Chinese Journal of Nephrology 2008;24(11):792-796
Objective To investigate the survival rate and the influencing factors in lupus nephritis (LN) patients with neuropsychiatric systemic lupus erythematosus (NPSLE). Methods Clinical characteristics and biochemical markers of 78 patients including 59 variances were analyzed. Patients were followed up from the onset of NPSLE to death. Patient survival rate was estimated by Kaplan-Meier method. Cox regression model was used to analyze influencing factors. Results Sixteen (20.5%) of 78 patients died of SLE or its complications. Infection was the main cause of death (31.3%). One-, 3-, 5- and 10-year survival rates were 83.2%, 81.7%, 76.7% and 76.7%, respectively. Hypertension (RR =6.965,95% CI:1.578-30.746, P= 0.010), pulmonary infection (RR=8.171,95% CI:1.954-34.177, P=0.004)and acute renal failure (RR=6.978,95%CI:2.063-23.609, P=0.002) were risk factors of mortality, while cyclophosphamide (CTX) impulse therapy (RR =0.130,95 % CI:0.031-0.541, P=0.005) and resolution of NPSLE (RR= 0.169, 95%CI:0.042-0.679,P=O.012)were protective factors. Conclusions Infection is the main cause of death in patients of LN complicated with NPSLE. Survival rate of LN patients with NPSLE in this study is lower than those of LN and NPSLE alone reported by other authors. Hypertension, pulmonary infection and acute renal failure are risk factors of mortality, while CTX impulse therapy and resolution of NPSLE reduce the mortality and improve the prognosis.

Objective To find a new indicator monitoring severe acute respiratory syndrome (SARS) by analyzing expression of lymphocytes and its subsets in SARS. Method Flow cytometer and hematology analyzer were used to detect the expression of lymphocytes and its subsets in 38 patients of SARS. Results Decreasing lymphocytes were found in 84 percent of the patients. The absolute value of lymphocyte subsets (T/B cell, NK cell) also decreased in a certain degree. Patients with decreasing T cell were mostly observed (95%). T helper/inducer (T 4)cells showed a higher rate of decreasing (100%) than T suppressor/killer (T 8)cells (87%). Based on the distribution of lymphocytes and its subsets, T cells demonstrated the greatest rate of decreasing (58%). Most patients had a relative normal B cells and NK cells with a rate of decreasing as little as 2.6% and 5.3%, respectively, more patients showed a decreasing T 4 cells (82%) than that of T 8 cells (34%). Forty-four percent of patients demonstrated a low ratio of T 4 to T 8. Conclusion The lymphocytes and its subsets were impaired in SARS, especially for T lymphocytes, T 4 cells were more seriously damaged than T 8 cells. The absolute counts of lymphocyte subsets showed more significant change of immune cells than relative counts of that in SARS.

Objective:To understand the employment situation of graduates of public health-related majors and explore the associated factors of turnover intention among public health practitioners in China. Methods:In April 2020, the convenient sampling method was used to recruit public health related graduates from 98 universities with public health-related majors from 31 provinces in China (excluding Hong Kong, Macao and Taiwan) and from Chinese Center for Disease Control and Prevention (only including working or unemployed status, no requirement for graduation years). A total of 1 322 questionnaires were collected, of which 1 165 (88.1%) were valid. Through the network questionnaire survey, the information of public health graduates′ work situation, job satisfaction and turnover intention were collected. Multivariate logistic regression model was used to analyze the associated factors of turnover intention, and structural equation model was constructed to explore the correlation between turnover intention and job satisfaction. Results:The age of 1 165 graduates was (28.1 ±4.8) years. Among them, 719 (61.7%) were female and 856 (73.5%) graduates were engaged in public health-related work after graduation. The results of multivariate logistic regression analysis showed that compared with those over 30 years old, graduates under 30 years old had higher turnover intention ( OR=2.531, 95% CI: 1.549-4.155). Compared with those with junior or no title, graduates with senior title had higher turnover intention ( OR=2.310, 95% CI: 1.047-5.162). The results of structural equation model analysis showed that the four factors of job satisfaction, ‘salary and welfare’, ‘promotion development’, ‘work itself’, and ‘internal and external environment’, were negatively correlated with turnover intention. The internal and external environment had the greatest impact on turnover intention (total effect =-0.539). Conclusion:Most graduates of public health-related majors are engaged in public health-related work in China. Age, professional title and job satisfaction are the associated factors of turnover intention.
