The shoulder pain is one of the most common problems to orthopaedic surgeons in clinic. Among therapeutic modality used to manage this pain, joint and periarticular injection, as well as suprascapular nerve block, show good clinical outcome. Ultrasound guidance is a safe technique, increasing the safety and accuracy of the procedure and reducing complications. An accurate understanding of the surface anatomy is important in performing the ultrasound-guided shoulder injections. This article aims to describe the surface anatomy and sono anatomy of both the shoulder and the surrounding structures and also summarize different infiltration techniques and peripheral nerve blocks.

3.Korean Pain Descriptors in Patients with Neuromusculoskeletal Pain

OBJECTIVE: To evaluate which Korean pain descriptors are frequently used in the patients with neuromusculoskeletal diseases and compare the frequency of Korean pain descriptor according to age, gender, pain pattern and intensity, and clinical diagnosis.METHOD: Two hundreds sixty nine patients with neuromusculoskeletal diseases were enrolled in this study. The patients were asked to fill out a pain questionnaire using Korean. The Korean pain descriptors were collected and classified according to neurophysiological mechanism. The frequency of Korean pain descriptor was analyzed by age, gender, pain pattern and intensity, and clinical diagnosis. They were divided into axial spine and peripheral joint pain group depending on the location of causal disease and shoulder pain descriptors were divided into intra-articular and bursa group.RESULTS: Among 24 Korean pain descriptors, ‘arida’ was the most common pain descriptor, followed by ‘ssusida’ and ‘jjireunda’. When the pain descriptors were classified according to neurophysiological mechanism, superficial somatic pain was the most common, followed by deep somatic pain. There was a significant difference in the frequency of the pain descriptor between axial spine and peripheral joint pain group (p=0.007). The pain descriptor ‘danggida’ was used significantly more in the patients with axial spine pain than peripheral joint pain (p=0.024). However, there was no significant difference in other factors.CONCLUSION: The patients with neuromusculoskeletal diseases expressed their pain using various Korean pain descriptors with stabbing nature and superficial somatic pain. Our results may be helpful to assess and develop a new Korean pain quality measure in the patients with neuromusculoskeletal diseases.

Hypermobility syndrome (HMS) was first described as a disease entity by Kirk et al. in 1967. It presents hypermobility of a joint without hereditary connective tissue disorders such as Marfan syndrome (MFS), EhlersDanlos syndrome (EDS), or osteogenesis imperfecta (OI). Arthralgia is a common symptom, and dislocation may occur with minimal trauma or even spontaneously. This symptom is often ignored or recognized as non-specific chronic pain because the diagnostic criteria for HMS varies among reserchers and because HSM has no typical radiologic and laboratory findings. We report a case of hypermobility syndrome, which was accompanied by spontaneous dislocation of both shoulders, along a review of appropriate literature on the subject.

6.Effects of Dan-jun Breathing Exercise Program Experienced by Women in Midlife.

PURPOSE: This study focused on affirming of the effects of Dan-jun breathing exercise experienced by women in midlife. METHOD: The data were collected via direct interview from 20 participants, who participated in a Dan-Jun exercise program during three months. Collected data were analyzed by content analysis. RESULT: 1. As for the health problems of women in midlife before the Dan-jun exercise program, 93 items were extracted in the content analysis. These items were categorized into 59 attributes and 15 higher attributes. 1) Of the 59 attributes of health problems, shoulder pain (30%) was the most dominant. 2) The 15 higher attributes consisted for the physical domain of, weakness, pain, joint stiffness, sexual problem, sleep disorder, gastro-intestinal disorder, menstrual irregularity, circulatory disorder, respiratory disorder, constipation, urinary incontinence, and for the psychological domain, anger, emptiness, depression, and anxiety. 2. As for the effects experienced by women in midlife after the Dan-jun exercise program, 169 items were extracted in the content analysis. These items were categorized into 85 attributes and 14 higher attributes. 1) Of 85 attributes, lightening of physical condition (55%) was the most dominant. 2) The higher attributes consisted in physical domain of, recovery of vigor, pain relief, menstrual regularity and improvements in flexibility, sexual problem, sleep disorder, gastro- intestinal disorder, circulatory disorder, constipation, respiratory disorder and urinary incontinence, and for the psychological domain, relaxation, cultured mind and self-confidence. CONCLUSIONS: Dan-jun breathing exercise program for three months showed positive effects on physical and psychological health in women in midlife. In this context, it can be also considered as a significant nursing intervention for maintenance and promotion of the health of these women.

