1.External iliac artery injury secondary to indirect pressure wave effect from gunshot wound.

In patients presenting with gunshot wounds, a high clinical suspicion of injury to vasculature and viscera remote from the projectile track is paramount. We present a case of a 17 year old male who sustained a gunshot wound to his abdomen and subsequently developed a right external iliac artery contusion requiring surgery as an indirect effect of the pressure wave from the bullet.

The aetiology, incidence and management of type II endoleaks in standard infrarenal endovascular aortic aneurysm repair is well described. Far less data is available for thoracic stent grafting. We present a rare and interesting case of a type II endoleak post thoracic aortic stenting and highlight the aberrant anatomy that can cause this phenomenon in such cases.

3.An Australian Evaluation of the Tick Program Awareness among University Students

Cardiovascular disease is a major cause of death in Australia. The Tick Program by the National Heart Foundation was designed to assist consumers in making healthier food choices. The aim of our study was to evaluate the awareness of university students regarding the Tick Program as a sustainable approach in preventing the onset of cardiovascular disease (CVD) in youth. Following Ethics Committee approval, a cross-sectional study was undertaken in 2006 to measure university students’ level of awareness of the Tick Program using a self-administered survey form. Inclusion criteria were full-time university students who have lived in Australia for a minimum duration of twelve months and do their own shopping. Students of less than 18 years of age were excluded from the study. Of 110 university students surveyed, 97 questionnaires were successfully completed (response rate: 88%). Overall there was a high level of awareness (72.2%) of the Tick program, which was also considered trustworthy by a majority of participants, with a mean rating of 3.87 (on a scale of 1 to 5). Tick-approved products were also considered a healthier choice by participants (mean 4.06 out of 5). Participants were also asked to identify potential barriers limiting the use of the Tick in making purchase decisions. The most important barrier identified to the Tick program was the limited range of Tick-approved products. A significant proportion of respondents also believed there was limited publicity of the program. The Tick Program is considered to be trustworthy and the approved products were regarded as healthy, with the results showing that participants have confidence in the Tick Program. This research also highlighted the potential areas for improvement of the Tick Program.

We present a rare case of a patient with aortoiliac occlusive disease on the background of type A crossed renal ectopia, for whom open surgical intervention was required. Aortic exposure in patients with concomitant crossed renal ectopia can present technical challenges to the vascular surgeon. The knowledge of variations in the ectopic renal blood supply is of paramount importance when performing surgery to treat this condition and affects the choice of surgical exposure. We present and discuss the operative details of our patient and outline an approach to this subset of patients.

5.Forty hours with a traumatic carotid transection: A diagnostic caveat and review of the contemporary management of penetrating neck trauma.

Although penetrating neck trauma (PNT) is uncommon, it is associated with the significant morbidity and mortality. The management of PNT has changed significantly over the past 50 years. A radiological assessment now is a vital part of the management with a traditional surgical exploration. A 22 years old male was assaulted by a screwdriver and sustained multiple penetrating neck injuries. A contrast CT scan revealed a focal pseudoaneurysm in the left common carotid artery bulb. There was no active bleeding or any other vascular injuries and the patient remained haemodynamically stable. In view of these findings, he was initially managed conservatively without an open surgical exploration. However, the patient was noted to have an acute drop in his hemoglobin count overnight post injury and the catheter directed angiography showed active bleeding from the pseudoaneurysm. Surgical exploration 40 hours following the initial injury revealed a penetrating injury through both arterial walls of the left carotid bulb which was repaired with a great saphenous vein patch. A percutaneous drain was inserted in the carotid triangle and a course of intravenous antibiotics for five days was commenced. The patient recovered well with no complications and remained asymptomatic at five months followup.

INTRODUCTION:
Despite adhering to criteria for extubation, up to 20% of intensive care patients require re-intubation, even with use of post-extubation high-flow nasal cannula (HFNC). This study aims to identify independent predictors and outcomes of extubation failure in patients who failed post-extubation HFNC.
METHODS:
We conducted a multicentre observational study involving 9 adult intensive care units (ICUs) across 5 public hospitals in Singapore. We included patients extubated to HFNC following spontaneous breathing trials. We compared patients who were successfully weaned off HFNC with those who failed HFNC (defined as re-intubation ≤7 days following extubation). Generalised additive logistic regression analysis was used to identify independent risk factors for failed HFNC.
RESULTS:
Among 244 patients (mean age: 63.92±15.51 years, 65.2% male, median APACHE II score 23.55±7.35), 41 (16.8%) failed HFNC; hypoxia, hypercapnia and excessive secretions were primary reasons. Stroke was an independent predictor of HFNC failure (odds ratio 2.48, 95% confidence interval 1.83-3.37). Failed HFNC, as compared to successful HFNC, was associated with increased median ICU length of stay (14 versus 7 days,
CONCLUSION
Post-extubation HFNC failure, especially in patients with stroke as a comorbidity, remains a clinical challenge and predicts poorer clinical outcomes. Our observational study highlights the need for future prospective trials to better identify patients at high risk of post-extubation HFNC failure.
