Spinal anesthesia with Marcaine 0.5%, for various kinds of operations was evaluated for complications. 90 patients were randomly allocated in to 2 groups: Group I: using isobaric Marcaine 0.5% and Group II: using hyperbaric Marcaine 0.5%, Patients were put on vigilant monitored for detecting effects on respiratory and cardiovascular system peri and postoperatively. Results: Group I and II showed significant different effects on cardiovascular system (P<0.05) by the deduction on blood pressure (>20%): group I: 2.22%; group II: 8.89% and the deduction on mean pulse: group I: 2.18 beat/minute; group II: 13.13 beat/minute. Affect on respiratory system was trivial in both groups. Other complications were not differ significantly (p<0.05) including: Nausea and vomit: group I: 2.22%; group II: 4.44%; Chill: group I: 2.22%; group II: 6.66%; Headache: group I: 4.44%; group II: 6.66%.

Comparision of analgesia and side effects in spinal anesthesia administered by bupivacaine combined with fentanyl in 66 patients with indicated abdominal hysterectomy in the Institute of Military Medicine No.103, from February to July, 2003. The results showed that: spinal anesthesia and the safe in surgeries have agained with the same dose 0.18mg/kg bupivacaine and 25mcg fentanyl at spinal interspace L2-3 and L3-4. However, spinal anesthesia was administered at the interspace L2-3 difference L3-4: onset time of sensory blockade at the T6 is faster, analgesia level after 20mins is higher, hemodynamic changing are faster and more serious

Epidural anesthesia was given to a 72-year-old female patient suffering from cholecystitis due to multiple gallstones, and chronic bronchial asthma. In the course of the procedure we inserted an epidural catheter (Deseret catheter with a plastic stylet) via a 17G Tuohy needle at the L1-2 intervertebral space. Since it was very difficult to insert the catheter through the Tuohy needle, we pushed it forward applying a considerable amount of force. Following the insertion of the catheter and prior to injection of a test dose of local anesthetic, it was found-that cerebrospinal fluid was flowing out through the catheter from a puncture of the dura. In a second attempt, we approached another intervertebral space, this time with a Portex catheter without a stylet, and succeeded in administering the anesthetic by means of a continuous epidural procedure.

Either tacrine (THA) with succinylcholine or alcuronium was used on a randomized basis for laparoscopic procedures in twenty young females. The technique using THA with succinylcholine was found to be more suitable and predictable for this procedure and gave a smoother anaesthetic course, brighter recovery and minimal postoperative complications.

OBJECTIVE: To observe the effect of electroacupuncture (EA) on laparoscope postoperative shivering in patients undergoing general anesthesia and explore its effect mechanism. METHODS: A total of 80 patients with elective laparoscopic resection of intestinal tumor under general anesthesia were randomly divided into an EA group and a tramadol group, 40 cases in each group. Thirty min prior to the end of the operation, in the EA group, EA was exerted at Neimadian and Zusanli (ST 36), with disperse-dense wave, 2 Hz/100 Hz in frequency, 1 mA in intensity, and lasting 30 min. In the tramadol group, tramadol hydrochloride injection was dropped intravenously, 1 mg/kg. The conditions of shivering, dizziness, nausea, vomiting and agitation were observed in the post-anesthesia care unit (PACU). Heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) were observed before treatment (T₀), at the moment of extubation (T₁), in 3 min of extubation (T₂) and 1 h after operation (T₃). Using ELISA, at T₀ and T₃, the expression levels of interleukin 6 (IL-6) and 5-hydroxytryptamine (5-HT) in plasma were detected separately. Choking and agitation were recorded during extubation. RESULTS: ① In the EA group, the incidence of shivering, dizziness, nausea, vomiting and agitation in the PACU was lower than that in the tramadol group (P<0.05). ②Compared with T₀, HR, SBP and DBP were increased at T₁ and T₂ in the tramadol group (P<0.05). HR, SBP and DBP in the EA group were lower than the tramadol group at T₁ and T₂ (P<0.05). ③Compared with T₀, the expression levels of IL-6 and 5-HT in plasma were increased at T₃ in the tramadol group (P<0.05). The expression levels of IL-6 and 5-HT in the EA group were lower than the tramadol group at T₃ (P<0.05). ④The incidence of choking and agitation during exudation in the EA group was lower than that in the tramadol group (P<0.05). CONCLUSION Electroacupuncture can reduce the incidence of laparoscopic postoperative shivering under general anesthesia. The potential mechanism mays related to the modulation of the expression levels of IL-6 and 5-HT caused by surgical trauma.

This paper attempts to explore and think about the harm and countermeasures of epidural anesthesia catheter fracture from the point of view of medical device supervision.
