Objective: To discuss the association between the length of stay at the intensive care unit (ICU) and sarcopenia among hemiplegic stroke patients. Methods: This study evaluated 66 hemiplegic stroke patients with history of ICU admission using handgrip strength and bioelectrical impedance analysis to obtain height-adjusted appendicular skeletal muscle mass. The diagnosis of sarcopenia was made according to the muscle mass based on the Asian Working Group for Sarcopenia. The patients were divided into sarcopenic and non-sarcopenic groups. The two groups were statistically analyzed, and the significant factors with differences were studied. A multivariate logistic regression analysis was performed to examine the association between length of stay in the ICU and sarcopenia, after adjusting for potential confounders. Results: Among 66 hemiplegic patients with an ICU admission history, 12 patients were diagnosed with sarcopenia. Sarcopenia patients showed lower scores on the Korean version of the Modified Barthel Index and the Korean version of the Mini-Mental State Examination. Additionally, patients with sarcopenia had a longer length of stay in the ICU, and univariate and multivariate analyses confirmed that the ICU length of stay was significantly related to sarcopenia (adjusted odds ratio=1.187; 95% confidence interval, 1.019–1.382; p=0.028). Conclusion The length of stay in the ICU was significantly associated with sarcopenia in hemiplegic stroke patients.

The aim of this study was to compare and quantify the spatiotemporal and gait parameters obtained by foot pressure analysis during the gait in a group of Parkinson's disease (PD) patients compared with other Parkinsonism diseases, especially multiple system atrophy (MSA). Thirty-seven out of ninety-three patients who visited the center of neurology or rehabilitation with features of Parkinsonism were recruited. Spatiotemporal gait parameters were collected using gait analysis system. The results did not differ in terms of the stride length, step width, double stance phase, stride time, cadence, velocity, gait line and single support line differences, anterior-posterior position of center of pressure, and maximal gait line velocity; the lateral symmetry showed a significant difference between the PD and the MSA groups (p < 0.05). The study evaluated the differences in terms of spatiotemporal parameters between the PD and MSA along with other Parkinsonism diseases; it showed that the PD patients had a gait tendency to deviate laterally compared to the MSA patients. The result suggests conducting the gait foot pressure analysis might help distinguish PD from other Parkinsonism diseases in early stage, aiding the early decision for the treatment plans.

The dentatorubropallidoluysian atrophy (DRPLA) is a neurodegenerative disorder with expansion of an unstable CAG trinucleotide repeat in a gene on chromosome 12 and a rare cause of progressive myoclonus epilepsy (PME). A 34-year-old female showed progressive myoclonus, choreoathetosis, generalized tonicclonic seizure, dementia and ataxia. Her uncle died during convulsion at the age of 19. Brain MRI revealed cerebral, cerebellar and brainstem atrophy accompanied by dilatation of the fourth ventricle. The demonstration of expanded CAG repeat (67/11) in the gene for DRPLA was used to confirm the diagnosis. (J Korean Neurol Assoc 19(2):173~175, 2001)

OBJECTIVE: The purpose of this study is to design the web-based dialogue expert system for preventing and managing of habitual substance disorders in adolescents. METHODS: Research process includes the requirement analysis, making of questionnaires, actual survey, and its analysis. Based on the analysis, we designed the web-based dialogue expert system. RESULTS: The habitual substance disorders in adolescents are variable. Our system for managing habitual substance disorders contains the diagnosis of alcoholism, nicotinism, and narcotism. And it analyzes the facts of them based the questionnaires and answer. As the results of the analysis, main reasons in order of the power of influence are belief, will of refusal, personal characteristics, family environment, and psychological personality. Our system suggests the customized solution to conquer that for each adolescent. Moreover, our system also includes the history tracking mechanism to manage the status of each teenager constantly and to make the survey and analysis information. CONCLUSION: With our system, young boys and girls can know their seriousness of alcoholism, nicotinism, and narcotism. And they can get all possible helps through our system. This system will be the interactive and familiar way that anyone all over the world can use for overcoming problems.

This retrospective study comprised of 141 patients with zygoma fracture caused by various types of accidents and treated in the department of Plastic and Reconstructive Surgery, Pusan National University Hospital during past 10 years from April, 1986 to March, 1996. The medical records of these 141 patients were reviewed and analysed retrospectively in order to obtain the annual variation of clinical pattern of zygomatic bone fracture and to help understand change of therapeutic tendency during 10 years in our hospital. The statistical items were the age, sex, distribution of cause, fracture sites, diagnostic method, the accompanied facial bone injury, intervals between onset of accident and time of operation, and the approach methods with fixation materials. The following results were obtained. 1. Mean age of patients was 33.6 years, and age range was 3 to 75 years. Most injuries occurred in young male with the highest incidence in the third decade of life. Male predominated more than female in the ratio of 4:1. Annual variation was not observed. 2. Traffic accident(47.5%) was the most common cause of injuries and incidence has been increased since 1993. Relative incidence of assault has been increased since 1994. Otherwise, incidence of industrial accident has been decreased since 1993. 3. The most common anatomical site of the zygomatic bone fracture was group III type fracture(44%) in Knight and North Classification. According to Larsen and Thomsen classification, type B(predicted unstable fracture : 68.1%) was the most common. Annual variation was not observed. 4. Associated facial bone fractures were mainly maxillary fracture followed by nasal, panfacial and blow-out(in odder of frequency). And head injury was the most common non-maxiilofacial bone injury accompanying zygomatic bone fracture. Annual variation was not observed. 5. The most prevalent time interval between onset and surgical intervention was within seven days and the most prevalent time interval between surgical intervention and discharge was within 2-3 weeks. 6. Open reduction was used for 75.1% of total cases. The most common reduction approach incision of the zygomatic bone fracture was bicoronal approach in conjunction with subciliary incision that had been mainly used since 1991 and the most common fixation material used was microplate and screw that had been used since 1989.
