1.Effects of corneal thickness and endothelial cells in patients with macular edema after intravitreal conbercept

AIM: To study the effect of intravitreal conbercept on corneal thickness and endothelial cells in macular edema patients.
METHODS:Intravitreal conbercept was performed on 30 eyes of 30 patients with macular edema due to different diseases from Jan. 2017 to Dec. 2017 in our hospital. The central corneal thickness was identified by ultrasonic biomicroscope(UBM)before and after surgery. Specular microscopy was also used to detect the density of central corneal endothelial cells and proportion of hexagonal cells before and after surgery.
RESULTS:The central corneal thickness were 551.68±12.80μm before surgery, 552.06±13.22μm at the first day after surgery and 552.49±13.83μm at 1wk after surgery(P>0.05). The density of central corneal were 2551.03±287.55/mm2 before surgery, and 2563.79±292.34, 2543.32±282.41, 2526.18±280.24, 2519.60±279.89/mm2 at 1d, 1wk, 3mo and 6mo after surgery respectively, meanwhile, the proportion of hexagonal cells were(50.23±7.51)% before surgery, and(50.93±8.23)%,(50.60±7.91)%,(50.40±7.50)%,(50.93±8.19)% at 1d, 1wk, 3mo and 6mo after surgery respectively(P>0.05).
CONCLUSION: Intravitreal injections of conbercept demonstrated a safety in the treatment of macular edema. It seems no affects on corneal thickness and corneal endothelial cells at 6mo after intravitreal conbercept.
2. Incidence and yrend of occupational pneumoconiosis in Ankang city from 1974 to 2017

Objective:
To understand the epidemiological feature and prevalent trend of pneumoconiosis in Ankang City, pertinently put forward the prevention and control strategy, and provide the basis for comprehensive prevention and treatment of pneumoconiosis.
Methods:
China Information System for Disease Control and Prevention has exported the network report data of "occupational diseases and occupational health information monitoring" from January 1, 2006 to December 312017 and has created the database; has investigated and supplemented the occupational disease reporting results from January 1, 1974 to December 31, 2005; has conducted the data cleansing through SPSS17.00; has conducted the statistical description and analysis according to pneumoconiosis diagnosis time, age, sex, length of service, industry, and etc.
Results:
From 1974 to 2017, Ankang City has accumulatively reported 1047 cases of various occupational pneumoconiosis; of which, 992 new cases and 55 stage development cases. Entities of pneumoconiosis include silicosis, coal workers pneumoconiosis, electric arc welders pneumoconiosis, cement pneumoconiosis and asbestosis; number of new cases of silicosis ranks first, the second is coal workers pneumoconiosis. Average length of employment of the occurrence of pneumoconiosis is (15.01±8.95) years; average length of age of the occurrence is (47.11±7.86) years. Distribution of pneumoconiosis involves 8 industries; the first is mining industry (54.23%) , the second is construction industry (27.52%) , and the third is public management, social security and social organization (12.70%) . Pneumoconiosis involves 24 types of work; the first is drillman (47.28%) , the second is main coal miners (15.52%) , and the third is blasters (13.31%) .
Conclusion
Prevention and treatment of pneumoconiosis in Ankang City gives priority to silicosis and coal workers pneumoconiosis, puts emphasis on strengthening the prevention of pneumoconiosis of drillman in the mining industry; the occurrence of pneumoconiosis is in a linear upward trend from 2012.
3.Comparison of Criminal Characteristics in Depression Patients and Schizophrenics with Homicide Behavior.

