid-derived suppressor cells in patients with multiple injury

Objective To explore the quantity change and significance of CD14-/CD11b+/CD33 + myeloid-derived suppressor cells (MDSCs) in patients with multiple injury. Methods Thirtyfour patients with multiple injury and seven healthy volunteers were enrolled in this study. Peripheral blood was collected and the factors of CD14-/CD1 1 b+/ CD33 + were taken as markers of MDSCs. The percentage of MDSCs was determined by flow cytometry (FCM) and serum interleukin-10 and C-reactive protein levels were determined by ELISA to analyze the quantity change and clinical significance of MDSCs. Results The percentage of MDSCs in peripheral blood of healthy volunteers was (1.13 +0. 25) %. At days 1,2, 3 and 7 after injury, the percentage of MDSCs in peripheral blood were (1.20 +0.22) %, (6.44 + 0.35) %, (13.84 ± 2.07) % and (15.60 ± 1.63) % respectively in patients with infection and multiple injury, whereas (1.29 ±0. 30)%, (4.93 +0. 32)%, (5.15 ±0. 21)% and (3.77 ± 0.34) % respectively in patients without infection. The percentages of MDSCs in two groups showed significant differences at days 2, 3 and 7 after trauma (P<0.05). No correlation was found between MDSCs percentage in peripheral blood and injury severity score, serum interleukin-10 or C reactive protein in patients with multiple injury (P > 0.05). Conclusions The increase of proportion ofMDSCs in peripheral blood correlates with the onset of infection in patients with multiple injury, indicating that the expansion of MDSCs in peripheral blood may play important roles in immune dysfunction after multiple injury.
2.The analysis of prognostic factors in treating variceal hemorrhage of liver cirrhosis and portal hypertension with transjugular intrahepatic portosystemic shunt

Objective To analyze the prognostic factors in treating variceal hemorrhage patients of liver cirrhosis and portal hypertension with transjugular intrahepatic portosystemic shunt (TIPS).Methods From January 2003 to December 2008, the data of 162 variceal hemorrhage patients with liver cirrhosis and portal hypertension treated with TIPS was collected, which included basic information, biochemical examination results within 7 days before the operation, regular follow-up observation after the surgery and survival data. The survival prognostic indexes were assessed with Cox regression model. Results The successful rate of TIPS was 99% (161/162). The median follow up duration was 21 months. Child-Pugh score and blood platelet count (PLT) were closely correlated with survival (P = 0. 003 and 0. 024). The total cumulative survival rate in patients with Child-Pugh score below nine (75%, 102/136) was higher than over nine (50%, 13/26) (χ2 = 9. 12,P=0. 003).The total cumulative survival rate of patients with PLT count over 47 ×109/L (74%, 82/112) was higher than below 47 × 109/L(66 %, 33/50, χ2 =4. 528, P = 0. 033). The one year after operation cumulative survival rate of liver function Child-Pugh class A, B, and C was 92%, 85%, 55% respectively. Conclusion Child-Pugh score and platelet count are independent predictable factors for the survival of variceal hemorrhage patients with liver cirrhosis and portal hypertension treated by TIPS. The risk increase after operation when Child-Pugh score over 9 and/or PLT count less 47×109 /L.

4.Intertransverse approach microendoscopic discectomy for far lateral lumbar disc herniation.

OBJECTIVETo explore the clinical effect of intertransverse approach microendoscopic discectomy for far lateral lumbar disc herniation.
METHODSFrom February 2005 to February 2010, 73 patients with far lateral lumbar disc herniation were treated with intertransverse approach microendoscopic discectomy. Their clinical data were retrospectively analyzed. There were 41 males and 32 females, aged from 19 to 80 years old with an average of 56.5 years; courses of disease ranged from 1 to 25 months with an average of 4.5 months. The main symptom was low back pain and sciatica, especially the sciatica was seriously. Herniation level was in L3,4 of 9 cases, L4,5 of 49 cases, L5S1 of 15 cases. Preoperative, 2 weeks after operation, final follow-up, conditions of pain relief were assessed by visual analogue scale (VAS); total life quality of patients were evaluated by Oswestry Disability Index (ODI) before operation and last follow-up.
RESULTSAll operations were performed successfully, operative time was from 40 to 115 min (mean of 50 min); and blood loss was from 50 to 150 ml (mean of 110 ml). Incision infection had 1 case and incomplete nerve root injury had 1 case. All patients were followed up from 3 to 8 years with an average of 4.5 years. Postoperative VAS and ODI had obviously improved (P < 0.01).
CONCLUSIONThe technique of intertransverse approach microendoscopic discectomy is a feasible and effective method for far lateral lumbar disc herniation.

