ObjectiveTo determine whether prophylactic cranial irradiation (PCI) has a role in the management of patients with non-small cell lung carcinoma (NSCLC)treated with radical intent.MethodsWe searched The Cochrane Library,MEDLINE,EMbase,CBM,CNKI and VIP.The quality of the includedstudieswascriticallyevaluated.Dataanalyseswereperformed usingtheCochrane Collaboration's RevMan 5.1 software.ResultsFour randomized controlled trials involving 905 patients met the inclusion criteria.The results meta-analyses showed the incidence of brain metastases was lower in PCI group compared with the observation group ( x2 =1.98,P =0.000 ) ; but there is no evidence of 1-year overall survival (OS) benefit ( x2 =1.12,P =0.880).Only RTOG 2009 provides prospective data:There were no significant differences in global cognitive function (P =0.600) or ADL ( P =0.880) after PCI,but there was a significant decline in immediate recall (P=0.030) and delayed recall (P =0.008 ) at 1 year,At 1 year,there was no significant differences in QOL after PCI ( P =0.050).Conclusions This systematic review show significantly decreases the risk of BM without improving 1-year OS in NSCLC patient receiving prophylactic cranial irradiation.There is insufficient evidence to support the use of PCI in clinical practice.Where possible,patients should be offered entry into a clinical trial.

Objective To systematically evaluate the influence of radiotherapy on triple-negative breast cancer (TNBC) patients treated with breast conservation surgery.Methods Electronic databases including PubMed,Ovid,CBM,VIP and CNKI and seven journals in Chinese (including the Chin J Breast Cancer,Chin J Clin Oncol,Chin J Radiat Oncol,Chin J Cancer,Chin J Oncol,J Pract Oncol and Tumor)were searched.Clinical trials comparing survival rates of patients with TNBC and non TNBC treated with breast conservation surgery and radiotherapy were reviewed.The quality assessment and data extraction were performed by two reviewers independently.RevMan5.1 software developed by the Cochrane collaboration was used for Meta-analysis.The 5-year distant metastasis-free survival (DMFS),overall survival (OS) and local recurrence-free survival (LRFS) rates were the primary end points.Results Five trials including 2345 patients with breast cancer were included in this systematic review.417 patients had TNBC and 1928 patients had non TNBC.Meta-analysis showed that the 5-year DMFS and OS rates of patients with TNBC were lower compared with non TNBC (Z =5.29,P =0.000 and Z =3.35,P =0.001).There was no statistical significant difference in 5-year LRFS rate between patients with TNBC and non TNBC (Z =1.35,P =0.180).Conclusion Radiotherapy provides good local control for patients with TNBC after breast conservation surgery.

Objective To study the changes in mRNA and protein expression of ubiquitin and to explore the relationship of apoptosis in the skeletal muscle of mouse with emphysema.Methods Emphysema model was established by passive cigarette smoking in mouse.Apoptosis was detected with TUNEL staining.The reverse transcriptase-polymerase chain reaction (RT-PCR) and immunohistochemical assay were used to determine the mRNA and protein level of ubiquitin.Results Apoptotic cells were increased in skeletal muscle of mouse with emphysema.The mRNA and protein level of ubiquitin were significantly higher in mouses with emphysema (0.48±0.02 and 0.23+0.05,respectively) than in control group (0.17±0.01 and 0.14+0.01,t=6.223、4.093,both P ＜0.05).Conclusions The increase of apoptosis in skeletal muscle of mouses with emphysema may be associated with high expression of ubiquitin.

Objective To develop a set of indicators for measuring the performance of China's Healthcare Improvement Initiative.Methods Such methods as literature review,expert consultation and onsite preliminary experiment were used,to study and demonstrate the indicators,the evaluation framework,the weight,evaluation indicators,and evaluation standards.Results The final evaluation indicators for medical institutions included nine class-1 indicators,29 class-2 indicators,and 56 class-3 indicators;those for healthcare administrations included six class-1 indicators,nine class-2 indicators,and 13 class-3 indicators.Conclusions The system takes into full account the special nature of healthcare,and mission of the Initiative,as the well as the quality,safety,efficiency and equity of healthcare,to make the system scientific and operational.

