1.Outer dense fiber 2 and sperm function: Progress in studies.

Oligoasthenozoospermia, teratozoospermia or low sperm motility is the main cause of male infertility. Low sperm motility can be induced by abnormalities of the sperm tail structure and sperm function. The outer dense fiber protein 2 (ODF2) is a protein fiber maintaining cytoskeleton, as a major component of the mammalian sperm tail and centrosome, and its abnormality is closely related to asthenospermia. Recent studies indicate that ODF2 includes many proteins of the same name and homologous splices located in the sperm centrosomes and spindles of cleaved-embryos, necessary for animal ciliogenesis and associated with sperm capacitation. The features of ODF2 indicate that it is not a single-structural protein. This paper reviews the known functions of ODF2, paving a ground for further studies of the relationship between the ODF2 protein and fertilization.

3.Initial observation on L-carnitine for asthenospermia with epididymal knob.

OBJECTIVETo evaluate the efficacy of L-carnitine on asthenospermia with epididymal knob.
METHODSThirty-five patients aged 25-39 years with asthenospermia with epididymal knob were received at our clinic of andrology from May 2003 to July 2004, and given oral L-carnitine 2 g/d for 3 months. The sperm parameters of the patients were analyzed by computer-assisted sperm analysis system before and after L-carnitine treatment.
RESULTSOf the 32 patients who accomplished the 3-month L-carnitine therapy, only 4 remained unimproved in sperm parameters, while the other 28 were significantly improved in sperm concentration, forward sperm motility, total sperm motility, straight line velocity, curvilinear velocity and average path velocity. Four of the patients wives were pregnant and one patient fathered a child.
CONCLUSIONOral L-carnitine therapy is efficacious for asthenospermia with epididymal knob.

4.Qilin pills combined with levofloxacin for asthenospermia accompanied with accessory sex gland infection: a clinical observation.

OBJECTIVETo observe the efficacy of the combination of Qilin Pills and levofloxacin in the treatment of asthenospermia accompanied with accessory sex gland infection.
METHODSWe randomly assigned 80 asthenospermia patients with accessory sex gland infection to groups 1 and 2 of equal number, the former treated with Qilin Pills + levofloxacin, and the latter with levofloxacin only. Qilin Pills were administered at the dose of 6 g tid for 30 days, and levofloxacin at the dose of 0.5 g qd for 20 days. We obtained semen parameters, including the percentage of progressively motile sperm and peroxidase-positive white blood cell (WBC) count, before and after medication, and compared the clinical effects between the two groups.
RESULTSAll the patients accomplished the clinical trial. The therapeutic effectiveness rates in improving progressive sperm motility were 60% in group 1 and 17.5% in group 2, with statistically significant differences between the two groups (P < 0.05). As for the peroxidase-positive WBC count in semen, the effectiveness rates were 87.5% and 82.5%, respectively, with no significant differences between the two groups (P > 0.05).
CONCLUSIONFor the treatment of asthenospermia accompanied with accessory sex gland infection, Qilin Pills combined with levofloxacin is evidently better than levofloxacin alone in improving sperm motility, and it has no obvious adverse effects.

5.High ligation of varicocele improves sperm DNA integrity in patients with asthenospermia.

OBJECTIVE:
To assess how high ligation of varicocele improves sperm DNA integrity in patients with asthenospermia.
METHODS:
The DNA fragmentation index (DFI) and semen analysis were conducted in 30 subjects with normal sperm samples, and the parameters were used as references. Fifty-six patients from the Department of Urology of the Second Affiliated Hospital of Chongqing Medical University from April 2009 to April 2011 all signed the informed consent. Semen was collected 1 week before the high ligation of varicocele and 3 months after the surgery. We compared the change of the integrity of sperm DNA and semen.
RESULTS:
Three months after the high ligation of varicocele, DFI and semen indexes significantly improved (P<0.05), but no significant difference was found between open surgery and laparoscopic surgery, unilateral varicocele and bilateral varicocele and Grade I, Grade II, Grade III varicocele (P>0.05). Six months later, 27 spouses were pregnant out of the 53 surgical patients (50.9%). DFI of the pregnancy group and the non-pregnancy group was (13.90±9.70)% and (20.10±10.27)%, with significant different (P<0.05).
CONCLUSION
Varicocelectomy can improve the sperm DNA integrity in patients with asthenospermia, whose effect is not related to surgical approach, unilateral or bilateral varicocele or varicocele grade.
