3.A Seroepidemiologic Study on Rubella Antibody Positivity: Primary School Students in Gyenggi-Do.

PURPOSE: The purpose of this seroepidmiologic study was to examine the immunization and infection history of rubella, positivity of rubella specific antibody, seroconversion rate of two rubella vaccines and to develop an adequate immunization program of rubella. Research Methods : Subjects of the study were 2.071 students of 8 priamary schools in Gyenggi-Do. Questionnaire and blood sampling were done. Test method of Rubella specific antibody was ELISA (Abbott IMx autoanalyzer). The anayltic methods for the study were chi-square-test, t-test and ANOVA. RESULTS: The results of the study were as follows : (1)The number of subjects who had infection history of rubella were only 28 students(1.5%). (2)The immunization rate of rubella was the highest in 6 year-old group(65%), it decrease as subjects grow older. (3)The positivity of rubella antibody was 76. 6%, it is the highest in 6 year old group and the lowest in 7 year-old group and it increase again as subjects grow older. (4)Average antibody titer was 53.3 IU(SD 79.2), the trend of antibody titer by age was similar to that of positivity. (5)The seroconversion rates of both vaccines, vaccine A,vaccine B, were 99.5% and 99.6%, the ratio of increase of antibody titer after immunization was higer than 400. it means that both vaccines are very efficacious. CONCLUSION: Results of the study suggest that it is necessary to increase the coverage of rubella immunization, and further studies are necessary to get more information for rubella.

5.A Study on the Relation between Glaucoma and Quality of Life.

PURPOSE: This study was performed to find the relation between glaucoma and the quality of life (QOL). METHODS: The questionnaire survey and ophthalmologic examination was made in 105 glaucoma patients under treatment at ophthalmology clinic of a university hospital and 120 normal persons who had undergone an routine health check-up from June 1st, 2000 and June 30th, 2001. The questionnaire was composed of general characteristics, health-related behavior and the short form-36 health survey for QOL. Using the t-test, analysis of variance (ANOVA), Pearson's correlation analysis and analysis of co-variance (ANCOVA), we examined the relationship between glaucoma and QOL. RESULTS: 1. By t-test or ANOVA various domains of QOL showed statistically significant difference by sex, age, coverage of medical insurance, family history of glaucoma, past history of systemic diseases, educational level, marital status, smoking and drinking status. Visual acuity showed significant positive correlation with QOL and horizontal and vertical cup-to-disk (C/D) ratio showed significant negative correlation with QOL. 2. By ANCOVA to find the association of glaucoma with QOL controlling other related characteristics of subjects, QOL of the glaucoma group showed lower than that of the normal group, but the differences were not statistically significant. CONCLUSIONS: QOL of glaucoma patients was lower than that of normal persons without glaucoma considering sex, age, health insurance system, family history of glaucoma, past history of systemic disease, educational level, marital status, smoking status, drinking status and visual acuity, so further study will be necessary to observe the change of QOL according to progression of glaucoma and treatment status.

7.A Case of Flavobacterium Indologenes Keratitis.

PURPOSE: Flavobacterium indologenes is known to cause keratitis very rarely. Authors have experienced 1 case of keratitis from Flavobacterium indologenes with history of diabetes mellitus, thereby reporting it. METHODS: History taking, slit lamp examination, staining and culture, sensitivity test about antibiotics were performed on 1 case of keratitis. RESULTS: Flavobacterium indologenes was detected in staining and culture that was performed on the first visit. Piperacillin was used based on the sensitivity test about antibiotics. Improvement of corneal lesion and symptom was observed with the use of piperacillin. CONCLUSIONS: Flavobacterium indologenes can be considered as a casual pathogen in keratitis with condition susceptible to opportunistic infection such as systemic illness or abnormal ocular immunity.

8.Properties of Dual AV Nodal Pathways in Patients with Paroxysmal AV Nodal Reentrant Tachycardia.

