1.The anatomical and clinical research of the meniscofemoral ligaments
Chinese Journal of Orthopaedics 2008;28(6):505-509
Objective To provide data on the incidence, shape, and sizes of the femoral attach.ments of meniseofemoral ligaments (MFLs),and to determine the function of MFIJs and the role in the femoral tunnel D0sitioning in the double-bundle reconstruction of PCL.Methods Anatomical study:Thirty cadaveric knees were used to investigate the incidence and tightening-slackening patterns during the knee nexion-extension of MFLs.Data were obtained to describe the size, shape, and the relationship between the position of the femoral attachment of MFLs and that of anterolateral bundle(ALB)and the posteromedial bundle(PMB) were detetrained.Clinical study:During arthroscopic reconstruction of PCL,the presence of MFIJs Was confirmed in 1 12 patients by observation and probing, the relationship between the femoral attachment of MFLs and that of ALB and PMB were determined. Results Anatomical study:The Humphry ligament and Wrisberg ligament were presented in l(3%)and 27(90%)case respectively. The femoral attachments were approximately ellipse both. The mean area was(18.14±3.05)mm2 and(25.63±7.92)mm2 respectively.Clinical study:The Humphry ligament and Wrisberg ligament were present in 2 cases(1.7%)and 67 cases(59.8%) respectively. The incidence of MFLs in knees with isolated PCL injury was highter than that in knees with multiple ligaments injuries. Conclusion The incident of MFLs is related to the trauma of the knee.and the Wrisberg ligament plays an important role in avoiding impingement between the ALB and the intereondylar roof during deep knee flexion. The Humphry ligament and the Wrisberg ligament are landmarks for the tunnel positioning of ALB and PMB in double-bundle PCL reconstruction respectively.
2.Effect of continuous blood purification on severe acute ancreatitis
Clinical Medicine of China 2010;26(9):931-932
Objective To investigate the effect of continuous blood purification(CBP) in the patients with severe acute pancreatitis(SAP). Methods Twenty-six patients with severe acute pancreatitis were divided into the control group and the observation group randomly. Fourteen cases in the control group were given conventional treatment only, 12 cases in CBP group were given continuous blood purification as well as the conventional treatment. The APACHE Ⅱ score and CRP of pre- and intra-treatment at the 2nd and 5th day were observed in the two groups. The duration of mechanical ventilation, the duration of ICU stay, the mortality of hospital were observed as well. Results The APACHE Ⅱ score and CRP of the second day and the fifth day of intra-treatment in the control group were 1.61±3.00,10.76±4.30, (149.71±60.06) mg/L and (101.21±33.98) mg/L, respectively, which were significantly different to those of (8.20±1.87) mg/L, (6.19±1.49) mg/L, (109.17±23.91) mg/L,(70.83±25.45)mg/L in the CBP group(P<0.05). The duration of mechanical ventilation, the duration of ICU stay and the mortality of hospital were (13.29 ± 6.23) d, (16.00 ± 6.86) d, 21.43% in the control group, which were significantly lower than those of (8.00 ±5.27)d,(10.33 ±4.81)d,8.33% in the other group (P <0.05).Conclusions CBP is an effective method for the patients with severe acute pancreatitis to shorten the duration of mechanical ventilation and the ICU stay, and decrease the mortality of hospital.
3.Multivariate Analysis of the “Small for Gestational Age” in Twins
Journal of China Medical University 2015;(12):1106-1109
Objective to analyze the influence obstetric factors of the small for gestational age(SGA)in twins,so as to direct the clinical prenatal supervision in clinic. Methods A retrospective study of the SGA was performed, the general data of 1 514 patients admitted in obstetrics and gyne-cology of Shengjing Hospital during 2001 to 2014 were analyzed by Logistic regression analysis. Results the incidence of the SGA in twins was 10.89%. the protective factors were male fetus, assisted reproductive technology(ARt)and gestational diabetes mellitus(GDM). the risk factors were the female fetus and hypertensive disorders in pregnancy. Conclusion the gender of fetus, assisted reproductive technology(ARt),gestation-al diabetes mellitus(GDM),hypertensive disorder complicating pregnancy are the influential factors of the SGA in twins. We recommend a further pre-pregnancy and pregnancy care which is of significance for the prevention of SGA.
4.The status and problems of forensic identiifcation standards
Chinese Journal of Forensic Medicine 2016;31(4):327-329,335
Standardization of Forensic Sciences is a worldwide judicial topic, with the implementation of judicial reform and the government’s intention on the standardization work, China, the golden age of the establishment of forensic identiifcation standards will becoming. However, the current standards of forensic identiifcation are less in quantity;uneven quality;slow updating; lack of scientiifcity in making standards procession and other issues, leading to the derailment of Forensic work and practice, many projects identiifed lack of identiifed standards. To this end, the authors suggested that a correct understanding of the legal status of forensic identiifcation standards and the presence of errors, committing to build forensic identiifcation standardization.
5.Clinical diagnose and significance of congenital sensorineural hearing loss combined with BPES.
Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2015;29(18):1660-1663
To analyze congenital sensorineural hearing loss combined with blepharophimosis-ptosis-epicanthus inversus syndrome (BPES). For the case of cochlear implantation to child with congenital sensorineural deafness combined BPES, accomplish routine examination and assessment, combining with literature to analyze the clinical diagnosis of this disease and its significance. Sensorineural hearing loss is a common congenital diseases with neonatal incidence of 1 per thousand - 3 per thousand, 50%-70% of deafness is associated with genetic factors, the incidence of congenital sensorineural hearing loss combined with eye disease is about 40%-60%, mainly reflected in ametropia and retinopathy. BPES's main clinical manifestations is blepharophimosis, ptosis, epicanthus inversus, and telecanthus. BPES is a rare autosomal dominant disease caused by FOXL 2 gene mutation, sometimes associated with retarded growth, delayed development, congenital heart disease, and microcephaly. Suffering from both sensorineural hearing loss and BPES is rare in reported literature. This case is diagnosed by clinical examination, without visual impairment. Facial nerve dysplasia has been found during the surgery. For congenital deafness patients with eye disease or other diseases, timely and correct diagnosis has important clinical significance, which can improve the diagnostic rate and make it coming true to early intervention, and then, effectively improve the quality of the patients. There are few literature reports, of patients with two kinds of genetic diseases. Our inference is that the cases are rare or the patients has visited different departments and ignored the other systems' signs. Therefore, in such doubtful cases, we should do the professional comprehensive examination in daily clinical work in order to avoid missed diagnosis or delayed treatment and intervention. By analyzing this case, the patient may also suffer from facial nerve dysplasia. Preoperatively viewing CT scan and operatively facial nerve monitor being used can avoid the occurrence of surgical complications.