OBJECTIVES:
To explore the criminal characteristics of forensic psychiatry expertise in depression patients and schizophrenics with homicide behavior.
METHODS:
A total of 40 depression (depressive episode) patients and 50 schizophrenics with homicide behavior were randomly assigned into the study group and control group, respectively. Data of demographic and criminal characteristic of the two groups were collected by a self-designed questionnaire, and then were compared.
RESULTS:
There were no statistical differences in age, education level and career between study and control groups (P>0.05). Compared with the control group, the victims in the study group were mainly the patient's children and parents, and most offenders had suicidal behavior after homicide (70%). In study group, the motives of crime were mainly extended suicide and indirect suicide, and most offenders had attempted suicide (85%) and diminished capacity of criminal responsibility (70%), which in control group had no capacity of criminal responsibility (56%). Except for criminal site, there were statistical differences in other criminal characteristics between two groups (P<0.05).
CONCLUSIONS
There are different criminal characteristics between depression patients and schizophrenics with homicide behavior in forensic psychiatry, and these characteristics should be considered when these two diagnoses are distinguished in forensic psychiatry expertise.

A case of 13-year-old female with three-pairs/six-lines of labial frenulums and thumb and forefinger's deformity was reported. The frenulums of inferior lip were bigger than frenulum of superior lip. 5.Clinical research of T tube implantation on children with chronic otitis media.

OBJECTIVE:
To study the appropriate time to take out T tube, long-term effect and adverse consequence of children with chronic otitis media subjected to adenoidectomy and tonsillectomy plus tympanostomy.
METHOD:
One hundred and eighteen children with 226 ears operated were analyzed and summarized.
RESULT:
T tubes taken out at 1, 3, 6 and 12 month were 6, 102, 88 and 23 ears, respectively, and T-tubes were not taken out in 7 ears.
CONCLUSION
At present, the treatment is curable and reliable with fewer complications. The appropriate time to take out the T-tube should be 3 months after the operation.

Taking microneurosurgery approach and applied surgical anatomy training as the core and combining theoretical teaching and perioperative training as the main contents , training program achieved significant effect among specialty degree neurosurgery postgraduates. In order to further improve the quality of training, it is proposed to set up micro-neurosurgery training center and more complete train-ing system based on micro-neurosurgery contents thus to improve clinical ability of specialty degree neuro-surgery postgraduates.
7.Effect of triptolide on fibrosis in a mouse model of cerulean-induced chronic pancreatitis

ObjectiveTo investigate the effect of triptolide on fibrosis in mice with cerulein-induced chronic pancreatitis (CP) based on the nuclear factor-kappa B (NF-κB)/interleukin-6 (IL-6) signaling pathway. MethodsA total of 15 male C57BL/6 mice were randomly divided into control group (treated with normal saline), cerulein group (model mice with cerulein-induced CP), and triptolide group (induced by cerulean and treated with triptolide), with 5 mice in each group. Samples were collected for detection after 6 weeks. The weight of the pancreas was measured; HE staining, picrosirius red staining, and Masson staining were used to observe pancreatic histomorphology and collagen deposition; ELISA was used to measure the expression of IL-6 in serum; immunohistochemistry was used to measure the expression of alpha-smooth muscle actin (α-SMA) and NF-κB/p65; Western blot was used to measure the expression of NF-κB/p65, IL-6, and α-SMA. A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups. ResultsThere were significant differences between the three groups in the weight of the pancreas, IL-6 concentration, pathological score, picrosirius red staining results, Masson staining results, and mean optical density of positive α-SMA signal and positive NF-κB/p65 signal(F=64.87, 15.85, 145.33, 141.80, 121.77, 250.22, and 69.22, all P<0.001). Compared with the cerulein group, the triptolide group had a significant increase in the weight of the pancreas and significant reductions in the expression of IL-6, pancreatic fibrosis, collagen deposition, and the expression of α-SMA and NF-κB/p65 (all P<0.05). Western blotting showed that there were significant differences in the expression of NF-κB/p65, IL-6, and α-SMA between the three groups (F=8.86, 6.74, and 16.23, all P<0.05), and compared with the cerulein group, the triptolide group had significantly lower expression of NF-κB/p65, IL-6, and α-SMA in the pancreatic tissue (all P<0.05). ConclusionTriptolide alleviates fibrosis in mice with cerulein-induced CP and inhibit the protein expression of NF-κB/p65 and IL-6.
8. Difference in expression of lymphatic markers D2-40 and Prox1 between angiokeratoma corporis diffusum and angioma serpiginosum

