1.Analysis of high risk factors of anastomotic leakage after colon cancer surgery

Objective To study the high risk factors of anastomotic leakage after colon cancer surgery ,so as to provide evidence for the prevention of anastomotic leakage. Methods Fifty-three patients with anastomotic leakage after colon cancer surgery of 916 cases were selected as research objects,then the correlation between systemic factors,local factors and occurrence of anastomotic leakage was analyzed and studied. Results The correlation between age, ASA grade, serum hemoglobin, red blood cell, total protein, fasting blood glucose of systemic factors except for sex and occurrence of anastomotic leakage was obvious (P< 0.05). There was correlation between the methods of anastomosis, anastomotic tension, setting position of drainage tube, intestinal obstruction, abdominal infection and degree of tumor differentiation except for with or without chemotherapy and radiotherapy and occurrence of anastomotic leakage(P < 0.05). Conclusion The high risk factors of anastomotic leakage after colon cancer surgery are multiple,and well various work should be done during perioperative period.
2.Cigarette smoke extract promotes senescence by reducing expression of HDAC2 in C2C12 myoblasts

AIM:To investigate whether the cigarette smoke extract (CSE) causes senescence of C2C12 myo-blasts and the relationship between senescence and histone deacetylase 2(HDAC2).METHODS: Murine C2C12 cells were induced to differentiate into myoblasts.The HDAC2 activator and inhibitor were used to investigate the effects of CSE in the myoblasts on cell senescence and the expression of HDAC2.The expression of HDAC2 at mRNA and protein levels was determined by real-time PCR and Western blot, respectively, and the positive cell rate of β-galactosidase staining for cell senescence was also detected.RESULTS:The optimal concentration of CSE was 60 mL/L and the intervention time was 24 h.After the intervention of CSE, the positive cell rate ofβ-galactosidase staining was increased, accompanied with the reduction of HDAC2 expression at mRNA and protein levels.The expression of HDAC2 at mRNA and protein levels was increased by 4, 5, 6, 7-tetrabromobenzotriazole (TBB), accompanied with the reduction of positive cell rate ofβ-galacto-sidase staining.Furthermore, when HDAC2 expression at mRNA and protein levels was reduced by HDAC2 inhibitor valp-roic acid, the positive cell rate of β-galactosidase staining was increased.CONCLUSION: CSE promotes the senescence by reducing the expression of HDAC2 in C2C12 myoblasts.
3.Detection of IgG4 and autoantibodies in patients with orbital disease of unknown origin

Objective To detect the serum IgG4 and autoantibodies levels in patients with orbital disease of unknown reasons,and to investigate their values in patients with orbital disease.Methods A total of 366 patients with orbital disease of unknown reasons recruited in the Department of Ophthalmology,Beijing Tongren Hospital Affiliated to Capital Medical University from October 2013 to October 2016 were retrospectively enrolled as orbital disease group,and 266 patients with autoimmune disease in the same period from the Department of Rheumatology of the hospital were selected as controls.The serum IgG4 was detected by rate scattering method,antinuclear antibody(ANA),anti-double-stranded DNA(dsDNA)antibody as well as anti-extractable nuclear antigen(ENA)antibody were measured by indirect immunofluorescence assay,and anti-neutrophil cytoplasmic antibody(ANCA)was detected by enzyme linked immunosorbent assay,all of which were compared between the orbital disease patients and the controls using chi-square test.Results The positive rate of the serum IgG4 in the patients with orbital disease was 36.1%(132/366),obviously higher than that in the controls(27.1%,72/266),the difference being statistically significant(x2 =5.705,P=0.017).And the positive rate of serum IgG4≥1 350 mg/L(29.0%,106/366)in the patients with orbital disease was higher than that in the controls(21.8%,58/266; x2 =4.107,P=0.043).The positive rate of ANA in the patients with orbital disease was 17.8%(65/366),obviously lower than that in the controls(28.6%,76/266),the difference also being statistically significant(x2 =10.389,P=0.001).The positive rate of anti-ENA antibody in the patients with orbital disease was 4.6%(17/366),also obviously lower that that in the controls(9.0%,24/266),with statistically significant difference as well(x2 =4.866,P=0.027).No anti-dsDNA antibody was detected in the patients with orbital disease.Only three patients with orbital disease(0.8%,3/366)were found ANCA positive,and no statistically significant difference was found in comparison with the controls(3.0%,8/266; x2 =3.127,P=0.077).Conclusions Elevated IgG4 level was commonly seen in the patients with orbital disease,where as autoantibodies were negative in the most of the patients,indicating that IgG4 might correlate with orbital disease,and part of orbital disease may belong to the IgG4-related orbital disease.
4.Community-based Rehabilitation(CBR) Network in Shijiazhuang