5.Study on rational daily administration frequency of Fufang Biejia Ruangan tablet based on integrated serum pharmacologic and pharmacokinetic model.

To observe in vitro the effect of rat drug serum on the proliferation of HSC-T6 hepatic stellate cells in the pharmacokinetic model for determining peoniflorin in Fufang Biejia Ruangan tablet, in order to discover the rational daily administration frequency of Fufang Biejia Ruangan tablet. Fufang Biejia Ruangan tablet was orally administered to rats with different daily administration frequency. Their blood was collected from veins behind eye sockets at different time points before the administration and after the first administration, in order to determine the concentration of peoniflorin in blood plasma and the effect of rat drug serums on the proliferation of HSC-T6. A comprehensive analysis was made on the relationship between pharmacodynamics and pharmacokinetics to determine the rational daily administration frequency of Fufang Biejia Ruangan tablet. The results showed a good correlation between the inhibitory effect of Fufang Biejia Ruangan tablet-contained serum on HSC-T6 and the concentration of peoniflorin in blood. The two-time administration group showed higher pharmacologic and pharmacokinetic AUCs than one-time administration and three-time administration groups. In conclusion, Fufang Biejia Ruangan table is recommended to be taken twice a day for treating liver fibrosis in chronic hepatitis.

6.Value of 2-18F-2-deoxy-β-D-glucose PET/CT in tumor detection of patients suspected as having paraneoplastic syndromes

Objective To determine the value of 2-18F-2-deoxy-β-D-glucose (18F-FDG)PET/CT in malignancy tumor detection of patients suspected as having paraneoplastic syndrome (PNS).Methods The clinical data of 54 patients suspected as having PNS, underwent PET/CT study in our hospital fiom June 2007 to December 2009, were retrospectively analyzed. The efficacy of 18F-FDG PET/CT on positive detection rate, positive predictive value of malignant tumors and on the detection of malignant tumor markers were analyzed with the results of pathological findings and clinical data;whether the course of disease could affect these detection was also analyzed. Results Positive results of 18F-FDG PET/CT were noted in 17 patients (31.5%), and 13 of them were confirmed as malignant tumors with pathological results. The positive predictive value of 18F-FDG PET/CT was 76.5%. The durations of patients with malignancies were not different from those of patients without malignancies.PET/CT enjoyed a significantly higher efficacy rate in identifying malignant tumors in patients with elevated tumor markers than in patients with tumor markers in normal range (P<0.05). Conclusion 18F-FDG PET/CT improves the tumor detection rate in PNS suspects, especially in patients enjoying elevated tumor markers.
7.Research on Chinese orthodontic specialists' subjective evaluation of orthodontic treatment outcome.

OBJECTIVETo analyze the results of multiple Chinese orthodontic specialists' subjective evaluation of orthodontic treatment outcome, to investigate the relevance of different experiment items and to explore the weight of each monomial material.
METHODSAs a randomized clinical trial, with six orthodontic treatment centers and Angle's classification being regarded as two stratification factors, it contained 108 cases with integrity data, which was random extracted from 2383 cases that received orthodontic treatment in six orthodontic treatment centers during the past five years, gathering post-treatment study casts, cephalometrics and photographs of 48 cases as the research subject. Similarly taking Angle's classification as a stratification factor, 108 cases were randomly divided into 9 groups. The randomization of sampling and grouping were both generated by a pseudo-random number generator. According to the monomial and combined subjects, 69 orthodontic specialists were regarded as the raters to rank the 12 cases in each group, and to judge whether the case was qualified.
RESULTSCorrelation analysis: the Spearman r between Post-M + C and Post-M + C + P and the Spearman r between Post-M + P and Post-M + C + P were both greater than 0.950. The Spearman r between Post-M and Post-P and the Spearman r between Post-M and Post-C were about 0.300. The Spearman r between Post-P and Post-C was 0.505. Regression analysis: the linear regression results: M + C = 0.782M + 0.308C - 0.150, M + P = 0.804M + 0.233P - 0.091, M + C + P = 0.764M + 0.243P + 0.131C - 0.291. The r(2) of above three models was greater than 0.9.
CONCLUSIONSIt was applicable to use M + C and M + P instead of M + C + P. Study casts could not replace cephalometrics or photographs when doing subjective evaluation. Cephalometrics and photographs could not substitute for each other either. In the combined materials evaluation, model accounted for the largest percentage. Based on the regression model, for the greater part, the integration of several monomial materials could replace the combined material assessment effectively.

