1.Giant Cell Tumour of the Tendon Sheath Masquerading as Trigger Finger

We report a case of a 59-year-old female who presented in the general orthopaedic clinic with triggering of her right middle finger. She did not respond to conventional treatment methods; subsequently she underwent surgical open release under local anaesthesia. Five months postoperatively, the patient presented with signs and symptoms of acute flexor tenosynovitis, and was thought to have a postoperative infection. Re-examination by a hand surgeon raised the possibility of a different aetiology. Based on clinical findings and response to initial treatment, giant cell tumour of the flexor tendon sheath was suspected and later confirmed following surgical biopsy. A high index of suspicion and knowledge of the variegated presentations of giant cell tumour in the hand are beneficial in these types of cases.
2.Heterogeneity in studies of p16 in cervical lesions in different Malaysian institutions: Time to consider collaborative study

Introduction: Clinical decision making becomes difficult when clinical and methodological heterogeneity does not permit synthesis of results of multiple small studies. Aim: For studies done in Malaysia, to identity the sample sizes and heterogeneity present in the various studies which used p16 in evaluating lesions of the cervix. To evaluate if it would be possible for a single study to answer the various questions posed by the original authors. To highlight areas where the design features of future studies can be optimised. Materials and Methods: Various databases were searched using synonyms for p16 AND cervix AND Malaysia. These were assessed for broad conformity to a Diagnostic Test Accuracy format. Methodological and clinical heterogeneity indicators were extracted into standardised fields. Results: There were 5 studies eligible for inclusion. Each sought to study different aspects of the disease such as diagnostic grade stratification and pathogenesis. The study type broadly conformed to a Diagnostic Test Accuracy format. The study design used was either consecutive or non-consecutive. Sample size ranged from 75 to 201. Clinical heterogeneity was present in the choice of controls with some using normal and some using inflamed tissue. Methodological heterogeneity in applying the reference test, index test and different antibody clones were present. Conclusion: There was both clinical and methodological heterogeneity making synthesis of studies difficult. It is possible to design a study which would be able to answer all the questions posed by the original authors with internal validity while at the same time increasing sample size.

3.Transforming growth factor-beta and liver injury in an arginine vasopressin-induced pregnant rat model

Objective:
Approximately 30% of preeclamptic pregnancies exhibit abnormal liver function tests. We assessed liver injury-associated enzyme levels and circulating transforming growth factor beta (TGF-β) levels in an arginine vasopressin (AVP)-induced pregnant Sprague-Dawley rat model.
Methods:
Pregnant and non-pregnant Sprague-Dawley rats (n=24) received AVP (150 ng/hr) subcutaneously via mini-osmotic pumps for 18 days. Blood pressure was measured, urine samples were collected, and all animals were euthanized via isoflurane. Blood was collected to measure circulating levels of TGF-β1-3 isomers and liver injury enzymes in pregnant AVP (PAVP), pregnant saline (PS), non-pregnant AVP (NAVP), and non-pregnant saline (NS) rats.
Results:
The PAVP group showed significantly higher systolic and diastolic blood pressure than both saline-treated groups. The weight per pup was significantly lower in the AVP-treated group than in the saline group (p<0.05). Circulating TGF-β1-3 isomer levels were significantly higher in the PAVP rats than in the NS rats. However, similar TGF-1 and TGF-3 levels were noted in the PS and PAVP rats, while TGF-2 levels were significantly higher in the PAVP rats. Circulating liver-type arginase-1 and 5'-nucleotidase levels were higher in the PAVP rats than in the saline group.
Conclusion
This is the first study to demonstrate higher levels of TGF-β2, arginase, and 5'-nucleotidase activity in PAVP than in PS rats. AVP may cause vasoconstriction and increase peripheral resistance and blood pressure, thereby elevating TGF-β and inducing the preeclampsia-associated inflammatory response. Future studies should explore the mechanisms through which AVP dysregulates liver injury enzymes and TGF-β in pregnant rats.

