2.Reducing radiation dose in liver enhanced CT scan by setting mAs according to plain scan noise

Objective To investigate the feasibility of setting mAs in liver enhanced CT scan according to plain scan noise with fixed mA CT scanner,in order to reduce the radiation dose.Methods One hundred continuous patients underwent liver enhanced CT scan (group A) prospectively.Two hundred and fifty mAs was used in plain and enhanced CT scans.Noises of plain and venous phase CT images were measured,and the image quality was evaluated.The equation between mAs of enhanced scan and noise of plain scan image was derived.Another 100 continuous patients underwent liver enhanced CT scan (group B).Enhanced scan mAs was calculated from noise on plain scan by using the equation above.Noises on venous phase images were measured and the image quality was measured.Based on body mass index (BMI),patients in groups A and B were divided into three subgroups respectively:BMI < 18.5 kg/m2,18.5 kg/m2 ≤ BMI < 25.0 kg/m2 and BMI ≥ 25.0 kg/m2.Image quality score was compared with nonparametric rank sum test,CT dose index (CTDI) and effective dose (ED) were measured and compared between each subgroup with 2 independent samples t or t' test.Results The equation between enhanced scan mAs (mAsX) and plain scan noise (SDp) was as follows:mAsX =mAs1 × [(0.989 × SDp + 1.06) /SDx]2,mAs1 =250 mAs,SDx =13.In patients with BMI < 18.5 kg/m2,ED of group A [(6.86 ±0.38) mSv,n =12] was significantly higher than group B [(2.66 ±0.46) mSv,n =10)] (t =18.52,P <0.01).In patients with 18.5 kg/m2 ≤ BMI < 25.0 kg/m2,ED of group A [(7.08 ± 0.91) mSy,n =66] was significantly higher than group B [(4.50 ± 1.41) mSv,n =73] (t' =10.57,P < 0.01).In patients with BMI ≥25.0 kg/m2,there was no significant difference between EDs of group A (7.54 ± 0.62 mSv,n =22) and group B [(8.19 ±3.16) mSv,n =17] (t' =0.89,P =0.39).Image quality of 5 patients in group A and none in group B did not meet the diagnostic requirement.Conclusion Setting mAs of enhanced scan according to plain scan noise could reduce the radiation dose with maintainence of image quality.
3.CFTR suppressing hyperhomocysteinemia-induced hepatocyte damage in ApoE-/-mice

Objective To investigate the function of CFTR in ApoE-/- mice with HHcy-induced hepato-cellular injury. Methods Thirty six 5-week old ApoE-/- mice were divided into three groups , including the ApoE-/- group, the HHcy group and the intervention group, (n = 12). Twelve normal C57BL/6J mice were fed with regular mouse diet as the normal control (SPF grade). HL-7702 human liver cells were intervened by Hcy (100 μmol/L) and 100 μmol/L Hcy + folic acid (100 μmol/L Hcy + F). The changes of Hcy, ALT and AST in the serum and the expression of CFTR mRNA and protein in liver and liver cells were detected. The concen-trations of ALT and AST in the liver cell intervened by VX-770 agonist and CFTR(inh)-172 inhibitor were mea-sured by ELISA. Results Compared with the control group , the levels of Hcy , ALT and AST were higher and the levels of CFTR mRNA and protein were lower in the Meth group (P < 0. 05 ) , while the reverse result in the Meth + F group (P < 0.05). Compared with the control group, the levels of CFTR mRNA and protein were de-creased and the levels of ALT and AST were increased in the 100 μmol/L Hcy group (P < 0.05). Compared with the 100 μmol/L Hcy group , the levels of CFTR mRNA and protein were increased and the levels of ALT and AST were decreased in the 100 μmol/L Hcy + F group (P < 0.05). Stimulated with VX-770 can reduce the concentrations of ALT and AST and the vice versa in the CFTR (inh)-17 group the concentration was increased in liver cells. Conclusion CFTR plays an important role in the regulation of hepatocellular injury by HHcy.
4.Hcy promotes the formation of atherosclerotic and effect of liver lipid metabolism disorder in ApoE-/- mice