Objective To introduce a community-based rehabilitation(CBR) network model in Shijiazhuang,Hebei province.Methods The work model in Qiaodong district in Shijiazhuang city was evaluated according to the national CBR standard and analyzed.Results Their work has met the national standard,the score of management section was the highest among all areas.Conclusion The work was featured by government dominate,strong management network and technical support.It implied the social model of CBR.
5.Predicting the radiosensitivity with Raf kinase inhibitor protein in nasopharyngeal carcinoma

Objective To investigate the relationship between RKIP expression and the efficiency of radiotherapy in NPC patients and evaluate the possibility of using RKIP as a predictor of radiosensitivity.Methods A total of 180 patients with NPC in Sun Yat-sen University Cancer Center without evidence of distant metastasis at initial diagnosis were enrolled in this study,who had received intensity-modulated radiotherapy alone.Patients were classified into 2 groups according to criteria below:patients with biopsy proven recurrent diseases occurring at nasopharynx and/or neck within 5 years after radiotherapy were classified as the radioresistant group.The pathological type at relapse was the same as the previous one before treatment.Patients with a minimum follow-up of 5 years after radiotherapy without evidence of recurrence at the original site of the tumor were classified as the radiosensitive group.Patients in the 2 groups were matched according to the factors related with radiosensitivity.RKIP was examined by immunohistochemical staining before radiotherapy.The relationship between RKIP expression and the effect of radiotherapy were analyzed.Results The positive rate of the RKIP expression in the radiosensitive group versus the radioresistant group was 80.0% versus 26.7%.The positive rate (x2 =12.498,P <0.01) and the intensity of the RKIP expression (x2 =51.429,P < 0.01) were significantly different between 2 groups with a negative correlation with radio-resistance to NPC (r =-0.344,-0.535,respectively,P < 0.01).Based on the RKIP expression,the radiosensitivity,specificity,accuracy,positive predictive value,negative predictive value,false positive and false negative were predicted as follows:80.0%,73.3%,77.2%,75.0%,78.6%,26.7%,and 20.0%,respectively.Conclusions RKIP protein shows negative correlation with radioresistance to NPC and could serve as a biomarker in preliminarily screening the intrinsic radiosensitivity of NPC.
6.Influence of different training methods in hand hygiene compliance of health care workers

Objective To train health care workers (HCWs)by method of conventional training plus survey of hand contamination status,the influence of two kinds of methods in hand hygiene compliance of HCWs was evalua-ted.Methods From November 2013 to April 2014,all HCWs in a cardiovascular internal medicine department were as trained subjects,they were divided into two groups (trial group and control group).Hand hygiene compli-ance status was investigated 1 month before training.In the first month after training,conventional training method was adopted by both groups,from the second to fifth month,conventional training plus hand contamination survey was adopted by trial group,hand hygiene compliance between two groups were compared.Results Hand hygiene compliance rates of trial group and control group was 42.63% (107/251 )and 41 .80% (102/244)respectively be-fore training,there was no significant difference(P >0.05 ).In the first and second month after training,hand hygiene compliance rate of trial group was 55.70% (132/237)and 63.11 % (154/244)respectively,control group was 56.52% (130/230)and 62.61 % (149/238)respectively,compared with pre-training,the differences were sig-nificant (both P <0.05),but the difference was not significant between two groups(P >0.05);From the third to fifth month,hand hygiene compliance rates of trial group was 60.73%(150/247),61 .44%(145/236),and 61 .22%(150/245)respectively,control group was 51 .68%(123/238),51 .02%(125/245 ),and 52.32% (124/237)respec-tively,there was significant difference between two groups(P <0.05).Conclusion Conventional training combined with survey of hand contamination status can promote hand hygiene compliance of HCWs.
7.EDA mutation screening and phenotype analysis in patients with tooth agenesis

