1.Diagnostic Significance of the Coronal Sectional Image of Computerized Tomography for the Renal Diseases.

The coronal sectional image of computerized tomography creates an image that is similar to an excretory urogram. It also provides more clearly delineated renal image and image of adjacent organs than an excretory urogram. Further more, the number, extent, and content of renal mats lesions used to discriminate density difference can be determined with relatively great accuracy. This image of computerized tomography is useful to understand and make a diagnosis of renal disease. Coronal sectional image of computerized tomography is more informative in evaluation of retroperitoneum space than conventional radiography. Recently we observed several cases of renal disease-renal cell ca, simple renal cyst, angiomyolipoma, retroperitoneal teratoma, pheochromocytoma, hydronephrosis which could be detected with coronal sectional image of computerized tomography.

One case of renal angiomyolipoma is reported in a 33 year-old house wife with right flank mass and pain, who was non-tuberous sclerosis, patient Diagnosis was made postoperately. This rare lesion posed a difficult differentiation from renal cell carcinoma. On the histologic examination, our case was found type 1 according to Smith's classification. The clinical, radiological and pathologic features are discussed.

4.Renal Angiomyolipoma: Report of 6 cases and Review of the Literature.

During the last 9 years, 6 patients of renal angiomyolipoma were managed at Catholic University Medical College. All were unilateral and were not associated with tuberous sclerosis. Preoperative diagnosis were hypernephroma (4), angiomyolipoma and renal stone. All patients were treated with nephrectomy. 17 patients were reported in Korean literature previously. In the review of the 26 cases (23 pts.), 16 cases were diagnosed as angiomyolipoma preoperatively and other as hypernephroma(9) and renal stone(1). Almost all cases(22) were managed with nephrectomy and only 4 cases were managed with conservative treatment (arterial embolization or observation).

Angiomyolipoma is a rare benign tumor and approximately 20 to 50% of patients with the diagnosis of angiomyolipoma have some or all of the other stigma of tuberous sclerosis. Patients with tuberous sclerosis tend to be younger, more likely to have multifocal and bilateral disease. Angiomyolipoma which is not associated with tuberous sclerosis mostly affects middle aged women. According to literature data, angiomyolipoma is very rare in child and almost exclusively associated with tuberous sclerosis. We report a case of 14 year-old girl who has a renal angiomyolipoma without clinical evidence of tuberous sclerosis.

6.Anal Myolipoma: A New Benign Entity in Patients with an Anal Tumor?.

A myolipoma is an extremely rare benign neoplasm, occurring most frequently in adults in the deep soft tissue of the abdomen or retroperitoneum. We experienced a case of an anal myolipoma occurring in a 30-year-old woman, and it was surgically resected. To our knowledge, this is the first reported case of a myolipoma arising from the anus, so such a possibility needs to be considered in the differential diagnosis.

8.A case of ruptured hepatic angiomyolipoma in a young male.

A 31-year-old male visited a local hospital due to sudden-onset severe abdominal pain. Abdominal computed tomography revealed a solid cystic mass with a size of approximately 12 cm and exhibiting both hemorrhage and fluid collection in the pelvic cavity. Emergency angiography and embolization were performed, and a large hepatic tumor was subsequently surgically resected. The tumor cells stained positive for human melanoma black-45 and smooth-muscle actin, and the pathologic diagnosis was hepatic angiomyolipoma. This case report also discusses the spontaneous rupture of a hepatic angiomyolipoma.

9.Spontaneous Rupture of Renal Angiomyolipoma Causing Hemorrhagic Shock.

We report a case of massive retroperitoneal hemorrhage from a spontaneous rupture of a renal angiomyolipoma in a 35 year-old man, that appeared unilateral and solitary tumor without stigma of tuberous sclerosis. He experienced a sudden onset severe pain on the right flank, but had no history of specific associated diseases or factors such as trauma or stone. The patient presented with a clinical picture of shock because of massive hemorrhage in despite of the emergency care and was treated by right total nephrectomy. Herein we review briefly the clinical presentation, diagnosis and management of renal angiomyolipoma.

10.Angiomyolipoma of Both Kidneys and Lymph Node.

We report on a patient with known tuberous sclerosis who has huge bilateral angiomyolipoma in both kidneys as well in the periaortic lymph node. Under preoperative diagnosis of angiomyolipoma, the problem was managed conservatively. Massive persistent hematuria was controlled by superselective angioinfarction with ethanol and functioning renal parenchyma was preserved in the right kidney. Bleeding within the tumor of the left kidney was treated by angioinfarction followed by nephrectomy.
