1.Partial androgen deficiency in aging males.

Partial androgen deficiency in aging males can be defined as a clincal condition characterized by a partial deficiency of androgen in blood and/or a decrease genomic sensitivity to testosterone or its active metabolites in target tissues. This state of hypogondism leads to a decline of physical energy, an altered state of well-being, a sexual dysfunction and various metabolic alterations. Testosterone supplement therapy can improve all these symptoms.

2.A comparative survey of sex hormones levels between elderly men with chronic heart failure and normal elderly subjects.

OBJECTIVETo understand the sex hormone levels in elderly men with chronic heart failure and the relationship between sex hormone levels and heart function.
METHODSOne hundred male patients diagnosed chronic heart failure clinically aged from 60 to 87 were selected in this study. All patients' left ventricular ejection fraction were less than 0.45. Four hundred elderly healthy men were studied as control. Blood samples were collected in the morning and TT, FT, DHEAS, SHBG, E(2), LH, FSH were measured. The results were compared with the hormone levels of healthy men with the same mean age.
RESULTS(1) In elderly men with chronic heart failure, the level of DHEAS was decreased with age (P < 0.05). However, the levels of SHBG, LH, FSH were increased with age (P < 0.05, P < 0.01). (2) Compared with healthy men the levels of TT, FT, E(2) and DHEAS were decreased significantly (P < 0.01) and the level of SHBG was significantly increased (P < 0.01). (3) The level of FT was positively correlated with LVEF (r = 0.279, P = 0.034).
CONCLUSIONThe androgen levels in elderly male patients with chronic heart failure were decreased significantly and the level of FT was negatively correlated with degree of heart failure.

3.Study of androgen and atherosclerosis in old-age male.

OBJECTIVETo observe the difference of androgen and inflammatory cytokines level in atherosclerosis and analyse their relations.
METHODBoth carotid arteries and arteries of lower extremity were subjected to ultrasonic examination by Doppler's method. Those with much atheromatous plaque formation were ranged into case group, and those with normal result formed control group. Total, free testosterone and estradiol were assayed by radioimmunoassay. C reactive protein (CRP) was assayed by nepheloturbidity. Tumor necrosis factor-alpha (TNF-alpha), Interleukin-6 (IL-6), Interleukin-8 (IL-8), Interleukin-10 (IL-10), Interleukin-18 (IL-18), soluble intercellular adhesion molecule-1 (sICAM-1) and soluble vascular cell adhesion molecule-1 (sVCAM-1) were assayed by ELISA. The mean difference between two groups and the correlation between free testosterone and cytokines were analysed.
RESULTSFree testosterone was (6.337+/-3.371) pg/L in case group and (11.375+/-4.733) pg/L in control group, P<0.01. No differences were found in total testosterone and estradiol. CRP was (27.294+/-10.238) mg/L in case group and (12.843+/-6.318) mg/L in control group, P<0.01. IL-6 was (41.700+/-31.385) pg/L in case group and (25.396+/-20.772) pg/L in control group, P<0.05. IL-8 was (89.249+/-58.357) pg/L in case group and (67.873+/-31.227) pg/L in control group, P<0.05. sICAM-1 was (470.491+/-134.078) pg/L in case group and (368.487+/-97.183) pg/L in control group, P<0.01. sVCAM-1 was (537.808+/-213.172) pg/L in case group and (457.275+/-157.273) pg/L in control group, P<0.05. There were no differences in TNF-alpha, IL-10 and IL-18. Correlation analysis showed that FT (free testosterone) had negative correlation with CRP, IL-6 and sICAM-1. Among them FT had well correlation with CRP, correlation index was -0.678.
CONCLUSIONFree testosterone was in negative correlation with atherosclerosis in old-age male. Free testosterone may have the role of anti-atherosclerosis, and this effect was not achieved by its transformation to estradiol. Low free testosterone level was followed by increased level of inflammatory cytokines. Low free testosterones coexist with inflammation and they both affect the process of atherosclerosis in old-age male.

4.A Case of Paroxysmal Nocturnal Hemoglobinuria.

