1.T and B Cells, T cell Subsets in Vitiligo Vulgaris.

The basic pathophysiology of vitiligo is still obscure. Most researchers emphasized that poasible immunologic role is very important in pathophysiology of vitiligo. Also the type of vitiligo is classified by various type baaed on clinical manifestations. These facts promoted us to analyse the immunologic state in each type of vitiligo in order to verify whether there is present any immunologic alteration in this permatosis or any differences of immune state in each type of vitiligo. The following immune cells were analysed, T cell, B cell, and T cell subsets such as helper T cell and suppresaor T cell. Vitiligo vulgaris in our study showed alteration of immune cell such as low level of T cell and helper T cell.

3.The Expression of CD40/CD40L in MM Patients and Its Significance to the Prognosis of MM Patients.

OBJECTIVE:
To explore the expression of CD40/CD40L in multiple myeloma(MM) patients and its influence on prognosis.
METHODS:
Thirty patients with MM treated in Cangzhou People's Hospital from May 2016 to June 2017 were selected and divided into MM group, then 30 healthy people with a physical examination in our hospital at the same time were selected as the normal group. The serum CD40/CD40L levels of the patients in the two groups was detected by flow cytometry, and its correlation with the lymphocyte population, pathological grade and prognostic significance of MM patients was anaysis.
RESULTS:
The expression of CD40 in serum of the patients in MM group was significantly higher than those in normal group (P<0.05). The expression of CD40L in serum of the patients in MM group showed no significant difference as compared with those in normal group (P>0.05). The levels of CD40 and CD40L in the patients before and after chemotherapy showed no difference(P>0.05). The levels of Ts and NK cells in the patients of MM group were lower than those in normal group (P<0.05). The proportion of total B lymphocytes, Th and Th/Ts cells between the two groups showed no significant difference (P>0.05). The CD40 level was correlated with the serum total B lymphocyte level of the patients in MM group (r=0.877, P=0.005). There was a correlation with CD40L and Th cells in the serum of MM patients (r=-0.783, P=0.035). The expression of serum CD40 in the patients at phase III-IV was higher than those of the patients at phase I-II, the levels of serum CD40L in MM patients at different periods showed no significant difference(P>0.05). CONCLUSION
The increasing of CD40 level in MM patients is related to the pathological grade of the patients. Chemotherapy can reduce the level of CD40. The increasing of CD40 is an important factor for the poor prognosis of MM patients. CD40L level is not meaningful for MM treatment and prognosis.

4.Application of lymphocytes test in peripheral blood of patients with systemic sclerosis during the treatment.

OBJECTIVE:
To explore the significance of lymphocytes in systemic sclerosis (SSc), by detecting the levels of T lymphocytes, B lymphocytes and natural killer (NK) cells, and analyzing the correlation between the lymphocytes and clinical laboratory indexes.
METHODS:
The numbers and proportion of T, CD4+T, CD8+T, B, and NK cells were detected by flow cytometry in peripheral blood of 32 SSc patients who had taken immunosuppressive drugs and 30 healthy controls (HC). The comparison of the lymphocyte subsets in SSc with them in the HC groups, and the correlation between the lymphocytes and other clinical and laboratory indicators were analyzed by the relevant statistical analysis.
RESULTS:
Compared with the HC group, the numbers of T, CD4+T, CD8+T, and NK cells in peripheral blood of SSc group, who had taken immunosuppressive drugs, were significantly decreased (P < 0.05). More-over, the proportion of NK cells in peripheral blood of the SSc group was also significantly lower than that in the HC group (P=0.004). In addition, all the lymphocyte subsets were decreased in peripheral blood of more than 65% of the SSc patients who had taken immunosuppressive drugs. Compared with CD4+T normal group, the positivity of Raynaud's phenomenon, erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) was significantly increased in CD4+T reduction group, respectively (P=0.024, P < 0.001, P=0.018). ESR was higher in CD8+T reduction group than CD8+T normal group (P=0.022). The prevalence of fingertip ulcer was significantly increased in B cell decrease group (P=0.019). Compared with NK cell normal group, the prevalence of fingertip ulcer was significantly increased in NK cell lower group (P=0.033), IgM was remarkablely decreased yet (P=0.049). The correlation analysis showed that ESR was negatively correlated with the counts of T lymphocytes (r=-0.455, P=0.009), CD4+T lymphocytes (r=-0.416, P=0.018), CD8+T lymphocytes (r=-0.430, P=0.014), B cells (r=-0.366, P=0.039).
CONCLUSION
The number of CD4+T, CD8+T, B, and NK cells significantly decreased in peripheral blood of SSc patients who had used immunosuppressive drugs, some lymphocyte subsets might be related with Raynaud's phenomenon and fingertip ulcer, and reflected the disease activity by negatively correlated with ESR and CRP; the numbers of lymphocyte subsets in peripheral blood should be detected regularly in SSc patients who had taken immunosuppressive drugs.