Objective:The research aimed to figure out the geographic availability of the residence in the aspect of receiving health service thereby studying the spatial distribution regulation and the distance attenuation coefficient of the inpatients.It also provides the decision-making basis for the regional health planning.Methods: Select 23 medical institutions and randomly pick up patients from each of them, calculate the distance between the inpatients home and the medical institutions, then simulate the corresponding probability patients using the Geographical Infor-mation System ( GIS) .Perform a variety of curve fitting models according to the determination and simulation graph-ics coefficient choosing the most appropriate model, and draw corresponding the distance attenuation coefficient.Re-sults:An exponential curve model is better than power curve model for the secondary medical institutions inpatients distribution and contrarily for those in the tertiary medical institutions.The mean distance attenuation coefficient was 0.51 for exponential curve model of secondary comprehensive medical institutions inpatients and its average was 0.50 for the secondary specialist medical institutions inpatients.The mean distance attenuation coefficient was 0.84 for the power curve model of the tertiary comprehensive medical institutions inpatient and its average was 0.53 for tertiary specialist medical institutions inpatients.Conclusion: The secondary medical institutions inpatient probable attenua-tion trends with distance are faster than in tertiary medical institutions, those for the comprehensive medical institu-tions inpatient are faster than in the specialist medical institutions.In the regional health planning, for different lev-els, categories of medical institutions should adopt different distance attenuation coefficients to calculate the health re-sources level.

Lysostaphin is highly effective on eliminating methicillin resistant Staphylococcus aureus (MRSA). In order to achieve controlled release of lysostaphin, a biocompatible drug carrier is needed. Hydroxyapatite/chitosan (HA/CS) composites were chosen to carry lysostaphin and sample composites with different weight ratios of HA to CS, including 80/20, 70/30, 60/40, and 40/60, were prepared. Multiple analyses were performed to determine the structural and physicochemical properties of the composites, including scanning electron microscopy, X-ray diffraction and Fourier transform infrared spectroscopy. We immersed HA/CS composites loaded with 1 wt% lysostaphin to test in vitro release activity and cultured MC3T3-E1 cells to carry out biocompatibility test. The result of the release behavior of the composites revealed that the controlled release of lysostaphin from 60/40 HA/CS composites was the highest release rate of (87.4 ± 2.8)%, which lasted for 120 hours. In biocompatibility testing, MC3T3-E1 cells were able to proliferate on the surface of these composites, and the extract liquid from the composites could increase the growth of the cells. These results demonstrate the controlled release of lysostaphin from HA/CS composites and their biocompatibility, suggesting the potential application of these composites to bone injury and infection applications.

Objective To systematically evaluate the efficacy and safety of stereotactic body radiotherapy (SBRT) for resectable stage Ⅰ non-small cell lung cancer (NSCLC).Methods Clinical trials of SBRT or surgery for resectable stage Ⅰ NSCLC were collected by computerized search of Cochrane Library,MEDLINE,EMbase,CBM,CNKI,and VIP.Literature selection,quality evaluation,and data extraction were performed by two inspectors based on the inclusion and exclusion criteria.A meta-analysis was performed on the enrolled studies using RevMan 5.3 software.Results A total of four clinical trials involving 410 patients were included.The results of meta-analysis showed that there was no significant difference in 3-year overall survival rate between SBRT and surgery (RR=1.13,95％ CI=0.66-1.94,P=0.66);there was no significant difference in local control rate between SBRT and surgery (RR=0.71,95％ CI=0.26-1.93,P=0.50);patients treated with SBRT had significantly lower incidence rates of grade 3-4 adverse reactions than those treated with surgery (RR=0.29,95％ CI=0.16-0.53,P=0.000).Conclusions SBRT shows equivalent efficacy to surgery in the treatment of resectable stage Ⅰ NSCLC.However,due to the limitations in this systematic evaluation,the conclusion needs to be further confirmed by large randomized controlled trials.

Usage of reverse genetic techniques in the research area of the fundamental etiology of foot-and-mouth disease virus (FMDV), has resolved the issue about the function of viral gene of FMDV on genomic integer level. At present, a further recognition and apprehension for the molecular etiology of FMDV based on the development in reverse genetics was made. Combined with the research work in our labs, we reviewed international advances about the molecular pathogenic mechanism, the relationship be-tween virulence and variation in the genomes, influencing factors for the viral replication, and the develop-ment of new-type gene vaccine of FMD in this article, and propose the potential research aspects in reverse genetics of FMDV in the future.