Objective To study the effect of homocysteine(Hcy)on the formation of atherosclerotic and acceleration of ApoE-/-mice liver lipid metabolism disorder .Methods 12 normal 5 weeks old C57BL/6J mice served as control group ,and 36 5 weeks old C57BL/6J A poE-/- mice were randomly divided into 3 groups(n=12 for each group) ,the model control group ,the hyperhomocys-teinemia(HHcy)group and the intervention group(intervened by folate and vitamin B12 ) .18 weeks later ,the blood of the mice was gotten using a Unilateral enucleation method ,and the Serum Hcy and lipid changes were detected by Biochemical analyzer .And the changes of plaque size were measured by HE staining .The liver tissues of the 4 groups mice were taken and the changes in hepato-cyte lipid were detected by oil red O staining ,and the hepatic lipid levels were measured by enzymatic determination(by Semi-quan-titative image analysis) .Results The results showed that ,when compared with the control group ,the serum Hcy ,LDL ,TG and CHOL levels of the HHcy group significantly increased by 2 .3 ,2 .8 ,5 .0 ,10 .7 fold(P<0 .01)and the content of HDL decreased by 64% (P<0 .01) ,and the result showed that ,conpared with the HHcy group ,the seram Hcy ,LDL and CHOL levels of the interven-tion group were significantly decreased by 43% ,34% ,21% (P<0 .05) .Atherosclerotic fatty plaque could be seen in the hyperlipi-demic ,model and intervention group .Meanwhile ,there was a large number of scattered fat in A poE-/-mice liver by oil red O staining in the HHcy group ,and the CHOL and TG levels were 2 .2 fold and 2 .8 fold higher in the HHcy than that in the normal control group respectively(P<0 .01) .And compared with the HHcy group ,the serum CHOL and TG levels of the intervention group sig-nificantly decreased by 34% ,33% (P<0 .01) .Conclusion It is found that Hcy can induce the formation of As and accelerate liver lipid metabolism disorder .
5.Effect of miRNA-143 on homocysteine induced vascular smooth muscle cell proliferation

Aim To explore the effect of miRNA-143 ( miR-1 4 3 ) on homocysteine ( Hcy ) induced-vascular smooth muscle cells ( VSMCs ) proliferation and the mechanism .Methods VSMCs were cultured and in-cubated with Hcy by using primary cultured method . Then, cells were treated with different concentrations of Hcy and folate .VSMCs proliferation was determined with MTT assay , miR-143 was measured by qRT-PCR, and methylation of miR-143 was determined with meth-ylated PCR.Results After cells were treated with dif-ferent concentrations of Hcy , the proliferation of VSMCs was significantly increased , mRNA expression of miR-143 was decreased and methylation of miR-143 was increased .The proliferation of VSMCs was signifi-cantly decreased when transfected VSMCs with miR-143 precursor , and cell proliferation was increased by using miR-143 inhibitor transfection .Conclusion Hy-pomethylation of miR-143 may inhibit VSMCs prolifera-tion.
6.Effect of Rosuvastatin on Prognostic of Patients with Paroxysmal Atrial Fibrillation after Circumferential Pul-monary Vein Ablation

OBJECTIVE:To observe the effect of rosuvastatin on prognostic of patients with paroxysmal atrial fibrillation (AF) after circumferential pulmonary vein ablation. METHODS:75 patients with paroxysmal AF were divided into observa-tion group(n=39)and control group(n=36)according to the admission single. All patients underwent circumferential pulmo-nary vein ablation antiarrhythmic treatment. Control group orally received warfarin sodium,amiodarone,metoprolol,and sub-cutaneous injected low molecular weight heparin. Observation group additionally received 10 mg Rosuvastatin tablet,qd on the basic of contol group. Changes of cardiac function,inflammatory factors,lipid levels in 2 groups before and after treat-ment were observed,and follow-up results were compared. RESULTS:There was no significant difference in left atrial diame-ter and left ventricular ejection fraction in 2 groups before and after treatment (P>0.05);atrial effective refractory periods (AERP)in 2 groups significantly prolorged,and observation group prolorged more significantly than control group,the differ-ence was statistically significant(P<0.05). Serum high sensitivity C-reactive protein,interleukin-6 48 h and 1 month after op-eration in 2 groups significantly increased,and control group was significantly higher than observation group,the difference was statistically significant (P<0.05);before operation and after 3 months of operation,there was no significant difference in inflammatory cytokine levels(P>0.05). Triglyceride and low density lipoprotein cholesterol in observation group 1 month after operation significantly decreased, HDL-C significantly increased, there were significant differences (P<0.05). Fol-low-up time was (23.91 ± 5.28) months,AF recurrence was 7.8%,which was significantly lower than control group (13.9%),the difference was statistically significant (P<0.05);and there were no significant differences in ablation-related atrial tachycardia (ATa) and the incidence of adverse reactions between 2 groups (P>0.05). CONCLUSIONS:Rosuvastatin can effectively inhibit the inflammatory reaction and prolong the AERP,thereby reducing the recurrence rate of AF,with good efficacy and safety.
7.Drug resistance characteristics and epidemic trend of Escherichia coli in Anning, Yunnan

Objective To investigate the drug resistance characteristics and epidemic trend of Escherichia coli in Anning, Yunnan, and to provide a reference for clinical rational use of antibiotics. Methods A total of 376 strains of Escherichia coli were isolated from June 2018 to June 2019 from Kungang Hospital of Yunnan Province. The ESBLs producing strains were screened by double disk method and the sensitivity of ESBLs to antibiotics was detected by K-B method. The genotyping of ESBLs strains and the class I, II and III integrase genes in integron 3' and 5' conservative regions was conducted using polymerase chain reaction (PCR). Results The detection rate of ESBLs producing Escherichia coli was 53.19% (200/376). ESBLs-producing Escherichia coli had a low resistance rate to carbapenem antibacterial drugs with a relatively high sensitivity. The resistance rates to amikacin and piperacillin were 7.50% and 12.50% respectively, which had good antibacterial activity. The resistance rate to other antibiotics was high. There were 74.00% (148/ 200) of ESBLs-producing Escherichia coli carrying CTX-M gene, 34.50% (69/200) carrying TEM genes, and 1.00% (2/200) of strains carrying SHV genes. In addition, there were 9.00% (18/200) of strains carrying both CTX-M and TEM genes, and 0.50% (1/200) of strains carrying both CTX-M and SHV genes. The detection rate of integrons accounted for 37.00% (74/200), all of which were class I integrons. Conclusion The prevalence of ESBLs-producing Escherichia coli was relatively high in Anning, Yunnan. ESBLs-producing Escherichia coli showed a trend of multi-drug resistance. The genotype was mainly CTX-M, and the integron was classified as class I.
8.Effect of FABP4 DNA methylation on abnormal lipid metabolism in trophocyte treated with L-NAME