5.The complement system in preeclampsia: a review of its activation and endothelial injury in the triad of COVID-19 infection and HIV-associated preeclampsia

This review assesses the complement system and its activation, with the pathological features of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), human immunodeficiency virus (HIV) infection and preeclampsia (PE). The complement system is the first defensive response by the host innate immune system to viral pathogens, including SARS-CoV-2. SARS-CoV-2 entry results in the release of pro-inflammatory cytokines and chemical mediators to create a “cytokine storm”. Endothelial cell (EC) dysfunction and cell-mediated injury are also present. These factors cause an exacerbated inflammatory state. During HIV infection and PE, various complement components are elevated, causing a hyper-inflammatory state. Furthermore, EC dysfunction and cell-mediated injury are also present. The similarities in pathological aspects of these three disorders may emanate from excessive complement activation. This review serves as a platform for further research on the complement system, coronavirus disease 2019, HIV infection and PE.
6.Alternate careers for medical graduates and house officers in Malaysia

Introduction: In recent years, many unresolved issues
pertaining to house officers in Malaysia have led to a longer
waiting time and a ‘glut’ of medical graduates with a
surprising 20% dropout amongst those who join the
housemanship programme. This appears to reflect the
changing times, mindsets and work expectations of
millennials who comprise this cohort reflecting a need to
consider possibilities of career shifts especially so in these
uncertain times. This study explores the perceptions,
awareness and interest in alternative career options
amongst recent graduates and house officers.
Materials and Methods: This was a study done between 2018
and 2019 using a questionnaire which was shared on
various social platforms. Data analysis was done using
Excel spreadsheet.
Results: A total of 450 house officers and 657 medical
graduates responded. Expectedly 66.8% claimed lifelong
passion whilst another 12.1% claimed family influence as
their reason to do medicine. Most were aware of their career
challenges and 40% of them were keen to consider career
change and reskilling indicating a possible shift from
traditional expectations of a medical career.
Conclusion: Whilst medicine is often considered a true
calling, current challenges will require mental and emotional
flexibility to explore other career opportunities. Thus,
engagement programmes should be directed at medical
graduates and house officers to identify and support those
open to career transitions. This will help address current
issues of internship bottleneck and rising dropout rates
amongst internees. Early career change engagements will
give them insight into their true career goals whilst opening
up opportunities for those who wish to change.
7.Factors Associated With Pressure Ulcer Risk Among Spinal Cord Injury Adults in Kuala Lumpur, Malaysia: A Community-based Study

Introduction: Pressure ulcers (PUs) are a leading health problem among spinal cord injury(SCI) patients. Therefore,
this comprehensive study was aimed at determining the association between potential factors and the occurrence
of PUs among SCI adults in community settings. Methods: A cross-sectional study was conducted among adults
with SCI aged 18 years and above from five selected locations in Kuala Lumpur, Malaysia. The study was carried
out through interviews based on validated questionnaires such as the Dietary History Questionnaire (DHQ), Spinal
Nutrition Screening Tool (SNST), Physical Activity Scale for Individuals with Physical Disabilities (PASIPD), Barthel
Index for Activities of Daily Living (ADL), and Instrumental Activities of Daily Living (IADL). Estimated weight and
height were obtained using recumbent formula. The stages of their PUs were assessed based on National Pressure
Ulcer Advisory Panel (NPUAP) grading system. Results: A total of 40 subjects participated in the study, 25% of
whom were having PUs. Their nutritional inadequacies were identified, whereby 90%, 70% and 85.5% of them
were receiving below their daily requirement of energy, carbohydrate and protein, respectively. The fat and sodium
intakes of 55% of them were above the Malaysian Recommended Nutrient Intakes(RNI). It was successfully found
that physical activity could decrease the risk of PUs(R2=0.654, OR=0.310, p<0.05). Conclusion: Overall, 25% of
the subjects had PUs that were associated with physical inactivity. The majority of the subjects did not meet their
nutrient requirements. Therefore, there is a need for increased physical activity and improved nutrient intake in order
to reduce the risk of pressure ulcers among SCI adults in communities.
