1.Effect of Jiuweiqianghuo pill combined with salmeterol fluticasone propionate on chronic obstructive pulmonary disease

Objective To observe the effect of of Jiuweiqianghuo pill combined with salmeterol fluticasone propionate on chronic obstructive pulmonary disease.Methods136 patientswith chronic obstructive pulmonary disease from January 2016 to December 2016 were grouped two groups by admission sequence.Two groups were treated with routine treatment and observation group was treated another with Jiuweiqianghuo pill to analyse its effect.ResultsThe total effective rate of observation group was 94.12%,higher than control group 79.41%(P<0.05).FEV1/ expected value and FEV1/FVC of observation group were(66.32±4.27),(71.15±4.41)%,higher than that of control group(58.37±4.19,65.16±4.30)%(P<0.05).6MWT and CAT score of observation group were(319.1±19.5m,19.15±2.97points),better than that of control group(287.4±18.0)m,(23.01±3.08)points(P<0.05).IL-8,TNF-α and hs-CRP level of observation group were(13.40±4.05)ng/L, (18.64±4.21)ng/L, (10.91±2.59)mg/L,lower than that of control group(18.92±4.27)ng/L, (25.13±4.48)ng/L, (14.67±2.75)mg/L(P<0.05).Adverse reactions rate of observation group was 2.94%,slightly lower than control group with 5.88%.ConclusionJiuweiqianghuo pill can decreased serum inflammation and improve lung function to improve the therapeutic effect of chronic obstructive pulmonary disease and it does not increase adverse reactions.
2.The study of mRNA expression of adiponectin in omental adipose tissue of patients with type 2 diabetes

Objective To study the adiponectin mRNA expression in omental adipose tissue of type 2 diabetes. Methods RT-PCR method was used to examine the adiponectin mRNA expression. Fasting levels of BG,TG,TC,HDL-C,LDL-C of all the subjects were checked and their blood pressure,height,weight,waist circumference,hip circumference were measured to calculate BMI,WHR. Results The adiponectin mRNA expression in omental adipose tissue was decreased in diabetes group versus non-diabetes group (P<0.05),and negatively correlated with WC,TG,FBG,WHR(P<0.05). Conclusions Type 2 diabetic patients show lower expression of adiponectin mRNA in omental adipose tissue than that of non-diabetic control.WC,TG,FBG, WHR and course of disease are correlated with adiponectin mRNA expression and insulin resistance
3.INTRATHECAL OXYGEN IN THE TREATMENT OF DELAYED NEUROLOGIC SEQUELAE OF CARBON MONOXIDE INTOXICATION: REPORT OF 8 CASES

8 patients, 6 males and 2 females, aged 26-56 years, with delayed neurologic sequelae following CO intoxication, were treated. The lucid intervals before the appearance of neurological sequelae varied from 5 days to 3 months, and the course of disease lasted for 7 days to 2.3 years. The clinical manifestations were mainly dementia, mutism, urinary or fecal incontinence, and gait disturbance. Among the 8 patients, clinical cure was obtained in 5 and improved in 3 after intrathecal oxygen administration.
4.In vitro activities of demestic macrolides against Chlamydia trachomatis and Chlamydophila pneumoniae

Objective To evaluate the activities of four demestic macrolides against C. trachomatis and C. pneumoniae by antimicrobiai susceptibility testing. Methods Cell culture and immunoflourescence staining of chlamydial inclusions were used to determine MICs of four demestic macrolides against C. trachomatis and C. pneumoniae. Results MIC (0.5 μg/ml) was found for acylspriramycin,erythromycin and azithromycin against C. trachomatis serovar B while it was 4 μg/ml for acetylspiramycin. Agaisnt C. trachomatis serovar D, MIC was 0.25 μg/mi in both acylspriramycin and azithromycin, and MICs were 0.5 μg/ml and 2 μml in erythromycin and acetylspiramycin, separately. Agaisnt C. pneumoniae TWAR, erythromycin was the most active with MIC≤0. 016 μg/ml, acylspriramycin and azithromycin were the second with same M1C of 0.032 μg/ml. However, acetylspiramycin was less active with 0.5 μg/ml of MIC. Conclusion Except acetylspiramycin, acylspriramycin erythromycin and azithromycin had reliable activities against both C. trachomatis (serovar B and D) and C. pneumoniae.
5.Risk factors of hypoparathyroidism after total thyroidectomy and bilateral central lymph node dissection in patients with papillary thyroid carcinoma

