1.Effect of age-related maculopathy on the quality of life

Objective To assess the vision-related quality of life(VRQL) in patients with age-related maculopathy (AMD) and the related factors in Shanghai. Methods The VRQL of 105 patiens with AMD and 105 without (the control group) in Shanghai was investigated via Chinese-version low vision quality of life questionnaire. Resutls The Cronbach ? coefficient of the questionnaire is 0.97 in the patients with AMD, whose scores of the various quality of life were significantly lower than those in the control group. The more the exudative defect happened, or the longer the disease course developed, the lower the sccores were. All the scores of the scales and the total questionnaire were significantly related to the better or worse monocular visual acuity. The better monocular visual acuity was the first independent risk factor affecting all aspects of VRQL in pateints with AMD. Conclusions Decreased visual acutiy may gradually impair the VRQL in patients with AMD. Chinese-version questionnaire of low vision quality of life can provide the general information of the VRQL in patients with AMD.
2.Correlation between vertebral artery hypoplasia and posterior circulation ischemic stroke

Objectve To investigate the correlation between vertebral artery hypoplasia (VAH) and posterior circulation ischemic stroke (PCIS).Methods The patients who were diagnose as ischemic cerebral stroke and underwent brain magnetic resonance imaging and cervical three-dimensional contrast-enhanced magnetic resonance angiography from March 2012 to March 2014 were enrolled.VAH was defined as vertebral artery diameter < 2 mm and thin or did not develop.They were divided into either an anterior circulation ischemic stroke (ACIS) group or a PCIS group according to the sites of disease.The clinical and imaging data were compared.Results A total of 137 patients were enrolled,including 96 patients (70.07%) with ACIS and 41 (29.93%) with PCIS.Thirty-seven patients were (27.01%) diagnosed as VAH,including 13 on the left and 24 on the right; 14 females and 23 males.The detection rate of females (29.17%) was higher than that of males (25.84%),but the difference was not statistically significant (x2 =0.175; P =0.676).There were significant differences in TOAST classificanon (x2 =6.710; P =0.035),combined with ischemic heart disease (14.6% vs.61.5% ; x2 =25.262,P < 0.001) and VAH (58.5% vs.13.5% ; x2 =9.505,P <0.001) between the PCIS group and the ACIS group.Multivariable logistic regression analysis showed that VAH was independently correlated with PCIS (odds ratio 10.788,95% confidence interval 3.863-30.131; P < 0.001),and combined with ischemic heart disease was independently correlated with ACIS (odds ratio 0.082,95% confidence interval 0.024-0.278; P < 0.001).Conclusions VAH is not rare in patients with ischemic stroke.It may promote the occurrence of PCIS.
3.Relationship between serum ferritin level and outcomes in patients with acute spontaneous intracerebral hemorrhage

