7.Review of 448 cases of acute appendicitis

Acute appendicitis accounted for 60-75% cases of abdominal emergencies. Because the clinical features were various, many patients received lately operation leading the severe complications, even death. A descriptive study on 448 patients with appendicitis in the military hospitals 103 and 175 during 12/1992 - 3/1994 has found that 40.6% of patients were lately hospitalized due to the use of antibiotics and analgesics. Other difficulty was atypical medical conditions. The study suggested that it should strengthen management of the private consultation and treatment and health education and communication.

8.Review of 620 cases of the acute appendicitis.

From the review of 620 cases with acute appendicitis, the author has shown that: 1. Abdominal pain is present in all patients with appenditis, the initial position of pain is difference: in the epigastrium (17.42%) or around the umbilicus (30.16%); the right lower quadrant (41.45%); the pain becomes steady: 77.58%. The pain in the right lower quadrant when palpation pressure is exreted: 80%, the rebound tenderness: 52.42%. The pain in the position of the appendix is the most important symptom for operation. 2. There was a 3.8% rate of negative appendectomies. The rate of late diagnosis is 16.55%. 3. Laparoscopic appendectomy offers advantages over the open technique by reducing wound infections as well as the other complications and a possible earlier return to normal activities. A laparoscopic procedure also allows complete evaluation of the abdominal cavity without an extension of the incision.

9.Using Alvarado practical score and rate of neutrophil to lymphocyte in diagnosis of acute appendicitis.

The study was conducted on 150 patients (82 males, 68 females) with acute appendicitis these were undergone appendectomy at the Military Central Hospital 108 between January 1998 and December 1999. These patients were examined retrospectively using Alvarado Practical Score combined with rate of neutrophil to lymphocyte more than 3.5. Mean age of patients was 3719.86 years (ranged from 92 to 2 years). Time from disease onset to operation is 20.8814.54 hours. The Alvarado practical score was applied easily, simple and appropriate for health clinics without modern equipment. For patient group in this study, the Alvarado practical score provided the sensitivity of 96.2% and specificity of 57.1% in diagnosis of acute appendicitis. Rate of neutrophil to lymphocyte produced the sensitivity of 73.6% and specificity of 85.7%. There was significant gender difference in diagnosis of acute appendicitis. Male is sevenfold more likely to be diagnosed accurately than female.