Paroxysmal Nocturnal Hemoglobinuria (PNH) is an uncommon hematologic disease characterized by an abnormal sensitivity of blood cells to the lytic action of serum complement. We experienced one case of PNH in Yeungnam University Hospital from May 1983 to May 1989. The patients was followed up without severe complications of 4 years since diagnosis with the only conservative treatments such as washed blood transfusion, adrenal corticosteroids, androgens, folate and iron preparation, intermittently.

5.Study on effects of acupuncture combined with medication on reproductive endocrines in the patient of PADAM.

OBJECTIVETo observe effects of acupuncture combined with medication on reproductive endocrines in the patient of partial androgen deficiency of aging men (PADAM).
METHODSSixty cases conforming to diagnostic criteria of PADAM were randomly divided into 3 groups, an observation group treated by acupuncture combined with medication, a control group I by simple acupuncture and a control group II by oral administration of Bushen Capsules, 20 cases in each group. Serum levels of testosterone (T), luteinizing hormone (LH) and follicle stimulating hormone (FSH) before and after treatment were determined with radioimmunoassay.
RESULTSAfter treatment, the serum levels of T in the 3 groups increased significantly, with more significantly increased in the observation group (P < 0.01); after treatment, the serum levels of LH in both the observation group and the control group I decreased significantly (P < 0.05), but in the control group II did not significantly change (P > 0.05); after treatment, FSH content in the observation group significantly increased (P < 0.05), but in the control group I and the control group II did not significantly change (P > 0.05); after treatment, the symptoms and signs in the 3 groups significantly improved and PADAM indexes decreased significantly (P < 0.05 or P < 0.01).
CONCLUSIONAcupuncture combined with medication has a better therapeutic effect on PADAM, and it can regulate secretion of hormones in the hypothalamus-pituitary-testicle axis and can increase blood T level, improve physical ability, vascular function, sexual function and quality of life in the patient of PADAM.

6.Vasectomy has no obvious longterm influence on the levels of serum androgens in aging males.

Objective:
To explore the longterm influence of vasectomy on the levels of serum androgens in aging males.
METHODS:
Using stratified random sampling, we conducted a questionnaire survey and physical examinations among 437 adult males aged ≥40 years, 232 with and 205 without the history of vasectomy. In addition, we measured the levels of serum total testosterone (TT), sexhormone binding globulin (SHBG), calculated free testosterone (cFT), testosterone secreting index (TSI), free testosterone index (FTI), and luteinizing hormone (LH).
RESULTS:
Compared with the nonvasectomy group, the vasectomy group showed significantly increased levels of serum TT ([16.01±5.41] vs [17.39±6.57] nmol/L), SHBG ([58.91±36.89] vs [70.28±40.90] nmol/L), and LH ([8.86±6.49] vs [10.85±11.73] IU/L) (all P< 0.05) and a decreased level of FTI (0.33±0.15 vs 0.30±0.12, P< 0.05). There were no statistically significant differences between the nonvasectomy and vasectomy groups in cFT ([0.24±0.07] vs [0.23±0.09] nmol/L) or TSI ([2.42±1.34] vs [2.46±1.51] nmol/IU) (both P>0.05), nor after adjustment for relevant factors in TT (β: 1.015, 95% CI: -0.180-2.210), SHBG (β: 5.118, 95% CI: -2.069-12.305), cFT (β: 0.003, 95% CI: -0.011-0.018), FTI (β: -0.012, 95% CI: -0.035-0.011), TSI (β: 0.138, 95% CI: -0.131-0.407), and LH (β: 1.011, 95% CI: -0.811-2.834) (all P>0.05).
CONCLUSIONS
Vasectomy has no obvious longterm influence on the levels of serum androgens in aging males.

7.Impact of Cryptotanshinone on the reproductivity and metabolism of male mice with Akt2 deletion.

