PURPOSE: The lateralization of cognitive functions in crossed aphasia in dextrals (CAD) has been explored and compared mainly with cases of aphasia with left hemisphere damage. However, comparing the neuropsychological aspects of CAD and aphasia after right brain damage in left-handers (ARL) could potentially provide more insights into the effect of a shift in the laterality of handedness or language on other cognitive organization. Thus, this case study compared two cases of CAD and one case of ARL. MATERIALS AND METHODS: The following neuropsychological measures were obtained from three aphasic patients with right brain damage (two cases of CAD and one case of ARL); language, oral and limb praxis, and nonverbal cognitive functions (visuospatial neglect and visuospatial construction). RESULTS: All three patients showed impaired visuoconstructional abilities, whereas each patient showed a different level of performances for oral and limb praxis, and visuospatial neglect. CONCLUSION: Based on the analysis of these three aphasic patients' performances, we highlighted the lateralization of language, handedness, oral and limb praxis, visuospatial neglect and visuospatial constructive ability in aphasic patients with right brain damage.

4.Efficacy observation of Thoroughfare Vessel theory in acupuncture for post-stroke dysphasia.

OBJECTIVETo compare the difference in the efficacy on post-stoke dysphagia between the point selection according to Thoroughfare Vessel theory and conventional point selection in treatment.
METHODSSeventy-eight patients were randomly divided into an observation group (n = 42) and a control group (n = 36). In observation group, acupuncture was applied to the acupoints selected according to Thoroughfare Vessel theory such as Dazhu (BL 11), Shangjuxu (ST 37), Xiajuxu (ST 39), Neiguan (PC 6) and Gongsun (SP 4) Mainly. In control group, acupuncture was applied to the conventional acupoints such as Fengchi (GB 20), Lianquan (CV 23), Tiantu (CV 22), Neiguan (PC 6) and Zusanli (ST 36), etc. Acupuncture was given once per day in either group, 12 treatments made one session and 4 sessions of treatment were required. The water swallow test was adopted to assess the swallowing function in two groups.
RESULTSThe effective rate was 100.0% (42/42) in observation group and was 77.8% (28/36) in control group. The efficacy in observation group was superior to control group (P < 0.05). The curative time was (28.65 +/- 10.42) days in observation group and was (38.74 +/- 21.30) days in control group. The time was shorter apparently in observation group as compared with control group (P < 0.05).
CONCLUSIONThe Thoroughfare Vessel theory in acupuncture treatment for post-stroke dysphasia achieves a superior efficacy as compared with the conventional acupoint selection, and this theory may quickly determine the point prescription in treatment.

5.Clinical application of Loewenstein Occupational Therapy Cognitive Assessment Battery-Second Edition in evaluating of cognitive function of Chinese patients with post-stroke aphasia.

OBJECTIVETo investigate the clinical application value of Loewenstein Occupational Therapy Cognitive Assessment battery in Chinese patients with post-stroke aphasia.
METHODSCognitive functions of 59 Chinese patients with aphasia following a stroke were assessed with the Chinese version of the second edition of LOTCA battery and their linguistic functions were tested with the Western Aphasia Battery (WAB) Scale, respectively. The Results of LOTCA were analyzed and compared across different groups, in the light of gender, age, educational background, the length of illness, and the degree of aphasia.
RESULTSNeither the score of subtests of the LOTCA nor the overall scores of LOTCA of aphasia patients with different gender and educational background differed (all P>0.05). In different age groups, apart from thinking operation (F=3.373, P=0.016), visuomotor organization (F=3.124, P=0.022), attention (F=3.729, P=0.009) and the total score (F=2.683, P=0.041), there was no difference in terms of the other subtest scores of LOTCA (all P>0.05). In the groups of different length of time with illness, apart from orientation (F=2.982, P=0.039) and attention (F=3.485, P=0.022), the score of other subtests and the total score of LOTCA were not different (all P>0.05). In the groups of different degree of aphasia, apart from attention (F=2.061, P=0.074), both the score of other subtests and the total score of LOTCA differed (all P<0.05).
CONCLUSIONLOTCA might be suitable to assessing the cognitive ability of post-stroke Chinese patients with aphasia.

6.Neurotoxic Manifestations of an Overdose Intrathecal Injection of Gadopentetate Dimeglumine.

The intravenous administration of gadopentetate dimeglumine (GD) is relatively safe and rarely causes systemic toxicity in the course of routine imaging studies. However, the general safety of intrathecal GD has not been established. We report a very rare case of an overdose intrathecal GD injection presenting with neurotoxic manifestations, including a decreased level of consciousness, global aphasia, rigidity, and visual disturbance.
