1.Recurrence of Left Atrial Myxoma.

We present a case of recurrent left atrial myxoma which occurred 7 years and 10 months after initial operation despite resection of originaltumor and adjacent atrial septum. This is the first case report of recurrence in Korea. Among the possible causes of recurrence, regrowth from pretumorous focus seems to be the most suggesting one in this case. Histopathologic findings of recurrent myxoma showed increased cellularity with active proliferation and nuclear hyperchromasia. These findings, together with the rapidity of regrowth, suggest that recurrent myxoma may have 'wilder' behavior. Prolonged postoperative observation is important, even if ample resection was done at the initial operation including atrial septum.

Myxomas comprise 30-50 percent of primary cardiac tumors making it the most common type. Most cases are sporadic, but familial occurrence has been described. Myxomas can occur as early as three years of age to as late as 83 years of age. Sporadic cases are known to occur at age groups ranging from 39-82 years, with a mean age of 56 years. Familial myxoma, on the other hand, are likely to occur in the younger age group. Seventy percent of cases occur among adult female patients. Five cases of atrial myxoma discovered during pregnancy have already been reported. It is therefore the objective of this report to document the first successful case of atrial myxoma excision in the Philippines with the aid of cardiopulmonary bypass under general anesthesia, during the second trimester of pregnancy. Cardiac myxoma is often discovered as an incidental finding and presents with various subtle symptoms and signs. Electrocardiographic findings are mostly non-specific. Atrial flutter or conduction abnormalities are known to be rare. We report a case of large left atrial myxoma that was diagnosed by transthoracic echocardiography at a primary care clinic. An asymptomatic, 71-year-old woman presented with serial abnormal electrocardiogram changes during a routine consultation. A diagnosis of left atrial myxoma was obtained through transthoracic echocardiography. We report this case with a review of literature on cardiac myxoma associated with arrhythmia.

Cardiac myxoma is the most common benign cardiac tumor and it presents various clinical symptoms and signs. Although two-thirds of patients have abnormal electrocardiographic findings, atrial flutter or conduction abnormalities are known to be rare. We report on a case of a large left atrial myxoma which was diagnosed by trans-thoracic, trans-esophageal echocardiography, chest computerized tomography, and histological examination. The myxoma was presented as atrial flutter in a 41-year old man who was complaining palpitation and dyspnea on exertion. After surgical excision of the mass, atrial flutter converted to normal sinus rhythm. We report this case with review of literatures on left atrial myxoma associated with arrhythmia.

A right atrial thrombus in structurally normal heart is very rare. A 66-year-old woman was admitted with chest discomfort and dyspnea. She was diagnosed of right atrial myxoma on echocardiography and chest computed tomography. We performed an excision of the mass attached to atrial septum, which was found to be an organized mural thrombus by pathologic examination. We report this rare case with a review of literature.

10.Biatrial Myxoma: A Case Report.

Biatrial myxomas are extremely rare, and only two cases have been reported in Korea so far. We report a 60-year-old male patient with decreased mentality due to embolization from myxoma. In this case, two pedicles grew into both atrial chamber separately from the different origins at the atrial septum. The patient underwent emergency operation immediately after the diagnosis by an echocardiography.