Objective To investigate the risk factors of hypoparathyroidism after total thyroidectomy and bilateral central lymph node dissection in patients with papillary thyroid carcinoma.Methods Data of patients with PTC who accepted total thyroidectomy and bilateral central lymph node dissection in the Department of Thyroid Surgery from Jan.2013 to Jun.2016 were collected and analyzed retrospectively.The patients were divided into normal group,transient hypoparathyroidism group and permanent hypoparathyroidism group according to the level of serum parathyroid hormone within 6 months after surgery.Clinical data were collected for comparison between the three groups.The risk factors of hypoparathyroidism were indentified with univariate analysis and multivariate analysis.Results A total of 468 patients,241 in the normal group and 227 in the hypoparathyroidism group (220 in the transient hypoparathyroidism group and 7 in the permanent hypoparathyroidism group),were included in the study.Univariate analysis showed that without application of carbon nanoparticles (P=0.04) and autotransplantation of more than one parathyroid gland (P<0.001) were risk factors of hypoparathyroidism,and without application of carbon nanoparticles (P=0.047),incidental parathyroidectomy of one parathyroid gland (P=0.04),gross extrathyroidal extension (P=0.006) and c N1a were risk factors of permanent hypoparathyroidism.Multivariate analysis showed that without application of carbon nanoparticles (OR,0.437;95% CI,0.243-0.789;P=0.006) and autotransplantation of more than one parathyroid gland (OR,3.025;95% CI,1999-4579;,P=0.000) were independent risk fact ors of hypoparathyroidism,and without application of carbon nanoparticles (OR,0.197;95% CI,0.039-0.982;P=0.048) and gross extrathyroidal extension (OR,12.381;95% CI,1.432-107.036;P=0.022) were independent risk factors of permanent hypoparathyroidism.Conclusion When total thyroidectomy and bilateral central lymph nodes dissection were performed,carbon nanoparticles were routinely applied.Although autotransplantation of more than one parathyroid gland can increase the incidence of transient hypoparathyroidism,it can reduce the incidence of permanent hypoparathyroidism.If extrathyroidal extension is suspected,hypoparathyroidism should be emphasized to patient,and accurate operation should be done to reduce the incidence of hypoparathyroidism.
6.Evaluation of the VIDAS chlamydia test detect Chlamydia trachomatis in samples from urogenital tract

Objectives To evaluate bioMerieux VIDAS(Vitek Immune Diagnositc Assay System) Chlamydia test (CHL) and to determine its performance(sensitivity and specificity) by comparing with cell culture. Methods C. trachomatis in urogenital samples was detected by both cell culture and VIDAS CHL. The different results were confirmed by direct fluorescent antibody assay (DFA). The sensitivity to C.trachomatis serotype D and E stocks was alsode tected with VIDAS CHL and cell culture. Results C.trachomatis was found in 33 (20.2%) of 163 urogenital samples by cell culture in coutrast to in 44(27.0%) by VIDA CHL. As the expanded gold standard was defined as either cell culture positive or cell culture negative and both CHL and DFA positive, the sensitivity was 80.5% and 95.3% and the specifiaty were 100% and 97.6% in cell culture and VIDAS CHL, respectively. In the sensitivity test, C. trachomatis serotype D was tested positive at the highest dilution of one to 102 400 dilution and serotype E was at one in 51 200 by cell culture. However, both serotype D and E were tested positive at the highest dilution of one to 6 553 600 by VIDAS CHL. Conclusions Comparing with the expanded gold standard, VIDAS CHL is sensitive and specific for C.trachomatis in urogenital specimens, with simple and short running hours (1 h). First catch urine (FCU), which avoids the painful male swab collectionin male patients, could also be used as specimen in VIDAS CHL test.
7.Monitoring of influenza by shell viral assay and direct immunofluorescent staining