Objective:To screen the ectodysplasin A (EDA)gene mutation in the patients with non-syndromic tooth agenesis and ectodermal dysplasia,and to analyze the phenotype of missing teeth pattern in these two groups of patients.Methods:In the study,174 patients with tooth agenesis (143:non-syn-dromic,31:ectodermal dysplasia)and 451 health control volunteers were enrolled from the clinic,and the genome DNA was extracted from either peripheral blood or oral mucosal swab.The coding region of EDA gene was then amplified by PCR,sequenced and blasted to online NCBI database.The missing teeth were recorded for all patients,and the missing teeth from patients with EDA mutation were com-pared among the different dentition sites.Results:33 patients were identified with EDA mutation.In the non-syndromic patients,13 /143(9.09%)were identified with EDA mutation,while in patients with ec-todermal dysplasia,20 /31 (64.52%)were found with EDA mutation.Ten novel EDA mutations were identified (c.769G >C[p.G257R],c.936C >G[p.I312M],c.223G >A[p.E75K],c.1166C >T[p. P389L],c.133G >C[p.G45R],c.1109G >A[p.E370K],c.914G >T[p.S305I],c.916C >T[p. Q306X],c.602G >T[p.G201V],c.88 -89insG[p.A30GfsX69]).For each dentition site there was no statistic difference in the number of missing teeth between the left and right sides,so the number from both sides were combined later in the analysis.In the patients with EDA mutation,the non-syndromic pa-tients had fewer missing teeth (15.9 ±6.4 missing teeth for each,207 /364 in total)than the patients with ectodermal dysplasia (23.9 ±4.3,478 /560).In the non-syndromic patients with EDA mutation, the maxillay central incisors and first molars were less affected,with the same missing rate as 19.2% (5 /26).While the mandibular central incisors (with a missing rate of 76.9%,20 /26),the maxillary late-ral incisors (the missing rate:88.5%,23 /26 ),the mandibular lateral incisors (the missing rate:80.8%,21 /26),and the maxillary first premolars (the missing rate:80.8%,21 /26)were more likely to be missing.In the ectodermal dysplasia patients with EDA mutation,only maxillary central incisors (the missing rate:60%,24 /40),maxillary canines (the missing rate:70%,28 /40),mandibular ca-nines (the missing rate:67.5%,27 /40),maxillary first molars (the missing rate:65%,26 /40)and mandibular first molars (the missing rate:72.5%,29 /40)had higher possibility of persistence.Teeth at other dentition sites were more likely to be affected (the minimum missing rate:87.5%,35 /40). Conclusion:The findings would help to reveal the EDA gene and its function in ectodermal organogene-sis.
8.Effect of oxymatrine on serum matrix metalloproteinase-2 and its inhibitor in patients with chronic hepatitis B and liver cirrhosis.

OBJECTIVETo observe the effect of oxymatrine on the level of serum matrix metalloproteinase-2 (MMP-2) and its inhibitor (TIMP-2) in patients with chronic hepatitis B (CHB) and post-hepatitis B liver cirrhosis (LC), as well as on liver fibrosis indexes as hyaluronic acid (HA), laminin (LN) and IV type collagen (IV C).
METHODSChanges of all the above-mentioned indexes in patients with CHB (n = 36) and LC (n = 36) before and after treatment were determined, and the relationship of MMP-2 and TIMP-2 with liver fibrosis indexes were analyzed.
RESULTSOxymatrine could decrease the levels of MMP-2, HA, LN and IV C in patients with severe or moderate CHB and LC of Child-pugh A, B and C grade, as compared with the data before treatment (P < 0.05). Serum level of MMP-2 and TIMP-2 was well correlated with the levels of liver cirrhosis indexes.
CONCLUSIONMMP-2 and TIMP-2 can be used as a reference for evaluating the degree of LC, and oxymatrine has a certain anti-LC effect.

9.Clinical significance of multiple tumor markers in the diagnosis of cholangiocarcinoma

Objective To investigate the clinical significance of multiple tumor markers in the early diagnosis of cholangiocarcinoma.Methods 165 patients with histopathologically confirmed cholangiocarcinoma and 25 patients with gallstone canceration were enrolled in this study.36 patients with gallstones and 46 patients with bile duct stenosis were enrolled as controls.The determinations of multiple tumor markers,including carcinoembryonic antigen (CEA),CA125,CA153,CA242,CA19-9,alpha-fIetoprotein (AFP) and Ferritin,of all subjects and the imageological determinations of all the patients with cholangiocarcinoma and with gallstone canceration were analysed retrospectively.Comparisons between the groups were performed using the likelihood ratio test of the Chi-square test.Results CA19-9 had the highest sensitivity and specificity (83.6% and 96.3% respectively) for cholangiocarcinoma,and CA242 had a high sensitivity for gallstone canceration though it was not so sensitive in cholangiocarcinoma.Multiple tumor markers increased the sensitivity and specificity in the early diagnosis of cholangiocarcinoma.Compared with imageological determinations,multiple tumor markers had a higher sensitivity for the early diagnosis of cholangiocarcinoma with a tumor mass <1 cm.Conclusion Multiple tumor markers have important clinical values in the early diagnosis of cholangiocarcinoma.
10.Comparison of the distribation of doses calculated with Monte Carlo N-particle transport code and those practically measured by 60Co therapy facility

Objective To discuss the feasibility of Monte Carlo N-particle transport code(MCNP)simulated calculation.Methods The calculation in water phantom was contrasted with the practical measurements and the reported values using the percent depth dose(PDD)curve and normal peak scatter factor.Results There Was no significant difference between calculated and measured results in the 10 cm×10 cm field(t=-0.41,P>0.05),however,there were significant differences in the 5 cm×5 cm field(t=7.2,P<0.05)and in the 12 cm×12 cm field(t=-4.6,P<0.05).There was no significant difierence between the calculated results and the reported values(t=-1.91,P>0.05).In the same radiation field,the PDD decreased as the depth increased,but increased as the size of the radiation field increased at the same depth.PDD and normal peak scatter factor were both important parameters for calculation of prescribed dose.Conclusions It is possible to establish a set of accurate and comprehensive percent depth doses and normal peak scatter factor parameters so as to provide the basis of clinical use, quality assurance and quality control for radiotherapy.
