8.Surgical Complications of Arteriovenous Malformations.

OBJECTIVES: The goal of surgical management of cerebral arteriovenous malformations (AVMs) is elimination of the lesions without development of new neurological deficits. To improve the surgical management of results of cerebral AVMs in the future, this article discusses about surgical complications of the AVMs and their preventive methods. METHODS: We investigate the methods to avoid the surgical complications of the AVMs based on the our experiences and review of literatures. RESULTS: Accurate anatomical knowledge about location of AVMs, proper reduction of nidus size by preoperative embolization, accurate dissection of peripheral margin of AVMs during surgery, and understanding and proper management of peculiar pathophysiological events of AVMs which occur during surgery and postoperative period are the key points for avoiding surgical complications of the AVMs. CONCLUSION: We conclude that refined skill for endovascular and microsurgical techniques are required to successful surgical treatment of cerebral AVMs.

The hemodynamic aspects of cerebral arteriovenous malformations(AVM) are currently the subject of increasing interest. Recently developed methods such as Doppler sonography, Xenon-enhanced Computed tomography Cerebral blood flow(CT CBF), single photon emission computed tomography(SPECT), and position emission tomography(PET) have promoted a more dynamic diagnostic and therapeutic approach to the complicated problem related to surgery of AVM. In this article, the literatures concerning the hemodynamics of cerebral AVM were reviewed.

10.Intracerebral artericvenous malformation in association with aneurysm

This association had been reported for long time with a frequency of 2.7-23% of cases of intracerebral arteriovenous connection. This association increases the risk of cerebral meningo hemorrhage. 3 cases of cerebral meningo hemorrhage associated with intracerebral arteriovenous connective malformation were studied retrospectively and treated at Viet Duc Hospital from January 1999 to January 2004. The presence of aneurysm in cerebral arteriovenous connection was an important predictive factor for the hemorrhage possibility, leading to cerebral hemorrhage
