1.Investigation on nutritional status and blood glucose control level in elderly patients with type 2 diabetes

Objective To investigate the nutritional status and glycemic control level in elderly patients with type 2 diabetes in urban community of Yangpu district in Shanghai.Methods The 24-hour dietary recall method for 3 consecutive days was used to investigate the dietary nutrients intake in 400 patients with type 2 diabetes.Results The ratio of patients with blood glucose level less than 6.1mmol/L was 23.3% (93/400).The energy intake in patients with type 2 diabetes was low,and only reached 62.81% of the recommended standard of energy intake for adults.The energy ratio of three diet nutrients (protein,fat and carbohydrate) were 14.41%,33.93%,51.66% respectively,which showed that the energy ratio of fat was relatively high.The energy ratio of breakfast,lunch and supper dinner were 23.37%,38.05% and 38.57% respectively.The average intake of vitamin E,iron,niacin,manganese,phosphorus and cholesterol in elderly patients was more than the recommended standard of adult diet nutrients,and reached 149.6%,138.1%,137.1%,132.5%,127.5% and 122.0% of the recommended standard respectively.But the average intakes of protein,fat,carbohydrate,dietary fiber,calcium,zinc,copper,magnesium,potassium,vitamin A,vitamin B1,vitamin B2 and vitamin C in elderly patients were lower than the recommended standard,and reached 77.9%,90.3%,57.1%,50.7%,49.3%,81.4%,75.7%,75.7%,77.5%,77.0%,55.1%,58.1% and 67.6% of the recommended standard respectively.Conclusions The status of dietary nutrients intake in elderly patients with type 2 diabetes in Yangpu district of Shanghai is unreasonable,the intakes of dietary fiber,calcium,vitamin B1,vitamin B2 etc.were insufficient,and reasonable diet instruction should be provided for the elderly patients.

Objective To compare the characteristics and abnormity of lipid metabolism among three groups of population in Yangpu.Methods The data of blood lipids of three subpopulations were collected by epidemiologic investigation and experimental examination.Results The abnormal rates of total cholesterol and triglycerides were 50.85% and 56.91% in patients with coronary heart diseases;41.28% and 46.82% in patients with diabetes,23.58% and 32.46% in staffs of enterprises.The abnormal rates of total cholesterol and triglycerides were 43.55% and 51.75% in over-weight subjects,and all of them were higher than those in subjects with normal body mass index.The abnormal rate of triglycerides was significant higher in subjects with abnormal ratio of waist-hip(46.35%) than that with normal ratio(39.77%,P

AIM: To study the effects of paecilomyces cicadidae total polysaccharides on the immune function and free radical scavenging of tissues in immunosuppressed rats.METHODS: The activities of lactate dehydrogenase(LDH) and arginase(Arg) in liver,kidney,spleen,thymus of immunosuppressed rats were detected by automatic biochemistry analyzer.The content of lipid peroxide(LPO) and the reduced glutathione(GSH) level in liver,kidney,spleen,thymus of immunosuppressed rats were observed.RESULTS: The activity of the LDH,Arg and GSH level were significantly increased,and the LPO content were significantly declined in liver,kidney,spleen,thymus of immunosuppressed rats induced by cyclophosphamide.CONCLUSION: Paecilomyces cicadidae total polysaccharides promote immune function and the ability of free radical scavenging of tissues in immunosuppressed rats.
5.Early diagnostic indices for melioidosis:report of 49 cases

Objective To study the early diagnostic indices of melioidosis by analyzing 49 cases. Methods Unconditional logistic regression analysis was used to analyze the clinical data of 49 cases of melioidosis and 98 cases of bacterial pneumonia who were admitted in our hospital and Peoples' Hospital of Hainan Province from December 1996 to December 2007. Their social characteristic,background diseases,clinical manifestations,laboratory findings,radiologic examination and complications were studied. Results Background disease (such as diabetes),hepatosplenomegaly,septic shock,sepsis et al were the main causes of melioidosis. Conclusion Chills,fever,hepatosplenomegaly,diabetes mellitus,septic shock and sepsis are all factors that should be considered with melioidosis.
6.Using Six Sigma Method to Reduce the Defects in Drug Delivery