7.A RETROSPECTIVE STUDY ON THE RELATIONSHIP BETWEEN MRI EVIDENCE OF TEMPOROMANDIBULAR JOINT EFFUSION AND CLINICO-SURGICAL FINDINGS.

PURPOSE: Although joint effusion caused by the inflammatory changes of synovium has been known to be associated with joint pain, it is still controversial. This study is to clarify the relationship between the evidence of joint effusion seen high signal intensity in T2-weighted magnetic resonance imaging and clinico-surgical findings. MATERIALS AND METHODS: All of the patients were diagnosed as temporomandibular joint internal derangement and received unilateral open surgery. The authors classified the joint effusion as four categories by size and shape: degree 0-no evidence; degree 1a-a line of high signal along articular surface in unfolded disc; degree 1b-high signal spot in the folded disc; degree 2-pooling in the anterior recess and folded disc; degree 3-pooling in the glenoid fossa and along articular eminence. The authors evaluated clinical and surgical findings. RESULTS: Two-hundred eighty-nine patients(242 females) with mean age of 30.2 years(range: 14 to 74) were included. High signal intensity was seen in 166 joints(57.4%): degree 1 in 82 joints, degree 2 in 69 joints(23.9%), degree 3 in 15 joints(5.2%). Maximal mouth opening of degree 0 group was 38.07mm, degree 1: 36.34mm, degree 3: 33.47mm. Mean TMJ pain score of the joint effusion groups was 1.58, no evidence group was 1.41. Click of degree 0 group was 44.7%, degree 3 was 43.4%. Crepitus of degree 0 group was 17.1%, degree 3 was 26.7%. Mean pain score of headache of degree 0 group was 1.80, degree 2 was 1.22. Neck and shoulder pain of degree 0 group was 50.4%, degree 1 was 39.0%. Perforation, adhesion, hyperemia of degree 0 was 27.6%, 35.8%, 18.7%, degree 2: 23.2%, 29.0%, 29.3%(degree 1). CONCLUSIONS: TMJ pain, TMJ sound and surgical findings did not relate significantly to the joint effusion of TMJ, but referred symptoms such as headache, neck and shoulder pain were less severe in the joint effusion groups.

9.Observation on therapeutic effect of pricking blood therapy combined with syndrome differentiation acupuncture on arthralgia syndrome.

OBJECTIVETo observe clinical therapeutic effect of pricking blood therapy combined with acupuncture on arthralgia syndrome.
METHODSOne hundred and ninety-two cases were randomly divided into a local pricking blood therapy combined with acupuncture group and a simple acupuncture control group, 96 cases in each group. Their therapeutic effects were compared.
RESULTSThe cured rate and the total effective rate were 83.3% and 100.0% in the treatment group and 31.0% and 87.0% in the control group with a significant difference or a very significant difference between the two groups (P < 0.05, P < 0.01); the mean times for cure in the treatment group was superior to that in the control group (P < 0.05, P < 0.01).
CONCLUSIONPricking blood therapy combined with acupuncture has an obvious and definite therapeutic effect on arthralgia syndrome.

10.Musculoskeletal Problems in Pregnancy.

OBJECTIVE: To investigate the frequency, nature and risk factors of musculoskeletal problems in pregnancy. METHOD: We obtained the information through a questionnaire and physical examination of full-term (gestational age over 38 weeks) pregnant women. Prevalence, onset time, severity and risk factors of musculoskeletal problems were analyzed. RESULTS: Among the 70 pregnant women, 37 women (53%) complained of low back pain which was the most common musculoskeletal problem in pregnancy. Other problems were hand symptoms (31%), lower extremity joint pain (31%), leg cramp (29%), chest pain, shoulder pain and neck pain. Previous history of low back pain was significantly correlated with the occurrence of low back pain in pregnancy. Weight gain during pregnancy and amount of time of housework during pregnancy were significantly correlated with carpal tunnel syndrome. Pregnant women suffering from leg cramp were significantly old in age. CONCLUSION: Prevalence of musculoskeletal problems during pregnancy was relatively high and several risk factors were related with occurrence of those problems. So, prophylactic risk factor control would be necessary to reduce musculoskeletal problems during pregnancy.
