1.Argon Laser Treatment of Trichiasis.

Recurrent trichiasis is a persistent annoyance and, too often, resists conventional treatment. We tried the application of argon laser in the treatment of trichiasis. The technique involves the destruction of the follicles of the abnormal cilia. This method proved safe, convenient, precise, and effective in 6 cases out of 6. The technique is most suitable when a few fine cilia are involved, but repeated treatment may be required for more and thicker eyelashes.

2.Clinical Evaluation of Laser Coreoplasty.

We performed coreoplasty on drawn pupils(17 eyes) by using Argon laser only or Argon laser combined with Q-switched Nd:YAG laser from June 1983 to May 1989. The results were as follows; 1. The causes were divided by 7 eyes(41.2%) with trauma and 10 eyes(58.8%) without trauma. 2. There were 15 eyes(88.2%) in up-drawn pupils and 2 eyes(11.8%) in downdrawn pupils. 3. The degree of preoperative drawn pupil was 1.42mm in average and enlargement of postoperative drawn pupil was 1.25mm in average. The corrected effect for preoperative drawn pupil and postoperative drawn pupil and postoperative drawn pupil was shown as 2.58mm in average. 4. Postoperative corrected visual acuity improved fairly. It was same as preoperative corrected visual acuity in 4 eyes(23.5%) and improved in 13 eyes(76.5%).

3.Transpupillary Argon Laaer Cyclophotocoagulation.

Transpupillary argon laser cyclophotocoagulation has been proposed as an alternative to conventional glaucoma surgery. The procedure involves phoptocoagulation of cilliary process to decrease intraocular pressure by reducing aqueous production. We report our experience with 3 advanced glaucoma patients, who received transpuillary cyclophotocoagulation. Their ciliary process was well visualized by gonioscopy. All visible process were treated up to a maximum of 1800 at one setting and treated again a few weeks later, so total transpupiJIary argon laser cyclophotocoagulation were performed 3 or 4 times. After surgery, each patient was maintained on preoperative glaucoma medication. As a result of this study, one of these patients was sucessfully controled and other one of them had effect of 20mmHg Intraocular pressure reduction.

4.Effect of Apraclonidine Hydrochloride on the Acute Intraocular Pressure Rise after Argon laser Iridotomy.

To determine the effect of apraclonidine hydrochloride on the acute intraocular pressure (IOP) rise after argon laser iridotomy (ALl), a double-masked comparative study was carried out. Twenty-nine eyes (20 patients) with angle-closure glaucoma underwent ALL Eighteen eyes were treated with apraclonidine and the remainder received placebo 1 hour before and immediately after ALl. The mean IOP increase in the apraclonidine group was lower than that in placebo group at each postlaser intervals (p<0.01). Although average value of maximal increases of IOP after ALl in apraclonidine group was 4 mmHg, that in placebo group was 16mmHg. 27.3% (3 out of 11 eyes) in placebo group experienced IOP rise greater than or equal to 10 mmHg, however, that kind of IOP rise was not found in apraclonidine group (0 out of 18 eyes) (p
5.The Effect of Indirect Laser Photocoagulation for Central Serous Chorioretinopathy.

In this study, we evaluated the effect of the argon indirect laser photocoagulation for patients with central serous chorioretinopathy (CSC), whose leaking points were indentified near the fovea or inside the papillomacular bundle. Among the 99 eyes of CSC, 62 eyes were assigned to group one, in which leaking points were located outside the papillomacular bundle or at more than 500 microdistance from fovea, and 37 eyes were assigned to group two, in which leaking points were located inside of papillomacular bundle or within 500 microdistance from fovea. Among the eyes in group one, 22 eyes had undergone direct laser photocoagulation treatment; among the eyes in group two, 27 eyes undergone indirect laser photocoagulation treatment; the rest 50 eyes were untreated. With theses results, We concluded that indirect laser treatment may not be in shortening the duration of central serous chorioretinopathy or in improving the vision. Its reccurence rate was higher than the rest of the modalities.

6.Endoscopic Treatment of Gastric Adenoma.

Gastric adenoma (dysplasia) is a precancerous lesion. Therefore, managements of gastric adenomas are important for preventing the development of gastric cancers and for detecting gastric cancers at earlier stages. The Vienna classification divides gastric adenomas into two categories: high-grade dysplasia and low-grade dysplasia. Generally, endoscopic resection is performed for adenoma with high-grade dysplasia due to the coexistence of carcinoma and the potential of progression to carcinomas. However, the treatments of adenoma with low-grade dysplasia remain controversial. Currently two treatment strategies for the low-grade type have been suggested; First is the ‘wait and see’ strategy; Second is endoscopic treatment (e.g., endoscopic mucosal resection, endoscopic submucosal dissection, or argon plasma coagulation). In this review, we discuss the current optimal strategies for endoscopic management of gastric adenoma.

