Objective: To explore the impacts of vitamin D deficiency on early carotid elasticity in patients with type 2 diabetes mellitus (T2DM) using ultrafast pulse wave velocity (UFPWV) technique. Methods: A total of 92 patients with T2DM were enrolled (T2DM group) and divided into vitamin D deficiency subgroup and vitamin D non-deficiency subgroup according to the level of 25-hydroxyvitamin D3 (25[OH]D3), while 1 544 normal adults were taken as controls(control group). UFPWV technique was used to measure carotid elasticity parameters, including pulse wave velocity at the beginning of systole (PWV-BS) and the end of systole (PWV-ES), and clinical data and laboratory indicators of both groups were collected. Then statistical analysis of the above data was performed. Results: ①Among T2DM group, the levels of body mass index (BMI), fasting blood glucose (FPG), systolic blood pressure, diastolic blood pressure, total cholesterol (TC), triglyceride, PWV-BS and PWV-ES of vitamin D deficiency subgroup were higher than those of non-deficient subgroup (all P<0.05), while the level of high density lipoprotein cholesterol (HDL-C) was lower than that of the vitamin D non-deficient subgroup (P<0.05). ②Pearson correlation analysis showed that PWV-BS and PWV-ES were positively correlated with age, BMI, FPG, systolic blood pressure, diastolic blood pressure and TC (all P<0.05) but negatively correlated with 25(OH)D3 and HDL-C (P<0.05). ③Multivariate linear stepwise regression analysis showed that FPG, systolic blood pressure, age and TC were risk factors for PWV-BS and PWV-ES (all P<0.05), whereas 25(OH)D3 was a protective factor for PWV-ES (P<0.05). Conclusion: PWV-BS and PWV-ES values in T2DM patients with vitamin D deficiency significantly elevated. Vitamin D may be a protective factor for carotid elasticity in patients with T2DM. Monitoring vitamins level in T2DM patients is promptly supplemented. Vitamin D may delay the development of T2DM complications.

ObjectiveTo summarize the experience on laparoscopic common bile duct exploration(LCDE). MethodsFrom Mar. 1992 to Mar. 2003, there were 693 cases undergoing LCDE. ResultsThe procedure was successful in 373 out of 391 cases with primary closure of duct incision and 272 out of 302 cases of T tube placement. Membrane stenosis dilation was successful in 68 out of 74 cases. Eleven out of 15 cases of malignant stenosis was successfully treated by biliary endoprosthesis and stent without bile leakage. Seven cases were shifted to open CBD exploration. Bile leakage was cured conservatively in 24 cases. Endoscopy failed to totally remove residual stones in 11 cases. Four cases died postoperatively. Conclusion LCDE was safe and effective in the hands of skilled endoscopists.

Objective To evaluate the efficacy, safety and compatibility of a new type of atrial septal defect ( ASD) occluder in atrial spetal defect mini-pigs model.Methods Five Tibet mini-pigs were selected as the ASD models which were established by the combination of atrial septal puncture and balloon dilation.Then the new type occluder was implanted for the therapy of ASD.Transthoracic echocardiography with color Doppler was used in all animals during closure and in follow up examinations.The animals were killed at 3 months after occlusion for electron microscopical observation and microscopic examination.Result The ASD models had been created in five piglets successfully without complication, all of whom were implanted successfully with the new device without shunts.Postmortem and microscopic examination of the 5 specimens 3 months after placement showed complete.Conclusion Transcatheter ASD occlusion with new type ASD occluder is safe,feasible and effective.This occlusion can repair the atrial septal defect successfully.

