1.Effects of bivalirudin versus heparin with tirofiban on thrombosis, hemorrhage and P-selectin in ST-segment elevation acute myocardial infarction patients with emergency percutaneous coronary intervention

Objective To study effects of bivalirudin and heparin plus tirofiban on plasma P-selectin in patients with ST-segment elevation acute myocardial infarction (STEMI) undergoing emergency percutaneous coronary intervention (PCI) and compare the clinical effects on bleeding and thrombosis.Methods 260 hospitalized patients with acute STEMI who accepted emergency PCI were enrolled in the study.They were randomly divided into the bivalirudin group (129 cases) and the heparin plus tirofiban group (131 cases).Blood samples were taken before operation,during operation (about 20 minutes after medicacion injection) and after operation (three hours after the end of operation).Serum P-selectin levels were measured.All patients were followed-up for 30 days.The main events included hemorrhage within 30 days,acquired thrombocytopenia and stent thrombosis.Results The level of P-selectin increased significantly in the heparin group during operation compared to the preoperative level and remained statistically higher at three hours after the end of operation than preoperative level (P < 0.05).Compared with preoperative level,the P-selectin level of the bivalirudin group was significantly lower than that of heparin group (P < 0.05).After 30 days of follow-up,bivalirudin had lower rates bleeding events compared to the heparin group(5.4% vs.15.3%,P =0.009).The need of medical intervention for bleeding events (BRAC2-5 hemorrhage) was less in the bivalirudin group(0.8% vs.5.3%,P =0.029).There was no significant difference in the major bleeding events between the 2 groups (BRAC3-5 hemorrhage) (0 vs.0.8%,P =0.32).No significant difference found between the two groups in acquired thrombocytopenia (P > 0.05).Conclusion Bivalirudin may reduce P-selectin levels in STEMI patients undergone emergency PCI during perioperative period.Heparin increases perioperative P-selectin.Bivalirudin may reduce the bleeding events.
2.Effect of xinglong pingchuan recipe on interleukin-5 and the inflammagen related to oxygen free radical in asthmatic mice.

OBJECTIVE:
To observe the effect of Xinglong Pingchuan recipe (XLPCR) on interleukin-5 (IL-5), superoxide dismutase (SOD), glutathione peroxidase (GPx), and malondialdehyde (MDA) in mouse asthma models, and to explore its mechanism in treating asthma.
METHODS:
The mouse asthma models were established by sensitization and challenge with ovalbumin (OVA). The asthma model was treated with XLPCR. At last, the number of white blood cells and eosinophil was counted, and the concentrations of inflammation factors such as IL-5, SOD, GPx, and MDA in the serum or the lung tissue of each mouse were detected.
RESULTS:
Compared with the asthmatic group, the number of eosinophil in the XLPCR group decreased significantly (P < 0.01); the concentration of IL-5 in the XLPCR group significantly decreased in the serum or the lung tissue (all P < 0.01); and the concentrations of SOD and GPx in the XLPCR group increased (P < 0.01 and P > 0.05, respectively). On the other hand, the concentration of MDA in the XLPCR group was significantly lower than that of the asthmatic group (P < 0.05).
CONCLUSION
XLPCR might inhibit the airway inflammation by decreasing the IL-5 level and adjusting the balance of oxidants/antioxidants.

3.Influence of astragalus injection on retina of simulated high altitude hypoxia rat

·AIM: To investigate the effects of astragalus injection on the metabolic and pathological changes of the retinal free radical metabolism and pathology in high-altitude hypoxic rats. ·METHODS: Sixty healthy SD rats without any eye disease were selected,and the random grouping method was divided into two groups: astragalus injection group (intervention group) and physiological saline group (control group),each group of 30 rats. In each group,6 were randomly selected for injection of astragalus injection (15mL/kg) and physiological saline (15mL/kg) in the pre-simulated chamber. The rats were sacrificed immediately and removed the eyeballs after the rats were taken out of the simulated module. We observed the changes of retinal morphology with the HE staining method, and determine the retinal SOD and MDA content with colorimetry. · RESULTS: HE staining showed no morphological changes in the two groups of retinas at 2h, and as the time of hypoxia was prolonged, the retinal edema gradually increased,but the intervention group was less edema than the control group. The activity of SOD in both groups decreased with the increase of anoxia time of high altitude, and the comparison of different time points in the group was statistically significant(P<0.05). The content of MDA in both groups increased gradually, and the comparison between different time points in the group was statistically significant (P<0.05). The SOD in the two groups of retinas was significantly different at same time point(P<0. 05), except for at 2h without statistical significance. The MDA showed the same situation as SOD. ·CONCLUSION: Astragalus injection can reduce the damage degree of retinopathy in rats under the high altitude hypoxia environment, that the mechanism may be related to free radicals, enhance the activity of SOD, reduce the MDA content of lipid peroxides,and enhance antioxidant capacity.
4.Study on the correlation of Xinjiang endometrial tumor and vascular endothelial growth factor