Objective:
To investigate the significance of lymphatic markers in the differential diagnosis of angiokeratoma corporis diffusum (ACD) and angioma serpiginosum (AS) .
Methods:
Totally, 9 patients with ACD and 6 with AS were enrolled from Department of Dermatology, Xijing Hospital between 2006 and 2017, and their clinical and histopathological features were retrospectively analyzed. Skin sections from all the patients were stained for CD31, D2-40 and Prox1.
Results:
In the 9 patients with ACD, abnormal vessels were weakly positive or negative for CD31, and positive for Prox1. Endothelial cells in abnormal vessels were locally positive for D2-40 in 4 patients with ACD, but negative for D2-40 in the other 5 patients. In the 6 patients with AS, the endothelia of hyperplastic small vessels were positive for CD31, but negative for D2-40 and Prox1.
Conclusion
The clinical features of a few patients with ACD are similar to those of patients with AS, and lymphatic markers have definite significance in the differential diagnosis of the two diseases.
9.Preliminary study on the effect of Pinggan huoxue powder on cerebral ischemia-reperfusion injury in rats and its mechanism

OBJECTIVE To investigate the effect and mechanism of Pinggan huoxue powder on cerebral ischemia-reperfusion injury (CIRI) in rats. METHODS CIRI model was induced by suture-occluded method. SD rats were divided into sham operation group, model group, positive control group (Tongxinluo capsule 0.325 g/kg), Pinggan huoxue powder low-dose, medium-dose and high-dose groups (3.02, 6.04, 12.08 g/kg), with 12 rats in each group. Each group was given constant volume of water or drug intragastrically, once a day, for consecutive 14 d. Longa score was used to evaluate neurological function of rats on the 1st, 7th and 14th day of intervention. After the last administration, the percentage of cerebral infarction volume in rats was measured, and neuronal injury (neuronal density) and neuronal apoptosis were observed; the protein expressions of p53, B cell lymphoma-2 (Bcl-2), Bcl-2 associated X protein (Bax), Caspase-3 and activated Caspase-3 (Cleaved caspase-3) in brain tissue were detected. RESULTS Compared with sham operation group, Longa score at the 1st, 7th and 14th day of intervention and the percentage of cerebral infarction volume in model group significantly increased (P<0.05), and the arrangement of cortical cells in ischemic side was disordered, Nissl bodies were absent and vacuolated, and a large number of cell apoptosis occurred. The neuronal density and protein expression of Bcl-2 at the lesion side were significantly decreased (P<0.05), while the protein expressions of p53, Bax, Caspase-3 and Cleaved Caspase-3 were significantly increased (P<0.05 or P<0.01). Compared with model group, the expressions of Bcl-2 in cerebral ischemia cortex of rats in Pinggan huoxue powder high-dose group were significantly increased (P<0.05). Longa scores of rats in Pinggan huoxue powder medium-dose and high-dose groups on the 7th and 14th day of intervention were significantly decreased (P<0.05), and other indexes were also significantly reversed (P<0.05 or P<0.01). CONCLUSIONS Pinggan huoxue powder can protect cerebral nerves and inhibit apoptosis of CIRI model rats, the mechanism of which may be related to down-regulating the protein expressions of p53, Bax and Caspase-3 and up-regulating the protein expression of Bcl-2.
10.The application of evaluation tools for criminal responsibility in forensic psychiatric expertise.

Criminal responsibility is divided into three types: full criminal responsibility, diminished criminal responsibility and criminal irresponsibility in China. In forensic psychiatric expertise, doctors often have different opinions about the responsibility in a given case because of lacking objective criteria. The evaluation of criminal responsibility is always unresolved problem in forensic psychiatric expertise. Application of these evaluation tools in forensic psychiatric expertise were reviewed in this article. The value of the tools were still controversial in the reliability and validity, but it is clear that these tools have the positive roles in ensuring the standardization and the uniformity of the forensic investigation.