5.The Distribution and Significance of Activated T Cells and Lymphocyte Subsets in Myelodysplastic Syndrome.

OBJECTIVE:
To investigate the distribution of bone marrow lymphocyte subsets in patients with myelodysplastic syndrome(MDS),the proportion of activated T cells with immunophenotype CD3+HLA-DR+ in the lymphocytes and its clinical significance, and to understand the effects of different types of MDS, different immunophenotypes, and different expression levels of WT1 on the proportion of lymphocyte subsets and activated T cells.
METHODS:
The immunophenotypes of 96 MDS patients, the subsets of bone marrow lymphocytes and activated T cells were detected by flow cytometry. The relative expression of WT1 was detected by real-time fluorescent quantitative PCR, and the first induced remission rate (CR1) was calculated, the differences of lymphocyte subsets and activated T cells in MDS patients with different immunophenotype, different WT1 expression, and different course of disease were analyzed.
RESULTS:
The percentage of CD4+T lymphocyte in MDS-EB-2, IPSS high-risk, CD34+ cells >10%, and patients with CD34+CD7+ cell population and WT1 gene overexpression at intial diagnosis decreased significantly (P<0.05), and the percentage of NK cells and activated T cells increased significantly (P<0.05), but there was no significant difference in the ratio of B lymphocytes. Compared with the normal control group, the percentage of NK cells and activated T cells in IPSS-intermediate-2 group was significantly higher(P<0.05), but there was no significant difference in the percentage of CD3+T, CD4+T lymphocytes. The percentage of CD4+T cells in patients with complete remission after the first chemotherapy was significantly higher than in patients with incomplete remission(P<0.05), and the percentage of NK cells and activated T cells was significantly lower than that in patients with incomplete remission (P<0.05).
CONCLUSION
In MDS patients, the proportion of CD3+T and CD4+T lymphocytes decreased, and the proportion of activated T cells increased, indicating that the differentiation type of MDS is more primitive and the prognosis is worse.

6.Change of Peripheral Blood CD5+ B Lymphocytes in Early Neonatal Period.

PURPOSE: This study was aimed to investigate age-related changes of CD5+ B lymphocytes in healthy Korean neonates. METHODS: Sixty healthy neonatal infants were enrolled in this study; at birth(n=10), day 1(n=10), day 2(n=10), day 3(n=10), day 4 (n=10) and day 5 (n=10). Phenotypic analysis of CD5+ B lymphocytes was performed on blood samples using standard flow-cytometric techniques. RESULTS: The percentage of lymphocyte was significantly increased from at birth to day 5. The percentage of CD19+ lymphocyte was decreased from at birth to day 4, but statistically not significant. The percentage of CD5+ /CD19- lymphocyte on day 5 was significantly lower than that of at birth. The percentage of CD5-/CD19+ lymphocyte was not changed. The percentage of CD5+ /CD19- lymphocyte on day 1 was significantly higher than that of at birth, then it gradually decreased with aging to the at birth figure. The percentage of CD5+ /CD19+ lymphocyte was decreased from at birth to day 5, but the changes were not significant. The percentage of CD5+ / CD19+ lymphocyte in CD19+ lymphocyte was decreased from at birth to day 5, but it was not statistically significant. CONCLUSION: CD5+ B lymphocyte counts are considerably higher than previously established adult values and the age-related changes are very important in interpretating CD5+ B lymphocyte. These data may serve as a reference range for studies in early neonatal period.

7.Flow cytometric analysis of lymphocyte subset in Korean Air forces pilots.

Major lymphocyte subsets were quantitated using immunofluorescence staining and flow cytometric analysis in Korea Air Force pilots undergoing continuous training and intensive situation both physically and psychologically. Total 197 subjects were included in this study. Young students assayed in parallel as group I (28 subjects) control group. The pilots were divided into two groups, group II 2nd and 1st lieutenants and group III(93) captain and major, according to the rank. The percentage of CD4 helper T lymphocyes was higher in group III (48.91+/-10.11%) Than group I (45.42+/-4.73%) or II(47.08+/-8.28). The percent of CD8 cytotoxic T lymphocytes of group III (23.53 5.64%) was lower than that of group I (28.91+/-3.37) or group II (25.95+/-6.36). Consequently, CD4/CD8 ratio was higher in group III (2.24+/-0.79) than in group I(1.59 +/-0.27) or group ~l.95+/-0.66). Total white blood count of group III (6969+/-1335 count/mm2) was lower In group III than that of groupI (7639+/-1788) or group II(7210+/-1653), but no statistical differences were noted in lymphocyte count and Percents of CD16 natural killer cells and CD19 B lymphocytes. In light of the functional significance of the lymphocyte subsets Investigated, It is suggested that the present stress-induced alterations may have practical implications.

