Antiphospholipid antibodies are increasingly investigated as a cause of arterial and venous thrombosis. These include anticardiolipin and lupus anticoagulant antibodies. The former was linked to several types of venous thrombosis, included deep venous thrombosis in upper and lower limbs, pulmonary embolism, intracranial venous and retinal venous thrombosis. Types of arterial thrombosis can be involved in coronary, carotid, cerebral or retinal arteries... Anticardiolipin antibody is found commonly in myocardial infarction, valve abnormalities and lupus erythematous.

Antiphospholipid antibodies (aPL) are commonly found in humans after an infection. Its action can promote thrombosis via the activation of endothelial cells, platelets, and neutrophils. This autoantibody is the leading cause of antiphospholipid antibody syndrome (APS), characterized by widespread thrombosis in various vascular beds. COVID-19 also causes acute ischemic stroke (AIS) in the younger demographic, who previously was not considered a population at risk for AIS, which may be related to APS. This narrative review will discuss the role of aPL in COVID-19 patients who experienced AIS during infection.

9.Retinopathy in a patient with systemic lupus erythematosus

Objective:
To descibe a case of bilateral severe ischemic retinopathy in a patient with systemic lupus erythematosus (SLE) and elevated antiphospholipid antibody.
Methods:
This is a case report.
Results:
An otherwise healthy 16-year-old Filipino female presented with bilateral diffuse retinal arteriolar occlusion, wide areas of capillary nonperfusion, and macular ischemia. The diffuse retinal ischemia eventually led to neovascularization and fibroproliferative-membrane formation. Despite laser panretinal photocoagulation, one eye developed vitreous hemorrhage requiring vitrectomy. She was diagnosed to have SLE with mildly elevated anticardiolipin antibody on serial measurements and treated with anticoagulant to prevent further thrombotic episodes.
Conclusion
This form of severe SLE retinopathy associated with elevated levels of antiphospholipid antibodies is rare. When it occurs, recognition is vital so that appropriate treatment can be instituted.

Antiphospholipid syndrome (APS) is an autoimmune disease that involves vascular thrombosis and pregnancy morbidity associated with elevated titers of antiphospholipid antibodies. APS can affect any organ and has a variety of clinical manifestations. Infections can be associated with thrombotic events in APS, including the potentially fatal subset catastrophic APS. We report a case of extensive symmetrical peripheral gangrene complicating Escherichia coli sepsis associated with antiphospholipid antibodies in a patient with lupus nephritis.