OBJECTIVE:To improve the drug delivery procedures and reduce the defects occurred in drug delivery.METHODS:The five procedures of six sigma including defining,measuring,analyzing,improving and controlling were applied to improve the drug delivery in our hospital's single dose dispensing(UDD)center.RESULTS:Due to the application of the six sigma method,the defect rate of drug delivery dropped from 25.1%(before the initiation of the six sigma method)to 5%(after the initiation of the six sigma method),which was significant lower as compared before the initiation of the six sigma method.CONCLUSION:Six sigma method contributed to the reduction of defect rate of drug delivery,so it deserves to be widely used in each management link in UDD center so as to improve the quality of dispensing.
7.Analysis of clinical characteristics of 26 patients with melioiclosis spleen abscesses in Hainan province

Objective To analyze the clinical features of melioidosis spleen abscess.Methods A retrospective analysis of 26 cases with melioidosis spleen abscess at Hainan People′s Hospital between October 2002 and March 2015 was performed.Data with general conditions, clinical characteristics, imaging features, treatment and outcome were all extracted and analyzed.Results High fever (100.0%) and splenomegaly (73.1%) were the main clinical symptoms and signs.Laboratory examinations showed that white blood cell and neutrophil percentage increased, with a mean value of 12.6×10.9/L and 0.86, respectively.88.5% cases had diabetes mellitus which was the main underlying disease.Apart from spleen abscess, pulmonary infection (73.1%) and liver abscess (38.5%) were the majority.Imaging showed multiple small abscess (92.3%).All patients had at least one positive blood culture of Burkholderia pseudomalleiand were treated with imipenem, ceftazidime and sulfamethoxazole alone or in combination.Hospitalization days ranged between 8 to 65 days with the mean of 29.5 days.Twenty-three cases (88.5%) were cured, the remaining 3 cases (11.5%) abandoned treatment and were all lost for follow-up.Conclusions Melioidosis should be suspected in patients from epidemic areas, who have diabetes and imaging shows spleen abscess and other organ infections including pulmonary infection and liver abscess.Microbiological culture is needed immediately.
8.Protective effects of extract of Ginkgo biloba on diaphragm of diabetic rats

AIM: To investigate the effects of extract of Ginkgo biloba (EGb) on diaphragm from diabetic rats. METHODS: Sprague-Dauley rats were divided into three groups: normal control, diabetic group and EGb treatment group. The morphologic changes of diaphragm tissues were studied by light and electron microscopy, the activities of succinate dehydrogenase (SDH), superoxide dismutase (SOD), nitric oxide synthase (NOS) and contents of malondialdehyde (MDA), nitric oxide (NO_2~-/NO_3~-) in the diaphragm mitochondria were assayed by spectophotometer, respectively. RESULTS: The activities of SOD, SDH decreased in diabetic diaphragm mitochondria, but the activitiy of NOS, the contents of NO_2~-/NO_3~-, MDA increased compared with control group. The activities of SOD, SDH were increased as well as NOS were decreased and the contents of NO_2~-/NO_3~-, MDA decreased in EGb treatment group compared with the diabetic group. CONCLUSION: EGb may protects the diaphragm mitochondria of diabetic rats by enhancing the function of respiratory chain, anti-oxidation and decreasing NO level. [

Objective:To study the effects of soybean isoflavone (SI) on metabolism of lipids and lipoprotein (a) [Lp(a)] in ovariectomized rats . Methods:Senventy female Sprague- Dawley (SD) rats were randomly divided into 7 groups according to the levels of total cholesterol (TC) in serum: high-lipid (HF) group, estrogen (EG) group, low-dose SI (L-SI) group, middle-dose SI (M-SI) group, high-dose SI (H-SI) group, sham group and normal control (NC) group. One week after bilateral ovaries were extirpated, except sham and NC group, estrogen, different doses of SI or deionized water were fed i.g. for 12 w. Except NC group, the other groups were fed high fat diet. Body weights were weighed every week and blood and heart were collected at the end of experiment. The serum TC, triglyceride(TG), high or low density lipoprotein-cholesterol (HDL-C, LDL-C), Lp (a), and antioxidative enzymes activities were assayed. Results:After SI intervention, the levels of LDL-C in SI groups were significantly lower than in HF group but higher than in NC group. The levels of Lp (a) were also changed, but there was almost no effect on HDL-C. Persistent intervention with SI can reduce TC, TG , and protect cadiocyte’s actin filament andmitochondrial ultramicrostructure from damage as shown in HF group. The ultramicroscopic pictures in EG and H-SI group almost resembled NC group. Conclusion:Persistent intervention of high-dose SI can reduce the levels of LDL-C, TC, TG and protect the myocardiac damages due to high fat diet in ovariectomized rats.