8.Analysis of intraoperative complications of microendoscopic disectomy and corresponding preventive measures.

OBJECTIVETo analyze the reasons of intraoperative complications of microendoscopic disectomy (MED) and corresponding preventive measures.
METHODSFrom October 2001 to January 2012, the data of 851 patients with lumbar disc herniation underwent MED were retrospectively analyzed. There were 469 males and 382 females with an average age of 42.5 years ranging 16 to 75. Course of disease was from 1 to 18 months with an average of 3 months. The segments of herniated disc including L3,4 of 24 cases, L4,5 of 418 cases and L5S1 of 409 cases . Main symptoms included low back pain with lower extremity radial pain and numbness. Of them,unilateral lower extremity symptom was in 729 cases and bilateral symptom was in 122 cases. There were at least 2 abnormal signs in the four signs which including feeling anormaly, muscle strength anormaly,dysreflexia and muscle atrophy. Distraction test of nerve was positive. CT or MRI findings must coincide with the clinical symptoms and signs. No lumbar instability,spinal stenosis,the upper lumbar disc herniation or combined with cauda equina nerve syndrome were found in 851 patients. The intraoperative complications were recorded and analyzed for the reasons of the intraoperative complication and related prevention measures.
RESULTSAccording to the Macnab standard,424 cases obstained excellent results, 321 good,106 fair,with excellent and good rate of 87.5%. The result was similar to the traditional open operation. One cases transferred to open operation due to equipment breakdown, case died for myocardial infarction at 11 days after the operation, 2 cases occurred acute epidural hematoma in 1 hour after operation. Injury of dura mate of spinal cord occurred in 28 cases and incidence rate was 3.29%(28/851); traction injury of nerve root occurred in 38 cases and incidence rate was 4.46% (38/851). One case occurred in retroperitoneal hematoma, 2 cases in incomplete cauda equina injury and 2 cases in incomplete nerve root breakage.
CONCLUSIONSkilled endoscopic hemostasis techniques,careful and meticulous operation is very important for the prevention of intraoperative complications. Moreover,timely finding and treating the complications was effective measures to prevent the coniplications.

9.Study of aniline's penetration capability through rats' complete skin in vitro and normal lab gloves.

OBJECTIVETo study the permeability of intact mouse abdominal skin to aniline and the protective capability of two typical lab gloves against aniline.
METHODSA Franz diffusion cell was used to perform in vitro transdermal absorption test and glove permeation test for aniline (0.102 mg/ml and 0.010 mg/ml). The permeabilities of intact mouse abdominal skin and gloves to aniline were measured by high performance liquid chromatography-diode array detection.
RESULTSThe transdermal penetration of the two concentrations of aniline followed zero order kinetics within 12 h, exhibiting total aniline permeabilities within 24 h of 51.71% and 48.31%, respectively. The absorption liquid had an aniline concentration of at least 18 µg/L. The medical disposable latex glove could not stop the penetration of 0.010 mg/ml aniline, but the industrial natural latex glove could.
CONCLUSIONThe penetration of 0.102 mg/ml and 0.010 mg/ml aniline through the mouse abdominal skin follows zero order kinetics within 12 h. The medical disposable latex glove cannot stop the penetration of 0.010 mg/ml aniline, but the industrial natural latex glove can.

10.Surgical treatment of hyperthyroidism.

OBJECTIVEIn order to explore the management of peri-operation and the therapeutic effect in the surgical treatment of hyperthyroidism.
METHODSFifty five cases of hyperthyroidism were undergone near-total thyroidectomy, during the operation recurrent laryngeal nerve was exposed, and the parathyroid was found with microscope when necessary. The third rank of inferior thyroid arteries were ligated to guarantee the blood supply for parathyroid.
RESULTSAll cases underwent near-total thyroidectomy. There was no mortality, and no permanent recurrent laryngeal nerve palsy occurred, and no permanent hypoparathyroidism, and no recurrent hyperthyroidism. Follow-up was carried out 16 months to approximately 5 years after near-total thyroidectomy patients, Hypothyroidism occurred in 15 cases (57.7%), serum calcium levels were 2.15-2.45 mmol/L.
CONCLUSIONSSpecial attention should be given to the management of peri-operation, the above the method can prevent operative complication in the surgical treatment of hyperthyroidism, with excellent result.