Objective To explore the relationships of serum vascular endothelial growth factor (VEGF) and endometrial cancer of Uighur Women in Xinjiang.Methods The serum of 50 endometrial cancer patient's and 70 healthy women's were collected.VEGF expressions were tested by ELISA method and the correlations of endometrial cancer with VEGF were analysed.The variety of serum VEGF in different clinical stages of endometriat cancer was analyzed.Results Serum VEGF expressions on endometrial cancer were significantly higher than normal ones (P<0.01);The serum VEGF level in late stage was significantly higher than early stage (P<0.01).The serum VEGF level significantly increased from well differentiated to the poorly differentiated cases (P<0.05).Conclusion high level expressions of VEGF are related to the endometrial cancer.Uighur Women in Xinjiang,particularly high expressed in advanced and poorly differentiated endometrial cancer.
5.Allogeneic peripheral blood hematopoietic stem cell transplantation in malignant hematopoietic diseases.

To evaluate the use of allogeneic peripheral blood stem cell transplantation (allo-PBSCT) for treatment of acute and chronic leukemia, from March 1997 to January 2003, 21 adult patients with malignant hematopoietic diseases underwent allo-PBSCT from HLA-identical siblings (19 patients) and haplo-identical mother (one) and one B point site mismatched sibling (one). All donors were mobilized with G-CSF for 4 days and peripheral blood stem cells were collected by CS-3000 separator. The conditioning regimen included the high dose combination chemotherapy and TBI. Cyclosporine-A (CsA) plus a short course of MTX was used for GVHD prophylaxis in all patients. The results showed that after trans plantation, median time for the recovery of granuocyte > or = 0.5 x 10(9)/L and platelets > or = 20 x 10(9)/L were 12 (10 - 20) and 15 (11 - 35) days, respectively. Acute GVHD was observed in 8/17 patients (47%), of which one transplanted from HLA-haploidentical mother. Chronic GVHD occurred in 12/17 patients (70%). All of four female survivals did not show acute and chronic GVHD. Day 100 transplantation-related mortality was 14% (3/21). Relapse occurred in two patients (9.5%) who underwent allo-PBSCT in stage of non-remission at one and six months. After follow-up of 40 (15 - 70) months, 11 patients (52.4%) are still disease-free survival. These results suggested that peripheral blood stem cells produce a faster hematopoietic recovery and a lower relapse of leukemia. The rate of aGVHD is not increased when using the peripheral blood as source of stem cells; however, cGVHD continues to be a significant problem. Donors tolerated the procurement procedure without complications.

6.Drug resistance and phylo-typing of ESBL-producing Escherichia coli from diarrheic lambs in Kashgar area,Xinjiang

The objective of this study was to determine the frequency and resistance patterns of ESBL-producing E.coli in lambs with diarrhea in the Kashi area,Xinjiang.The findings may provide guidance for the prevention and control of clinical E.coli disease.We collected 385 samples of perianal feces from lambs with diarrhea in the Kashgar area.From these samples,we isolated 371 strains of E.coli.We then used the double-paper-sheet synergistic method to screen for ESBL-producing E.coli.Additionally,we conducted analyses to identify drug-resistance genes,analyze drug resistance,and study the phylo-typing of the screened strains.Of 371 E.coli strains,204 were identified as ESBL-producing strains.The prevalence rates of blaCTX-M,blaCTX-M-1G,blaCTX-M-9G,and bla TEM resistance genes was 67.65%,69.12%,30.39%,and 63.73%,respectively.All ESBL-pro-ducing strains were resistant to multiple drugs,with resistance rates ranging from 90.69%to 100%for eight specific drugs:ampicillin,cefotaxime,gentamicin,enrofloxacin,azithromy-cin,tetracycline,chloramphenicol,methotrexate,and amitrazine.The phylogenetic subgroups of the strains were distributed primarily in groups A and D.Among group A strains,41.11%exhibited resistance to ten drugs,whereas among group D strains,40%exhibited resistance to 11 drugs.ESBL-pro-ducing strains of Escherichia coli are the main pathogens cau-sing diarrhea in lambs in the Kashgar region;group A is the main group,and all groups are multi-drug resistant.
7.Long-term follow-up of treatment outcome and prognosis on 46 children with acute promyelocytic leukemia.