Objective To investigate the change regulation of the serum ferritin level and its correlation with the clinical outcomes.Methods The patients with spontaneous intracerebral hemorrhage were enrolled.Their clinical data were collected.The serum ferritin levels were detected within 24 h after admission and on day 3,7,and 14,respectively.The modified Rankin Scale (mRS) were used to evaluate the clinical outcomes at day 90 after onset.mRS 0 to 2 was defined as good outcome and > 2 was defined as poor outcome.Results A total of 32 patients with spontaneous intracerebral hemorrhage were enrolled,including 18 in the good outcome group and 14 in the poor outcome group.There were significant differences in age (66.33 ±12.57 years vs.76.50 ±6.55 years;t =-2.908,P =0.007),baseline National Institutes of Health Stroke Scale (NIHSS) scores (median [interquartile range];9.50 [4.75-11.25] vs.15.00 [11-20];Z=-3.499;P=0.001),diastolic blood pressure (82.16± 13.79 mmHg vs.94.76± 12.17mmHg,1 mmHg=0.133 kPa);t=-1.145,P=0.045),amount of bleeding (4.50 [2.75-9.00] mlvs.11.00 [7.50-15.00] ml;Z=-2.613,P=0.008],blood glucose (5.82±0.87 mmoFL vs.7.09±1.72 mmoFL;t =-2.548,P =0.020),and white blood cell count ([7.26 ± 2.36] × 109/L vs.[10.94 ±5.83] × 109/L;t =-2.440,P =0.021) between the good outcome group and the poor outcome group.The serum ferritin levels of the good outcome group were 139.81 ± 98.50 μg/L,181.77 ± 97.29 μg/L,198.17 ±96.63 μg/L,and 159.59 ±72.43 μg/L,respectively,within 24 h on admission and at day 3,7,and 14.There were no significant differences at each time point (F=1.397,P =0.251),and those of the poor outcome group were 226.07 ± 119.22 μg/L,297.36 ± 81.48 μg/L,305.45 ± 97.05 μg/L,and 307.74 ± 82.54 μg/L,respectively,and they increased progressively over time (F =4.245,P =0.044);at each time point,the good outcome group was significantly lower than the poor outcome group (within 24 hon admission:t =-2.242,P=0.033;at 3 days after onset:t =-3.234,P =0.003;at 7 days after onset:t =-3.149,P =0.004;at 14 days after onset:t =-3.628,P =0.001).Multivariate logistic regression analysis showed that the serum ferritin level within 24 h on admission (odds ratio 1.048,95% confilence interval 1.004-1.095;P=0.034)and baseline NIHSS score (odds ratio 1.021,95% confidence interval 1.004-1.039;P =0.016) were the independent risk factors for affecting the outcomes in patients with acute intracerebral hemorrhage.Conclusions The serum ferritin level increases in the poor-outcome patients with acute intracerebral hemorrhage.The increased serum ferritin level is an independent risk factor for poor outcome in patients with acute intracerebral hemorrhage.
4.Role of 5-HT5A receptors in activation of astroglia in the spinal dorsal horn in a rat model of neuropathic pain by vincristine

Objective To evaluate the role of 5-HT5A receptors (5-HT5A R) in activation of astroglia in the spinal dorsal horn in a rat model of neuropathic pain induced by vincristine. Methods Forty adult male SD rats weighing 180-200 g were randomly divided into 4 groups ( n = 10 each): control group (group C);neuropathic pain group (group P);Ad-X-HK group (group B) and Ad-5-HT5A-siRNA group (group S). Neuropathic pain was induced by repeated intraperitoneal (IP) injection of vincristine 0.1 mg/kg according to the method described by Weng et al in group P, B and S. On the 2nd day after the last IP injection, the animals received artificial cerebrospinal fluid, Ad-X-HK and Ad-5-HT5A-siRNA 25 μl administered intrathecally (IT) in group P, B and S respectively. Paw withdrawal threshold to mechanical stimulus was measured before and on the 7th day after IT administration. The animals were then sacrificed. The lumbar segment ( L4.5 ) of the spinal cord was removed for determination of 5-HT5A R and GFAP expression. Results Body weight and paw withdrawal threshold were significantly decreased after repeated IP vincristine administration in group P compared with group C. IT Ad-5-HT5A-siRNA reduced pain threshold further in group S compared with group P. Repeated IP vincristine significantly increased the expression of 5-HT5A R and GFAP in spinal dorsal horn, and IT Ad-5-HT5A-siRNA significantly decreased the expression of 5-HT5A R while increased the expression of GFAP in spinal dorsal horn in group S compared with group P. Conclusion 5-HT5AR is involved in the inhibition of astrocyte activation, resulting in reduction of vincristineinduced neuropathic pain.
5.Design and Implementation of A Portable ECG Monitor for Multi-user