OBJECTIVETo investigate the impact of protein kinase B (Akt2) allele deletion on testicular reproductive function, and to discuss the regulatory effect of Cryptotanshinone on the reproductivity of male mice with Akt2 allele deletion and its molecular mechanism.
METHODSFifteen Akt2 +/+ male mice were randomly divided into Groups A (baseline control, n = 7) and B (stimulation, n = 8), and another 29 Akt2 -/- male mice into C (baseline control, n = 7), D (stimulation, n = 8), E (solvent, n = 7) and F (Cryptotanshinone, n = 7). Groups B and D underwent human chorionic gonadotropin (HCG) stimulation tests at 5 IU / 20 g, while A and C received physiological saline, all for 4 hours; Group F were given gastric lavage of Cryptotanshinone, while E solvent only, at 600 mg/kg twice a day for 8 weeks, both subjected to oral glucose tolerance tests (OGTT) at 2 g/kg before and after the treatment. The body and bilateral testis weights were obtained, the serum testosterone (T) level measured, and the expressions of testicular steroid hormone synthesis and glycometabolism-related genes determined by RT-PCR.
RESULTSOGTT showed that the level of blood glucose was significantly higher in Groups C and D than in A and B ([10.38 +/- 1.42] and [10.96 +/- 1.81] mmol/L vs [7.92 +/- 0.63] and [8.32 +/- 0.44] mmol/L, P < 0.05), but had no significant differences at different time points in E and F (P > 0.05). The testis weight was remarkably higher in Groups C and D than in A and B ([0.17 +/- 0.01] and [0.17 +/- 0.01] g vs [0.15 +/- 0.01] and [0.15 +/- 0.02] g, P < 0.05), but exhibited no obvious difference in E and F, nor were there any significant differences in body weight among different groups (P > 0.05). The serum T level was markedly higher in Group C than in A ([9.08 +/- 1.59] nmol/L vs [6.42 +/- 0.95] nmol/L, P < 0.05), but evidently lower in F than in E ([5.94 +/- 0.49] nmol/L vs [8.18 +/- 1.44] nmol/L, P < 0.05). The baseline expression levels of Cyp11, Cyp17, 3B-HSD, Star, Gsk3beta, Erk-1, and MCM2 mRNA were significantly higher in Group C than in A (P < 0.05). After HCG stimulation, the expressions of Cyp11, Cyp17, 3B-HSD, and Star mRNA were remarkably increased in B and D, but with no obvious difference between the two groups (P > 0.05), while the expressions of Cyp11, Cyp17, 3B-HSD, Star, Gsk3beta, Erk-1, and MCM2 mRNA markedly decreased in F as compared with E (P < 0.05).
CONCLUSIONAkt2 gene deletion may affect glycometabolism and testicular function, and cause abnormal glycometabolism and androgen secretion in male mice, whose molecular mechanism is associated with the elevated expressions of the key glycometabolic molecules and of the key enzymes for androgen synthesis. Cryptotanshinone can reduce the levels of androgens by down-regulating the expressions of the key enzymes for androgen synthesis.

8.Atherosclerosis and androgen levels in elderly males.

OBJECTIVETo observe the relation of androgen levels and atherosclerosis (AS) in elderly males.
METHODSBoth carotid arteries and arteries of lower extremity were examined with Doppler ultrasonography. Those with arteriosclerosis and much atheromatous plaque were designated as case group, and those with normal results formed control group. Total testosterone (TT), free testosterone (FT) and estradiol (E2) were measure by radioimmunoassay, HDL-C, LDL-C, TC and TG were assayed by colorimetry, vascular endothelium growth factor (VEGF), tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL-6), intercellular adhesion molecule (sICAM-1) were determined by ELISA.
RESULTFT was significantly lower in case group than in control group (P<0.01), no differences were found in TT, E2. HDL-C in control group was higher than that in case group (P<0.01), TC and TG were higher in case group than those in control group (P<0.05). HDL-C was correlated positively and LDL-C was negatively with FT level, while both TC and TG in case group had negative relation with FT. VEGF was higher in case group (P<0.05), and it had negative relation with FT in both groups. TNF-alpha and IL-6 were significantly higher in case group (P<0.05), and they had negative relation with FT. sICAM-1 was significantly lower in control group than it in case group (P<0.01).
CONCLUSIONThe normal androgen levels, especially FT, have beneficial effect in AS development in elderly males. Low FT level may be an independent risk factor in AS development.