OBJECTIVEAcute promyelocytic leukemia (APL) is a specific type of hematopoietic malignancy, accounting for 10% of the de novo acute myeloid leukemia (AML). The data on long-term outcome of APL in children are limited. The aim of this study was to investigate the clinical biological features, diagnosis, prognosis and long-term survival of childhood APL.
METHODSA total of 46 children with newly diagnosed APL from April 1998 to October 2005 were enrolled into this study. Induction treatment containing all-trans retinoic acid (ATRA) plus daunorubicin (DNR) or pirarubicin (THP) was performed on these patients, followed by 6 courses of chemotherapy consolidation: DNR, homoharringtonine or etoposide plus Ara-C. A maintenance therapy was then administered once 3-6 months. The total period of treatment was 2.5 years.
RESULTSOf the 39 patients who had completed the regular treatment, 36 (92.3%) achieved a complete remission. The 5-year cumulative incidence of relapse (CIR) was 28.6%. The estimated overall survival (OS) rates at 1, 3 and 5 years were (86.1 +/- 5.8)%, (76.1 +/- 7.5)% and (70.2 +/- 8.9)% respectively, while the event free survival (EFS) rates were (78.4 +/- 6.8)%, (63.6 +/- 8.7)% and (53.1 +/- 10.0)% respectively. The 5-year OS rate of patients with WBC less than or equal to 10.0 X 10(9)/L was (81.4 +/- 10.3)%, which was significantly higher than that with WBC greater than 10.0 X 10(9)/L[(51.6 +/- 14.7)%, P < 0.05]. Five patients with RT-PCR positive for PML/RARalpha S (short) subtype died eventually although all of them achieved CR, but none of the 13 patients with PML/RARalpha L (long) subtype died.
CONCLUSIONSRemission induction therapy with ATRA + DNR or THP is effective and safe for newly diagnosed childhood APL. The remission induction therapy combined with chemotherapy containing high/intermediate dose Ara-C can improve the long-term survival rates of APL patients. High WBC count and S subtype of PML-RARa are two poor prognostic factors for children with APL.

8.The Uptake and Distribution Evidence of Nano-and Microplastics in vivo after a Single High Dose of Oral Exposure

Objective Tissue uptake and distribution of nano-/microplastics was studied at a single high dose by gavage in vivo.Methods Fluorescent microspheres (100 nm, 3 μm, and 10 μm) were given once at a dose of 200 mg/(kg·body weight). The fluorescence intensity (FI) in observed organs was measured using the IVIS Spectrum at 0.5, 1, 2, and 4 h after administration. Histopathology was performed to corroborate these findings.Results In the 100 nm group, the FI of the stomach and small intestine were highest at 0.5 h, and the FI of the large intestine, excrement, lung, kidney, liver, and skeletal muscles were highest at 4 h compared with the control group (P < 0.05). In the 3 μm group, the FI only increased in the lung at 2 h (P < 0.05). In the 10 μm group, the FI increased in the large intestine and excrement at 2 h, and in the kidney at 4 h (P < 0.05). The presence of nano-/microplastics in tissues was further verified by histopathology. The peak time of nanoplastic absorption in blood was confirmed.Conclusion Nanoplastics translocated rapidly to observed organs/tissues through blood circulation;however, only small amounts of MPs could penetrate the organs.

10.Effects of the interleukin-21 expression in patients with immune thrombocytopenia and its regulation by high-dose dexamethasone.

OBJECTIVETo investigate the correlation of immunologic thrombocytopenia(ITP) pathogenesis with the abnormal expression of IL-21, and to explore the association of high-dose dexamethasone (HD-DEX) treatment with the IL-21 expression.
METHODS26 newly diagnosed ITP patients and 24 healthy controls were enrolled in this study. The mononuclear cells and serum were obtain from density gradient centrifugation in the newly diagnosed ITP patients before HD-DXM treatment, and the samples of healthy controls were also used for assays. The protein and mRNA expression of IL-21 on peripheral blood mononuclear cells(MNC) were determined by flow cytometry and real-time reverse-transcription polymerase chain reaction. Plasma levels of IL-21, IFN-γ and IL-4 were determined by enzyme-linked immunoabsorbent assay (ELISA).
RESULTSIL-21 expression on mononuclear cells was significantly higher in ITP patients (13.07%) than that in normal controls (8.2%), the ratio of IL-21/GAPDH mRNA expression on MNC was significantly higher in ITP patients (9.524±0.97) than that in normal controls (3.701±0.60, P<0.01). After HD-DXM therapy, the ratio of IL-21/GAPDH mRNA decreased significantly (5.87±1.21) as compared with the level before treatment. Significantly high levels of serum IL-21, IFN-γ and lower IL-4 were found in ITP patients, as compared with healthy controls. Serum IL-21 and IFN-γ levels in ITP patients decreased significantly after HD-DXM administration (P<0.01), while post-treatment levels of IL-4 were increased significantly, compared with the levels before treatment (P<0.01).
CONCLUSIONTherapeutic effect of DXM on ITP associates with down-regalation of IL-21 expression. The increased expression of IL-21 involves in the pathogenesis of ITP.