Objective:Observed activity of adjuvant arthritis ( AA) rats Hedgehog signaling pathway and explore the effect of Cyclopamine on rat arthritis and kidney injury.Methods:40 rats were randomly divided into control group,Cyclopamine group,AA+Cyclopamine group and AA model group.We used Freund′s complete adjuvant rat model with Cyclopamine intraperitoneal injection.By measuring paw swelling, systemic inflammation and arthritis semi-quantitative assessment methods of rats to evaluate the model.HE staining was used to detect the renal pathological changes of rats.Western blot was used to detect kidney Gli1 protein expression levels of the rats.Immunohistochemical staining was used to detect the rat kidney TNF-α,IFN-γ,IL-6 expression.Results:After using Cyclo-pamine,the AA rat paw swelling reduced and arthritis refers relieved significantly.Compared with the control group,Cr,BUN and organ coefficient increased significantly in AA model group, and the difference was statistically significant.Meanwhile renal TEM detection appeared obvious pathological changes in rats of AA model group.After using cyclopamine,the content of Cr,BUN and organ coefficient change reduced significantly, so did the pathological.Western blot detected kidney tissue Gli1 protein in each group.Compared to control group,there was no significant difference in Cyclopamine group.AA model group and AA+Cyclopamine group Gli1 protein expression was significantly higher compared with Cyclopamine Gli1 protein group,the difference was statistically significant.AA model group and AA Cyclopamine group Gli1 protein expression was significantly higher,the difference was statistically significant.Compared to the AA model group,AA+Cyclopamine group Gli1 protein levels have decreased significantly,the difference was statistically signifi-cant.Immunohistochemical assay detection found that kidney tissue proinflammatory cytokines TNF-α,IFN-γ,IL-6 expression and semi-quantitative score changed.Compared with the control group, TNF-α, IFN-γexpression of AA model group were significantly increased.After using Cyclopamine,the expression of TNF-α,IFN-γin kidney of AA model reduced significantly.IL-6 expression in AA model group was significantly higher than the control group.Conclusion:Cyclopamine AA can relieve arthritis and kidney injury in rats with arthritis AA,the Hh pathway was on activity state in the process,may altered expression of inflammatory factors.

Objective To investigate the drug resistance characteristics and epidemic trend of Escherichia coli in Anning, Yunnan, and to provide a reference for clinical rational use of antibiotics. Methods A total of 376 strains of Escherichia coli were isolated from June 2018 to June 2019 from Kungang Hospital of Yunnan Province. The ESBLs producing strains were screened by double disk method and the sensitivity of ESBLs to antibiotics was detected by K-B method. The genotyping of ESBLs strains and the class I, II and III integrase genes in integron 3^' and 5^' conservative regions was conducted using polymerase chain reaction (PCR). Results The detection rate of ESBLs producing Escherichia coli was 53.19% (200/376). ESBLs-producing Escherichia coli had a low resistance rate to carbapenem antibacterial drugs with a relatively high sensitivity. The resistance rates to amikacin and piperacillin were 7.50% and 12.50% respectively, which had good antibacterial activity. The resistance rate to other antibiotics was high. There were 74.00% (148/ 200) of ESBLs-producing Escherichia coli carrying CTX-M gene, 34.50% (69/200) carrying TEM genes, and 1.00% (2/200) of strains carrying SHV genes. In addition, there were 9.00% (18/200) of strains carrying both CTX-M and TEM genes, and 0.50% (1/200) of strains carrying both CTX-M and SHV genes. The detection rate of integrons accounted for 37.00% (74/200), all of which were class I integrons. Conclusion The prevalence of ESBLs-producing Escherichia coli was relatively high in Anning, Yunnan. ESBLs-producing Escherichia coli showed a trend of multi-drug resistance. The genotype was mainly CTX-M, and the integron was classified as class I.

OBJECTIVETo discuss the relationship between the onset of peptic ulcers (PU) and meteorological factors (MF).

METHODSIn reviewing records from 17 hospitals in the city of Nanning from 1992 to 1997, we found 24, 252 cases of PU in 104, 121 samples of gastroscopic examinations. We then calculated the detectable rate of PU (DRPU) during each season every five days (FD) and made a correlated analysis with the seasonal MF during the same period in Nanning. Finally, we made a multiple regressive correlated analysis of DRPU and the 5MF for the same period of the year. A forecast model based on the MF of the previous FD was established. The real value and the forecast value was being tested and verified.

RESULTSFrom 1992 to 1997, the DRPU is: winter and spring > summer and autumn (P < 0.005). There is a close relationship between the DRPU and the average temperature (AT), the average highest temperature (AHT), the average lowest temperature (ALT), average air pressure (AAP) and the average dew point temperature (ADT) of the five days of the same period of the year (the correlated coefficients are -0.5348, -05167, -0.5384, 0.4579 and -0.4936, respectively), with P < 0.01. The AT, AHT, ALT, AAP and ADT of the previous FD are of great value in forecasting the onset of PU, with its real value and forecast value corresponding to 66.6%.