Objective To design and implement a portable electrocardiogram (ECG) monitor for multiple users. Methods Multilevel menu on liquid crystal displayer(LCD) for switching among users based on digital signal processor(DSP) was developed. Record header in ECG records for different users was appended, and communication protocol between monitor and hospital center was made. Results This monitor could record three users’s ECG signal and transmit the ECG records to remote hospital center via digital communication method. The hospital center could receive and file the ECG records of different users. Conclusion This monitor can be used for three users and is more efficient in the application of ECG monitoring.
7.Reform on training mode of clinical medicine postgraduates with professional degree

The enrollment scale of clinical medicine postgraduates with professional degree was expanded each year in order to meet the needs of the society.How to train high quality medical talents in line with modem medical education and people's health needs was the problem we were confronted with.The affiliated hospital of Inner Mongolia medical university lunched reform on training mode of clinical medicine postgraduates with professional degree; the concrete measures included increasing postgraduate pre-service training,implementing tutor responsible system,applying PBL teaching and focusing on the overall quality training.Students' clinical competence was improved after the reform.
8.Roles of BCG in establishing collagen-induced arthritis mice

Objective:To determine the correlation of BCG concentration in Freund complete adjuvant ( FCA) and severity of arthritis during arthritis establishment in DBA 1/j male mice induced by bovine type Ⅱcollagen.Methods:CIA was induced by the im-munization of DBA1/j mice with bovine type Ⅱcollagen and BCG of various concentrations dissolved in FCA.To ascertain the effects administering the collagen booster,CIA-related features(including body weight,clinical scoring of arthritis),TNF-αin serum and the histopathological changes in the spleen and the joints regions were measured.Results:4 mg/ml BCG induced more serious arthritis than 1 mg/ml in DBA1/j mice after collagen exposure.Conclusion:There is a positive correlation of arthritis severity with BCG.Which indicates that selection of adjuvant with suitable BCG concentration could determine pathological outcome in CIA mouse model .
9.The influence of apolipoprotein E gene polymorphism on brain volume in healthy young adults

Objective To explore the effect of apolipoprotein E (ApoE) gene polymorphism on brain volume in healthy young adults.Methods One hundred and ninety-three healthy young volunteers (age:20-40 years) were recruited in Shengjing Hospital of China Medical University between August 2013 and May 2014.Two milliliters of peripheral blood were collected for the determination of ApoE genotype using direct sequencing of polymerase chain reaction (PCR) products.Brain MRI scans were performed using three-dimensional T1-weighted turbo field echo imaging sequence.The diffcrences of brain morphometry between the ε4 carriers group (ε3/ε4 and ε4/ε4 genotype) and the ε4 non-carriers group (ε3/ε3 genotype) were analyzed using voxel based morphometry (VBM).Results The ε4 carriers accounted for 14.51% (28/193) of the total population.Twenty of ε4 carriers and 45 of ε4 non-carriers were included in the final images analysis.VBM analysis showed that the ApoE ε4 carriers had 10 atrophic brain areas compared with the ε4 non-carriers (P < 0.005,uncorrected,10 continuous voxels),which mainly located in the right anterior cingulate,the bilateral caudates,parietal lobes and lateral temporal lobes.Conclusions The influence of ApoE gene polymorphisms on brain volume has appeared in the youth.The ε4 gene is related to the reductions of the gray matter volume in multiple brain areas.
10.Research progress on nanobody-drug conjugate

Antibody-drug conjugate (ADC) has become an effective method for treating various diseases, especially cancer, due to its clear target and good selectivity in clinical practice. However, the monoclonal antibodies in traditional ADC have poor tissue permeability, high modification costs, pose risks such as immunogenicity and immunotoxicity. The nanobody (Nb) which is extracted from the blood of camel animals, is the smallest antibody fragment known to have complete antigen binding ability. It has advantages such as strong tissue permeability, strong specificity, low immunogenicity, and high stability, and can replace traditional monoclonal antibodies to participate in the construction of nanobody-drug conjugate (NDC). This article reviews and discusses the advantages of Nb structure, the construction and application of NDC in the hope of providing ideas for the research and development of NDC.
