Septic arthritis is a surgical emergency. Prompt diagnosis
and immediate treatment reduce the destruction of articular
cartilage and give better outcome. We describe a simple,
minimally invasive closed tube irrigation system for the
initial treatment of septic arthritis of the knee in a patient
with complex medical problems who was unfit to undergo
surgery.

2.Evaluation of method of bone drilling and irrigation of antibiotic solution in treatment of osteomyelitis and suppurative arthritis in children.

The osteomyelitis and arthritis are common surgical emergency. During 10 years(from 1988 to 1997) we operated 214 cases with 143 males and 71 females with Haematogenous osteomyelitis by technique of drilling bone and irrigation of antibiotic solution, surgical drainage of subperiosteal pus. The purpose of those papers are to evaluate method of drilling bone with irrigation of the medullar canal by antibiotic solution and to examine some early signs of osteomyelitis in children. 115 patients were operated by drilling bone with irrigation of antibiotic solution, and 69 patients of them were operated by drainage of subperiosteal pus. The early signs of heamatogenous osteomyelitis with fever, localized paint, swelling, tenderness and warm are usual.

8.Pyogenic Arthritis of the Shoulder in Patient with Infective Endocarditis: A Case Report.

PURPOSE: We report a case of pyogenic arthritis of the shoulder secondary to infective endocarditis. MATERIALS AND METHODS: A 70 year-old male who had suffered from pyogenic arthritis of the left shoulder secondary to infective endocarditis was treated with artificial valvuloplasty, arthroscopic synovectomy and drainage. RESULTS: Infection was cured and the patient achieved a good functional outcome. CONCLUSION: Pyogenic arthritis of the shoulder is rarely associated with infective endocarditis. However, if the symptoms are misdiagnosed as musculoskeletal symptoms associated with infective endocarditis, serious complications may arise. As such, musculoskeletal symptoms associated with infective endocarditis should be paid careful attention.

Treatment of pyogenic arthritis in hip joints in adults usually required debridement through early arthrotomy. However, domestically arthroscopic management is not widely performed. Recent development of operation techniques and equipments helped to improve the value of arthroscopic approach and its diagnostic value and therapeutic usefulness. We report the clinical results from the 4 cases of pyogenic arthitis of hip joint in adults treating with arthroscopic irrigation and debridement.

10.Bilateral Wrist Fracture-Dislocation by Fall from Height : A Case Report.

Wrist fracture-dislocation including perilunar injury is an uncommon high energy trauma, especially cases occurred simultaneously on bilateral wrist have been quite rarely reported in the orthopaedics literature. We encountered a case of open transtriquetral dorsal perilunar dislocation on right wrist and transstyloid dorsal dislocation on left wrist by fall from height. We performed open reduction and percutaneous K-wire fixation with interosseous ligament repair for both wrist. On the left wrist, flexion-extension arc, radio-ulnar deviation arc, grip strength were recovered satisfactorily at 1 year after the injury. On the radiographs, there were no intercarpal instabilities and scapholunate angles were within normal range. But clinical results on right wrist were dissatisfied by septic arthritis sequelae, and patient complained discomfort at motion. We report this unusual pattern of injury with review of the relevant literature