9.Value of the serum free testosterone level in detecting erectile dysfunction.

Objective:
To investigate the values of serum calculated free testosterone (cFT), testosterone secretion index (TSI), and free testosterone index (FTI) in the diagnosis of ED with androgen deficiency by observing their changes in the patient.
METHODS:
We conducted this study among 185 men complaining of ED and 35 20-40 years old healthy males presenting at the clinic for premarital medical checkup. We asked them about their medical history, to fill in the International Index of Erectile Function (IIEF-5) Questionnaire, and to complete the nocturnal penile tumescence (NPT) test. According to the data obtained, 150 of the complainants were diagnosed as ED patients and 25 of the healthy examinees were included in the control group. We determined the levels of total serum testosterone (TT), luteinizing hormone (LH), sex hormone-binding globulin (SHBG), serum albumin (ALB), cFT, bio-available testosterone (bio-T), TSI, and FTI in the two groups of subjects. Using cFT ≤0.3 nmol/L, TSI ≤2.8, and FTI ≤0.4 as the critical values and TT ≤11.5 nmol/L as the gold standard for androgen deficiency, we calculated cFT-, TSI-, and FTI-related rates of missed diagnosis, misdiagnosis, and diagnostic coincidence.
RESULTS:
With TT ≤11.5 nmol/L as the criterion, the cFT-, TSI-, and FTI-related rates of coincidence in the diagnosis of androgen deficiency in the ED patients were 90.8%, 85.8%, and 80.8%, those of missed diagnosis were 4.0%, 33.3%, and 44.0%, and those of misdiagnosis were 10.5%, 19.4%, and 12.6%, with the Kappa of values 0.755, 0.564, and 0.427, respectively (P <0.05). The levels of serum TT, cFT, Bio-T, TSI, and FTI were decreased with increased age of the 20-40 years old ED patients, with statistically significant differences among different age groups except the serum TT level. However, no statistically significant differences were found in the levels of TT, cFT, Bio-T, TSI, and FTI among the patients with different IIEF-5 scores.
CONCLUSIONS
The level of cFT has a higher value than those of TT, TSI, and TSI in the diagnosis of ED with androgen deficiency in 20-40 years old men.

10.Preliminary study on androgen dependence of calcitonin gene-related peptide in rat penis.

UNLABELLEDTo study the androgen dependence of the neurotransmitter, calcitonin gene-related peptide (CGRP) in rat penis.
METHODSForty-four Sprague-Dawley rats were randomly divided into Group A (intact controls), Group B (castrated) and Group C (gavaged with finasteride 4.5 mg.kg(-1).day(-1)). Four and ten weeks later respectively, half of rats in each group were anaesthetized. Blood samples were taken for the measurement of serum testosterone and dihydrotestosterone (DHT) by means of radioimmunoassay. Penile samples were harvested for the investigation of calcitonin gene related peptide (CGRP)-immunoreactive nerve fibers with immunohistochemistry. The computer-assisted imaging analysis system was applied to calculate the area proportion of the CGRP-positive nerve fibers (CGRP-PNF) in each group.
RESULTS1) Both 4 and 10 weeks later, testosterone and DHT levels in Group B decreased significantly compared with those in Group A, (P <0.05, P <0.01, respectively); DHT level in Group C was also significantly decreased in comparison with that in Group A for both 4- and 10- week animals (P <0.05); 2) There was no significant differences in area proportion of CGRP-PNF among Groups A, B and C 4 weeks after treatments (P >0.05); However, 10 weeks later, the proportion of CGRP-PNF in Groups B and C was significantly less than that in Group A (P <0.01); 3) The proportion of CGRP-PNF of 4-week animals in Groups B and C was significantly higher than that of 10-week animals (P <0.05).
CONCLUSIONThe expression of neurotransmitter, CGRP may depend on androgens, including testosterone and DHT in rat penis.