Objective To investigate the effect of fatty acid binding protein 4(FABP4)DNA methylation on abnormal lipid metabolism in placental trophoblastic dyslipidemia. Methods Human placental trophoblast cell line(HTR-8)was treated with L-NAME of 100 μmol/L for 48 h. The lipid content in placental trophoblasts was detected by chemical enzyme-colorimetry. The FABP4 DNA methylation level in placenta trophoblasts was detected by nested-touch down methylation specific PCR (NT-MSP). the mRNA and protein expression of DNMT1 and FABP4 were detected by qRT-PCR and Western Blot,respectively,in trophoblast cells. Results The lipid content in trophoblasts significantly increased as compared with the control(P < 0.05). Expression of FABP4 mRNA and protein increased(P < 0.05),while FABP4 methylation level and expression of DNMT1 significantly decreased (P<0.05)after treatment with L-NAME. Conclusions FABP4 DNA methylation is involved in the regulation of lipid metabolism in placental trophoblastic cells of hypertensive disorder complicating pregnancy.
9.A feasibility study on “Tri-Low” technology in combination with iterative model reconstruction (IMR) algorithm in CT angiography(CTA) of the head-and-neck vessels

Objective To evaluate the feasibility of low-tube-voltage, low injection rate, low contrast agent dosage in combination with iterative model reconstruction ( IMR) algorithm in CT angiography ( CTA) of the head-and-neck vessels .Methods Sixty patients who underwent CT angiography of the head-and-neck vessels were randomly divided into groups A and B with 30 cases in each group .Patients in group A received a conventional scan with 120 kVp and filterback projected ( FBP) reconstruction .Patients in group B received a low-dose scan with 80 kVp, and image reconstruction with FBP ( group B1) and IMR (group B2)algorithm.The contrast agent protocol were as follows: the injection time in all patients was 10s, the injection rate was 4.5-5.5 ml/s in group A while 3.5-4.0 ml/s in group B.The CT values of artery, image noise, signal to noise ratio ( SNR) and contrast to noise ratio ( CNR) were measured and compared among three groups with One-way ANOVA analysis . Image quality was evaluated by two radiologists with five scale method , and compared with Kruskal-Wallis test .The CT dose index volume (CTDIvol) and dose length product ( DLP) were recorded and compared between groups with two independent samples t-test.Results The image quality scores of groups A , B1and B2 were 3-5, 2-4 and 3 -5, respectively .Image quality of twelve patients in group B 1 couldn′t meet the diagnostic requirements but none in group A and B 2.The objective image parameters SNR and CNR for group B 2 were equal to group A ( P >0.05), while those for group B1 were lower than group A (t=13.39, 9.45, P<0.05) and group B2 (t=-12.14, -9.96,P<0.05).CTDIvol and DLP for group B were separately 80.9%, 81.3%lower than those of group A(t=39.1, 32.2,P<0.05).The injection rate and contrast agent volume for group B were separately 22.0%, 22.1% lower than those of group A ( t=20.8, 20.8, P<0.01) .Conclusions It is feasible in CT angiography of the head-and-neck vessels with lower tube-voltage, lower injection rate, lower contrast agent dose and combining with iterative model reconstruction algorithm.This protocol can reduce the radiation dose by 81.3% while maintaining image quality .Trial registration Chinese clinical trial registry ,ChiCTR-BOC-16010060.
10.Livedoid vasculopathy and its association with factor V Leiden mutation.

Livedoid vasculopathy is a rare chronic relapsing disorder characterised by recurrent painful thrombotic and vasculitic ulcers on the legs. We present the cases of two Indian women with livedoid vasculopathy that were found to be associated with an underlying factor V Leiden heterozygous mutation. There were no other thrombotic manifestations, and livedoid vasculopathy was the sole presenting feature of the factor V Leiden mutation, although this could also be coincidental. Initial treatment with high-dose immunosuppressive therapy was suboptimal, and the addition of pentoxifylline and antiplatelet therapy was crucial in achieving disease control and remission. These cases highlight the possible association with an underlying prothrombotic disorder, such as factor V Leiden mutation, in patients with livedoid vasculopathy. Although this association is relatively uncommon, it is more relevant to Indian patients, as the presence of factor V Leiden mutation is highest in this ethnicity as compared to the local Malay and Chinese populations.