8.A Case of Group O without Anti-A,B due to hypogammaglobuluinemia.

An ABO discrepancy was observed in a 19-year-old man clinically diagnosed as congenital hypogammaglobulinemia. The patient's red cell was typed as group O, and his serum had no ABO isoagglutinins. Absence of A antigen and B antigen on patient's RBC was confirmed by adsorption- elution test and saliva test. A-transferase and B-transferase activities were not demonstrated in patient's serum. Patient's serum protein electrophoresis revealed hypogammaglobulinemia pattern, and immunoglobulin levels were markedly decreased. Complete absence of B lymphocytes was observed on patient's lymphocyte subset profile. Patient's father and mother were typed as blood group O and no abnormalities were recognized in their serum protein electrophoresis patterns and immunoglobulin concentration.

9.Relationship of anxiety state with lymphocyte subsets and the effect of Chinese medical treatment on anxiety in patients with chronic hepatitis B.

<b>OBJECTIVEb>To analyze the relationship of anxiety state with CD4(+) level and CD4(+)/CD8(+) ratio and to observe the effect of Chinese medicine (CM) treatment on anxiety in chronic hepatitis B (CHB) patients.
<b>METHODSb>The anxiety state of 120 CHB patients was evaluated based on Hamilton Anxiety Scale (HAMA) scoring. According to the scores, 63 patients with scores ≥14 were classified to anxiety and 57 patients with scores <14 to non-anxiety. The differences in CD4(+) cells and CD4(+)/CD8(+) ratio between patients with anxiety and non-anxiety were analyzed. Moreover, 63 patients with anxiety were randomized into two groups: 31 in the control group were treated with lamivudine (100 mg per day) alone and 32 in the observation group were given equal dosage lamivudine combined with CM treatment depending on syndrome differentiation, all for 12 weeks. The effects of treatment on anxiety state and T-lymphocyte subsets as well as its impact on some CHB-related indices were observed and compared.
<b>RESULTSb>The anxiety state of CHB patients was negatively correlated with CD4(+) and CD4(+)/CD8(+); the level of CD4(+) in patients with anxiety was significantly lower than that in non-anxiety patients (P<0.01 or P<0.05). After treatment, anxiety state in the observation group was significantly improved, with their HAMA scores significantly lowered (P<0.01), and the levels of CD4(+) and CD4(+)/CD8(+) were significantly higher than those in the control group (P<0.05 or P<0.01). Moreover, the alanine transaminase recovery rate and the HBV-DNA-negative conversion rate in the observation group were significantly higher than those in the control group, respectively (P<0.05).
<b>CONCLUSIONSb>The anxiety state of CHB patients was related to CD4(+) and CD4(+)/CD8(+) levels. CM treatment could improve the anxiety state and showed certain regulatory effect on the patients' immune system.

10.The Characteristics of Residual Leukocyte in Sepacell PLS-5A Filtered Platelet Concentrates.

BACKGROUND: We evaluated residual leukocytes characteristics of white cell(WBC) reduction filter in platelet concentrates. Differential count and lymphocyte subset changes were measured before and after leukocyte filtration in platelet concentrates. MATERIAL AND METHODS: Ten units of platelet concentrates were prepared and were filtered with WBC-reduction filter(Sepacell PLS 5A, Japan). After filtration of blood products, WBC and differential leukocyte count and lymphocyte subsets were counted by microscopic examination of Wright-Giemsa stained smear and Facscan(Becton-Dickinson, USA). Monoclonal antibodies used for lymphocyte subset test were CD3(FITC), CD4(FITC), CD8(PE), CDl4(PE), CDl6(PE), CDl9(PE), CD33(PE), CD56(PE), IgGl(FITC), IgG2(PE). RESULTS: The main population of residual leukocytes after filtration was mainly lymphocytes(96.7%), and CD3 positive T lymphocytes showed 23.8% positivity of residual leukocyctes and the next were NK cell(8.7%). B lymphocytes were rarely found(<0.01%) and CD4/ CD8 ratio was within normal limits. CONCLUSION: The leukocyte reduction filters(Sepacell PLS-5A) would be effective for prevention of platelet alloimmunization but not sure about the effect for prevention of TA GVHD.
