Anterior Cerebral Artery

We report a patient with a left anterior cerebral artery (ACA) infarct showing representational neglect. Representational neglect is some form of neglect to areas of imagined space. We believe that the description of our case gives further information about neglect syndrome and the functional asymmetry of the cerebral hemispheres because these observations are rare.

Several studies have described the anatomy of the anterior cerebral vascular complex involving the proximal anterior cerebral artery(A1), the anterior communicating artery, and the recurrent artery of Heubner. The anatomic data of these vessels with special reference to the cerebrovascular artery were reviewed. The unilateral hypoplasia of the proximal anterior cerebral artery and fenestration of the anterior communicating artery were most frequent vascular anomalies of each vessel which were associated with aneurysms of the anterior communicating artery.

The authors report four cases of aneurysm of the anterior cerebral artery proximal to the anterior communicating artery(A1 segment). In three of these cases, the aneurysms arose from the distal A1 segment. In one case the aneurysm arose from the proximal A1 segment. The aneurysms were saccular in three cases and fusiform in a case. Hypoplasia of A1 segment was not present in any of these cases. Three saccular aneurysms were clipped and a fusiform aneurysm was trapped. Prognosis was good in two cases, poor in other two cases.

In a series of 375 bilateral carotid angiographies, two accessory middle cerebral arteries, one duplication of the middle cerebral artery demonstrated on the angiography were reviewed relative to the pertinent literature. These anomalous middle cerebral arteries are very rare. We reported a case of duplication of the middle cerebral artery at 1993 which was first one to be reported in Korea. At present, we report two additional anomalous accessory middle cerebral arteries. Among these, one was accessory middle cerebral artery originated from the horizontal portion of the anterior cerebral artery, and the other originated from the A2 segment of the anterior cerebral artery. Both of these gave rise to some perforating arteries.

The accessory middle cerebral artery (MCA) is a variation of the MCA branching, which originates from the anterior cerebral artery (ACA). It is deemed as an incidental anatomical finding and the incidence is reported to be 0.3% to 4.0%. Although the accessory MCA is infrequent it nevertheless may have important implications in the interpretation of transcranial Doppler (TCD) findings. We report two patients having an accessory MCA and its clinical significance.

Fenestrations of anterior cerebral arteries are rare anomalies, which may be associated with saccular aneurysms at the fenestration itself or other intracranial vessels. This association is not infrequently the cause of angiographic diagnostic problem, as well as surgical difficulties. We report a case of aneurysm arising from a fenestrated proximal anterior cerebral artery that was confirmed at surgery and discuss together with a review of the literature.

The authors described different type of aneurysms in the distal anterior cerebral artery(DACA), which is difficult to approach surgically. there are anomalies of distal anterior cerebral artery in about one fourth and narrow space, which made more careful surgical dissection. We experienced 7 cases of DACA aneurysm, 6 cases of which were treated surgically through the interhemispheric approach via the unilateral frontal paramedian craniotomy on the non-dominant side. Surgical results were anatomical complexities, modern microsurgical technique aided by microscope make the approach more exactly and safely.

Seven cases of A2 anomaly including azygous A2 and accessory A2 were observed during aneurysm surgery. We present case histories, angiograms and operative descriptions, and discuss with the clinical significance of the anomaly. The recognition of the anatomic variations prior to clip placement for anterior cerebral artery aneurysm is emphasized.
