1.Optimization of Extraction Technology for Practolol Oral Liquids

Objective:To optimize the extraction technique for practolol oral liquids. Methods:An orthogonal test was adopted in the study. The content transfer rate of ginsenoside Rg1 and ginsenoside Re, and the extraction rate were used as the indices, the com-prehensive scoring method was applied central to optimize the experimental data, and SPSS 17. 0 statistical software was employed to process the data. The extraction process was optimized. Results:The optimized technology was as follows:8-fold amount of 70% etha-nol, and extracting three times with 60min for each. Conclusion: The preferred process is scientific, reasonable, economical and practical, and suitable for the industrial production.
2.Inhibitory effect of combination of cetuximab and irradiation on colorectal carcinoma CL187 cells

Objective To investigate the combination effect of cetuximab and irradiation on colorectal carcinoma CL187 cell line and underlying molecular mechanism.Methods CL187 cells with or without cetuximab treatment were irradiated by 0,4 and 8 Gy X-rays,then cell death percentage was determined by MTT 24 and 48 h post-irradiation.Clone forming assay was used to evaluate the cell reproliferation ability.Cell cycle distribution,apoptosis,and necrosis were analyzed by flow cytometry.Western blot was used to detect the protein expressions of DNA-PKcs,Ku70 and Ku80.Results The cetuximab enhanced the percentage of radiation-induced cell death,while descreased the cloning formation capacity and increased radiosenvtivity (t =-6.14、-6.53,P <0.05).The SER of cetuximab on CL187 cell line approached to 1.38.In addition,cetuximab also increased radiation-induced G0/G1 phase arrest (t=-4.64,P<0.05) and the percentage of apoptosis and necrosis (t=-9.16,P <0.05),but it descreased the expression levels of DNA-PKcs,Ku70 and Ku80 proteins.Conclusions The cetuximab treatment might enhance the inhibitory effect of irradiation on colorectal carcinoma CL187 cell line by influencing cell cycle distribution,cell apoptosis,and the expression of DNA repair proteins.
3.Clinical implications of thyrotropin receptor antibody measurement

It has been 50 years since the discovery of thyrotropin receptor autoantibody (TRAb). Advances in the knowledge of thyrotropin receptor ( TSHR) structure and function, combined with the elucidation of TSHR signaling and TSHR-autoantibody interaction have greatly facilitated our understanding of TRAb and their clinical applications. Measurement of TRAb activity plays an important role in the diagnosis of Graves' disease ( GD) and Graves' opthalmopathy. It has also been well recognized that TRAb is an effective predictor of GD relapse or remission after antithyroid drug and radioactive iodine treatment. TRAb test is of particular help in pregnant women and lactating mothers with recent iodine load, where radioactive iodine or technetium tests are contraindicated. In addition, it is useful in the diagnosis and differential diagnosis of fetal and neonatal hyperthyroidism as well as some rare forms of thyrotoxicosis in clinical practice. Accumulating evidence also indicates the possible correlation between thyroid cancer occurring in GD patients with positive TRAb and adverse outcomes. However, further innovation and standardization of TRAb tests are required to help pave the way for clinical applications.
4.The different biological effects of single, fractioned and continuous low dose rate radiations on CL187 colorectal cancer cell line

Objective To investigate the effect and underlying mechanism of single,fractioned and continuous low dose rate radiation on CL187 colorectal cancer cell line.Methods CL187 cells were exposed to 6 MV X-rays at a high dose rate of 4 Gy/min and 125Ⅰ seed at a low dose rate of 2.77 cGy/h with three groups:single dose radiation group (SDR),fractioned dose radiation group (FDR) by 2 Gy/f,and continuous low dose rate radiation group (CLDR).The radiation doses were 0,2,4 and 8 Gy.Total cell number and cell viability were determined by trypan blue.Clone forming assay was used to evaluate the cell proliferation ability.The percentage of apoptosis cells was analyzed by flow cytometry.Western blot was used to detect the protein expression levels of PHLPP2,PTEN and Bax.Results Compared with SDR and FDR groups,the total cell number and survival fraction of CLDR group decreased.The relative biological effect (RBE) for 125Ⅰ seeds compared with 6 MV X-rays was 1.41.The percentage of apoptosis cells of CLDR group was significantly increased (t =-15.08,-11.99,P < 0.05).The expression level of Bax increased in CLDR group,while no obvious changes were observed on PHLPP2 and PTEN among three groups.Conclusions The expression level of PHLPP2 increaseS in SDR,FDR and CLDR group,while it seems that it was not influenced by dose rate.The expression level of Bax increased in three groups,while more colorectal CL187 cells in CLDR group may be killed due to the increase of Bax expression.
5.Comparison of incisions between two approaches in areolar reduction surgery