CONCLUSIONSThere exists a close relationship between DRPU and the AT, AHT, ALT, AAP and ADT of the FD for the same period. A mid-term medical meteorological forecast of the onset of PU can be made more accurately and reliably according to the close relation between the DRPU and some MF of the previous FD.

Objective:To identify the key convergence points in the medical-nursing combined care among literature, and establish a mechanism for medical-nursing combined care integration from the perspective of the synergy theory, for the purpose of promoting the integrated development of such care in China.Methods:Relevant literature on the medical-nursing combined care were retrieved from the CNKI database from September 2013 to September 2021. The search formula used was TI= "the medical-nursing care integration" AND SU=(convergence+ transformation+ coordination+ mechanism), while supplementary searches were made using " pkulaw.com database" and Baidu Scholar database. The method of literature content analysis was used to screen key points of the medical-nursing combined care, and the convergence mechanism was built based on the perspective of the synergy theory.Results:A total of 42 literatures were included in this study, 8 key convergence points of medica-nursing combined care integration were identified. Namely the 4 key horizontal convergence points of medical care, aging care, rehabilitation and nursing, the 3 key vertical convergence points of home care, institution care and community care, and the one key convergence point of institutional medical care. Based on the synergy theory and the 8 key convergence points, the " 431" convergence mechanism of medical-nursing combined care was constructed. This mechanism refered to the horizontal linkage of healthcare, nursing, rehabilitation and aging care, the vertical redirection among home care, institution care and community care modes, and the development mode focusing on institutional aging care.Conclusions:At present, the poor convergence between aging care and nursing care in China is a key roadblock hindering the integrated development of aging and nursing care. Medical-nursing combined care should be conducive to the " 431" convergence development, achieving horizontal linkage, vertical redirection, and resource sharing, for the purpose of high-quality development of China′s aging care system.

Objective To analyze the relationship of serum S100 calcium binding protein A4(S100A4)and pentraxin-3(PTX3)levels with left atrial appendage thrombosis in patients with NVAF.Methods A total of 120 elderly NVAF patients treated in our hospital from March 2020 to March 2023 were enrolled in this study.According to their echocardiograms,they were divided into a left atrial appendage thrombosis group(40 cases)and a non-thrombosis group(80 cases).Serum S100A4 and PTX3 levels were detected.Spearman correlation analysis was applied to ana-lyze the relationship between serum S100A4 and PTX3 levels and left atrial appendage thrombo-sis.Logistic regression analysis was conducted to analyze the factors affecting left atrial appendage thrombosis.Results The serum levels of S100A4 and PTX3 were higher in the thrombosis group than the non-thrombosis group(P＜0.01).The serum levels of S100A4 and PTX3 were positively correlated with left atrial appendage thrombosis(r=0.497,P=0.000;r=0.555,P=0.000).Heart failure,CHA2DS2-VASc score,B-type natriuretic peptide,uric acid,S100A4 and PTX3 were risk factors for left atrial appendage thrombosis in NVAF patients(P＜0.05,P＜0.01).Combination of serum S100A4 and PTX3 in predicting left atrial appendage thrombosis formation in NVAF patients had an AUC value of of 0.949(95％CI:0.893-0.981).Conclusion Serum S100A4 and PTX3 levels are increased in NVAF patients,they are related to left atrial appendage thrombosis,and their serum levels have certain predictive value for left atrial appendage thrombosis.

