Bronchial asthma is a common chronic respiratory disease, which is involved in a variety of cells and cellular components. In 2019, the guidelines for the diagnosis and treatment of asthma issued by the Global Initiative for Asthma (GINA) Committee put forward the concept of type 2 inflammatory asthma for the first time. The updated evolution of GINA guidelines has promoted the development of biological agents and disease treatment, providing effective prevention and treatment for patients with severe asthma and improving disease outcome. This paper expounds the disease mechanism and management suggestions of type 2 inflammatory asthma in GINA guidelines, and analyzes the relevant clinical studies on targeted treatment of type 2 inflammatory asthma in recent years, in order to provide reference for in-depth understanding of level 3 prevention and management of patients with type 2 inflammatory asthma.

8.Role of exercise in asthma management in children.

OBJECTIVETo study the role of exercise in asthma management in children and formulate exercise prescriptions for asthmatic children.
METHODSA total of 112 asthmatic children aged 7-14 years were randomized into control group (n=56, with medications only) and exercise group (n=56, with medications and exercise prescription) for a trial lasting for 4 months. An asthma diary was used for all children to record the morning and evening peak expiratory flow (PEF), daytime and nighttime asthma symptom score, use of short-acting beta agonists, participation in physical activity, and exercise-induced asthma symptoms.
RESULTSOf the 112 children, 5 dropped out before the trial completion. From the 13th week, PEF variation in exercise group was significantly lowered compared to that in the control group (P<0.05). Upon the completion of the trial, the daytime and nighttime symptoms score and emergency medication score were all significantly lower in the exercise group than in the control group (P<0.05). No severe adverse events occurred in the children during the trial.
CONCLUSIONExercise for no less than 20 min, 3 days a week for 4 months is safe and beneficial for asthma control in children.

The revision inincludes both the diagnosis of asthma and the control-based asthma management. It points out that asthma is a heterogeneous disease, and the diagnosis of asthma should be based on the characteristic pattern of symptoms and evidence of variable airflow limitation, emphasizing the diagnosis of atypical asthma. Besides, the epidemiology of asthma, assessment of asthma, management severe asthma, special type of asthma and asthma in special populations have been added in this version. The revised guideline provides an important reference for the standardized management of asthma.

Inhaled corticosteroid (ICS) is the best effective medicine in asthamatic management. Inhale corticosteroid and vigorous 2 medicine for long term is basical preventation in the next of 20 years