Objective To compare the perinipple round-block technique with the conventional periareolar round-block technique in areolar reduction operations.Methods A total of 37 patients who underwent areolar reduction operations in our department were randomly allocated into two groups:Group A (19 patients) received a conventional periareolar round-block technique,while group B (18 patients) through a new perinipple round-block technique.In the latter approach,an intraareolar donut of pigmented skin was deepithelialized,and the exposed areolar dermis was then telescoped inward and stretch-anchored to an imaginary circular line situated beneath the breast skin areola junction.The roundblock technique was then utilized to approximate the skin edges,resulting in a minimal scar,totally inconspicuous,confined to the immediate perinipple area.The operative time,perioperative complications,scars,absorption time of the skin folds,the sensation of the nipple-areolar complex and the patients' satisfaction were compared between the two groups.Results The mean operative time of group A and group B was 40.5 min and 45.8 min,respectively (P<0.05).No perioperative complications were found in these two groups.No patient had experienced permanent nipple sensory changes.The skin folds absorption time of group A was 4.34 months,while the time of group B was 8.78 months (P<0.01).The perinipple round-block technique produced a more inconspicuous scar with a better aesthetic results.Conclusions The traditional periareolar round-block technique leaves a sharply demarcated areola by a circular scar which has a totally unnatural and artificial look.As for the perinipple round-block technique,the incision as well as any residual cutaneous wrinkling or puckering is well concealed by the dome of the nipple and the areolar,but it needs a longer skin folds absorption time.
6.Clinical significance of muscle sparing verticle rectus abdominis myocutaneous flap for vaginal reconstruction

Objective To evaluate the method for vaginal treconstruction with muscle sparing verti-cle rectus abdominis myocutaneous flap(MS-VRAM flap). Methods From September 2006 to April 2008, MS-VRAM flaps were used for vaginal reconstruction in 9 patients (20 to 35 years old), ancl all ca-ses were congenital absence of vagina. Before operation, the inferior epigastrie arteries were detected by the Doppler and the flaps based on the perforators ranged from 6 cm×20 cm. MS-VRAM flaps were ele-vated and then transferred to reconstruct the vagina. Results All 9 cases of MS-VRAM flaps survived completely. No complications occurred at donor site of abdominal wall. With 2-12 months' follow-up, the patients were satisfied with the results. Conclusion Despite technical difficulties in elevating the MS-VRAM flap, the flap is a good choice for vaginal reconstruction.
7.Application of Management Information System for Medical High-value Consumables in Operation Room

Objective To make the management responsibility clear for guarding against the lacking of legal basis in patient lawsuits.Methods An implantable coding database for medical consumables was set.Barcode on packaging bag was scanned in 'information system'.'secondary management database for high-value consumables' was set in operation room.According to a warehouse-out list,the warehouse was checked monthly.Results The flow of all consumables was controlled.Conclusion Operational links are reduced and work efficiency is improved.Besides,the transparency of charges is increased.[Chinese Medical Equipment Journal,2008,29(2):64-65]

9.Effects of osteoblasts on the formation of osteoclasts in vitro

Objective By culturing the osteoclasts together with the osteoblasts directly to investigate the effect of osteoblasts on the formation of mature osteoclasts.Methods The bone marrow mononuclear cells of rats were treated with 30?g/L M-CSF and 50?g/L RANKL and cultured for 6 days.Subsequently,the primary osteoblasts which were of the same quantity as the osteoclasts were co-cultured directly.In the co-culture system,we added the liquid containing 1,25-(OH)2D3 1?10-8mol/L and PGE2 1?10-6mol/L.The morphological observation,TRAP staining and pit staining were adopted to identify osteoclasts.Results When the osteoclasts were co-cultured with primary osteoblasts,the growth of osteoblasts had more preponderances.After staining,we could see more osteoblasts than osteoclasts.Conclusion The relationship between osteoblasts and osteoclasts is related to the relative quantities of the two cells.When osteoblasts outnumber osteoclasts,osteoblasts would inhibit the formation and differentiation of osteoclasts.
10.Role of maximal sterile barrier precaution on preventing catheter-related bloodstream infection

Objective To investigate the effect of maximal sterile barrier precaution during deep venous/arterial catheterization on preventing catheter-related bloodstream infection (CRBSI)in patients in the department of critical care medicine.Methods 996 patients who were hospitalized in Beijing Friendship Hospital and underwent deep ve-nous/arterial catheterization from September 2011 to April 2014 were analyzed retrospectively,patients were divided into standard sterile barrier precaution group (SSB group,September 2011-December 2012,n=560)and maximal sterile barrier precaution group (MSB group,January 2013-April 2014,n=436)based on whether they received maximal sterile barrier precaution,incidence of CRBSI and mortality were compared between two groups.Results There was no statistical difference in constitute of intubation sites between patients in SSB group and MSB group(χ2=6.750,P=0.08).The incidence of CRBSI per 1000 catheter days in SSB group and MSB group were 2.41‰(1.64‰-4.02‰)and 1.91‰(0‰-4.56‰)respectively,rank test revealed no significant difference(Z=-0.057, P>0.05);24 (4.29%)patients in SSB group and 26(5.96%)patients in MSB group developed CRBSI,difference between two groups was not statistically significant(χ2=1.447,P>0.05).The mortality in SSB group and MSB group were (18.43±5.53)% and (11.68±4.14)% respectively,independent sample t-test revealed that difference was significant(t=3.907,P<0.05).Conclusion Maximal sterile barrier precaution did not reduce the incidence of CRBSI.