Anterograde and retrograde conduction properties of dual AV nodal pathways were analyzed in 15 patients having no accessory pathway and showing dual AV nodal pathways during extrastimulation. Four patients in whom the supraventricular tachycardia of common type (common type SVT) was induced during electrophysiological study were classified into group A. The remained 11 patients in whom the common type SVT was not induced were classified into group B. There was a history of paroxysmal supraventricular tachycardia(PSVT) in all of group A but none of group B. Among the properties of dual AV nodal pathways, effective refractory period (ERP) of anterograde fast pathway (FP) and retrograde FP were not different in both group. Maximal AH interval of anterograde slow pathway (SP) was significantly shorter in group A than in group B (P<0.01). In group A, all patients had intact ventriculoatrial (VA) conduction, but in group B, 5 patients had no VA conduction. The paced atrial cycle length producing Wenckebach block (WBCL) and the paced ventricular cycle length producing retrograde VA block (VABCL) were significantly shorter in group A the in group B (p<0.05). There were two patients showing retrograde dual AV nodal pathways. The common type SVT was not induced in both of them probably due to long ERP of retrograde FP. In conclusion, in patients with anterograde dual AV nodal pathways, the occurrence of common type SVT is highly related to anterograde slow pathway refractoriness (WBCL) and retrograde fast patway refactoriness (VABCL).

9.The Prognostic Role of Body Temperature in Acute Ischemic or Hemorrhagic Stroke Patients.

BACKGROUND: Hyperthermia is known to be one of the important poor prognostic factors of ischemic stroke. However, the prognostic role of fever in hemorrhagic stroke is not clear. We investigated whether the prognostic role of fever is influenced by the type of stroke. METHODS: We analyzed the linical data of patients who were admitted to our hospital within three days after the onset of stroke during 28 months. We divided the patients into three groups according to the degree of maximum temperature recorded during the first seven days; no fever (< OR=37.2 degrees C), low grade (37.3~38.0 degrees C) or high grade (> OR=38.1degrees C) fever. Using the modified Rankin disability scale (MRDS) at discharge, clinical outcome was dichotomized into favorable (MRDS 0~2) and unfavorable (MRDS 3~5) categories. The size of lesion, age, sex, alcohol, diabetes, hypertension, initial glucose level, hypercholesterolemia, heart disease, smoking, previous stroke, and initial National Institutes of Health Stroke Scale (NIHSS) were investigated. RESULTS: Of 217 patients, 46.1% had no fever, 36.9% had low grade fever, and 17.1% had high grade fever. Fever was strongly correlated with outcome in patients with stroke of any type (p<0.05). The size of lesion was also significantly correlated with the degree of fever in patients with ischemic stroke (p=0.01). Multivariate analysis in the logistic model revealed that fever and NIHSS were independent predictors for a poor prognosis. CONCLUSIONS: Fever in patients with acute stroke is an independent poor prognostic factor regardless of stroke type.

10.Change in Electromyographic Activity of Wrist Extensor by Cylindrical Brace.

PURPOSE: To verify the effect of a newly-developed cylindrical type forearm brace, which was designed to give focal counterforce perpendicularly on the muscle belly of the wrist extensor. MATERIALS AND METHODS: The dominant hands of 24 (12 males, 12 females) healthy subjects were tested. Two types of forearm braces (focal cylindrical type and broad pneumatic type) were examined. The braces were applied at the extensor carpi radialis brevis, 5 to 7 cm distal to the lateral epicondyle. Two surface electrodes were attached to the proximal and distal parts of the brace. By quantitative electromyography, the mean amplitudes of voluntary extensor carpi radialis brevis contraction before and after applying each brace were recorded and analyzed. RESULTS: The mean amplitudes of the focal cylindrical brace and broad pneumatic brace were reduced significantly compared to no brace (p<0.05), with a larger reduction for the cylindrical brace than the pneumatic brace (p<0.05). There was no significant difference between the proximal and distal mean amplitudes with each brace. CONCLUSION: A cylindrical type brace decreased electromyographic activity in the wrist extensor more effectively than did the pneumatic type brace.