7.A Case of Optic Disc Pit.

Optic disc pits are an uncommon congenital anomaly of the optic nerve head. Serous detachment of the macula is a well known complication in patients with an optic disc pit Although spontaneous reattachment has been reported, most eye with macular detachment associated with optic disc pit have a poor visual prognosis inspite of treatments. We have experienced a case of optic disc pit associated with serous macular detachment, that has been treated with argon pure green laser photocoagulation, reporting it with a review of literature.

8.Influence of nonthermal argon plasma on the shear bond strength between zirconia and different adhesives and luting composites after artificial aging

PURPOSE: Plasma activation of hydrophobic zirconia surfaces might be suitable to improve the bond strength of luting materials. The aim of this study was to analyze the influence of nonthermal argon-plasma on the shear bond strength (SBS) between zirconia and different combinations of 10-MDP adhesive systems and luting composites after artificial aging. MATERIALS AND METHODS: Two hundred forty Y-TZP specimens were ground automatically with 165 µm grit and water cooling. Half of the specimens received surface activation with nonthermal argon-plasma. The specimens were evenly distributed into three groups according to the adhesive systems ([Futurabond U, Futurabond M, Futurabond M + DCA], VOCO GmbH, Germany, Cuxhaven) and into further two subgroups according to the luting materials ([Bifix SE, Bifix QM], VOCO GmbH). Each specimen underwent artificial aging by thermocycling and water storage. SBS was measured in a universal testing machine. Statistical analysis was performed using ANOVA and Scheffè procedure with the level of significance set to 0.05. RESULTS: Surface activation with nonthermal plasma did not improve the bond strength between zirconia and the tested combinations of adhesive systems and luting materials. The plasma-activation trended to reveal higher bond strength if the self-etch luting material (Bifix SE) was used, irrespective of the adhesive system. CONCLUSION: Plasma-activation seems to be suitable to improve bond strength between zirconia and self-etch resin materials. However, further research is necessary to identify the influence of varying plasma-parameters.

9.A Case of Argon Plasma Coagulation Therapy for Hemorrhagic Radiation-induced Gastritis.

Hemorrhagic radiation-induced gastritis is a rare but serious complication of upper gastrointestinal radiation treatment, and no simple and effective treatment method has yet been developed. Studies on effective treatment methods for achieving hemostasis in patients with hemorrhagic radiation-induced gastritis are necessary, because the new indications for upper gastrointestinal radiotherapy in the field digestive oncology can potentially lead to an increased incidence of radiation-induced gastric vasculopathy. For the first time in Korea and to the best of our knowledge, we report here on a 59-years-old male patient with hemorrhagic gastritis that was induced by external radiotherapy for ampullary adenocarcinoma. This was all well-treated using Argon plasma coagulation (APC).

10.The Usefulness of Argon Plasma Coagulation Compared with Endoscopic Submucosal Dissection to Treat Gastric Adenoma.

BACKGROUND/AIMS: To date, the best therapeutic modality for gastric adenoma, especially low-grade adenoma, has not been established. The aim of this study was to assess the usefulness of argon plasma coagulation (APC) in treating gastric adenoma compared with endoscopic submucosal dissection (ESD). METHODS: We included 210 patients with gastric adenoma, who underwent treatment with either APC (97 patients) or ESD (113 patients). The clinical and pathologic characteristics, mean duration of hospital stay, complications, and recurrence rates between the two groups were compared. RESULTS: The mean adenoma size was 0.9 cm and 1.1 cm in the APC group and ESD group, respectively (p<0.001). The mean duration of hospital stay was significantly shorter in the APC group than in the ESD group (1.6 days vs. 5.8 days, p<0.001). Complications did not occur in the APC group. However, one case of perforation (0.9%) and 6 cases of bleeding (5.3%) occurred in the ESD group. The recurrence rates were 15.3% (15/97 patients) in the APC group and 3.5% (4/113 patients) in the ESD group (p=0.003). The proportion of hospitalization was less in the APC group (43.3%, 42/97) than in the ESD group (100.0%, 113/113) (p<0.001). Medical expense was less in the APC group (377,172 won) than in the ESD group (1,430,610 won) (p<0.001). CONCLUSIONS: The findings of this study suggest that APC is a safe treatment method for gastric adenoma without serious complications. However, regular endoscopic follow-up is necessary to detect any residual or recurrent lesions due to the relatively high rate of local recurrence after APC.