Objective To establish a rapid culture method for monitoring of influenza circulation and laboratory diagnosis of individual patients with influenza. Methods Nasal aspirate specimens were spun onto 24-well plate containing confluent monolayers of Madin-Darby Canine Kidney (MDCK) cells. Fluorescein labeled monoclonal antibodies to influenza A and B viruses were used to stain and type the isolates. Results A total of 713 nasal aspirates specimens from patients experiencing influenza-like symptoms was collected in Beijing between December of 2000 and March of 2001, 187(26 2%) specimens were found with influenza A virus and 109(15 3%) with influenza B virus. 2 2% with influenza A and 11 2% with influenza B virus were detected in 89 nasal aspirates specimens between Jan-Apr 2002. Conclusion Prevalence of influenza was low during 2000~2002 year non-epidemic period in Beijing. The combination of shell viral assay and direct immunofluorescent staining can provide a rapid laboratory diagnosis of influenza, which makes possible for the patients to receive treatment of anti-influenza virus drugs.
8.Clinical Observation of Short-segment Percutaneous Pedicle Screw Non-fusion Fixation Plus Electro-acupuncture for Treatment of Single Thoracolumbar Fracture

Objective To investigate the feasibility of short-segment percutaneous pedicle screw non-fusion fixation plus electro-acupuncture for the treatment of single thoracolumbar fracture.Methods Forty-three patients with single thoracolumbar fracture were treated with short-segment percutaneous pedicle screw fixation without fusion,and then were given electro-acupuncture on points such as Jiaji (EX-B2),Huantiao (GB30),and Zhibian (BL54) after the operation.Before and after operation and during the follow-up,X-ray images of the spine at positive position and lateral position were taken for the measurement of vertebral anterior and posterior diameter ratio and the vetebral kyphosis Cobb's angle.Lumbago visual analogue scale (VAS) scores and Oswestry scores for lumbago function disorder were used for the evaluation of clinical efficacy.Results (1) All of the 43 patients were followed-up,and the follow-up period lasted for 10-27 months.(2) After the operation and at the end of follow-up,vertebral anterior and posterior diameter ratio and Cobb's angle were much improved (P < 0.01compared with those before the operation),and the two indexes at the end of follow-up were improved as compared with those after the operation (P < 0.05).(3) One week after the operation and at the end of follow-up,VAS scores and Oswestry scores were obviously improved as compared with those before treatment (P < 0.01),and the two kinds of scores at the end of follow-up were also improved as compared with those after the operation (P <0.01).Conclusion Short-segment percutaneous pedicle screw fixation without fusion plus electro-acupuncture at points such as Jiaji is effective for the treatment of single thoracolumbar fracture by relieving pain in the waist and back,which ensures the satisfactory effect of operation.

Objective To investigate the level of soluble interleukin 7 receptor (sIL-7R) in serum of lupus nephritis(LN)patients and evaluate its clinical significance. Methods sIL-7R level in serum of LN patients and healthy controls were measured by ELISA , while total 24 hours urinary protein and complement C3 of LN patients were measured by BN ProSpec. The level of sIL-7R correlation with SLEDAI, total 24 hours urinary protein and complement C3 were analyzed respectively. Results The levelof sIL-7R was higher in serum of LN patients than healthy controls (P < 0.01). Moreover, its expression in serum was increased in LN patients in active stage than in LN patients in stable stage (P < 0.05). The level of sIL-7R was positively assosicated with SLEDAI, total 24 hours urinary protein(P < 0.01, P < 0.05) and negatively with complement C3 (P < 0.05). Conclusion The level of sIL-7R is upregulated in serum in LN patients and correlated with disease activity and progression, so it may be expected to become a potential marker of disease in prediction.