ObjectiveTo investigate the clinical features and risk factors of primary biliary cholangitis （PBC） comorbid with metabolic associated fatty liver disease （MAFLD） and the interaction between the two diseases. MethodsA total of 187 patients who were diagnosed with PBC， MAFLD， or PBC with MAFLD in The First Affiliated Hospital of Kunming Medical University from January 2019 to December 2022 were enrolled and divided into PBC group with 70 patients， PBC+MAFLD group with 38 patients， and MAFLD group with 79 patients. Related data were collected， including general information， clinical symptoms， serological parameters， transient elastography （FibroScan）， and non-invasive fibrosis markers， which were compared between the three groups. A one-way analysis of variance or the Kruskal-Wallis H test was used for comparison of continuous data between groups， the chi-square test or the Fisher’s exact test was used for comparison of categorical data between groups， and the binary Logistic regression analysis was used for multivariate analysis. ResultsThere were significant differences between the three groups in sex， age， height， weight， body mass index （BMI）， and history of autoimmune diseases （P<0.05）. In the PBC+MAFLD group， female patients accounted for 89.5%， with a mean age of 57.26±12.72 years and a BMI of 23.35±3.70 kg/m²， and in the PBC group， the detection rate of autoimmune diseases was 25.7% （18 patients）. There were significant differences between the three groups in the incidence rates of weakness， poor appetite， pruritus， jaundice， varices， ascites， and splenomegaly （all P<0.05）. The PBC+MAFLD group had the common symptoms of weakness in 18 patients （47.4%）， poor appetite in 15 patients （39.5%）， abdominal pain in 14 patients （36.8%）， and abdominal distension in 16 patients （42.1%）； the MAFLD group had the common symptoms of abdominal pain in 34 patients （43%） and abdominal distension in 32 patients （40.5%）； the PBC group had the common symptoms of weakness in 37 patients （52.9%）， poor appetite in 25 patients （35.7%）， jaundice in 25 patients （35.7%）， abdominal pain in 18 patients （25.7%）， abdominal distension in 25 patients （35.7%）， varices in 19 patients （27.9%）， ascites in 23 patients （32.9%）， and splenomegaly in 44 patients （62.9%）. The PBC+MAFLD group had a controlled attenuation parameter （CAP）， which was higher than that of the PBC group， and the PBC group had significantly higher levels of liver stiffness measurement， aspartate aminotransferase-to-platelet ratio index （APRI）， and fibrosis-4 （FIB-4） than the MAFLD group （all P<0.05）. The factors without multicollinearity were included in the regression analysis， and with the PBC group as the reference group， FIB-4 （odds ratio ［OR］=0.218， 95% confidence interval ［CI］： 0.069 ‍—‍ 0.633， P<0.05） and history of autoimmune diseases （OR=0.229， 95%CI： 0.067‍ — ‍0.810， P<0.05） were influencing factors for the onset of PBC with MAFLD； with the MAFLD group as the reference group， ALT （OR=0.157， 95%CI： 0.025‍ —‍ ‍1.000， P<0.05） and TBil （OR=0.995， 95%CI： 0.990‍‍ — ‍0.999， P<0.05） were influencing factors for the onset of PBC with MAFLD. ConclusionPBC with MAFLD lacks specific clinical manifestations， and PBC patients tend to have more severe clinical manifestations and a higher incidence rate of liver function decompensation. PBC comorbid with MAFLD may not aggravate the disease progression of PBC.

Objective:To evaluate the efficacy and safety of lactulose combined with polyethylene glycol for bowel preparation before colonoscopy in patients of different risks.Methods:A total of 208 patients undergoing colonoscopy were enrolled, including 108 high-risk and 100 low-risk patients. The high-risk patients were divided into group A (54 taking lactulose + polyethylene glycol) and group B (54 taking polyethylene glycol), and the low-risk patients were divided into group C (49 taking lactulose + polyethylene glycol) and group D (51 taking polyethylene glycol). The Boston bowel preparation score, cecal intubation time, withdrawal time, the detection rate of colonic polyps and adenoma, and the incidence of adverse reactions were observed.Results:Among the high-risk patients, the Boston bowel preparation score and adenoma detection rate in group A [(6.35±1.15) scores, 46.3%] were significantly higher than those in group B [(5.76±0.89) scores, 22.2%, both P<0.05], and the first defecation interval in group A was significantly shorter than that in group B [(1.20±0.85) h VS (3.29 ± 2.93) h, P<0.05]. There was no significant difference in adequate bowel preparation rate, polyp detection rate, frequency of defecation or incidence of adverse reactions between group A and B. In the low-risk patients, the first defecation interval in group C was significantly shorter than that in group D [(1.65 ± 1.35) h VS (3.42 ± 2.64) h, P<0.05], and the incidence of adverse reactions was significantly lower than that in group D (44.9% VS 64.7%, P<0.05). There was no significant difference in adequate bowel preparation rate, Boston bowel preparation score, adenoma detection rate, polyp detection rate or frequency of defecation between group C and D. Conclusion:For the high-risk patients, the effect of lactulose combined with polyethylene glycol for bowel cleansing is better than that of traditional polyethylene glycol in the improvement of the Boston bowel preparation score, adenoma detection rate, and the first defecation interval. For low-risk patients, lactulose combined with polyethylene glycol regimen has few advantages over traditional polyethylene glycol regimen.
