1. Assessment and analysis of nursing workload of tertiary general hospitals based on work sampling method

Objective:
To assess and analyze clinical nursing workload of tertiary general hospitals in Tianjin.
Methods:
4 tertiary general hospitals in Tianjin were recruited by convenience sampling. 13 representative departments of the medical system and the surgical system were selected as the investigation departments. All the clinical duty nurses of every day work in selected departments were taken as the observation objects. Unified training investigators used work sampling method to observe and record the "operation" , "movement", "waiting" and "others" four types of activities of nurses, taking an observation point in a minute; SPSS17.0 was used for analysis.
Results:
Totally 172 800 sampling points were selected. Besides 2.62% (4 534/172 800) of other affairs, the total nursing workload in one day was 97.38% (168 264/272800), with 85.03% (146 923/172 800) operating time including direct nursing accounting for 33.64% (58 144/172 800), indirect nursing accounting for 8.92% (15 422/172 800), and 42.46% (73 363/172 800) of related nursing. Movement accounted for 11.71% (20 228/172 800), while waiting accounted for 0.65% (1 113/172 800).
Conclusion
Generally, nurses in tertiary general hospitals have long working time and heavy workload, but direct nursing which can reflects the value and connotation of nursing work is low. In nursing work, the "waiting" caused by the lack of personnel, material, equipment, environment problems made a waste of time and human resources to certain extent. Therefore, results of the present study can provide reference for tertiary hospitals to optimize the allocation of nursing human resources, improve the quality of nursing service, reduce the workload of nurses and construct lean management system.
2.Renal depth measured by CT optimize the glomerular filtration rate using the Gates method in living donor kidney transplantation

Objective To evaluate the significance of kidney depth obtained by computed tomography (CT) in measuring glomerular filtration rate (GFR) by Gates method in living kidney transplant donors.Methods Individual kidney depth was compared among the estimates of Tφnnesen,Taylor and Li Qian formulas and CT measurements in 167 living-related kidney transplant donors respectively.While maintaining the active region of interest of kidney and background unchanged in 137 99mTe-DTPA renal dynamic imaging cases,GFR was measured by Gates' method and individual kidney compared among the estimates of Tφnnesen,Taylor and Li Qian formulas and CT measurements.Results Left/right kidney depth obtained by CT,Tφnnesen,Taylor and Li Qian formula was 6.82 ± 0.96/7.02 ± 1.00,5.67 ± 0.58/5.71 ± 0.59,6.43 ± 0.77/6.81 ± 0.72 and 7.03 ± 0.76/7.06 ± 0.70 cm;GFR 45.44 ± 9.04/46.61 ± 9.06,37.54 ± 6.34/37.37 ± 6.02,43.39 ± 7.59/44.62 ± 6.94 and 46.99 ± 8.04/46.70 ± 7.30 ml/min respectively.Individual kidney depth and GFR calculated by Taylor and Li Qian were higher than those of Tφnnesen formula (P<0.01).Individual kidney depth and GFR calculated by CT were higher than those of Tφnnesen and Taylor formulas (P<0.01).Left kidney depth and GFR calculated by Li Qian formula were higher than those of CT measurements (P<0.01).And no significant difference existed in right kidney(P>0.05).Conclusions Kidney depth measured by CT improves the accuracy of kidney depth estimated by Gates method and optimizes GFR in living donors for renal transplant.
3.Roles of complement system in psychiatric disorders

The complement system is an important part of the innate immune system,including more than 50 secretory proteins and membrane-bound proteins,and it contributes to the clearance of apoptotic cells and invading pathogens to limit inflammatory immune responses and maintaining brain homeostasis.Complement activity is strictly regulated to protect cells from random attacks or to prevent the deposition of complement proteins in physiological cases.However,overactivation or abnormal regulation of the complement cascade in the brain can lead to neuronal damage and brain dysfunction.Recent studies have pointed out that changes in complement molecules exist in patients with psychiatric diseases and play an important role in the occurrence and development of diseases by regulating the function of neurons and glial cells.Therefore,summarizing the latest research progress of complement system in psychiatric diseases such as schizophrenia,autism spectrum disorder,major depression,bipolar disorder and anxiety disorder can provide new ideas for preventing and controlling psychiatric diseases caused by abnormal activation of complement system.
